Diagnostic Approach for the Primary Cardiac Angiosarcoma – A Case Report 原发性心脏血管肉瘤的诊断方法- 1例报告

We report a 25-year-old male who presented to an outside institution with progressive dyspnoea. He had increased jugular venous pressure and muffled heart sounds, suggesting a pericardial tamponade. A diagnosis of a cardiac angiosarcoma was put forward. Primary cardiac tumours are rare, and patients can present with constitutional symptoms, posing a diagnostic challenge. The objective of this report is to suggest a diagnostic standard for early recognition of these fatal tumours.A chest radiograph demonstrated an enlarged cardiac contour and signs of pericardial tamponade. Pericardiocentesis was performed, complicated by a pneumopericardium and the pericardial fluid was haemorrhagic and exudative. Echocardiography confirmed a right atrial mass featured by heterogeneous enhancement in subsequent chest computed tomography and magnetic resonance imaging concerning a sarcoma. No metastatic lesions were found. The tumour was excised completely, and the following echocardiogram indicated normokinetic biventricular function. Histological examination of the tumour cells confirmed the presence of a cardiac angiosarcoma. Patient was scheduled for combined chemotherapy following discharge.Primary cardiac angiosarcomas are difficult to diagnose due to the rarity of the tumour as well as the non-specific symptoms. An early diagnosis may help improve prognosis which emphasizes the importance of establishing a standard diagnostic protocol for such tumours.

Polyarteritis nodosa with recurrent skin ulcers. A case report 结节性多动脉炎伴复发性皮肤溃疡。病例报告

Polyarteritis nodosa or PAN is a rare disease. It is diagnosed frequently in middle age patients and the elderly. Reduction in hepatitis B virus infection has been associated with a reduction in the prevalence of PAN.We present a 78-year-old patient with extensive skin lesions, weight loss, high blood pressure (HBP), polyneuropathy, evidence of hepatitis B virus infection, and a biopsy showing vasculitis of a small or medium-sized arteries. Corticosteroid and antiviral treatment were prescribed to the patient and with great results.PAN is a necrotizing vasculitis that typically affects medium-sized vessels with occasional involvement of small vessels. It can be found in elderly male patients over the sixth decade of life with manifestations of recurrent skin ulcers and HBP.

Clinical profile and Risk Factors of Central Serous Chorioretinopathy in Al-Ain, United Arab Emirates 阿拉伯联合酋长国Al-Ain中枢性浆液性脉络膜视网膜病变的临床特征和危险因素

Based on the adverse event reporting system, the authors observed unique occupational and pharmacological trends in central serous chorioretinopathy (CSC); despite being a common condition in the region, there are no large epidemiological studies.To describe the clinical profile, risk factors and co-medications of CSC in a multispecialty hospital in UAE.Hospital based retrospective, observational study in which all the confirmed cases of CSC (272) seen between 2010 and 2019 were included. Supplementary data was collected on follow up visits or through telephonic calls.The male: female ratio was 17:1; low socio-economic status was (155; 73.45%), and occupations such as drivers (61; 28.9%) and outdoor laborers (59; 27.96%) were the most commonly affected. The majority (78.05%; 185) were single expatriates. Financial worries (105; 50.72%) constituted the leading cause of stress. The usage of corticosteroids (83; 30.51%), nasal decongestants (14.70%) or both (17; 6.25%) within one year was common; mostly (82; 30.14%) for rhinitis/respiratory states, though grossly under-recognized. Muscle relaxants and psychotropic medications were the other major medications used by 24 (8.82%) and 25 (9.19%) cases prior to CSC. Seven of 15 women had at least one condition that alters the endocrinal milieu: Pregnancy (3), recent child birth (1), erratic oral contraceptive intake (1), menopause with liver dysfunction (1), hormone replacement therapy for menopause and endometriosis (1), ovarian cyst and infertility (1) hypothyroidism (3), and Cushing syndrome (1). Chronic/ recurrent forms were seen in 31.08%. There were too few glaucoma patients despite both CSC and glaucoma being common among our patients.CSC in Al-Ain predominantly affected males with a specific occupational predisposition. Emotional distress, financial worries, allergic disorders, and endocrinal disorders (among females) were common. Adverse drug events were under-reported compared to the usage of steroid or decongestant users. Suspected pharmacological associations included drugs that modulated the vasomotor tone.

Pituitary Stalk Interruption Syndrome in a Newborn with Encephalopathy: ACase Report 脑病新生儿垂体柄中断综合征1例报告

Pituitary Stalk Interruption Syndrome is a rare midline brain anomaly involving the pituitary gland that leads to a variable degree of pituitaryhypofunction with a classic neuroimaging finding of the ectopic posterior pituitary, thin or absent pituitary stalk, and anterior pituitary hypoplasia.We present the case of a term newborn who developed encephalopathy and respiratory distress during the neonatal period and was later diagnosedwith Pituitary Stalk Interruption Syndrome.Genetic defects have been identified in 5% of cases, and at times there are associated extra pituitary malformations. In spite of being a welldescribed syndrome, a detailed description of its neonatal presentation and course is lacking in the literature.

Healthcare Professional’s Perceptions of E-Learning during the COVID-19 Pandemic COVID-19大流行期间医疗保健专业人员对电子学习的看法

COVID-19 has a significant impact on lives worldwide. Owing to the adverse effect of this pandemic, there has been a major shift from in-personlearning activities to virtual learning. Different methods of virtual learning or e-learning, such as online classes and webinars, have emergedrapidly.The aim of this study is to identify the perceptions of healthcare professionals regarding e-learning during the COVID-19 pandemic.An online survey was conducted using Microsoft Forms sent via a text link to mobile phones and emails to healthcare professionals. Thequestionnaire had multiple-choice questions and five-point scaling to determine perceptions about virtual learning. Data in the form of responseswere collected, analyzed, and summarized as mean ± standard deviation and percentage.Responses were received from a total of 410 participants, out of which 240 (58.54%) were females, and 170 (41.46%) were males. Among allparticipants, 294 (71.71%) were doctors. A vast majority of participants (90%) attended online webinar/e-conferences since the pandemic. Themobile phone was the most commonly used device for e-learning. More than half of the participants opined that in-person meetings are moreeffective than virtual meetings. In terms of medical learning, 40.48% of participants believed that virtual conferences are more convenientcompared to conventional in-person attendance. According to 42.2% of participants, lack of personal interaction in virtual meetings affected theirability to acquire knowledge and experience.Our survey demonstrated the acceptance of virtual learning by healthcare professionals as a new learning method. The majority of participantsseem willing to adapt to this new medium.

EBV Reactivation in A Case of DRESS Syndrome Associated with Lamotrigine – A Case Report 拉莫三嗪所致DRESS综合征1例EBV再激活1例报告

Drug reaction with eosinophilia and systemic symptoms (DRESS syndrome) is a rare, T-cell mediated hypersensitivity reaction that develops secondary to a drug reaction. Several drugs have been associated with DRESS syndrome, most commonly carbamazepine. The mechanism is not clearly understood. It is a life-threatening condition that can present with skin rash, hematologic abnormalities, lymphadenopathy, and organ failure.The authors report a case of 43-year-old gentleman who developed DRESS syndrome secondary to lamotrigine and was found to have EBV reactivation. Patient was managed with supportive care; topical steroids and the culprit drug were discontinued. He had full recovery almost 2 weeks following treatment. DRESS syndrome can occur 2 weeks following exposure to an offending drug in susceptible individuals.Lamotrigine and EBV reactivation are not frequently reported in patients with DRESS syndrome. Therefore, physicians should be vigilant about this rare drug related hypersensitivity reaction in order to prevent life threatening complications.

Preventing the Catastrophe: Aberrant Carotid in Adenoid and Tonsil Surgery:Two Case Reports 预防灾难:腺样体和扁桃体手术中颈动脉异常:两例报告

Adenoid and tonsil surgeries are the most commonly done surgical procedures, with haemorrhage being an important complication. Fatalities dueto haemorrhage almost never occur, but the occasional devastating outcomes that are life-threatening become a frequent source of litigations andaudits. Catastrophic bleeds are usually due to an aberrant vessel or carotid protruding in the pharyngeal airway. The aberrant carotid poses a riskduring routine pharyngeal surgeries like adenoidectomy, tonsillectomy, Quincy and Para-pharyngeal abscess drainage, UPPP, pharyngeal biopsies,etc. and injuries during routine pharyngeal surgical procedures can be catastrophic due to massive bleeding.We report two cases of routine Adeno-tonsillectomy surgeries where aberrant carotid vessels protruding in the adeno-tonsil surgical area wereidentified by endoscopic transoral evaluation intraoperatively. The surgery in these cases was postponed for further investigation due to the risk ofinjury and catastrophic bleeding.Aberrant carotids are usually clinically silent, and adenoid removal in most cases is a blind procedure done by curettage. There are no guidelines toidentify aberrant vessels pre or intraoperatively, and preventing injury and catastrophic bleeding depends on surgeon’s experience and caution.These cases underline the importance of due vigilance and taking steps before starting the procedures, particularly pre-and intraoperative transoralendoscopic assessment for identifying aberrant vessels in the upper airway area, thus preventing injury and avoiding devastating complications. Wereport these cases and underline the steps to identify aberrant vessels in the upper airway surgical field before starting surgery to prevent apotentially catastrophic complication.

Good Uptake of the COVID-19 Vaccine Among Patients with Rheumatic Diseases in the United Arab Emirates (UAE) 阿拉伯联合酋长国风湿病患者COVID-19疫苗的良好接种率

Many patients with rheumatic diseases are immunosuppressed and take forms of immunomodulator medication. Hence, these patients might be at a heightened risk of Covid-19 and have special concerns in regard to vaccines, such as safety and efficacy. Vaccines have been available for the population of UAE as of January 2021, these being Pfizer (the United States and Europe), Sinopharm (China), and AstraZeneca (United Kingdom). The aim of our paper was to examine the rate of vaccine uptake among our patients with rheumatic conditions in the United Arab Emirates.A questionnaire was distributed among patients with rheumatic diseases attending a rheumatology specialised clinic in the United Arab Emirates. We collected data on 149 consecutive patients who were seen during a one-month period from 10/06/2021 to 10/07/2021 in Dubai.The mean age of the patients was 45.9 +/- 41.3. The majority of the patients had underlying rheumatic diseases (86.9%), the rest having conditions such as soft tissue pain or to rule out rheumatic conditions. 48.5% were taking immunomodulators. 41.7% of respondents had other chronic diseases. It was also found that 18.9% had Covid-19 previously, all of whom recovered, with the average duration of the illness being 11 days. 99.1% of individuals reported that receiving the Covid-19 vaccine was extremely important, and 95.0% had already received the vaccination. Their reasoning for receiving the vaccination varied from medical advice (17.5%), general worry about infection/ reinfection of Covid-19 (65.3%), in order to travel (8.4%), employee requirement (5.6%), and other reasons (3.2%). Specifically, 48.3% chose the Pfizer vaccine, 39.8% had Sinopharm, and 11.9% had AstraZeneca. These decisions on which vaccine to choose were influenced by the availability of the vaccine (35.1%), medical advice (16.9%), effectiveness (15.5%), safety and side effect profile (18.2%), age restrictions (2.7%), experience/ advice from friends and family (8.1%) and other reasons (3.4%).Overall, an overwhelming majority of our patients (95.0%) with rheumatic diseases in the specialised muscular-skeletal clinic in the UAE have received the Covid-19 vaccine. This reflects good public health service messaging and concentrated efforts from the medical community, UAE government, and the patient’s rheumatologists.
