Hygiene as a science in modern clinical therapeutic practice: from observation to digitalization (part one) 现代临床治疗实践中的卫生学:从观察到数字化(上)

Building on the global medical historiography, this review attempts to demonstrate the continued interest and involvement of doctors in investigation of the influence of environmental factors, their epidemiological and pathological aspects, on life expectancy and health of human beings, as well as to cover the most significant domestically developed prevention measures applicable in everyday life, during epidemics and against occupational hazards. We have also attempted to outline the history of interinfluence of the two medical specializations, including the new round of their transformation as they merge into the digital reality of today. The review shows that when medicine, as science and trade, in Russia was going through its establishing phases in Russia, the prominent Russian experts underscored the need for integrated application of therapeutic and hygienic approaches, development of the most effective combination thereof with the aim of qualitative improvement of public health care. The article considers the historical prerequisites for development of the system of preventive and anti-epidemic measures, which are the key safeguards against diseases, and development of the hygiene, including occupational hygiene, from the moment of inception to the age of digital medicine we live in currently.

Self-assessment of health and lifestyle as a basis for understanding health preservation by schoolchildren 学童对健康及生活方式的自我评估,作为了解健康保养的基础

The article describes the possibility of using the data on the health status and lifestyle self-assessment obtained during the questionnaire survey of high school students in the teacher's preventive activities aimed at preserving and improving the schoolchildren's health. The main indicators that according to the respondents determine their quality of life have been revealed, and health status is ranked only third. It has been found that 40% of students consider their health to be “good”, while 53% currently do not worry about their health at all. Certain characteristics of physical activity, nutrition, sleep schedule, prevalence of bad habits in students attending schools of various types are provided. Despite high subjective assessment of physical activity, only 8% of schoolchildren do morning excercises every day, most of students do not attend sports sections. More than 40% of students sleep less than seven hours, which has a negative effect on the adolescents' health. Evidence has been obtained that 19% of schoolchildren have tried vaping and 13% vape regularly, which is definitely worrisome.

Medical and social problems of optimizing the nutrition of children and adolescents under the conditions of today 在当今条件下优化儿童和青少年营养的医疗和社会问题

The article reviews publications devoted to the issue of optimization of nutrition of children and adolescents including school nutrition, various ways of formation of eating habits, succession of home and school nutrition, nutrition of children with alimentary-dependent diseases and children who go in for sports, quality of raw materials and products for school nutrition, and nutrition using new products. The information was searched for with eLIBRARY.RU, PUBMED, Web of Science and Scopus information portals and platforms from 2007 to 2021. Publications related to the issue were assessed while studying at school and during distance learning. Modern issues of nutrition of schoolchildren such as disturbances of alimentary behavior and underdeveloped skills of nutritional culture, dietary habits of schoolchildren with alimentary-dependent diseases and approaches to nutrition optimization at educational institutions are highlighted. The ways of prevention of alimentary-dependent diseases are estimated by way of inclusion of functional products into the diet of schoolchildren. Principal methodical and regulatory documents that can help reduce risks of alimentary-dependent diseases are mentioned. Pressing trends of researches regarding the reviewed topic were suggested to preserve health of schoolchildren as far as healthy nutrition goes.

Awareness of and adherence to measures of influenza and ARVI prevention among the citizens of Yekaterinburg 叶卡捷琳堡市民对预防流感和急性呼吸道感染措施的认识和遵守情况

According to experts, yearly flu rate can constitute 5–20% in adults and 20–30% in children; and in case of development of a pandemic, a number of those suffering from flu‑like infection can be increased by 50%. Vaccination against influenza and properly used non‑specific prevention can exclude seeking medical aid and hospitalization due to high incidence of COVID‑19 and reduce the load to the healthcare system. Research purpose: examine awareness of children, young people and adults about signs of flu, methods of specific and non‑specific prevention and their use in life. Materials and methods: the research was performed in three groups based upon online questioning: 270 adults aged 25 to 60, 1,112 students from universities and colleges aged 15 to 24, 101 schoolchildren aged 11 to 17. The questionnaire included sets of questions that reflected awareness of and commitment of population to measures of specific and nonspecific prevention. The students were questioned from January to February 2022. The obtained results show high flu and ARVI morbidity, insufficient hygienic education and commitment of citizens to the measures of specific and non‑specific flu and ARVI prevention. To improve the situation, not just hygienic education is required, but also education of medical workers, parents, healthcare workers, journalists and correspondents of mass media regarding flu and ARVI prevention, availability of vaccine prevention centers, and consultations of pediatricians and therapists regarding the issues of specific prevention.

The features of social and autonomic adaptation to study conditions depending on age, gender and socio-pedagogical educational environment 根据年龄、性别和社会教育环境对学习条件的社会适应和自主适应特征

The study is concerned with a pressing issue of assessing social adaptation (SA) and autonomic stability (AS) in the young people studying in various educational institutions. It is necessary to determine the students’ social adaptation and autonomic stability at different ages. The study included 100 schoolchildren who living in the big city; 89 schoolchildren living in rural areas; 70 schoolchildren living and studying in the city boarding school. The schoolchildren were aged 14–15. The group of youth included 248 first‑year students aged 17–18 and 136 6th year students aged 22–23. The study was performed with the use of the two‑factor personality questionnaire by M. Gavlinova approved by the European Union for School and University Health and Medicine and adapted to Russian conditions. The authors provide data for determination of social adaptation and autonomic stability in young people aged 17–23. When analyzing the findings, it was found that the differences in social adaptation and autonomic stability between schoolchildren were partially dependent on their gender, type of learning, and place of residence. Given equal starting opportunities in young males and females in the beginning of high school training, young males showed higher SA and AS values compared to young females by the end of training. A group was distinguished that included students showing low SA and AS values, which, in our view, required psychological and medical rehabilitation. Given the findings, we believe that physicians should further examine the patients with low AS values, and psychologists (neuropsychiatrists) should further assess patients with low SA values during medical check‑ups in order to ensure prevention and management of the disorders identified in schoolchildren and students.

On assessment of physical development of university students 论大学生身体发育的评价

Health of the younger generation determines the prospects of social and economic development of Russia. So, concern for healthcare of the youth is displayed in fundamental state documents (including the ‘Fundamentals of the state youth policy of the Russian Federation for the period up to 2025’) [1]. In accordance with its relevance, we conducted research aimed at examination of physical development of 1,820 students of the first and fourth years from four Universities of the Republic of Bashkortostan. Based on the past experience, standards of physical development for students from the Republic of Bashkortostan were developed and registered. A retrospective analysis was done comparing data related to modern students from the Republic of Bashkortostan and the ones related to the standard of physical development of students from the Republic of Bashkortostan obtained 25 years ago. The results reveal a tendency (during the last quarter of a century) to asthenization of modern youth from the Republic of Bashkortostan. The functional reserves of the cardiovascular and respiratory systems of students were assessed using the circulatory and respiratory Skibinski’s index. It is established that in students, the mean Skibinski’s index was 24.8 ± 0.3, which corresponds to the ‘satisfactory’ range only. The program called ‘Software to assess physical development and adaptation capabilities of the body’ was developed and registered at the Federal Institute of Industrial Property aimed at determination of a risk group of diseases among students depending on their physical development, functional condition and adaptive capabilities of the body.

Based on the past experience, standards of physical development for students from the Republic of Bashkortostan were developed and registered. A retrospective analysis was done comparing data related to modern students from the Republic of Bashkortostan and the ones related to the standard of physical development of students from the Republic of Bashkortostan obtained 25 years ago. The results reveal a tendency (during the last quarter of a century) to asthenization of modern youth from the Republic of Bashkortostan. The functional reserves of the cardiovascular and respiratory systems of students were assessed using the circulatory and respiratory Skibinski’s index. It is established that in students, the mean Skibinski’s index was 24.8 ± 0.3, which corresponds to the ‘satisfactory’ range only. Methodological aspects of studying the actual nutrition of athletes 研究运动员实际营养的方法学方面

Analysis of actual nutrition of athletes and its correspondence to energy expenditure seems relevant as a balanced diet produces a direct effect on physical endurance and professional performance of athletes. It is meaningful to estimate not just nutritional and energy value of the diet, but also a degree of diversity of basic groups of food products as sources of essential nutritional substances in the diet of athletes along with dietary fibers to optimize the gut microbiome diversity. Regular studies of actual nutrition conducted with certain time intervals and simultaneous assessment of biomarkers characterizing the alimentary status enable to trace their effect on professional performance, effectiveness of the training process, adequate restoration and health for every athlete and the whole team.

Comparative assessment of physical development of schoolchildren and children involved in swimming 小学生与游泳儿童身体发育的比较研究

Health of children and adolescents is an essential criterion of well-being of the society. Monitoring of health among children is impossible without assessment of physical development of children and adolescents. Individual-based analysis of anthropometric characteristics is required to develop personalized preventive activities. Considering different levels of physical load of schoolchildren in case of a disturbed nutritional status, an approach to body mass constituent study that deviates from the normal values requires examination. The study purpose was to assess physical development and component composition in schoolchildren during a medical examination and in children involved in classic swimming at a sports club in the city of Samara. The study object involved children studying at a general educational institution and not engaged in any sports sections (first group), and children involved in classic swimming at a sports club in Samara (second group. Significant differences in basic values of body composition were found during the conducted study of physical development and component composition of schoolchildren who do not go in for sports and junior athletes.

Awareness of medical college students about the formation of reproductive health 医科大学生生殖健康意识的形成

The study objective is to assess awareness of medical college students about formation of reproductive health. 80 female students from a medical college were surveyed concerning formation of their menstrual function; this survey was followed by an interview of 126 female students and 81 male students from medical colleges about their awareness of reproductive health protection and attitude to hygienic education regarding this topic. Data were processed using Statistica 13.0 (StatSoft, USA). 83.8% girls had their periods between 11 and 14, whereas in 7.5% of girls they started at the age of 15 and older. During the interview, 22.5% of girls had an irregular menstrual cycle. The conducted study revealed an insufficient awareness of students from a medical college of reproductive health formation. It produces a negative effect on their own health and will prevent subsequent effective hygienic education of different categories of population as far as this issue goes.

Metabolic syndrome and obesity in children as a social and hygienic issue 儿童代谢综合征和肥胖作为一个社会和卫生问题

The problem of pediatric obesity focused on risk factors at early stages because their modification can reduce the risk of later metabolic diseases and disability. It is the metabolic syndrome that describes a set of cardiometabolic risk factors, including abdominal obesity, insulin resistance, elevated blood pressure, high level of triglycerides and low level of high-density lipoproteins that increase the risk of cardiovascular diseases and type 2 diabetes mellitus. Obesity mainly determines statistical values that characterize the case with endocrine gland diseases among children and adolescents in the Republic of Tatarstan. Obesity is not growing at the same rate any more. The current level of excessive weight in children is too high and requires intervention at the level of the community and school. Inactivity and non-rational nutrition can contribute to formation of excessive body mass among children. Regular physical activity and better nutrition of adolescents is a worthy investment in the health of future generations.
