Purpose To Check the importance of Intraocular Pressure Asymmetry In Rural-Based Hospitals In North India. Methods A prospective study was conducted on all the patients above the age of 40 years who attended ophthalmology OPD of NC Medical College and Hospital, Israna. The study was conducted for a period of 6 months from December 2022 to May 2023. A total of 200 patients were taken for the study. The purpose of this study was to investigate the importance of IOP asymmetry in patients visiting hospitals in north India. Intraocular pressures were measured by Non-contact tonometry (NCT) for both eyes in each case two times. The ones with IOP asymmetry of more than 3mmHg is measured again with Goldmann applanation tonometry (GAT). The amount of absolute IOP asymmetry between the 2 eyes was calculated by subtracting the lower IOP from the higher IOP, which were hence compared to see the percentage difference of IOP asymmetry present in glaucomatous and non-glaucomatous patients. Glaucoma was diagnosed if the intraocular pressure asymmetry difference was > 5 mm hg with signicant glaucomatous optic disk and eld changes. Results A difference of >3mmHg is associated with 19% probability of having glaucoma and a difference of >5mmHg with 50% probability of having glaucoma. Conclusions Inter-eye Intraocular pressure (IOP) asymmetry is a regular nding in glaucoma patients. There is a direct relationship between the amount of IOP asymmetry between fellow eyes and the probability of having glaucoma.

An insurgency is a violent, armed rebellion against government. In Assam various sub-national groups like ULFA, NDFB, DHD etc. seem to have been involved and involved for fullment of demands for safeguarding their identity, rights over land and resources. It has often been manifested as ethnic terrorism; sometimes, deliberate violence to advance their cause; sometimes, seen as proto-guerrilla terror acts directed against symbolic targets, designed largely to foster self identity as well as development. Whereas, stability for a country where there belongs to different communities, castes, religions, all recognize the fact of unity and integrity for much needed peace and security for development. Insurgency and stability are the two relative terms, though one challenges the other. The insurgency phenomenon has been hampering the process for a long period of time in Assam. The question for its emergence seems to have been the most enigmatic, most destructive; arguably second to none in terms of brutality, savagery, wanton callousness, scope of operation and in terms of challenges it paused to stability and integrity. The conict dynamics ranges from autonomy to secession and from sponsored terrorism to ethnic violence; mostly generated due to continuous ow of illegal migrants across the borders. The failure of the government in safeguarding the interest of indigenous people by deporting illegal migrants brought obviously insurgency vis-a-vis the question of stability in the state. This paper tries to understand the root causes of insurgency problem in Assam and the concomitant stability problem for bringing permanent solution it created which hampers the socio-economic development in the light of focusing much needed harmony, peace and security in the state.

The case study is presented of a 32 year old man that presented with intermittent pus discharge from a recurrent boil in his peri-anal region that was diagnosed as stula-in-ano. The case was well managed with Apamarga Kshar Sutra and Ayurvedic medications. The patient recovered very well within 4 weeks. Ayurveda treatment modalities possess promising result in the cases of Bhagandara (stula-in-ano). The use of Kshar Sutra in treating stula-in-ano is characterized by its simplicity, low complication rates and cost effectiveness. Therefore, Kshar Sutra is proved to be a highly efcacious, minimally invasive surgical approach for managing stula-in-ano.

Lower urinary tract symptoms (LUTS) are common and severe in men, causing them to avoid social situations and struggle with home and work activities. A prospective study investigated the use of the Synthesis repertory in managing cases of male LUTS in older adults. 30 cases of LUTS were selected, and medicines were prescribed based on symptoms. Out of 30 cases, 19 showed good improvement, 6 showed moderate improvement, and 5 failed to improve. The study concluded that Synthesis Repertory has a role in managing cases of male LUTS in older adults, as evidenced by clinical trials and observations from homoeopathic similimum outcomes.

In today's obstetrics, the ultrasound is one of the most often utilized diagnostic tools for identifying fetal developmental abnormalities. However, it is crucial to understand which defects may be diagnosed with great certainty during pregnancy and which ones can only be discovered partially or not at all before giving birth. Our study aims to evaluate the role of ultrasound in detecting congenital fetal anomalies and to nd out the most common system involved / most common fetal anomaly encountered in our institution. Prenatal diagnostics also requires a great deal of medical professional responsibility because the effects of an unintentionally incorrect diagnosis can have a negative impact on both the fetus and the family.

Gonadal dysgenesis is a group of heterogeneous disorders with very rare presentation. The clinical manifestation of disease includes primary amenorrhoea, failure to develop secondary sexual characters and also infertility. Here we are reporting a case of 29 year old phenotypic female who presented with primary infertility with 46, XX karyotype with hypoplastic uterus and streak ovaries (on imaging), with high gonadotropins level and low estradiol. After evaluation of the couple and proper counseling plan was made to go for cyclic hormone therapy for 3 months followed by donor oocyte and husband sperm IVF due to hypoplastic uterus with gonadal dysgenesis. 3 Day 3 embryos were transferred after hormonal therapy of 3 months. She was conceived with twins and later delivered 2 healthy babies at 9 months. We conclude that, pregnancy can be achieved even in gonadal dysgenesis patients after proper diagnosis of the condition and counseling the couple regarding the best treatment options available.

Background: Hollow Viscus Perforation is characterized by loss of gastrointestinal wall integrity with leakage of enteric contents. To Study the Role of Mannheim Peritonitis Index (MPI) scoring system i Aim: n predicting the Mortality and Morbidity in peritonitis due to Hollow Viscus Perforation. This is a hospital Materials and Methods: based cross sectional study which was carried out at Department of General Surgery from November 2017 to October 2019 at Katuri Medical College and Hospital, Chinakondrupadu, Guntur, Andhra Pradesh. Mannheim Peritonitis Index score was used. Results: Mean age group of the patients was 52.35 ± 11.09. Majority of the patients i.e. 19(31.7%) were seen in 51 – 60 years age group. Among the 39(65%) male patients, majority were identied to be in 51-60 years. Among the 21(35%) Female patients, majority i.e. 8 patients were identied to be in 61-70 years. The present st Conclusion: udy suggests that Multiple attempts at developing a scoring system to predict the risk of mortality and morbidity in patients with peritonitis due to hollow viscus perforation undergoing emergency laparotomy. Most of the scoring systems cannot assess individual patient risk. MANNHEIM PERITONITIS INDEX scoring system is an easy and very useful tool for predicting the risk of morbidity and mortality in patients with peritonitis due to hollow viscus perforation.

Introduction:- The aim of this retrospective observational study was to compare the potential efcacy of epidural steroid injection along with other conservative measures for relieving pain and improving function in patients with lumbosacral pain with radiculopathy caused by lumbar disc herniation. The route of administration (transforaminal, caudal or translaminar) were chosen on basis of pattern of radiculopathy and mri ndings of disc herniation. Methods:- This study included 131 patients who presented with low back pain and radiculopathy due to lumbar disc herniation(at levels of L4–L5 or L5–S1) diagnosed clinically and conrmed by means of MRI. 4 patients were lost to follow up remainig 127 patients at nal follow up.(127 patients)All patients were given fair trial of conservative treatment with drugs and physiotherapy. All patients were assessed at presentation and after starting the treatment at the second week and then third and sixth month using the visual analogue scale (VAS) for pain and the Oswestry Disability Index (ODI) for function status. Results:- In the transforaminal group (45 patients), there was a statistically signicant improvement in the ODI scores from before the injection (ODI mean 62.4) to 2 weeks after the injection (ODI mean 24.4, P < 0.01), and upon follow-up at 3 months. (ODI mean 20.8, P < 0.01). 21 patients (16.5%) required 1 or 2 repeated injections, 4 (3.1%) patients underwent surgery. In the interlaminar group (19 patients), there was a statistically signicant improvement in the ODI scores from before the injection (ODI mean 60.7) to 2 weeks after the injection (ODI mean 30.1, P < 0.01), but not upon follow-up (ODI mean 43.2, P = 0.09).5 (26.3%) patients required 1 or 2 repeated injection, 2 (10.5%) patients underwent surgery and 4(21%) patients developed transient paraparesis.In caudal group(63 patients) there was statistically signicant improvement in ODI SCROES from before the injection (ODI mean 39.6) to 2 weeks after the injection (ODI mean 29.6) which deteriorate at 3 months( ODI mean 31.8).There was an insignicant difference (P > 0.05) between the transformainal and caudal groups in the VAS, except at the third month (P < 0.05). Epidural steroid injection could be a preferable choice in chronic lowback and radicular Conclusion:- pain due to LDH. It has shown good short term outcomes and can be safe, cost effective and minimally invasive treatment and alternative to surgery.

The route of administration\u0000(transforaminal, caudal or translaminar) were chosen on basis of pattern of radiculopathy and mri ndings of disc herniation.\u0000Methods:- This study included 131 patients who presented with low back pain and radiculopathy due to lumbar disc\u0000herniation(at levels of L4–L5 or L5–S1) diagnosed clinically and conrmed by means of MRI. 4 patients were lost to follow up\u0000remainig 127 patients at nal follow up.(127 patients)All patients were given fair trial of conservative treatment with drugs and\u0000physiotherapy. All patients were assessed at presentation and after starting the treatment at the second week and then third\u0000and sixth month using the visual analogue scale (VAS) for pain and the Oswestry Disability Index (ODI) for function status.\u0000Results:- In the transforaminal group (45 patients), there was a statistically signicant improvement in the ODI scores from\u0000before the injection (ODI mean 62.4) to 2 weeks after the injection (ODI mean 24.4, P < 0.01), and upon follow-up at 3 months.\u0000(ODI mean 20.8, P < 0.01). 21 patients (16.5%) required 1 or 2 repeated injections, 4 (3.1%) patients underwent surgery. In the\u0000interlaminar group (19 patients), there was a statistically signicant improvement in the ODI scores from before the injection\u0000(ODI mean 60.7) to 2 weeks after the injection (ODI mean 30.1, P < 0.01), but not upon follow-up (ODI mean 43.2, P = 0.09).5\u0000(26.3%) patients required 1 or 2 repeated injection, 2 (10.5%) patients underwent surgery and 4(21%) patients developed\u0000transient paraparesis.In caudal group(63 patients) there was statistically signicant improvement in ODI SCROES from before\u0000the injection (ODI mean 39.6) to 2 weeks after the injection (ODI mean 29.6) which deteriorate at 3 months( ODI mean\u000031.8).There was an insignicant difference (P > 0.05) between the transformainal and caudal groups in the VAS, except at the\u0000third month (P < 0.05). Sickle cell disease (SCD) is an inherited genetic disorder characterized by the production of abnormal hemoglobin, known as hemoglobin S. This alteration leads to the formation of sickle-shaped red blood cells, impeding their passage through blood vessels and causing blockages, ischemia, and pain. Additionally, individuals with SCD are more susceptible to infections and vascular complications. Acute pain crises are a common manifestation of SCD, triggered by blood vessel obstruction. These crises may necessitate immediate medical intervention to alleviate pain and prevent complications. Long-term complications include chronic organ damage, such as kidney failure, lung injuries, and cardiac disorders. COVID-19 has raised additional concerns, as SCD patients may face an elevated risk of severe complications. Hydroxyurea and blood transfusions are therapeutic strategies used to reduce pain crises and prevent complications. Ongoing research seeks more effective therapies and comprehensive management strategies to enhance the quality of life for those living with this genetic disorder.
