Inequities in Cities and in Urban Health 城市和城市卫生方面的不公平现象

As cities grow and thrive, urban populations have access to more resources and better health than has ever been the case in human history. However, at the same time, the material gaps between the haves and have-nots are also starker in cities, where large populations live in close proximity, than they are anywhere else. These social divides map onto health divides, creating health haves and health have-nots, thus threatening to create two classes of population in close geographic proximity. This chapter discusses the challenge of resource inequities in cities and how these inequities become health inequities.

The Partnership for Healthy Cities is a global network of cities that is committed to reducing morbidity and mortality from noncommunicable diseases among their populations. Funded by Bloomberg Philanthropies, in partnership with the World Health Organization and Vital Strategies, the Partnership engages cities that are willing to take on high-impact innovative approaches to prevent and ameliorate the burden of noncommunicable diseases (e.g., cancer, cardiovascular disease, diabetes, etc.). This chapter discusses the vision behind, the goals of, and the successes and challenges of the Partnership for Healthy Cities, with a particular focus on lessons learned that can broadly be applied to urban health.

The growth of urban health, perhaps in parallel with the global growth of urban living, represents an exciting evolution in an interdisciplinary field. As more scholarship emerges in this area, the opportunity for transdisciplinary synthesis increases, making way for a whole new scholarship that will help us better understand the particular challenges and opportunities presented by characteristics of cities as it adopts methods and approaches from long-established disciplines. This chapter brings together concepts presented in the preceding chapters of this book, summarizing key points with an eye to pointing the way to the next decade of scholarship in urban health.

The production of health in cities is driven by a range of features of the urban environment, including characteristics of interpersonal interactions, of neighborhoods, and of entire cities themselves as well as citywide policies. This rests directly on multilevel causal frameworks that aim to understand how forces at different levels of influence and produce health. This approach, now firmly embedded in many quantitative social sciences, provides a conceptual and analytical framework through which we can understand the production of health in cities. This chapter introduces a multilevel approach and its attendant methods to the end of understanding core questions in urban health.
