Introduction: Osteogenesis imperfecta (OI) is an autosomal dominant disorder that affects about 6-7/100,000 patients. Case report: We describe a term neonate, with a supervised pregnancy and fetal ultrasonography at 23 weeks with bone dysplasia suspicion. Physical exam revealed axial hypotonia, increased anterior fontanel with distant cranial sutures and small and tapered nose. Skeletal radiography showed hypomineralization and wormian bones of cranial calotte, thin ribs and short and curved femurs. Genetic test revealed heterozygous variant of p.(Gly328Ser) in exon 19 of the COL1A2 gene, of the missense type. By 2 months of age, parents noticed unexplained left lower limb edema, associated with severe pain and crying, and he was diagnosed with aligned fracture of the left femur diaphysis. Discussion: There are several genetic disorders and congenital defect conditions, such as OI, that have been associated with bone fragility and fractures that can be misdiagnosed as child abuse. possible. This case report highlights the importance of valuing infant’s bone fractures, especially if the fracture is unexplained and located in places like long bones of the arms and legs, ribs and small bones of the hands and feet. Conclusion: Clinicians should always look for fragile bones and exclude causes such as OI. Never forget to exclude, as well, child abuse, as part of differential diagnosis.

Background: Although cancer mortality rates in Africa are higher than in high-income countries, the disease has received little attention in the region. We aimed to highlight the rising cancer burden in the Republic of Benin, trends, characteristics, controversies, and their implications for national development. Methodology: A review of published studies and documents was conducted using keywords including cancer, incidence, mortality, Africa, The Republic of Benin, risk factors, behavioral issues, cultural disparity, psychosocial and ethnic background in Medline, Scopus, Pubmed, and Google. Some of the incidence and mortality figures retrieved from published papers and the GLOBOCAN website were subjected to graphical and frequency analyses. Findings: Breast cancer and lung cancer accounted for approximately 12% and 18% of all new cancer cases and deaths worldwide, respectively. Africa currently has the fourth highest age-standardized cancer mortality rate globally, with Sub-Saharan Africa having the highest incidence rates. Certain factors, such as westernized diet, urbanization, and possibly increased awareness, had been implicated, though their precise contributions had yet to be determined. Conclusion: Cancer will compound The Republic of Benin’s disease burden, increase poverty and gender inequality, and reverse global gains in maternal and neonatal mortality unless urgent action is taken.

Nous rapportons le cas d’une localisation atypique de l’histiocytome au niveau du seuil narinaire chez une patiente âgée de 57 ans.

INTRODUCTION: Facial neuritis is a disease of the nervous system, characterized by shallowing or disappearing wrinkles of the frontalis muscle, incomplete eyelid closure, shallowing nasolabial groove, skewing mouth angle, showing teeth to the healthy side, and skewing mouth angle to the affected side when relaxing. CASE REPORT: In this article, it is reported that a 27-year-old male patient was diagnosed as facial neuritis, and the patient appeared after injection of the COVID-19 vaccine (Modena mRNA-1273) on May 17, 2022. After inspection, no obvious abnormality was found on the MRI plain scan of brain. Electromyography: the left and right facial nerve conduction were normal, and the amplitude of evoked potential on the left side was significantly lower than that on the right side. House-Brackmann Facial Nerve grade IV, the patient's condition improved 14 days after hormone treatment. CONCLUSION: The causal relationship between vaccination and the development of Facial neuritis uncertain. The temporal relationship to the vaccination should encourage health care professionals to be vigilant toward people developing Bell’s palsy after SARS-CoV-2 vaccination with mRNA vaccine. Diagnosis and treatment should be swift and decisive.

Concomitant chemoradiation (CRT) for locally advanced cervical cancer has become an established treatment based on randomised trials. Major concerns, however, remain over the acute and late toxicity and hence the generalisability of the conclusions of these studies. The study period was from October 2020 to May 2022. A total of 126 patients with Stage IIB to IIIB Cervical Cancer were taken for the study from Patna Medical College Hospital, Patna, Bihar with consent from the patients. Most of the patients were of stage IIIB (47.60%) in group A and (54%) in group B. Most of the patients had parity ˃5 (68.30%) in group A and (87.30%) in group B. Most of the patients (58.73%) in group A and (66.66%) in group B completed EBRT within the schedule time of 35 days. Twenty seven (42.85%) patients in group A and twenty three (36.50%) patients in group B had break during EBRT due to toxicity. In conclusion, observation during follow up after 6 months group A patients 74.60% had no evidence of disease and in group B 76.19% had no evidence of disease or no residual disease. In group A 25.40% had residual disease and in group B 24.60% had residual disease.

Abstract Background: Depression is usually managed in primary care and antidepressants are often the first-line treatment, but only half of those treated respond to a single antidepressant. Objectives: To compare efficacy of Escitalopram monotherapy versus combination with L-methyl folate or Mirtazapine in first episode of Depression Design: The comparative follow-up study was conducted from June 2021 to May 2022 in the Department of Psychiatry, MGM medical college, Indore (M.P.). Setting: Participants were recruited from Psychiatry outpatient department (OPD) in Mental Hospital Indore and MY Hospital, Indore. Participants: The study included 40 patients of age 18-45 years, attending Psychiatry outpatient department (OPD in Mental Hospital Indore and MY Hospital Indore, fulfilling the inclusion criteria. Interventions: Participants were divided into three groups- A, B and C. Group A was given Escitalopram as drug monotherapy, and Group B was given Escitalopram with L-methyl folate 7.5mg and group C was given Escitalopram with Mirtazapine 7.5mg combination therapy. Follow up was done on 2nd and 4th to assess improvement in the symptoms. Results: On comparing HAM scores for R50 (50% reduction in initial HAM score) in each group. In group A, R50 was achieved in 20% patients in the 4th week. In group B, R50 was achieved at 12.5% in the 2nd week, 30% in the 4th week. In group C, R50 was achieved 37.5% in 4th week. The majority of the population in our study belongs to lower socio-economic status and were unemployed resulting in dietary deficiencies hence responded well with L-methyl folate which can cross blood brain barrier. Conclusions: On the basis of available literature and result of the index study, it can be concluded that combination therapies showed better response suggested by HAM scores at the end of study. Moreover, the response achieved with combination therapies were earlier as compared to monotherapy. This substantiate the findings of previous studies. Thus, the study suggest earlier response rates when patients received L-Methylfolate 7.5mg. When compared with monotherapy both the combination therapies achieved earlier and better response.

Low-grade stromal endometrial sarcoma (ESS) is a rare type of uterine cancer that arises from the stromal cells of the endometrium, which is the inner lining of the uterus. It is a subtype of endometrial stromal sarcoma, which is a group of tumors that are characterized by the presence of cells that resemble the normal stromal cells of the endometrium. The optimal management of low-grade ESS is not well established, but surgical resection followed by adjuvant hormonal therapy or chemotherapy may be considered depending on the patient's individual characteristics. we report the case of a 66 years old women, diagnosed with a low grade stromal endometrial sarcoma, treated in our tertiary hospital with surgery and endocrine therapy.

Introduction: Multifetal pregnancies (MFPs) are high risk pregnancies and are associated with higher complications and adverse outcomes. Aims and Objectives: To assess the incidence, risk factors, comorbidities, and outcomes of MFPs. Materials and Method: This prospective, observational study was performed over a period of 12 months (December 2018 to November 2019) and involved 212 women with MFP admitted in antenatal ward and labor room of a tertiary care center located in Central India. Maternal characteristics including age, parity, gestational age, mode of delivery, and maternal comorbidities and complications were recorded. Finally, neonatal morbidity and mortality was studied. Results: The incidence of MFP was 1.25%, with that of twin and triplet pregnancy being 1.22% and 0.03%, respectively. Majority of the women belonged to the age group of 25-30 years (43.3%), were multigravida (53.3%), and had a gestational age of 34-36 weeks (41.03%). Women most commonly underwent vaginal delivery (58.01%). The predominantly comorbidity and complications were hypertensive disorders (36.8) and preterm labor (58.49%), respectively. Majority of the neonates had low birth weight (LBW, 80.3%). There was no significant difference between the first and second twin in terms of LBW (P-value >0.05). Perinatal mortality was statistically associated with both prematurity and LBW (both P-values <0.0001). Conclusion: Overall, MFP was associated with high maternal and perinatal morbidity and mortality. Increased parity, low SES, rural area of residence, and advancing maternal age were the major determinants of MFPs. Perinatal mortality was significantly associated with LBW and prematurity.

Aims and Objectives: To assess the incidence, risk factors, comorbidities, and outcomes of MFPs. Materials and Method: This prospective, observational study was performed over a period of 12 months (December 2018 to November 2019) and involved 212 women with MFP admitted in antenatal ward and labor room of a tertiary care center located in Central India. Maternal characteristics including age, parity, gestational age, mode of delivery, and maternal comorbidities and complications were recorded. Finally, neonatal morbidity and mortality was studied. Results: The incidence of MFP was 1.25%, with that of twin and triplet pregnancy being 1.22% and 0.03%, respectively. Majority of the women belonged to the age group of 25-30 years (43.3%), were multigravida (53.3%), and had a gestational age of 34-36 weeks (41.03%). Women most commonly underwent vaginal delivery (58.01%). The predominantly comorbidity and complications were hypertensive disorders (36.8) and preterm labor (58.49%), respectively. Majority of the neonates had low birth weight (LBW, 80.3%). There was no significant difference between the first and second twin in terms of LBW (P-value >0.05). Perinatal mortality was statistically associated with both prematurity and LBW (both P-values <0.0001). Conclusion: Overall, MFP was associated with high maternal and perinatal morbidity and mortality. Increased parity, low SES, rural area of residence, and advancing maternal age were the major determinants of MFPs. As a heterogeneous category of malignant neoplasms with a variety of histological characteristics, chondrosarcomas are distinguished by the production of cartilaginous matrix. Oncological surgical excision is the preferred form of treatment due to the disease's inherent resistance to chemotherapy and radiation. Histological grading and tumor localisation localization are the most crucial prognostic indicators having a huge impact on treatment plan. The prognosis for proximal humerus chondrosarcoma has marginally improved, particularly for low and moderate grade tumors. This case reports a 32-year-old female who presented with shoulder pain for the past few days and was diagnosed with grade-2 chondrosarcoma. She was successfully treated with surgical excision of the growing tumor with reconstruction of the humerus.

Delayed tracheal perforation is a rare occurrence post thyroidectomy. We present a 72 year-old female patient that underwent elective total thyroidectomy and right upper parathyroidectomy, with bilateral central lymph node dissection and modified radical neck dissection. The post-operative period was complicated by high-risk pre-morbid status and multiple failed extubations, in addition to ischaemic tracheal areas visualised on the bronchoscopy on post-operative day (POD) 8, with subsequent computerised tomography (CT) scan showing anterior neck collections and perforation. A tracheal perforation involving the anterolateral wall (2nd and 3rd rings) was repaired surgically, followed by a tracheostomy. Suspicion of tracheal injuries should always be considered if patient fail to liberate from the mechanical ventilation post-operatively, warranting prompt conservative and/or surgical management. Potential devascularisation, secondary to diathermy-induced injury and pressure exerted by endotracheal tube cuff onto already weakened/friable trachea, should be considered as probable aetiological factors.
