One of the main causes of cognitive dysfunction in a late stage of HIV infection is HIV-associated encephalopathy. The direct neurotropic impact of HIV creates the pathogenic basis for this clinical syndrome. The purpose — to evaluate the cognitive status and neuron-specific enolase (NSE) level in cerebrospinal fluid (CSF) in HIV-encephalopathy patients. Material and methods. 35 patients were included in the study. They were divided into two groups: 20 patients (9 females and 11 males) with cognitive dysfunction and confirmed diagnosis of HIV/AIDS and 15 patients (6 females and 9 males) were in comparison group. The cognitive status was investigated by mini-mental state examination (MMSE), international HIV-dementia scale (IHDS) and frontal assessment battery (FAB). CSF level of NSE were investigated by ELISA test. Results. Correlation between cognitive dysfunction severity and NSE level in CSF was determined in HIV-encephalopathy patients. Conclusion. NSE level changing in CSF is an objective indicator of clinical severity of HIV-encephalopathy and neurotropic impact of HIV.

Despite the fact that the new coronavirus infection in children proceeds much milder than in adults, the pediatric practice is facing a problem associated with the general concept of «post-COVID conditions», including Multisystem Inflammatory Syndrome in Children (MIS-C) associated with COVID-19 and a complex of disease processes with various manifestations, united by the concept of postacute consequences of treatment or hospitalization for SARS-CoV-2 infection (PASC). The article describes a clinical case of combined post-COVID syndrome, in the form of a MIS-C and the subsequent development of PASC in the form of long-term recurrent urticarial rash elements in a 10-year-old child, simulating reactions similar to serum sickness and chronic urticaria.

The purpose — to assess the acute coronary syndrome (ACS) clinical aspects in patients with aortic stenosis. Material and methods. This is a retrospective single-center study that included comparison of hospital and midterm outcomes of ACS and aortic stenosis (n = 95, divided into the following groups: group 1 — severe aortic stenosis + ACS (n = 39); group 2 — moderate aortic stenosis + ACS (n = 28); group 3 — ACS without aortic stenosis (the control group selected using «Propensity matching» method) (n = 28)). Results. Hospital mortality did not differ statistically between the groups. In group 1, there was a tendency to worse survival within 1 year. In groups with aortic malformation, ACS with ST segment elevation was less common — p = 0.0002 and p = 0.001, respectively. In group 1, 23.1% of patients had intact coronary arteries. Conclusions. Severe aortic valve stenosis in some cases «mimics» acute coronary syndrome, which is indicated by significantly more frequent detection of intact coronary arteries in this cohort. Aortic stenosis of varying degrees does not lead to an increase in the frequency of early postoperative complications and 30-day mortality after PCI.

The purpose — clinical assessment of the algorithm of terms of the restoration of the finger tendons’ flexor after an injury with a profiled metal sheet. Results. Based on the study results, an algorithm for restorative operations on tendons after an injury with a profiled metal sheet was proposed. When the skin-tendon flap width-to-length ratio is 1:3 or more, one-stage reconstruction of the deep flexor tendon is possible. If the flap width-to-length ratio is less than 1:3, it is necessary to conduct a primary delayed (in 7–10 days) or staged reconstruction of the deep flexor tendon with skin restoration. Conclusion. The presented graphical model of «profiled metal sheet injury» allows giving a clinical assessment of the blood supply to the skin-tendon flap and differentiating treatment of patients with varying degrees of injury severity. In turn, the restoration of normal blood circulation in the injured segment accelerates the healing process, reduces the percentage of complications, and ultimately determines the success of the surgical intervention.

Patients with COVID-19 often suffer from a variety of gastrointestinal disorders. Often this is directly related to the effect of the virus on the epithelial cells of the gastrointestinal tract, as well as the prescription of drugs for the new coronavirus infection treatment. In some patients, gastrointestinal disturbances precede the onset of respiratory symptoms and remain for a long time after recovery, significantly reducing the quality of life. Approximately every second patient who had a COVID-19 infection suffers from stool disorders and dyspeptic symptoms. The most common variant of post-covid lesions of the gastrointestinal tract is functional dyspepsia and irritable bowel syndrome, which outpatient doctors have recently encountered more and more often. In most cases, the attending physician pays more attention to the pulmonary and cardiological manifestations of coronavirus infection, while the symptoms of gastrointestinal lesions are considered minor. The purpose of this article — is to consider possible methods for correcting the functional pathology of the gastrointestinal tract in patients after a COVID-19 infection.

The acromioclavicular joint is the link between the collarbone and the scapula, responsible for synchronizing movements in the upper shoulder girdle. Chronic instability of the acromioclavicular joint leads to changes in the orientation of the scapula, which provokes kinematic disorders leading to chronic pain. In case of acute dislocation of the acromial end of the clavicle, minimally invasive techniques are usually used, such as fixation with spokes or a hook-shaped plate, or arthroscopic fixation. Surgical techniques for the treatment of patients with acromioclavicular joint chronic symptomatic instability differ significantly from those used to treat patients with acute dislocation of the acromioclavicular joint. In this case, reconstruction of torn ligaments is necessary, because the biological potential of restoring these structures is exhausted after 3–4 weeks. The range of possibilities includes anatomical and non-anatomical techniques, open and minimally invasive procedures using arthroscopy, as well as biological and synthetic transplants. This article presents the authors’ approach to treating such patients and describes the modern reliable surgical technique.
