We present a case of a 6-month-old male infant, presenting two sequential cranial swelling with hard consistency, non-adherent to deep planes, without inflammatory signs. A soft tissue ultrasound showed two well-delimited, round formations with surrounding sclerosis, located in the bone diploe. It was not conclusive. Investigation was completed by cranioencephalic magnetic resonance imaging, that revealed two formations suggestive of Dermoid cysts (DCs). It Currently maintains an expectant attitude and clinical stability. Dermoid cysts are benign tumours frequently located in the head. They are among the most common paediatric skull tumours and usually occur as solitary lesions. DCs manifest as a painless, consistent and non-pulsatile swelling. In their natural history, DCs can enlarge and erode the cranial bone, rupture or become infected. Cranioencephalic magnetic resonance imaging can reveal intracranial or intraspinal extension. The clinical approach is controversial.

Idiopathic Intracranial Hypertension is a condition characterised by raised cerebrospinal fluid pressure of unknown etiology seen predominantly in obese, middle aged patients. The typical clinical features include headache, pulsatile tinnitus, diplopia and transient visual obscurations. Seizures are a rare symptom seen in IIH patients. We present a case of a young, obese female patient who presented with headache and seizures. Imaging showed evidence of Idiopathic Intracranial Hypertension with temporal lobe encephalocoeles and brain herniation into arachnoid granulations. The diagnosis was subsequently confirmed by clinical workup and she was started on treatment for the same.

Vitiligo has been reported in 2% to 8.3% of patients followed for melanoma. Other studies have reported the rarity of this event, especially in patients followed for other metastatic cancers. We report a rare case of vitiligo which occurred in a 67-year-old patient followed for metastatic bronchopulmonary cancer treated with immunotherapy such as Pembrolizumab. Vitiligo occurring with the use of pembrolizumab in bronchopulmonary cancer has been exceptionally reported in the literature. Immunotherapy in bronchopulmonary cancer is associated with many side effects, vitiligo is very rare in this context and can have an impact on the quality of life of patients.

Introduction To remove tears from people’s eyes’ – is one of the greatest challenges of lacrimal surgery. Epiphora is defined as watering eye due to inadequate drainage of normally produced tears. Dacryocystitis is the inflammation of the lacrimal drainage system which presents as acute dacryocystitis or a much commonly as chronic dacryocystitis. Other common causes of ephiphora are blepharitis, conjunctivitis, trichiasis, punctal eversion and foreign bodies. The conventional treatment for watering eyes due to Naso Lacrimal Duct (NLD) is the procedure of external dacryocystorhinostomy (DCR) which has its own limitations and disadvantages. With the introduction of high resolution endoscopes for paranasal sinuses, endoscopic endonasal DCR (EnDCR) is gaining popularity. The main advantages of the endoscopic technique are minimal morbidity, less intraoperative bleed, shorter operative time, preservation of pump function of orbicularis occuli muscle, presaccal fibers and medial canthal tendon and avoidance of cutaneous scar. In EnDCR the most common cause for failure of surgery is closure of rhinostoma and to prevent this silicone tube insertion is the most preferred method. It has also been claimed that silicone tubing would improve surgical results. But studies also indicate that silicone tubing itself results in certain problems like, foreign body sensation, increased granulation tissue leading to failure, prolapse of tube leading to injury to cornea, nasal migration of tube, fistula formation, pharyngitis and associated sinusitis & recurrent nose bleed. The present study was undertaken to compare the surgical results of endoscopic endonasal DCR with and without silicone stent. Aims & Objectives To compare the success rates of endonasal dacryocystorhinostomy and evaluate the presence of post-operative changes and their effect on the outcome of the surgery in endoscopic dacryocystorhinostomy with and without stenting. Material & Methods This Prospective randomized comparative study was carried out amongst the patients attending department of ENT and referred cases from ophthalmology department for chronic nasolacrimal duct obstruction, both as inpatients and outpatients with complaint of chronic epiphora. 50 patients divided into two groups of 25 each fulfilling inclusion and exclusion criteria were included using convenient sampling. Participants were divided into two groups of 25 each based on randomization technique. Group 1 includes patients undergoing EnDCR with stenting and Group 2 includes patients undergoing EnDCR without stenting. Patients were assessed for post-operative complications like granulations, size of the neo ostium and patency of the neo ostium were assessed on nasal endoscopy. Data was entered in MS Excel work sheet. Data was analyzed using appropriate statistical methods. Result This study had Female predominance in both the groups. No predominance for any side was observed. No major intra operative complications were observ

Context: Serum uric acid is a potential risk factor for development of type 2 Diabetes Mellitus. Hyperuricemia is frequently found in persons with metabolic syndrome. Aim: To compare the serum uric acid in the normal population and in those with diabetes mellitus. Settings and Design: A comparative study was conducted in a tertiary-care teaching hospital in South India, with 100 patients with type 2 diabetes mellitus and 100 healthy controls. Methods and Material: Serum uric acid levels were compared in patients with diabetes and the controls. The serum uric acid levels of the cases and controls were correlated with GFR. Association with factors such as age, gender, BMI, duration of diabetes, proteinuria and hypertension were looked for. Statistical analysis used: Appropriate test of significance such as Student t test, one-way ANOVA test and Pearson’s correlation were used. A p-value of less than 0.05 was considered significant. Results: The mean GFR in cases were 82.6 and in controls were 116.76. The mean serum uric acid levels were significantly higher among cases as compared to controls (4.8 v/s 3.7 mg/dl. A weak negative correlation was obtained between serum uric acid levels and GFR, both in diabetics as well as healthy controls. Conclusions: Serum uric acid levels were higher in patients with diabetes mellitus than in the normal population. Those with elevated serum uric acid levels were found to have reduced GFR.

Kimura Disease (KD) is a rare chronic inflammatory disorder that involves the subcutaneous tissues, occurring predominantly in the head and neck region, frequently associated with regional lymphadenopathy or salivary gland involvement. The etiology is largely unknown and it most commonly affects Asian men. KD is characterised by painless subcutaneous masses in the head and neck region, blood and tissue eosinophilia, and elevated immunoglobulin E (IgE)

Introduction: The presence of glucosuria is usually a red flag that leads to diabetes mellitus screening in pediatric patients. Despite this correct approach, high levels of glucosuria without hyperglycemia should raise suspicion for other diagnoses, namely renal tubular disease, such as Familial Renal Glucosuria (FRG). A large number of mutations on the solute carrier family 5 member 2 (SLC5A2) gene, which encodes sodium-glucose cotransporter 2 (SGLT2), have been reported to be responsible for the increase in renal excretion of glucose.(1) Case Report: We report a case of a teenager, followed in our outpatient clinic for Hypokalemic Periodic Paralysis type 2 (HyPP2), in which persistent renal glucosuria was fortuitously found. Genetic testing for SLC5A2 gene was conducted, leading to the identification of two mutations in the SGLT2 gene, both in heterozygosity, one in exon 14 (N654S), previously reported, and another in exon 2 (A94C), a novel mutation not yet described in population databases. Discussion: The fact that our patient had mutations in two different genes from different chromosomes, both related to sodium channels and one of these not previously described, makes this case interesting. The key point is that it is important to be aware of these diseases and assess the child as a whole. Despite the apparent benignity of these two diseases, clinical surveillance must be tight. We advise avoiding prolonged fasting, a diet rich in carbohydrates and intense physical exercise. Conclusion: In the presence of isolated glucosuria, without hyperglycemia, we should suspect Familial Renal Glucosuria. Familial Renal Glucosuria is a rare disease caused by mutation in SGLT2 gene. Exon 2 (A94C) in heterozygosity is a novel mutation not yet described in population databases. This is the first description of a patient with both Familial Renal Glucosuria and Hipokalemic periodic paralysis type 2. The association of both these diseases had not yet been reported.

Conclusion: In the presence of isolated glucosuria, without hyperglycemia, we should suspect Familial Renal Glucosuria. Familial Renal Glucosuria is a rare disease caused by mutation in SGLT2 gene. Exon 2 (A94C) in heterozygosity is a novel mutation not yet described in population databases. This is the first description of a patient with both Familial Renal Glucosuria and Hipokalemic periodic paralysis type 2. Background: Surgery for intra-articular distal humeral fractures can be difficult. Total elbow arthroplasty (TEA) versus open reduction and internal fixation (ORIF) for treating elderly patients is still up for debate. In order to compare ORIF with TEA for treating intra-articular distal humerus fractures in patients, this study compared the clinical and functional results. Methods: Between one and half years at Sri Krishna Medical College Hospital, Muzaffarpur, Bihar, India, 70 willing patients with intra-articular distal humeral fractures who underwent osteosynthesis by orthogonal locking compression plating using posterior approach with olecranon osteotomy participated in a prospective study. Results: With a mean age of 38.5 years, our study included 38 male patients and 32 female patients. A motor vehicle collision accounted for 65.2% of instances, an accidental fall accounted for 21.7%, and a fall from height accounted for 13%, with the right side being more frequently afflicted (73.9%). The average operation lasted 100 minutes. Following surgery, the Mayo Elbow Performance Score was 83.3 percent, and the mean arc of mobility was 117°. 82% of cases had excellent to good outcomes. Conclusions: ORIF demonstrated superior results to TEA in patients with intra-articular distal humerus fracture.

This case report presents a unique and unreported association between synchronous hepatocellular carcinoma (HCC) and liposarcoma in a 30-year-old male petrochemical industry worker. The patient's main concerns were asymptomatic hyperferritinemia and isolated high gamma-glutamyl transferase levels. Clinical findings revealed mild hepatic steatosis, nonspecific nodular images, and fibrosis grade F4 on imaging and elastography. Further investigations confirmed the presence of HCC and subsequent liposarcoma in different organs. The patient underwent SIVB-VI hepatectomy followed by a nephrectomy to remove the tumors. The case report adds to the scientific literature by presenting a clinical scenario that has never been described before—the synchronous occurrence of HCC and liposarcoma in a patient with occupational risk exposure to aromatic organic compounds. The patient's occupation, along with other risk factors such as alcohol consumption, cirrhosis, and dyslipidemia, may have contributed to the development of these tumors. This report underscores the need for further research with larger sample sizes and extended follow-up periods to better understand the underlying mechanisms and outcomes associated with multiple primary neoplasms. The findings emphasize the significance of environmental and occupational chemical exposure as potential risk factors for developing HCC and numerous primary malignancies. Enhanced knowledge in this area can improve clinical understanding and inform optimal treatment approaches for patients with similar presentations.
