Effect of Angle Alpha on Visual Quality after Multifocal Intraocular Lens Implantation 多焦人工晶状体植入术后α角对视觉质量的影响

Objective: To evaluate the visual quality of patients with different angle alpha sizes after a multifocal intraocular lens (MIOL) implantation. Methods: This was a prospective case series study. Fifty-six patients (74 eyes) who had phacoemulsification with MIOL implantation were enrolled in the study from December 2016 to October 2018. The patients were divided into 3 groups based on the size of the preoperative angle alpha. There were 12 eyes in group A (0 mm 0.4 mm). Monocular far, intermediate, and near uncorrected visual acuities were measured during a 3-month follow-up. Other outcome measurements taken were the modulation transfer function (MTF) cutoff, the incidence of glare, and halo. The data were analyzed with an independent χ2 test or a ANOVA test. Results: The incidence of halo and glare in groups B and C was higher than that in group A (χ2=38.372, P=0.01). There were no significant differences in the far, intermediate, or near vision between the three groups (P>0.05). Angle alpha had no significant effect on the MTF, night hyperopia, double vision, mixed focus or starburst (P>0.05). Conclusions: When angle alpha is greater than 0.4 mm, the incidence of glare and halo increases. In clinical work, for patients with a larger angle alpha, the choice to implant MIOLs should be carefully evaluated. 目的:评价多焦人工晶状体植入术后不同α角大小患者的视觉质量。方法:前瞻性病例系列研究。2016年12月至2018年10月,56例(74只眼)行超声乳化术合并MIOL植入术。根据术前α角大小将患者分为3组。A组12只眼(0 ~ 0.4 mm)。在3个月的随访期间测量单眼远、中、近未矫正视力。其他测量结果包括调制传递函数(MTF)截止、眩光发生率和光晕。数据分析采用独立χ2检验或方差分析检验。结果:B、C组患者晕眩发生率高于A组(χ2=38.372, P=0.01)。三组患者的远、中、近视力差异无统计学意义(P>0.05)。α角对MTF、夜间远视、复视、混焦和星爆均无显著影响(P>0.05)。结论:当α角大于0.4 mm时,眩光和光晕的发生率增加。在临床工作中,对于alpha角度较大的患者,应仔细评估植入miol的选择。关键词:白内障超声乳化术;多焦人工晶状体;角α;调制传递函数

Effect of WeChat Intervention on Physical Activity in Patients with Glaucoma 微信干预对青光眼患者身体活动的影响

Objective: To study the effect of WeChat reminder intervention for increasing physical activity (PA) in glaucoma patients. Methods: In this prospective, randomized, controlled study, 102 glaucoma patients were enrolled from the outpatient department of the Eye Hospital, Wenzhou Medical University from June 2018 to December 2018. Participants were randomly divided into a control group and an intervention group using random numbers generated by Excel. All the participants were taught the benefit of exercise and were advised to increase their daily steps when they were enrolled in the outpatient department. Only patients in the intervention group were invited to join the WeChat group for a reminder of exercise intervention. The PA of all patients was monitored by ActiGraph (WGT3X-BT) for a 1-week baseline and 1-month follow-up during the intervention period. Statistical analyses were performed using a Chi-square test, independent samples t test, Mann-Whitney U test, paired samples t test and Wilcoxon signed-rank test. Results: Thirty patients with high daily exercise levels at baseline (>12 000 steps/day), poor compliance or other reasons during the follow-up were excluded. Seventy-two patients finally met all requirements, including 42 patients in the control group and 30 patients in the intervention group. For the intervention group, the mean amount of PA during the 1-month follow-up was significantly higher than that at the 1-week baseline, in which the mean daily steps (t=4.94, P<0.001), calories consumed during exercise (Z=-2.87, P=0.004), metabolism equivalents (METs) (Z=-3.30, P=0.001), moderate physical activity (MPA) (Z=-2.89, P=0.004), vigorous physical activity (VPA) (t=2.57, P=0.016) and moderate-to-vigorous physical activity (MVPA) times (Z=-3.01, P=0.003) had significant differences. Light physical activity (LPA) and the number of sedentary breaks were reduced compared with those before intervention (t=-2.14, P=0.041; t=-2.76, P=0.022). For the control group, the average daily steps in the 1-month follow-up were also higher after intervention than the average at baseline (t=3.29, P<0.001). LPA time was also reduced compared to baseline (t=-2.57, P=0.014). In addition, in the intervention group, the increase in VPA time (follow-up-baseline) (Z=-3.04, P=0.002) and very vigorous physical activity (VVPA) time (Z=-2.06, P=0.040) were significantly higher than those in the control group. Conclusions: WeChat intervention can increase the daily steps and MVPA time of glaucoma patients and decrease the LPA time and the number of sedentary breaks. Advances in Visual Quality after Posterior Chamber Collamer Lens Implantation 后房型人工晶体植入术后视觉质量的研究进展

With the development of posterior chamber intraocular lens implantation and the improvements in design, function and material of the implantable collamer lens (ICL), ICL implantation has become one of the main methods to correct moderate and high myopia. However, the requirement for visual quality of myopic patients is increasing. Not only visual improvement after removal of eye glasses to achieve the desired corrected visual acuity, but also improvement in corrected visual acuity to achieve a high-definition visual effect. Therefore, this article reviews research on visual quality after ICL implantation. Key words: implantable collamer lens; visual quality; ametropia

Analysis of the Causes of Non-Amblyopic Functional Low Vision in Children 儿童非弱视功能性低视力原因分析

Objective: To analyze the causes of non-amblyopic functional low vision in children. Methods: One hundred and seven non-amblyopic functional low vision children aged 8.3±1.9 years were selected from Puyang Eye Hospital from June 2015 to May 2019 in this serial case study. The initial best near-far corrected visual acuity (BCDVA, BCNVA), monocular and binocular accommodative sensitivity (MAF, BAF), accommodation amplitude (AMP), accommodative convergence (BCC), positive and negative relative accommodation (PRA, NRA), near-far horizontal oblique lens power (DLP, NLP), accommodative convergence/accommodation (AC/A), and spherical equivalent (SE) were measured. Decimal visual acuity was converted into a 5-point expression by Muir's method. The univariate multiple linear regression analysis was used to study the relationship between corrected vision before training and independent variables of related visual function parameters. Results: Seventy-nine patients (73.8%) had a BCDVA lower than 5.0 in both eyes and 28 patients (26.2%) had a BCDVA lower than 5.0 in one eye. Seventy-one patients (66.4%) had a BCNVA below 5.0 in both eyes and 22 patients (20.6%) had a BCNVA below 5.0 in one eye. Visual function examination showed that the accommodative range of all eyes with low vision was lower than normal, 59 patients were diagnosed with convergence insufficiency and 18 patients were diagnosed with simple exotropia, accounting for 55.1% and 16.8% of the total cases, respectively, accounting for 97.5% of all eyes with a BCDVA lower than 5.0. There were 27 patients with a simple accommodative abnormality, accounting for 25.2% of the total patients, and 96.4% of those had a monocular BCDVA lower than 5.0. The best corrected visual acuities of far and near were positively correlated with AMP (r=0.563, P=0.035; r=0.587, P=0.029), both negatively correlated with NLP (r=-0.413, P=0.031; r=-0.409, P=0.030), which had no significant correlation with BCC, PRA, NRA, DLP, AC/A, or SE (P>0.05). Conclusions: ①Patients with convergence insufficiency and simple exotropia are characterized mainly by binocular BCDVA abnormalities, while patients with simple accommodation abnormalities are mainly characterized by monocular BCDVA abnormalities. ②The formation of the children's non-amblyopic functional vision impairment is related to the AMP and the near horizontal cryptogamy anomaly, of which the AMP anomaly is the root cause and the near horizontal cryptogamy anomaly is the secondary cause. The initial best near-far corrected visual acuity (BCDVA, BCNVA), monocular and binocular accommodative sensitivity (MAF, BAF), accommodation amplitude (AMP), accommodative convergence (BCC), positive and negative relative accommodation (PRA, NRA), near-far horizontal oblique lens power (DLP, NLP), accommodative convergence/accommodation (AC/A), and spherical equivalent (SE) were measured. Decimal visual acuity was converted into a 5-point expression by Muir's method. The univariate multiple linear regression analysis was used to study the relationship between corrected vision before training and independent variables of related visual function parameters. \u0000 \u0000 \u0000Results: \u0000Seventy-nine patients (73.8%) had a BCDVA lower than 5.0 in both eyes and 28 patients (26.2%) had a BCDVA lower than 5.0 in one eye. Seventy-one patients (66.4%) had a BCNVA below 5.0 in both eyes and 22 patients (20.6%) had a BCNVA below 5.0 in one eye. Visual function examination showed that the accommodative range of all eyes with low vision was lower than normal, 59 patients were diagnosed with convergence insufficiency and 18 patients were diagnosed with simple exotropia, accounting for 55.1% and 16.8% of the total cases, respectively, accounting for 97.5% of all eyes with a BCDVA lower than 5.0. There were 27 patients with a simple accommodative abnormality, accounting for 25.2% of the total patients, and 96.4% of those had a monocular BCDVA lower than 5.0. Changes in Pupil Diameter and Reactions during the Phacoemulsification Stage with Posterior Chamber Intraocular Lens Implantation 后房型人工晶状体植入术后超声乳化期瞳孔直径变化及反应

Objective: To investigate changes in pupil diameter and reaction parameters following phacoemulsification with posterior chamber intraocular lens (IOL) implantation. Methods: This prospective study included patients who underwent consecutive cataract and high myopia transparent crystalline lens replacement and were scheduled to undergo phacoemulsification from January to September 2016 in Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine. Pupil diameter under four controlled illumination levels (0, 1, 10, and 100 cd/m2) and reaction parameters (initial diameter, amplitude of contraction, latency of contraction, duration of contraction, velocity of contraction, latency of dilatation, duration of dilatation, and velocity of dilatation) were measured preoperatively and 1 week and 1 month postoperatively using computerized dynamic pupillometry (Metro-vision MONCV3). A Pearson correlation coefficient, Spearman correlation coefficient, and repeated measures analysis of variance were used to analyze the data. Results: In total, 53 eyes from 53 patients were included in this study. Under the four conditions of illumination, pupil diameter decreased as age increased (r0=-0.467, P0<0.001; r1=-0.383, P1=0.005; r10=-0.374, P10=0.006; r100=-0.312, P100=0.023). The overall differences in pupil diameters before and after surgery under 4 kinds of illumination intensity were statistically significant (F0=206.9, P0<0.001; F1=106.8, P1<0.001; F10=41.7, P10<0.001; F100=36.7, P100<0.001). Compared to preoperative measurements, postoperative pupil diameters were reduced at 1 week and 1 month under the four conditions of illumination (P<0.001). The overall differences in the initial diameter and constriction amplitude of pupil reaction were reduced, and the differences in pupil contraction and dilation velocity reaction were statistically significant (Finitial diameter=99.5, Pinitial diameter<0.001; Fcontraction amplitude=36.2, Pcontraction amplitude<0.001; Fcontraction velocity=51.9, Pcontraction velocity<0.001; Fdilation velocity=23.840, Pdilation velocity<0.001). Compared to preoperative measurements, the initial diameter and constriction amplitude of pupil reaction were reduced, and pupil contraction and dilation velocity reaction declined at 1 week and 1 month postoperatively (P<0.001). Conclusions: Pupil diameters are smaller and pupil constriction and dilation velocity are slower within 1 month postoperatively. Effect of 0.01% Atropine on Accommodation in Myopic teenages 0.01%阿托品对近视青少年适应的影响

Objective: To study the effect of 0.01% atropine on accommodation in myopic teenages. Methods: A prospective non-randomized controlled study was conducted. Sixty-one children (122 eyes) with low to moderate myopia were selected at the Second People's Hospital of Liaocheng from April 2015 to June 2018. Patients (aged 13-16 years old (average age, 14.4±0.9 years) were divided into an atropine group (30 patients, 60 eyes) and a control group (31 patients, 62 eyes) according to the wishes of the guardians. Sodium hyaluronateeye drops were used once in the control group and the atropine group was treated once with 0.01% atropine eye drops. In the atropine group, near vision, accommodative amplitude and accommodative facility were measured at 2 h, 4 h and 6 h after atropine treatment, and the same parameters were measured in the control group at 2 h and 6 h after treatment with sodium hyaluronate eye drops. The data were analyzed by repeated measures analysis of variance and a t-test. Results: The difference in near vision between the two groups and at the same time-points were not statistically significant (F=0.209, P=0.650; F=0.292, P=0.594) and there was no interaction between group and time (F=0.097, P=0.756). The accommodative amplitude of the atropine group was lower than that of the control group at 2 h after treatment and the difference was statistically significant (t=-13.09, P<0.001). The difference at 6 h after the atropine treatment was not statistically significant compared with sodium hyaluronate treatment (t=-0.26, P=0.797). In the atropine group, the accommodative amplitude at 2 h after treatment (10.09±0.69 D) was significantly lower than the baseline value (12.58±0.88 D) (P<0.001). At 4 h after treatment, the accommodative amplitude (10.98±0.77 D) was higher than that 2 h after treatment (P<0.001), and the difference between the baseline value (12.45±0.77 D) and 6 h after treatment was not statistically significant (P=0.911). The difference in accommodative facility between the two groups was not statistically significant at 2 h and 6 h after treatment. The accommodative facility at different time points in the atropine group was not statistically significant (F=0.09, P=0.965). Conclusions: After a single treatment with 0.01% atropine eye drops, the amplitude of accommodation decreases, but sufficient residual accommodation is retained without affecting near vision or accommodative facility. Effectiveness of Surgical Treatment for Dissociated Vertical Deviation with Different Clinical Characteristics 不同临床特征解离性垂直偏差的手术治疗效果分析

Objective: To investigate the effectiveness of surgery for dissociated vertical deviation (DVD) with different clinical characteristics. Methods: A retrospective study was conducted on 49 patients who underwent surgical correction for DVD at Qingdao Eye Hospital of Shandong Eye Institute from January 2013 to December 2016. Patients were divided into four groups according to their angle of vertical deviation. Different surgical designs were used, and surgical efficacy was analyzed. In group A, the deviation angle was ≥20△ in both eyes of 18 patients with a difference of <5△. In group B, the deviation angle was ≥20△ in both eyes of 14 patients with a difference of ≥5△. In group C, 17 patients had a large deviation angle ≥20△ in one eye and a small deviation angle <5△ in the other eye. Small vertical strabismus (<5△) meant a slight rotation or no vertical strabismus. Group C was divided into group C1 and group C2 based on different surgical methods used. A 7-to-10 mm recession of the superior rectus muscle was reserved for patients in group A. A 7-to-10 mm recession of the superior rectus muscle was reserved for eyes with a large vertical angle and a 5-to-6 mm recession of the superior rectus muscle was reserved for the other eyes in group B. A 7-to-10 mm recession of the superior rectus muscle was reserved for eyes with a large vertical angle, and a 4 mm recession (10 patients) was reserved in group C1, or no surgery (7 patients) was performed on eyes with a small deviation angle in group C2. Results: No eyelid retraction was observed after large recessions of the superior recuts muscle. The rate of surgical satisfaction was 94% in group A and was 93% in group B. Among the patients with bilateral surgeries in group C1, the rate of surgical satisfaction was 100%. Among the patients with unilateral surgical treatment in group C2, the rate of surgical satisfaction was 29%. There was a statistically significant difference between group C1 and group C2 (Z=-3.656, P<0.001). Conclusions: For DVD, usually a binocular disease, bilaterally equal or unequal recession of the superior rectus muscle could be reserved according to the symmetry or asymmetry degree of vertical strabismus. A 4 mm recession of the superior rectus muscle may be reserved in the preferred eye of patients with a small vertical angle or no vertical strabismus. If the non-preferred eye with a large vertical angle is operated on, the preferred eye with a small vertical angle may show obvious vertical strabismus or a new compensatory head position. 目的:探讨不同临床特征游离性垂直偏差(DVD)的手术治疗效果。方法:对2013年1月至2016年12月在山东眼科研究所青岛眼科医院行DVD手术矫正的49例患者进行回顾性研究。根据患者的垂直偏离角度将患者分为四组。采用不同的手术设计,分析手术效果。A组18例患者双眼偏角≥20△,差值<5△。B组14例患者双眼偏角≥20△,差≥5△。C组17例患者单眼大偏角≥20△,另眼小偏角<5△。小垂直斜视(<5△)是指轻度旋转或无垂直斜视。C组根据手术方式的不同分为C1组和C2组。7到10毫米上直肌肌肉的衰退是预留给病人在A组的7到10毫米衰退上直肌肌肉是预留给眼睛一个大垂直角度和5到6毫米上直肌肌肉的衰退是留给另一个眼睛在b组7到10毫米上直肌肌肉的衰退是预留给眼睛一个大垂直角度,和4毫米的经济衰退(10例)保留在C1组,C2组为小偏斜眼或不手术(7例)。结果:上切肌大范围萎缩后,未见眼睑挛缩。A组手术满意率为94%,b组为93%。C1组双侧手术患者手术满意率为100%。C2组单侧手术治疗患者,手术满意率为29%。C1组与C2组比较,差异有统计学意义(Z=-3.656, P<0.001)。结论:DVD通常为双眼疾病,可根据垂直斜视的对称或不对称程度,保留双侧等或不等的上直肌后退。对于垂直角小或无垂直性斜视的患者,其首选眼可保留上直肌后退4mm。如果对大立角的非首选眼进行手术,小立角的首选眼可能会出现明显的垂直斜视或新的代偿头位。关键词:游离垂直偏差;手术矫正;上直肌衰退

Patients were divided into four groups according to their angle of vertical deviation. Different surgical designs were used, and surgical efficacy was analyzed. In group A, the deviation angle was ≥20△ in both eyes of 18 patients with a difference of <5△. In group B, the deviation angle was ≥20△ in both eyes of 14 patients with a difference of ≥5△. In group C, 17 patients had a large deviation angle ≥20△ in one eye and a small deviation angle <5△ in the other eye. Small vertical strabismus (<5△) meant a slight rotation or no vertical strabismus. Group C was divided into group C1 and group C2 based on different surgical methods used. A 7-to-10 mm recession of the superior rectus muscle was reserved for patients in group A. A 7-to-10 mm recession of the superior rectus muscle was reserved for eyes with a large vertical angle and a 5-to-6 mm recession of the superior rectus muscle was reserved for the other eyes in group B. A 7-to-10 mm recession of the superior rectus muscle was reserved for eyes with a large vertical angle, and a 4 mm recession (10 patients) was reserved in group C1, or no surgery (7 patients) was performed on eyes with a small deviation angle in group C2. \u0000 \u0000 \u0000Results: \u0000No eyelid retraction was observed after large recessions of the superior recuts muscle. The rate of surgical satisfaction was 94% in group A and was 93% in group B. Among the patients with bilateral surgeries in group C1, the rate of surgical satisfaction was 100%. Among the patients with unilateral surgical treatment in group C2, the rate of surgical satisfaction was 29%. There was a statistically significant difference between group C1 and group C2 (Z=-3.656, P<0.001). \u0000 \u0000 \u0000Conclusions: \u0000For DVD, usually a binocular disease, bilaterally equal or unequal recession of the superior rectus muscle could be reserved according to the symmetry or asymmetry degree of vertical strabismus. A 4 mm recession of the superior rectus muscle may be reserved in the preferred eye of patients with a small vertical angle or no vertical strabismus. Objective Assessment of Optical Quality in Mild Keratoconus with a Double-Pass System 目的用双通道系统评价轻度圆锥角膜的光学质量

Objective: To analyze the difference in optical quality between a normal cornea and keratoconus using an optical quality analysis system (OQAS). Methods: This cross-sectional study included 32 normal eyes and 27 keratoconus eyes. Optical qualities were measured by OQAS. Modulation transfer function cutoff frequency (MTFcutoff), Strehl ratio and OQAS values (OV-100, OV-20, OV-9) in artificial pupil diameters of 3 mm, 4 mm and 5 mm were analyzed by independent t test or Mann-Whitney U test, the difference among groups was analyzed by ANOVA for repeated measurement and Bonferroni post hoc t test. Results: MTFcutoff, Strehl ratio and OQAS values of the keratoconus group in artificial pupil diameters of 3 mm, 4 mm and 5 mm were all lower than those of the control group (3 mm: t=6.658, 6.695, 6.660, 6.493, 6.778; 4 mm: t=7.106, 6.567, 7.109, 6.802, 6.788; 5 mm: t=6.148, 7.254, 6.158, 6.446, 7.210; all P<0.001). Both in the control group and in the keratoconus group, the MTFcutoff, Strehl ratio, OV-100, OV-20 and OV-9 varied from artificial pupil diameters (control group: F=42.205, 28.335, 42.559, 32.621, 30.372; keratoconus group: F=15.732, 11.533, 15.625, 13.870, 10.495; all P<0.001). Conclusions: The optical qualities of keratoconus measured by OQAS are worse than normal eyes. Both in keratoconus and normal eyes, the optical qualities decline as pupil size increases.
