Tuberculosis in children is still a challenge for world health problems, especially in establishing a diagnosis. Under diagnosis or over diagnosis can occur in some cases, so getting accurate data is still a challenging task. Clinical symptoms that are not typical in children add to the difficulty of diagnosis enforcement, especially in areas with a high incidence of TB infection. There has been much development of diagnostic TB methods in children, including genetic approach. Some studies report that miRNAs play an essential role in tuberculosis pathogenesis. MicroRNA as one of the diagnostic genetic methods can distinguish the incidence of latent and active infection in tuberculosis cases in children.

Owing to vascular malformations, it is difficult to perform catheter operation following the radial artery approach in percutaneous coronary intervention for acute ST-segment elevation myocardial infarction (STEMI) with Kommerell’s diverticulum on the right-sided aortic arch. However, only few studies have reported the use of the femoral artery approach to achieve early reperfusion; however, there is no established approach for patients with a right-sided aortic arch. In this study, we retrospectively analyzed the usefulness of the femoral artery approach in patients with STEMI and a right-sided aorta from January 2010 to March 2021.The total number of computed tomography (CT) cases was 180,514, of which 2 involved STEMI. In one of the two cases, the right radial artery approach was used. Therefore, only one patient with STEMI with a right-sided aortic arch underwent and operation using the femoral artery approach. In this patient, early revascularization was achieved with a door-to-balloon time of 70 min, suggesting the usefulness of the femoral artery approach.

It was established for the first time that not only the resonance of destruction is presented in nature, but also the resonance of creation. In modern medicine, resonance and destruction resonance are widely represented. But only the first steps are being taken in describing and using the concept of “resonance of creation” for diagnostics and therapy (8-13). There is no description and physical interpretation of this concept. This review is an attempt to discuss the phenomenon of "resonance of creation" and the results of its use in the treatment of various diseases.

Objective: This study aimed to explore the magnitude and related factors of the unmet need for postpartum family planning among Saudi women in Riyadh. Methods: The cross-sectional survey was conducted in 2018 among postpartum women of less than 12 months. A structured questionnaire was used to collect the required data from 400 participants in primary health care (PHC) settings in Riyadh. Logistic regression model was used to determine the factors of unmet need. Results: The proportion of unmet need for postpartum family planning among women was found to be 20.5%. Around half (55.8 %) of the participants reported that they had used family planning methods in the past. About 62% of the participants reported using modern contraceptive methods. Women over 30 years were 3 times more likely to be pregnant without using contraceptives (AOR:3.17; CI: 1.55-6.50; p: 0.002). Women who have 3 or more kids were 2.51 times more likely to experience unwanted pregnancies than women with fewer kids (OR: 2.55, CI: 1.17-5.56, p = 0.018). Conclusion: The unmet need for postpartum family planning is almost double the global average and neighbour countries. There is a need to educate women about the importance of contraceptives use in the kingdom.

The causes of chronic civilisation diseases are still unclear. This article examines and supports the hypothesis that chronic silent inflammation is likely to be the underlying cause. Reactions of the immune system that are excessive and can no longer be controlled by the organism are involved. Triggers can be pathogens, but then the processes continue as autoaggressions. A number of natural remedies are presented that are more than symptomatically effective.

Background: Tenotomy for residual equinus is commonly required in the management idiopathic club. The need to reduce complication and cost has made the advancement from mini open to needle tenotomy. This is even more pertinent in low- and middle-income country where clubfoot is more common. The study compared the outcome of needle and blade tenotomy for management of idiopathic club foot. Patients and Methods: it is a prospective randomized study of patient requiring tenotomy for residual equinus from July 2015 to June 2020 at a tertiary Orthopediccenter. Results:A tenotomy rate of 60 percent was noted. It involves thirty-one patients with forty-three feet were include in the study. The mean follow up was 27,1 months. There was no significant difference in the outcome of equinus correction. Three patients had relapse of deformity, two in the blade group and one in the needle group. All were resolved by further manipulation. Conclusion: Needle tenotomy offers similar outcome to blade tenotomy. It is less expensive and more acceptable to those scared of the surgical blade.

Objectives: We aim to present the retrospective results and statistical analysis of a year under COVID19 pandemic. Such analyses might be of great importance for management of individual cases of the disease on a day-to-day basis, as well as deciding upon certain restrictive measures as a whole; border-crossings movement, diagnostic testing and limiting travels in order to contain the pandemic of Covid 19. We hope our data contributes for further epidemiological investigations and future reference information. Design and methods: We combined collected information from our rtPCR test results, statistical data within our hospital and epidemiological data according to our Regional Health Inspection. The collected data is cumulative and statistical analysis incredibly helpful and necessary. Our Molecular Diagnostics Unit is licensed within the territory of Bulgaria. Our methods were strict protocols from the guidelines of National Center for Infectious and Parasitic Diseases-Bulgaria; the kits we use are all CE, the overall concept in sync with the global regulations as from the CDC and the WHO. Statistical analyses were done with ANOVA tool. Results: We give an overview of the collected data over the months of pandemic for the year 2020, as we observe several increases in the numbers of infected patients, but instead of having graphs with distinctive peaks we demonstrate a steady increase, giving a glimpse of the magnitude of the pandemic that followed after. The rtPCR test results are presented by pool data for both healthy travelling individuals and patients for confirming Covid diagnosis. Discussion: The current report represents retrospective data and suggests the possibilities, available to nations and applicable locally and of immense importance near border-crossing regions in a situation of pandemic with such magnitude and without the immediate access to vaccines and reliable treatment resources for novel infectious outbreaks.

Pregnant women are usually worried about pain during childbirth. It is caused by the force of contractions in the uterus and the pressure that occurs. Pain will increase when the cervix is fully dilated due to fetal pressure on the pelvic structures followed by stretching and tearing of the birth canal. The fear of pain during childbirth is a reason for women to choose to give birth by caesarean section. Every woman has a different pain threshold. Handling labor pain needs to be considered by health workers when helping deliveries. One of the interventions that can be done to reduce pain is pain management. Pain management is an action to reduce and relieve the pain that is felt. Pain management aims to help patients control pain so that the pain is lost or reduced. Pain management can be done with pharmacological and non-pharmacological techniques. Both of these techniques aim to reduce or eliminate labor pain by blocking the pain nerves. Related to this problem, a literature review is needed to dig deeper into the information in scientific research journals, so that we can get an overview from various points of view of research, especially regarding the names of pain management performed on maternal patients. This type of research is a literature review with a rapid review technique from journals on the Cochrane library and pubmed data base.

BACKGROUND :- Lung cancer is the leading cause of cancer-related mortality over word wide, Although the pathological diagnosis of lung carcinoma is limited as only small specimen available for diagnosis.the availability of targeted therapies has created a need for precise subtyping of non-small cell lung carcinoma . Several recent studies have demonstrated that the use of Immunohistochemical markers can be helpful in differentiating lung squamous cell carcinoma (LSCC) from lung adenocarcinoma (LAC) not on surgically resected material but also on small biopsy samples and cytology. AIM (1) To classify the non small cell lung carcinoma into major categories like squamous cell carcinoma (LSCC) and adenocarcinoma (LAC) and other categories by applying immunohistochemicalmarker like p40 (truncated p63) and Napsin A (2) To analyse the sensitivity and specificity of p40 and Napsin A in light of histomorphology and/or other relevant immunohistochemical markers available, using appropriate statistical tests. Material and methods:- This study was a one and half year (18 months) prospective study from Jan 2017 to June 2018, conducted in department of pathology on patients attending the outpatient and inpatient department of TB and respiratory disease, a total of 210 bronchoscopic guided biopsies / transthoracic (CT/MRI /guided) small tissue biopsies from the patients suspected of lung malignancy were incorporated in the study. 20 corresponding resection specimens (wedge resection and lobectomy) were also included in the study for correlation of morphology and immunohistochemical findings on small biopsies. RESULTS:-In our study IHC for both p40 and napsin –A aided in subtyping of 71.9% cases of non small cell lung carcinoma and this diagnostic accuracy was found to be statistically significant with p-value < 0.05.,on statistical analysis we found that napsin-A had a sensitivity of 90% and specificity of 80%. Also, positive predictive value and negative predictive value were seen to be 88.0% and 81.8% respectively.

Objective This study aims to observe whether Kryptonite Bone Cement combined with a standard Kirschner wire, instead of autogenous bone grafts, increases bone healing and mechanical strength in rats with tibia fractures. Methods The study included sixteen rats, which were divided into two groups as a control group (n=8) and an experimental group (n=8). After segmental fractures were made in both groups, intramedullary fixation of tibia procedures were conducted with the use of a Kirschner wire. No additional procedures were performed inthe control group, but Kryptonite Bone Cement was applied in the experimental group. The rats were evaluated clinically, radiologically and histologically4 times; immediately after the operation, and atthe 1st, 3rd, and 6thweeks following surgery. Results One rat from the experimental and one from the control group had both wound dehiscence and wound site infection. No recovery was observed in any rats either in the control or experimental group immediately after surgery. During week 1, callus formation was identified in 5 rats in the experimental group (p=0.0072), and during week 3, fracture lines disappeared in 4 rats in the experimental group (p=0.064); the differences between control and experimental groups were statistically significant. During week 6, no statistical significance was observed in radiological assessments for the control group and experimental group (p=0.71). The rate of non-union was higher in the control group (37.50%) than in the experimental one, while the rate of complete fusion was higher in the experimental group (87.50%) than that in the control group. Conclusion Osteoconductivity and ergonomic qualities of Kryptonite Bone Cement prove helpful in bone repair. Future studies to be conducted in a prospective and randomized manner will be effective on demonstrating the effectiveness of Kryptonite Bone Cement.

After segmental fractures were made in both groups, intramedullary fixation of tibia procedures were conducted with the use of a Kirschner wire. No additional procedures were performed inthe control group, but Kryptonite Bone Cement was applied in the experimental group.
