In April 2020, the spread of COVID-19 infectious disease in Japan issued a state of emergency, making it impossible for people to go out except for unnecessary and urgent matters. It has been reported that the declaration of self-restraint will significantly reduce the volume of physical activity (PA). However, since the volume of change in the volume of PA after 6 months or more after the declaration of self-control was lifted is unknown, the volume of PA was compared. The participants were 13 elderly people living in Hiroshima City. In October 2020, an activity meter with a 3-axis accelerometer was attached to the participant's waist, and PA and physical function were measured for one week and compared with the PA measured in October 2019. Six months after the declaration was issued, there was changes volume of moderate intensity walking PA, were no changes in the number of steps, activity time, volume of light intensity PA, and volume of total PA (p ≥ 0.07%). Six months have passed since the self-restraint of activities due to the state of emergency, and now that the number of infected people has decreased, it is possible that the volume of PA in the elderly has improved due to the improvement in the sense of security and happiness. This suggests that it is important to continue daily activities, hobbies, and exercises as much as possible while taking measures against infection in order to reduce the risk of cardiovascular events.

Introduction: Lacunar stroke accounts for nearly a quarter of all ischemic strokes and is an important cause of vascular cognitive impairment and dementia. Case Presentation: A 52-year-old man with lacunar infarct presented with weakness in his left thumb and forefinger since one day ago when he was sitting down. The patient’s fingers appear bent as if holding a pencil and could not be straightened. Physical examination showed an high blood pressure, and paresis of the right facial and hypoglossal nerve. A non-contrast CT scan of head using the parenchymal window showed a hypodense lesion on the left pons, interpreted as a lacunar infarct. Discussion: Lacunar strokes represent approximately 25% of ischemic strokes and most are associated with good outcomes. However, in 20% to 30% of patients with lacunar stroke, early neurologic damage occurs within the first days after stroke onset. Lacunar stroke was defined as acute lacunar syndrome with infarction <15 mm in diameter in the basal ganglia, deep white matter, thalamus or pons that is not associated with large vessel atherosclerosis or a cardioembolic source. Conclusion: Lacunar infarction is a potentially reversible. These conditions can potentially be well managed with adequate treatment.

This research aimed to study the effectiveness of the Public Health Volunteer Participation Program in the home care of stroke patients. Methods: This was quasi-experimental research; the samples consisted of 30 public health volunteers in each comparison and experimental group by simple sampling. The experimental group participated the Public Health Volunteer Engagement Program that consisted of 1) improving the environment at home and supporting the physiotherapy by increasing movement to improve the body balance, 2) Stress management and social support of stroke patients at home, 3) Taking care the nutritional status among stroke patients at home, 4) Following on a daily within 12 weeks by nurse and public health volunteer. Data were collected by using the knowledge interview form on stroke and care of stroke patients. Data were analyzed by using frequency, percentage, mean, standard deviation, pair-sample t-test, and independent-sample t-test. Results: The results found that the knowledge average score and the home care practice score of the health volunteers were significantly higher than the comparison group (p < 0.001). When comparing the results before and after program implementation, the cerebrovascular disease care in public health volunteers were significantly higher than before program implementation. (P < 0.001). Conclusion: The Public Health Volunteer Engagement Program in Home Care for Stroke Patients was effective. This program was suitable for promoting the public health team to care stroke patients at the community level.

The common knowledge of elderly posyandu cadres in Nagari Tapakis hurts their performance in the field. The initial survey results showed that the elderly posyandu cadres had just been formed; so far, the assigned cadres were toddler’s posyandu cadres. This makes the cadres have a double burden so that the activities of the elderly posyandu are less active even though they already exist. Since establishing the elderly posyandu cadres, no training has been held for cadres related to their primary duties and functions. The service method used is lecture, discussion, practice, and pre-test and post-test work. The training material focuses on techniques for managing the elderly posyandu and health services for the elderly that must be carried out during the Covid-19 pandemic era. Participants in the activity included 18 elderly posyandu cadres in Nagari Tapakis. On August 27, 2021, the action took place at the Tapakis Mayor's Office. The results obtained in training involving 18 elderly posyandu health cadres generally showed encouraging things. The participants responded positively and were very enthusiastic about participating in the training activities during the material presentation and discussion sessions. The pre-test and post-test values also showed a change in the form of an increase in respondents' knowledge; this was evidenced by the difference in the average understanding of respondents before counseling (13.2) and after counseling ( 16.8) with p = 0.000.

Background: Breast cancer is the most common cancer among women. Therefore, it is very important to determine prognostic parameters and biomarkers related to these parameters. Recent studies have shown that folate metabolism is effective on cell division and invasiveness. One of the most important regulators of folate metabolism is the methylene tetrahydrofolate dehydrogenase enzyme (MTHFD2). Method: This study, immunohistochemical staining with MTHFD2 was performed in 194 cases diagnosed with invasive breast carcinoma. The relationship between the immunohistochemical expression of MTHFD2 in normal ductal areas and invasive tumoral areas of these cases and some clinicopathological parameters was investigated. Results: While no expression was observed in non-tumor areas with MTHFD2, varying degrees of expression were observed in all tumor areas (p<0.0001). Significant correlations were also observed between important clinicopathological and prognostic parameters such as MTHFD2 nuclear grade, hormonal receptor, Ki67 and etc. (all p<0.05). Conclusion: High MTHFD2 expression in breast carcinomas with invasive and poor prognostic values was determined to be very significant. Therefore, it can be predicted that detecting and lowering MTHFD2 levels in these cases may prevent tumor development, invasion and metastasis. We think that more studies on the subject are needed, including the development of chemotherapeutics targeting MTHFD2.

When smoking Reactive Oxygen Species (ROS), which is a reactive oxidizing agent, can cause damage to the DNA structure of liver cells and increase the occurrence of liver necrosis. The research aims to determine the histopathological picture of the liver in male mice (Mus musculus) exposed to cigarette smoke. This research is an experimental laboratory with the sample used is male mice (Mus musculus). The sample with the most damage in treatment I obtained 5 (55.6%) samples with a score of 2 who had damage to hepatocytes. , 7 (77.8%) samples with a score of 3 had central venous dilation, 3 (33.3%) samples with a score of 1 had fatty liver, and 3 (33.3%) samples with a score of 1 had liver necrosis. While in treatment II, 5 (55.6%) samples with a score of 2 had hepatocyte cell damage, 8 (88.9%) samples with a score of 3 had central vein dilation, 5 (55.6%) samples with a score of 3 had fatty liver, and 5 (55.6%) samples with a score of 2 had liver necrosis. Based on these results, the histopathological changes in the liver of male mice (Mus musculus) were exposed to cigarette smoke.

Pregnant women can present with a wide variety of neurological conditions. Conditions including epilepsy, eclampsia, facial nerve palsy, pituitary tumor, cerebrovascular disorders, myasthenia gravis, multiple sclerosis, and nonpituitary intracranial tumors could be encountered. (Cheung, 1997). We describe a patient who presented with Acute Symptomatic Seizure Associated Brain Metastasis In Pregnant Woman. She came to the hospital because of a fit event and sudden left hemiparalysis and was diagnosed as a stroke, but further examinations revealed unexpected results. Brain imaging suggested brain metastasis was encountered, but histopathological examination did not support this data. More investigations are needed to determine the true etiology of her fit attacks. This patient’s diagnosis and management were even more challenging since she’s now at nine weeks gestation. Background: The use of multimedia-based electronics during this Covid pandemic is the right solution fordistance learning. Electronic media that are generally known by the public are cellphones and laptops/computers, and television which are communication media that use electromechanical power. Aims: To find the association between the use of electronic media with primary headaches in students of the Faculty of Medicine, Baiturrahmah University class 2017. Method: This research was an observational analytic study with a cross-sectional design. The sample of this study was the 2017 class of students who met the inclusion criteria and exclusion criteria—sampling using a non-random sampling method, namely consecutive selection. Data collection was done by distributing questionnaires. Results: In this study, the gender of the respondents, who were primarily women, was 50 people (72.5%). Respondents who experienced migraine headaches were 13 people (18.8%), and Tension-Type Headachewas 56 people (81.2%). This study showed no significant association between the use of cellphones and primary headaches in terms of frequency, duration, length of possession, position, and brightness (p>0.05). Likewise, for the use of laptops/computers, there was no significant association with primary headaches in terms of frequency, duration, length of stay, position, and brightness (p>0.05). Furthermore, there was no significant association with primary headaches for television use in terms of frequency, duration, length of stay, and position. Conclusion: There is no significant association between the use of electronic media with primary headache.

Electronic media that are generally known by the public are cellphones and laptops/computers, and television which are communication media that use electromechanical power.\u0000Aims: To find the association between the use of electronic media with primary headaches in students of the Faculty of Medicine, Baiturrahmah University class 2017.\u0000Method: This research was an observational analytic study with a cross-sectional design. The sample of this study was the 2017 class of students who met the inclusion criteria and exclusion criteria—sampling using a non-random sampling method, namely consecutive selection. Data collection was done by distributing questionnaires.\u0000Results: In this study, the gender of the respondents, who were primarily women, was 50 people (72.5%). Respondents who experienced migraine headaches were 13 people (18.8%), and Tension-Type Headachewas 56 people (81.2%). This study showed no significant association between the use of cellphones and primary headaches in terms of frequency, duration, length of possession, position, and brightness (p>0.05). Likewise, for the use of laptops/computers, there was no significant association with primary headaches in terms of frequency, duration, length of stay, position, and brightness (p>0.05). Rickettsia is a group of Gram-negative intra cellular obligatory bacteria. Rickettsia species are an important cause of infectious disease in people and animals. Fleas, ticks, mite, lice etc., serve as vectors who transfer the rickettsia from animal to humans.Rickettsiosis is one of the oldest known vector-borne diseases(Blanda et al., 2020). Usually it is a nonspecific self-limiting febrile illness without major complications. Fever, headache, mild-rash, myalgia's and anorexia are the most common presenting signs and symptoms. Laboratory findings include elevated transaminase levels, hyponatremia, thrombocytopenia and anemia (Merhej et al., 2014). The disease is usually of benign nature. Severe complications are reported in about 6% of cases. Patients may present with organ involvement, severe vasculitis and multi organ failure (Oteo and Portillo, 2012) and the mortality rate is up to 2.5%(Giammanco et al., 2005; Raoult et al., 1986).

Introduction: Hyperthyroidism has been known to cause a variety of cardiovascular manifestations. Isolated right heart failure (RHF) occurs occasionally, is usually due to pulmonary hypertension or tricuspid valve abnormalities. In rare cases, hyperthyroidism could be the underlying disease. Case Presentation: A 72-year-old woman with suspected but untreated hyperthyroidism presented with progressive dyspnea and lower extremity swelling in the last ten days. Physical examination showed an irregular and high heart rate, increase in JVP, enlargement of the thyroid gland, systolic murmur, and clear lungs. The laboratory findings showed an elevated level of free T4 (2.51 ng/dL) and a low level of TSH (<0.003 uIU/mL). Electrocardiogram revealed atrial fibrillation with a rapid ventricular response. Echocardiography showed right atrial and right ventricular dilatation with moderate tricuspid regurgitation. Left ventricular size and systolic function were normal. Chest x-ray showed a cardiothoracic ratio of 53% with organized left pleural effusion. Symptoms resolved as her thyroid hormone levels normalized with adequate treatment. Discussion: The most common changes that result from hyperthyroidism to the cardiovascular system are increased cardiac preload, decreased peripheral vascular resistance, direct injury, increased heart rate and contractility, which together produce a hyper dynamic circulatory state that leads to increased blood volume and venous return resulting in the increased risk of RHF. Conclusion: Hyperthyroidism is a potentially reversible cause of heart failure and should be ruled out in every heart failure patient, especially in those with isolated right heart failure, tricuspid regurgitation, and pulmonary hypertension. These conditions can potentially be well managed with adequate treatment.
