Cross-Cultural Adaptation of English Version of Vertigo Symptom Scale in Punjabi 旁遮普语眩晕症状量表英译本的跨文化适应

Introduction: The Vertigo Symptom Scale mainly focuses on symptoms of vestibular disorders, that are anxiety and autonomic symptoms. Till now valid and culturally adapted Punjabi version of Vertigo Symptom Scale has not been produced yet, this study will describe the translation of the original 34-item version of VSS into Punjabi language and its Psychometric analysis in individuals with Benign Paroxysmal Positional Vertigo. The Cross-Cultural adaptation of a health status self-administered questionnaires for use in a new region, culture or language requires a distinctive methodology in order to reach equivalence between the original source and target languages. The adaptation of psychological scale is a complex task that requires orderly planning regarding its content maintenance, psychometric properties, and general validity for the intended population. Methods: The study has been split into two parts. The English version of Vertigo Symptom Scale will be translated into Punjabi language following guidelines given by of Beaton. In the second phase the translated Vertigo Symptom Scale will be then tested for its reliability in persons with Benign Paroxysmal Positional Vertigo. Results: Test retest reliability and internal consistency was calculated to check internal consistency Cronbach’s alpha, Split-Half (odd-even) correlation and Spearman-Brown Prophecy were calculated from the reading taken on Punjabi Vertigo Symptom Scale twice after the 48 hour. Calculated readings of Cronbach’s alpha are 0.998, Split- Half (odd even) correlation is 0.996, Spearman-Brown Prophecy is 0.997. Test retest intra-rater reliability calculated for Punjabi Vertigo Symptom Scale in group A is 0.998 (95% CI: 0.996-0.998). Conclusion: The translated version of Vertigo Symptom Scale has been demonstrated to be reliable, valid, acceptable and simple to understand by the patients. Data analysis show satisfactory high internal consistency and also the translated version was able to differentiate population with dizziness and without dizziness.

Comparison of Motor and Cognitive Performance of Educated Mothers with and without Cerebral Palsy Children 受过教育的脑瘫母亲与非脑瘫儿童运动和认知能力的比较

Introduction: The physical and psychological health of the parents, especially the mother, as the principal person responsible for the care, is influenced by the behavior and needs of children. Motor performance refers to a temporary status of motor behaviour, for example assessed during a motor practice session. Cognition is act for process of knowing including both awareness and judgment. Objectives: The main objective of this study is to compare motor and cognitive performance of educated Mothers with and without cerebral palsy children. Methods: A total of 100 mothers in the age group of 25-40 years were selected. Mothers should only have Cerebral Palsy child with aged 3-6 years. After the selection on the basis of criteria, mothers were divided into two groups of 50 each. Group A included the mothers of Cerebral Palsy child while Group B included mothers of normal child. Both groups were measured for motor performance by Figure of 8 and Rhythmic coordination test. Cognition of both groups was measured by Stroop test and Digit Symbol Substitution Test. Results: Unpaired ‘t’ test was used for analysis. Conclusion: The present study concluded that the Motor performance and cognition of mothers with cerebral palsy children is better than the mothers without cerebral palsy. However the result of motor performance and cognition between mothers of cerebral palsy child and mothers of normal child were statistically significant.

Functional Neuroimaging Guided Active Rehabilitation for PCS: A Retrospective Comparative Study of an NVC-Targeting Therapeutic Approach 功能性神经影像学引导的PCS主动康复:一种针对nvc的治疗方法的回顾性比较研究

Background: Recent work has demonstrated several advancements in therapeutic options for patients with postconcussion syndrome (PCS). Specifically, active rehabilitation has emerged as a promising direction for the best treatment outcomes. Enhanced Performance in Cognition (EPIC) is one form of active rehabilitation for PCS that uses a multi-disciplinary clinical approach focusing on two primary aims: the objective diagnosis of PCS using a quantitative, biomarker-based form of fMRI, and targeted neurorehabilitation. The targeting and rehabilitation of the neurovascular coupling (NVC) unit is an essential and novel component of this approach. This study seeks to contribute to the current field of active PCS therapy by demonstrating the usefulness of targeting NVC dysfunction. Further, we compare the EPIC protocol to treatment as usual using a retrospective comparative study design. Methods: The principal cohort was designated as all patients who received EPIC treatment from the time original pilot data was published to the time of retrospective data analysis and chart review (June 2016-October 2017) (N=375). Pre- and post-EPIC post-concussion symptom scales (PCSS) and severity index scores (SIS) were measured and compared. Then, based on pre-specified inclusion criteria for chart review, two patient cohorts were retrospectively assigned to treatment order 1 (TO1: N=15) and treatment order 2 (TO2: N=28). SIS was measured at pre-/post-EPIC and pre-/post-treatment as usual (TAU) functional neurocognitive imaging scans. SIS results from the treatment periods were compared and reported. Results: For the principal cohort (N=375), there was a statistically significant reduction in post-EPIC SIS and PCSS. Concerning the treatment cohorts, there was a highly significant (p<0.0001) overall effect of treatment type on SIS but no such effect of treatment order. However, analysis of TO2 alone revealed a significant effect on SIS reduction (p<0.001) if TAU occurs after EPIC therapy. Conclusion: The findings in this study appear to agree with the current body of research that demonstrates the effectiveness of active rehabilitation strategies compared to standard symptom-modulating and rest-based PCS therapies. Further, EPIC-style therapies contribute to the field of active PCS treatment by addressing NVC disruption in addition to the autoregulatory/vasoreactive aspects of PCS pathophysiology.

Suggestions for Treatment of Schizophrenia 精神分裂症的治疗建议

I am a scientist at the University of Hull in the Social Sciences, and a patient myself with schizophrenia since 2002 [1]. In my free time, I have researched schizophrenia and alternative treatments, and I would like to make some suggestions on how to improve the treatment of schizophrenic patients based on my own experience. I am not trained in psychiatry or biology, but I am a scientist, and I had the advantage that I could test all treatments on myself and experience any effects first hand.

Challenges of Parents with Autistic Children in Ghana 加纳自闭症儿童父母面临的挑战

This study looks into the challenges that parents with autistic children face in Ghana. The study looks specifically at challenges that being a parent in Ghana entails. Semi structured interviews were used as a form of data collection. Prominent themes the study highlights are the challenges of parents, which was associated with the inability of the parents to have meaningful conversations with their children, the financial burden of taking care of them, and stigmatization of their autistic children as well as themselves. The study was measured against already done work on the topic. From the study, parents major challenge had to do with finances, they explained the various sources in which these expenses were incurred to include transportation, medical bills and school fees. The study brought out transportation as a financial expense because of the where the study was conducted. Parents went on to address how people stigmatize them and their children whenever they are in public. The study also highlighted the challenge of not being able to communicate with their children with disability.

Downregulation of Ubiquitin Carboxyl-terminal Hydrolase L1 (UCHL1) Expression in the Pathogenesis of Alzheimers Disease 泛素羧基末端水解酶L1 (UCHL1)表达下调在阿尔茨海默病发病机制中的作用

The pathogenesis of Alzheimer’s disease (AD) has not been definitely confirmed so far. As a result, a cure for AD is lacking, and current treatments are limited to modest symptomatic relief. Accumulation of damaged proteins and formation of protein aggregates are found in AD brains, suggesting impairment of protein degradation is contained by the pathogenesis of AD. Impairment of ubiquitin-proteasome system has been found in Alzheimer Disease. Here we demonstrate ubiquitin carboxyl-terminal hydrolase L1 (UCHL1) which plays a role in neuronal ubiquitination/de-ubiquitination machinery participates in the pathogenesis of AD.

Clinical Investigation of Patients with Brain Damage and Incidental Occurrence of Simultanagnosia 脑损伤伴偶发同时失认症的临床研究

Purpose: The aim of this study was to investigate the clinical characteristics of patients with brain damage and incidental occurrence of simultanagnosia. Methods: Among patients with brain damage who visited Fukui General Clinic for undergoing rehabilitation between 2012 and 2017, 16 patients with incidental occurrence of simultanagnosia were investigated. With reference to Ohigashi's classifications, brain damage was classified into the following forms: attentive form, perceptual form, or semantic form; clinical characteristics, symptoms in daily life, and imaging characteristics in patients with each form of brain damage were investigated. Results: There were nine patients in the attentive form group, four in the perceptual form group, and three in the semantic form group. All patients in the attentive form group had right parietal lobe lesions, with six exhibiting unilateral spatial neglect and three exhibiting optic ataxia. All four patients in the perceptual form group had left temporo-occipital lobe lesions and exhibited slow processing speeds that affected their daily lives. All three patients in the semantic form group were elderly and had brain atrophy.Conclusions: We investigated lesion distribution and complicating symptoms in patients with brain damage and incidental occurrence of simultanagnosia based on the form of brain damage. In the attentive form group, unilateral lesions in the right parietal lobe had developed. Detailed interviews regarding daily life activities and detailed evaluation performed with the likelihood of simultanagnosia kept in mind are necessary when examining patients with brain damage.

Traumatic brain injury (TBI) has become a worldwide public health problem due to its high mortality and morbidity. Refractory intracranial hypertension (ICH), secondary to TBI, always results in poor prognosis and sometimes even death. Sedation, intracranial pressure (ICP) monitoring and mild hypothermia have been proven effective in monitoring and controlling ICH after TBI. Here we present a case of 55-year-old male suffering from acute TBI.

Indeed, I am now very busy in concluding my new book (second edition) about the “calcium paradox” discovery, including its impact in neurological and psychiatric diseases. While I am writing this editorial, this new book has been finalized. The contract has been signed, and everything has been running properly. What can we expect from this new book?
