One of the central tenets of the Mental Capacity Act 2005 is ‘assume capacity’, which aims to enshrine the rights of adults to make decisions about their social and medical care. In this article, Jane Fisher explores how the principle of assumed capacity is often misappropriated, misunderstood and misapplied when attending to mental health service users with suicidal ideation.

Community rehabilitation for people with severe and enduring mental health difficulties: a description and evaluation of a new community enhanced rehabilitation team 严重和持久精神障碍患者的社区康复:对新的社区强化康复团队的描述和评估

The community enhanced rehabilitation team was implemented in 2021 as part of the NHS Long Term Plan. This evaluation assesses whether the team is meeting its aims and objectives.. Qualitative and quantitative methods were used to assess the team's progress. This included semi-structured interviews with service users, staff questionnaires and a review of data collected as part of routine clinical care.. Findings showed 117 interventions had been provided via the community enhanced rehabilitation team since May 2021. Service user anxiety had reduced following intervention, and service users were less likely to be re-hospitalised if they were under the community enhanced rehabilitation team compared to other teams. The community enhanced rehabilitation team is meeting its aims and objectives, suggesting this is an effective model for community rehabilitation teams. Community enhanced rehabilitation teams can be created using this model as an example.

How carers' needs can be met by staff within inpatient settings: a qualitative study 住院环境中的工作人员如何满足照护者的需求:一项定性研究

Inpatient admissions can increase carer stress rather than provide respite. The aim of this study was to gain carers' perspectives about how their needs can be met during an inpatient admission. Semi-structured interviews were conducted with 12 carers whose relatives had been admitted to inpatient care at least once. Interviews were analysed thematically. Carers described a fear of relinquishing responsibility for care, which was exacerbated by concerns about the quality of care provided by staff. Better experiences were possible when carers were given information, ranging from generic information about symptoms or how the mental health system works, information about patients' needs and carer support systems. Communication is key and needs to be a two-way process, with carers and staff learning from each other to support patients. Inpatient staff need training in carer involvement, including ways of engaging carers when patients do not give consent to information sharing.

The experiences, views and beliefs of emerging adults and their carers of receiving adult mental health services in the UK: a systematic review and meta-ethnography 英国新兴成年人及其照护者接受成人精神健康服务的经历、观点和信念:系统回顾和元人种学研究

Media coverage has highlighted the difficulties encountered by young people in the UK in accessing appropriate mental health care after turning 18 years old. Little appears to be known about the experiences of 18–25-year-olds and their carers of receiving adult mental health services. This systematic review aimed to synthesise the qualitative evidence on the experiences, views and beliefs of emerging adults and their carers of using mental health services in the UK, using meta-ethnographic methods to identify the current research, any gaps in research and directions of future research. A systematic review and meta-ethnographic synthesis of nine qualitative studies was conducted. Three overarching themes were found: locus of control, embarking into adulthood, and accessibility and engagement. Emerging adults view embarking into adulthood as an essential part of their recovery. A reappraisal of the balance between confidentiality and collaboration with family, for this age group, is suggested. Emerging adults perceive stigma from service providers, which creates a barrier to accessibility and engagement with services. A coaching approach to care could support social inclusion, facilitate choice and promote self-determination for this cohort Addressing embarking needs, the locus of control and the accessibility of services may improve emerging adults’ experience of using mental health services. There is an impetus for mental health nurses to consider a coaching approach to care planning.

Evaluation of the home care service: a remote inpatient service for people requiring mental health care 家庭护理服务的评估:为需要心理保健的人提供的远程住院服务

In response to the COVID-19 pandemic, St Patrick's Mental Health Services introduced a home care package, offering all the elements of the inpatient programmes, but provided to the service user remotely in their own home. A survey was used to evaluate service user experiences of receiving remote inpatient mental health treatment via the home care service. All participants who had a virtual admission were invited to complete a quantitative survey online. Data were inputted into the Statistical Package for Social Sciences software and a descriptive analysis was completed. A total of 88 participants completed the online survey, the majority of whom were women (62.5%) and between the ages of 61 and 70 years (28.4%). Diagnosis was most often depression (60.2%) or anxiety (29.4%) and almost 66% of participants had previous inpatient experience. Staff rated most highly by participants were the consultant psychiatrist (96.7%), ward-based nursing staff (95.2%) and their psychologist (92.3%). The highest rating statements, which participants rated as part of a questionnaire, were around technology options (87.1%), one-to-one work with a clinician (81.3%) and arranging collection of medicine (81%). Preliminary scores on this survey demonstrate that remote care is an effective way to improve access, enhance quality and provide efficient care. Further research should consider clinical outcomes with this model compared to standard care.

Examining the factors that influence medication administration errors and the reporting of these among mental health nurses in the hospital setting: a systematic review 检查影响药物管理错误的因素和报告这些在医院设置的精神卫生护士:一个系统的回顾

A medication administration error is defined as ‘any difference between what the patient received or was supposed to receive and what the prescriber intended in the original order’. Medication administration errors are associated with increased morbidity and mortality. They occur frequently in the mental health setting and pose a serious risk of patient harm. The factors leading to medication administration errors in the mental health care setting is under researched and the potential for mental health services to prevent harm from medication administration error is unknown. The aim of this study was to understand factors that influence medication administration errors, and the reporting of these, among mental health nurses in the mental health hospital setting. A systematic review was conducted following the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols (2015) checklist. A total of 3625 studies were identified, nine of which were included for final review. The CINAHL, Cochrane, EMBASE, Medline, Medline Ovid, Psych Info and PubMed databases were searched, as well as grey literature. Medication administration errors and their reporting are complex, multifaceted and show organisational and individual overlap. Heavy workload, staffing shortages, differing levels of experience, ward dynamics, lack of knowledge, interruptions, communication and patient interaction contributed to the occurrence of medication administration errors. Key factors included rationale on why not to report an error, lack of knowledge regarding how to report, what to report, as well as fear and stigma. Many factors lead to the occurrence of a medication administration error or why they are not reported. More education is needed to help reduce medication administration errors. Further research may understand the behavioural patterns of mental health nurses and at-risk behaviour. This may help identify adequate interventions to reduce the risk of medication administration errors in this setting.

Medication administration errors are associated with increased morbidity and mortality. They occur frequently in the mental health setting and pose a serious risk of patient harm. The factors leading to medication administration errors in the mental health care setting is under researched and the potential for mental health services to prevent harm from medication administration error is unknown. The aim of this study was to understand factors that influence medication administration errors, and the reporting of these, among mental health nurses in the mental health hospital setting. A systematic review was conducted following the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols (2015) checklist. A total of 3625 studies were identified, nine of which were included for final review. The CINAHL, Cochrane, EMBASE, Medline, Medline Ovid, Psych Info and PubMed databases were searched, as well as grey literature. Medication administration errors and their reporting are complex, multifaceted and show organisational and individual overlap. Heavy workload, staffing shortages, differing levels of experience, ward dynamics, lack of knowledge, interruptions, communication and patient interaction contributed to the occurrence of medication administration errors. Key factors included rationale on why not to report an error, lack of knowledge regarding how to report, what to report, as well as fear and stigma. Many factors lead to the occurrence of a medication administration error or why they are not reported. More education is needed to help reduce medication administration errors. Further research may understand the behavioural patterns of mental health nurses and at-risk behaviour. Preliminary evaluation of a multidisciplinary pathway for people who self-harm in acute inpatient mental health care 多学科途径对急性住院患者自我伤害的初步评价

Inpatient mental health settings are required to provide biopsychosocial interventions to support service users who self-harm. This is important as self-harm has been associated with increased admissions and risk of suicide. A multidisciplinary self-harm pathway was developed in an acute inpatient mental health ward for adult women to help staff support patients who self-harm. The aim of this project was to evaluate the pathway in relation to service user distress, confidence in managing mental health, therapeutic alliances with the ward team and self-harm incidents on the ward. Data were collected from the record-keeping systems and from questionnaires completed before and after the pathway was introduced. The pathway was well received by service users and contributed to improvements in distress, confidence and to more effective management of self-harm. The self-harm pathway was found to be a useful framework within which to structure psychosocial interventions for self-harm and least restrictive practice.
