Coffee Pit Stop is one of the coffee shops in the city of Gresik, with its segmentation of consumers being middle and lower markets at prices that are very affordable for every community. Based on the initial survey, some complaints were felt by the customer, including seating for only one person so that if they came together with a large number they had to adjust their place first, the service was less representative; the taste variants were limit to each menu offered. This study aims to measure customer satisfaction by using the Servqual, IPA, and PGCV method with 100 respondents. The results showed that there were three attributes in quadrant A namely P4 very complete support facilities with PGCV values of 5.99, P5 wide and safe parking area with a PGCV value of 5.54, P10 immediately providing the right service according to customer desires with amount PGCV is 5.50. The corrective action taken to improve service quality is to carry out routine cleaning of the supporting facilities owned by Gresik Pit Stop. Suitable parking arrangements by parking officers must always consider so that the effectiveness of land use can be maximized, and the waiter must pay attention to the accuracy and speed in serving customers so that customer satisfaction will be creating.

The production process of API 5L PSL2 pipes in Pipe Companies in 2017 happened to defect products at 7%. This defect is still above the standards set by the Company at 4%. The study aims to determine the value of sigma and provide recommendations for improvement with the six sigma method. Based on the results of the DMAI analysis, the amount of the pipe sigma level was 3.65 with the value of DPMO 14448. Types of defects identified in the process such as open welding defects, high lows, contact marking, scarring, and under dimension Proposed improvements were made by designing recommendations for improvements using 5W + 1H by checking the condition of the forming machine and changing bearing components before the bearing lifetime, performing preventive maintenance every week and adding coolant bursts to the cutting tool.

The success of the project can be judged by the timeliness of the planned target. The problem in the highway project is in the difference between cross drainage age implementation and the age of the project plan that has been set, so that, the project becomes not timely. This research aims to evaluate the critical path of cross drainage project, and what improvements are appropriate in the framework of project acceleration. The Critical Path Method (CPM) method is used to determine the critical path of the cross drainage project activity, from which critical path will be analyzed by using the Fishbone Diagram. Furthermore, borda technique is used to determine the appropriate improvement.

Inventory is one of the things that must be considered by the company. One way is to manage inventory management. The problem that occurs in the research location is the lack and excess stock for packaging raw materials. This lack of capital is due to the absence of calculations for the number of safety stock and reorder points, the cause of excess inventory is the excess in the absence of groupings of packaging raw materials to find out which products are more critical according to demand. Analysis of system improvements focuses on packaging raw materials. Based on the results of data processing obtained from 304 existing packaging raw materials, there are 15 packaging raw materials in class A. Processing is focused on class A only. Forecast calculations using SPSS software. And processing is continued by using the size-reorder point systems lot method with quantity results, reorder margin and safety stock that varies 15 raw materials according to the calculations performed.

The development of the business world is currently experiencing a very rapid growth rate. Planning for the proper procurement of materials will be able to facilitate the company's production activities. Production process activities in companies often experience problems such as inaccurate production, which is caused by delays in the arrival of raw materials, and damage to raw materials due to too long storing in warehouses. This study aims to plan future needs with the Material Requirement Planning (MRP) approach by doing forecasting, making the Master Production Schedule (MPS), Material Requirement Planning (MRP) and conducting a cost analysis of the five methods used. The results of this study are forecasting product demand will be used linear regression method, and effective material requirements planning method with the lowest cost is to use the Lot For Lot (LFL) method of $ 164.48. The application of the Lot For Lot method can reduce costs by 90.06% compared to the Fixed Period Requirements (FPR) method used by the company.

Deaths and injuries from traffic accidents have become health problems for people throughout the world including Indonesia. The saddest data from the victims who died due to traffic accidents found that 10,428 people were killed in 2017 because the drivers did not use seat belts. This research aims to see the relationship between the perception of safety risk i.e. the ability, knowledge and environmental factors with the behavior of the use of safety belts in truck drivers in mining companies. This research is a comparative causal research i.e. research that states the relationship of one variable causes other variables. What is affected is the dependent variable, namely the use of safety belt behavior and the influencing variable is the independent variable, namely the perception of the risk of driving safety. Participants are 25 mining company truck drivers. The data obtained is then processed and analyzed using the SPSS version 16. Based on the results of data processing and analysis, it is found that the ability, knowledge and work environment factors have an influence on the safety belt usage behavior.

A company that serves loading and unloading services which includes demolition and loading of goods carried out in the Pier Port area also has a high potential for workplace accidents. From interviews with safety officers, accidents that occur are still limited to minor accidents such as eye irritation due to dust and irritation to the feet due to fertilizer leaking and about the body of the worker. Based on the workers' side, the potential for workplace accidents can be overwritten by fertilizers, irritation due to leaky fertilizers, respiratory problems from chemical conditions that smell of fertilizers and fall from trucks. This study aims to determine the activities that have the potential for workplace accidents and carry out risk assessments that occur with the HIRARC method. The object of research is a company engaged in loading and unloading operations operating in East Kalimantan. Data collection is done by direct observation by observing the location and activities of workers. Also, by direct interview with the Safety Officer (1 person), Field Coordinator (1 person), Tally Man (1 person), Rager Crane Ship (1 person) and field workers totaling 33 workers. The results of the study obtained risk factors from 8 activities such as placing cranes, preparing loading and unloading equipment, installing slings and hooks to crane hooks, preparing tally man tables on the dockside, workers entering barges, directing transport trucks, fertilizer loading, heading vehicles to leave the dock and 14 potential hazards. From the risk assessment obtained three sources of extreme value hazard, nine sources of hazard that have high-risk values and two sources of hazards that have moderate risk values. Risk control can be done to providing K3 guidance, make good working procedures, and use complete PPE in carrying out loading and unloading activities.

This study aims to reduce the waste that occurs on each workstation and has an impact on increasing productivity and reducing production costs caused by waste that occurs during production. The data used for this study is quantitative data obtained by direct observation to the relevant departments regarding the problem of identifying waste that occurs — data taken from observations as much as 30 times the collection of data and historical data used from January 2016 to June 2017. By using the value stream mapping method on lean manufacturing, it can be seen that there are five types of waste from the existing seven waste. The five wastes are unnecessary motion, waiting, reject, transportation and in-process processes. From the results of the value stream mapping carried out a reduction in waste on each workstation with a total reduction of waste that occurs at 66.97 tons / year or 18.6% on waste gel and 88.8 tons / year or 19.3% at waste powder and there was a reduction in the changeover process for 45 minutes or 12.16% of the total changeover time before the improvement. Also, there was also an increase in the production process speed of 2 minutes 47 seconds or 4.52% from the lead time of the previous production process, 61 minutes 34 seconds to 58 minutes 47 seconds.

Pelabuhan Banten is a company engaged in port services in Indonesia, one of the essential aspects of which is the process of loading and unloading ships which are part of the landing process that will affect the queue of vessels entering or exiting the port. In this case, the variables observed were data on the time of arrival, service time, and the number of services. The method used in the retrieval of data on arrival time is purposive sampling. Data analysis methods include the detection of data distribution, determination of the queue model, and performs the calculation of the queue model. Foaming Agent company is a company engaged in the chemical industrial manufacturing, especially the additive industry for plastic and synthetic rubber. In the production process, the Foaming Agent Company has a problem with one of its products, Azodicarbonamide (ADCA). The number of defects caused becomes a problem primarily related to the course of production and the relationship with the customer. The research was conducted using the Six Sigma method by implementing DMAIC (Define, Measure, Analyze, Improve, Control). This study aims to identify the factors that cause defects and the most optimal choice of options to increase the level of sigma in the ADCA production process. Based on the results the most significant aspects that cause defects in ADCA products is Average Particle Size; 66.77% of the overall production defects. The sigma level that obtained before and after the improvisation is 3.68 and 4.03 with the average achieved savings per month are Rp.600 million. The results of the gradual decrease in defects showed that the options implemented at the improvisation stage were successful, but still needed improvisation so that the company's performance can be even better
