The purpose — to present the data of a clinical case of non-compact left ventricular myocardium complicated by severe cardiac arrhythmias and heart failure. Material and methods. The diagnosis of non-compact myocardium of the left ventricle became possible since 1990s after the introduction of echocardiography as a diagnostic method. The disease genesis is still unclear, starting with clinical manifestations of complications: cardiac arrhythmia or heart failure or thromboembolism, leading to fatal outcomes. Conclusions. The article is of interest to practitioners, since non-compact myocardium of the left ventricle is a rare disease, which presumably has both a «family» and a «non-family» character, with the possible occurrence of a triad of syndromes or an asymptomatic course without sufficient evidence for the treatment of this pathology.

The article presents the clinical experience of conservative delivery of 1124 women with a scar on the uterus at Republican Clinical Perinatal Center in Ufa. It was established that a woman’s age less than 40 years, certain indications for the previous caesarean section and the absence of indications for the previous cesarean section at this gestation, the history of vaginal delivery, the development of spontaneous labor should be considered criteria associated with the success of «trial» births. Pregnant women who meet these criteria can be delivered through the natural birth canal. Such a tactic allows avoiding unnecessary caesarean sections and improving maternal morbidity and mortality.

At the moment, there is a lot of data demonstrating the effectiveness of anticoagulants in the acute period of COVID-19; as for their effectiveness in patients after discharge, there are relatively few data, especially in patients with concomitant cardiological pathology. The purpose — is to evaluate the relationship of taking anticoagulants, within one month after discharge, with the development of cardiovascular events and deaths in patients with cardiovascular pathology during 3 months of follow-up. Material and methods. A retrospective, cohort, observational study of 317 patients who had COVID-19 and had a history (before COVID-19) of cardiovascular pathology was conducted. Results. Within three months after discharge from the hospital, 15 (4.73%) cardiovascular events were registered. The frequency of arterial thrombotic complications was 3.79% (n = 12), venous thrombotic complications 0.95% (n = 3). All cardiovascular events were reported in patients not using anticoagulants (n = 15; 6.28%), p = 0.026. When analyzing concomitant cardiovascular pathology in patients with registered events, it was revealed that the incidence of atrial fibrillation and ischemic stroke in their anamnesis was statistically significantly higher (33.33 and 40%), relative to the comparison group (10.03 and 7.9%; p = 0.002; p = 0.001, respectively). Conclusions. The use of anticoagulants in patients with cardiovascular pathology during treatment in a COVID hospital and within one month after discharge was associated with a significant decrease in thromboembolic events over 3 months of follow-up. Predictors of thromboembolic events in patients with cardiovascular pathology and those who have had COVID-19 are: senior age; atrial fibrillation and ischemic stroke in the anamnesis.

We evaluated intestinal permeability by triple sugar test in patients with autoimmune liver diseases. Patients were divided into groups with autoimmune hepatitis, primary biliary cholangitis and overlap syndrome (autoimmune hepatitis/primary biliary cholangitis). 102 people were included in the cross-sectional study: 72 patients with autoimmune liver diseases and 30 healthy volunteers. Diagnose based on recommendations — EASL (2017), AASLD (2019), IAIHG (2021). Small intestine permeability was evaluated by the ratio of lactulose/mannitol in urine, large bowel permeability by the level of sucralose in urine (nmol/l). We used the method of high-performance liquid chromatography–mass spectrometry. Patients with autoimmune liver diseases had increased small intestine permeability – 0.12 [0.07; 0.234] (p < 0.001) compared to the control group — 0.013 [0.01; 0.025]. Large bowel permeability — 760.8 [386.4; 1640] nmol/l did not differ from group control — 785.6 [408; 1027.2]. Increased small intestine permeabilityin patients with autoimmune liver diseases: stage of hepatitis — 0.17 [0.118; 0.253] (p < 0.001), liver cirrhosis — 0.106 [0.07; 0.23] (p < 0.001) compared with the control group. Large bowel permeability in patients with hepatitis and in liver cirrhosis did not differ from control group. Increased values of the triple sugar test were detected in patients with autoimmune hepatitis (increased permeability of the small intestine (0,09 [0,07; 0,17] (p < 0.05), increased permeability of large bowel — 1091,2 [760; 2352] (p < 0.05)) and overlap syndrome (increased small intestine permeability — 0.18 [0,086; 0,3] (p < 0,05) and increased large bowel permeability — 880 [424; 1680] (p < 0,05)) at the stage of liver cirrhosis. Patients with autoimmune liver diseases show the increased permeability of the small intestine. The presence of increased permeability of the small and large intestine may indicate the progression of the disease.

The article presents a clinical case of a patient with subcutaneous calcification. Deposits of calcium salts in the skin and subcutaneous fat can be a manifestation of a number of diseases. The article describes the characteristics of the clinical course of the disease and the laboratory and instrumental diagnostic methods. Judging by the dynamics of changes in the skin and subcutaneous fat, the presence of an immuno-inflammatory disease in the anamnesis, slow progression against the background of immunosuppressive therapy, the diagnosis was made: low activity dermatomyositis (as an outcome of juvenile dermatomyositis) with lesions of skin and muscles in the anamnesis. It should be noted that subcutaneous calcification is more characteristic for juvenile dermatomyositis.

Chronic sinusitis is one of the most common pathologies in the structure of the incidence of ENT organs. The purpose — to study and compare the features of the histological structure of the sinus mucosa in normal and chronic pathological conditions. Material and methods. The study included 30 patients with maxillary sinus cyst and 30 patients with chronic maxillary sinusitis. Immunomorphological studies of the normal mucosa of the maxillary sinus, cyst membrane, hyperplastic mucosa of the maxillary sinus in chronic sinusitis were carried out. All samples were subjected to immunohistochemical staining with further comparison of the results obtained. Results. When comparing all samples, no statistically significant differences were found in the content of macrophages when comparing the normal mucosa and cyst membrane, as well as the content of T-lymphocytes and neutrophils when comparing the cyst membrane and mucosa in chronic sinusitis. All other parameters (the content of neutrophils, macrophages, T and B lymphocytes, mast and goblet cells, the area of non-cellular tissue structures) had significant differences when compared in all groups. Conclusions. Inflammatory changes are observed in the cyst shell, which have significant differences both from the normal mucous membrane and from the mucous membrane in chronic sinusitis.

The purpose — to demonstrate the capabilities of computed tomography and ultrasound in the diagnosis and treatment planning of a patient with a rare case of necrotizing fasciitis. Material and methods. A clinical observation of a 57-year-old patient K. with complaints of pain in the area of the right foot, reddening of the skin on the right foot is presented. T — 37.2 ºC. To determine the nature of the revealed changes, the patient was referred to the Department of Radiation and Functional Diagnostic Methods for soft tissue ultrasound and computed tomography (CT) of the vessels of the lower extremities to assess the lesion and plan surgical treatment. Results. After an ultrasound of the soft tissues of the right leg posterior surface, around the gastrocnemius muscle, an inhomogeneous liquid rim (an echo picture of fasciitis), soft tissue edema was visualized. CT of the lower extremities vessels showed a decrease in blood flow in the lower leg arteries, infiltration and a moderate amount of gas in the soft tissues of n/3 of the lower leg and foot. Discussion. Necrotizing fasciitis (fasciitis necrotisans) is a representative of a large group of surgical infections of soft tissues, the morphological basis of which is progressive necrosis of the superficial fascia (fascia superficialis) and subcutaneous adipose tissue, and the clinical course is particularly severe and is accompanied by high mortality. NF is an emergency surgical situation. Early and accurate diagnosis is critical to provide the necessary aggressive treatment. Conclusion. This clinical observation demonstrates the importance of conducting a study using computed tomography and ultrasound in patients with necrotizing fasciitis for the earliest possible diagnosis and immediate surgical treatment, supported by effective antibiotic therapy.

Hepatoblastoma is a common primary malignant tumor in children aged 0 to 4 years. In the structure of liver neoplasms, the pathology occupies 80% and has been steadily growing over the past 30 years with an annual increase of 4–5%. Risk factors for the development of hepatoblastoma include an increase in the survival rate of deeply premature babies, oxidative tissue damage associated with intensive therapy of low birth weight premature babies, and IVF. The disease has no specific complaints and characteristic clinical manifestations. The initial manifestations of hepatoblastoma are often hidden under a variety of mask diseases. It is accompanied by the absence of «oncological alertness» of local pediatricians, which determines the late diagnosis of the liver malignant neoplasm. The article presents an analysis of hepatoblastoma cases in young children over a ten-year period. The main initial clinical manifestations of the disease in early children are indicated. The effectiveness of modern methods of diagnosis and therapy of the liver malignant neoplasms in children is shown. A clinical example of successful treatment of this disease is given.

The purpose of the study was to evaluate the effects of interval hypoxic therapy in women with autoimmune thyroiditis (AIT) and hypothyroidism. Material and methods. 136 women with newly diagnosed AIT and hypothyroidism were examined and divided into two equal groups. The first group received hormonal replacement therapy with levothyroxine sodium for 12 months. The second group received levothyroxine sodium in combination with interval hypoxic therapy. Results. Levothyroxine sodium therapy in women with newly diagnosed AIT and hypothyroidism was associated with a further increase in the thyroperoxidase autoantibodies blood serum content (p < 0.001). The combination with hypoxic therapy led to the lower autoantibodies values than levothyroxine sodium monotherapy (p < 0.05). In addition, therapy with levothyroxine sodium was characterized by an increased need for medications (p < 0.001). Hypoxic therapy supplementation in AIT provided the medication dose stabilization. Conclusions. It is appropriate to combine levothyroxine sodium with normobaric hypoxic therapy sessions (every 3 months for 10 days) in order to improve the effectiveness of autoimmune thyroiditis and hypothyroidism treatment in women.
