Introduction. "Health and well-being of people" is the first national development goal of the Russian Federation until 2030. The Sustainable Development Goals (SDGs), also known as the Global Goals adopted by the United Nations (UN) in 2015, are comprehensive and indivisible and are being implemented everywhere [1]. At the 2016 Shanghai Conference on Healthy Cities, people's health and well-being were placed at the center of the UN's 2030 Agenda for Sustainable Development (“Agenda 2030”). A preliminary assessment of the current state and current trends using the example of Moscow can be a “starting point” for making decisions in the field of public health management in large cities. Goal. To assess the progress of the city of Moscow towards achieving the third sustainable development goal (SDG 3). Materials and methods. For the analysis, data from open sources of the Federal State Statistics Service of Russia, the Rospotrebnadzor department for the city of Moscow were used. Results. Despite the pandemic, Moscow has maintained positive trends on a number of SDG targets and is advancing ahead (the proportion of births attended by skilled health workers; reducing infant and child mortality and tuberculosis incidence). There is also a decrease in mortality from traffic accidents, diseases of the circulatory system and neoplasms, primary incidence of malaria and hepatitis B. The assessment of public health indicators at the Moscow level is a new step in regional and territorial management through the use of the SDG 3 operational tool.

Introduction. The high prevalence of dental diseases and their complications, congenital anomalies, traumatic injuries and various neoplasms of the maxillofacial region among children and adults necessitates the improvement of the medical care system. Purpose. Improvement of specialized medical care by optimizing the routing system for patients with maxillofacial pathology in a metropolis on the example of Moscow. Materials and methods. The study was conducted on the basis of the GBUZ “Maxillofacial Hospital for War Veterans of the DZM”. A search was made for regulatory documents regulating the routing of patients with maxillofacial pathology. The statistical data of the specialized medical care provided to patients with maxillofacial pathology in institutions subordinate to the Moscow City Health Department for the period from January 2021 to December 2021 were analyzed. Results and discussion. The article presents a list of regulatory documents governing the routing of patients. The structure of diseases of the maxillofacial region among the children and adults of the city of Moscow was determined. Routing schemes for patients with planned and emergency maxillofacial pathology are given. Problems are identified and solutions are proposed, as well as the continuity of the provision of specialized care in organizations in the field of “maxillofacial surgery”. Conclusions. A well-functioning routing system will improve the timeliness, availability and quality of specialized medical care. In addition, an optimized routing system will contribute to proper planning and resource allocation in healthcare.

In addition, the study analyzed in detail the activities of dental clinics in the context of the administrative districts of Moscow.

Introduction. The state and analysis of the revenue part of the budget of organizations of the state healthcare system in Moscow, providing primary medical care to the adult population, is of particular interest in the context of a shortage of financial resources. First of all, management specialists are interested in the formation of a source of their own income in the form of income from the provision of paid medical services, since it is one of the most effective tools for ensuring the financial sustainability of medical organizations. Purpose. To analyze the indicators of the volume of own incomes of polyclinics subordinate to the Moscow Department of Health (hereinafter referred to as DZM) in dynamics to assess the possibility of implementing the provision of paid medical services by state budget organizations providing primary medical care to the adult population. Materials and Methods. The study was based on the data of the annual budget reporting of medical organizations for 2019 – 2021 from the accounting system DZM "Parus Budget 8". Results. The volume of own income in 2021 compared to 2020 increased by 67,3 %, and in 2020 there was a sharp decrease in this indicator compared to 2019 – by 32,4 %. In the context of administrative districts in 2021, there is a significant variation in the values of the indicator "growth rate of own income" in relation to 2020 – from 136,6 % in the Central Administrative District to 255,1 % in the Eastern Administrative District. In 2020, compared to 2019, the difference between the maximum and minimum values was insignificant – 13,6 %. Based on the totality of indicators, it can be concluded that, despite the equal rights enshrined in Russian law, medical organizations of the Moscow healthcare system, due to territorial differences, have different opportunities to replenish the revenue side of the budget through the provision of paid medical services. Conclusion. The dynamics of general incomes and the dynamics of own incomes have a multidirectional trend. Among the administrative districts, significant differentiation was noted, both in terms of the “share of own income in the total income of medical organizations”, and in terms of the “growth rate of own income” and “growth rate of income from the provision of paid medical services”, which requires a more in-depth analysis.

Introduction. Chronic non-communicable diseases (CNCDs) remain the most important medical and social problem and cause significant damage to the economy of the capital. In this situation prevention, early detection and timely treatment of diseases among the working-age population are of particular importance. In an urban environment, a person is constantly exposed to the following risk factors that negatively affect health: reduced physical activity and a sedentary lifestyle, high levels of stress, poor nutrition. The latter is also one of the reasons for the increase in the incidence of obesity and diabetes mellitus type 2 (hereinafter – DM2) and the prevalence of metabolic syndrome (hereinafter – MS). The purpose of the study was to assess the level and dynamics of the incidence of the working-age population of the city of Moscow by the class of endocrine diseases, DM2 and obesity in the administrative districts. Materials and methods. For the analysis data from medical organizations of the Moscow City Health Department on registered morbidity for 2017–2021 were taken. The method of direct ranking was used, the calculation of indicators of the dynamic series. Results. For the study period 2017–2021 in the city of Moscow, the overall morbidity rate of the workingage population in the endocrine diseases class increased by 36.4 %, DM2 – by 17 %, obesity – by 38,3 %. The primary morbidity rate of the working-age population in the endocrine diseases class decreased by 5,1%, DM 2 – by 24,7 %, obesity – by 45,9 %. The rates of general morbidity in the class of endocrine diseases and DM 2 decreased most rapidly in the Zelenograd administrative district (ZelAO) - 13.3% and 18%, respectively, and obesity – in the South-Western administrative district (SWAO) by 23.3%. The decrease in primary morbidity in the endocrine diseases class was most rapidly recorded in the North-Western Administrative District (NWAO) – by 36 %, DM 2 in the Eastern Administrative District (EAO) – by 50.4 %, obesity in the Troitsky and Novomoskovsk administrative districts (TiNAO) – by 83 %.

Introduction. Prostate cancer (PC), according to the results for 2019, is the second most frequently diagnosed malignant tumor in men living in the Russian Federation. There is a significant proportion of patients with advanced (III-IV stages) of prostate cancer - up to 50 %, while metastatic cancer accounts for up to 18.1 % of patients. Popularization in the Moscow City Healthcare clinical practice of innovative technologies for managing patients with metastatic castration-resistant prostate cancer (mCRPC) directly meets the goals and objectives of the Federal project aimed at increasing the life expectancy of cancer patients. Materials and methods. The article reviews the results of clinical developments in the field of mCRPC treatment presented at The ASCO Annual Meeting, June 3-7, 2022: VISION sub-studies, TheraP study, study of the prognostic role of circulating tumor DNA (ctDNA) in the early choice of a strategy for the use of abiraterone or enzalutamide in patients with initial mCRPC - treatment naïve - status. Results and discussion. The volume of clinical information on the results of theranostic antitumor radiation technologies implemented with the participation of prostate-specific membrane antigen (hereinafter referred to as PSMA) is growing rapidly. PSMA - mediated radio-imaging is considered as a potentially effective tool for selecting patients who are sensitive to specific therapy. When studying radionuclide therapy mediated by a ligand to PSMA (PSMA-L), the main efforts are focused on determining the optimal regimens for its clinical application. The possibility of expanding the current indications for prescribing 177Lu-PSMA-617 to patients with prostate cancer at an earlier stage is also being studied. Until final results are obtained from clinical trials that provide clear answers to open questions regarding various aspects of the application of innovative PSMA - mediated theranostic technologies, it remains very important to reach an expert consensus, within the framework of an exchange of opinions, re arding their appropriate use in various clinical situations.

The development and establishment of postgraduate training in the Russian Federation has a long history. A retrospective comparison of all periods of postgraduate training showed that, as a training program, postgraduate studies have always been such, but with regard to the tool for preparing a qualified scientist, the question still hovers among the professional community - what is the educational component and what is the scientific component in the specified educational program. New regulations that came into force in 2022 that change the system for training scientific (scientific and pedagogical) personnel are the result of a search for a solution to create an optimal system for training postgraduate students in the Russian Federation.

The development of an evaluative approach to determining the effectiveness of medical organizations, the introduction of various indicators for assessing the provision of medical care is one of the mechanisms for improving its quality and accessibility for the population. Correction of regulatory legal documents regulating the activities of the sphere of compulsory medical insurance, taking into account changes, including those taking place in the healthcare of the Russian Federation as a whole, is a necessary requirement for updating approaches to the selection of adequate assessment criteria.

Introduction. The article is devoted to the consideration of the directions of continuing education in healthcare. Of course, the development of a system of continuous medical education (further – CME) and its various forms, such as distance learning, is a necessary and timely measure for the development of healthcare in the Russian Federation. Purpose. to consider the current aspects of the development of distance learning in the system of continuing professional education of doctors. The article discusses statistical data in the field of distance learning in the system of continuing education of medical workers. There are a number of problems in this direction and the ways to solve them depend not only on the functioning of the system itself, but also on the recognition of its importance by practitioners. Materials and methods. Statistical indicators from the websites of Rosstat, the Ministry of Health of the Russian Federation and other medical structures, including medical educational institutions, were used as materials in the study. Results and discussion. The NMO increases the speed with which information reaches doctors. In electronic form, in the format of webinars, seminars and trainings, new knowledge gets into medical offices faster than with the help of approved training programs, which often do not meet the needs of practical healthcare.

Introduction. Children's health is an important integral indicator and criterion for the socio- conomic wellbeing of society and the state, and is also included in the system of global indicators for achieving the goals in the field of sustainable development of countries. In terms of statistical research and forecasting in health care, children's health indicators can be assessed as the most stable and reliably reflecting the actual situation. The article contains information that allows one to form an idea of the health status of children of various age groups in Moscow in conditions of social isolation and distance learning during the COVID-19 period. Purpose of the study. To identify the main trends in the level of general morbidity among children aged 0-17 in Moscow during the COVID-19 pandemic, both in general and by age groups. Assess how the lockdown associated with the coronavirus outbreak has affected the health of children. Materials and methods. The review used population data, as well as data from federal statistical observation form No. 12 “Information on the number of diseases registered in patients living in the service area of a medical organization” for 2016–2020. Morbidity was analyzed for age groups 0-4 years, 5-9 years, 10-14 years, 15-17 years per 100,000 of the corresponding population. Results and discussion. In 2020, the overall level of child morbidity in the capital amounted to 189.1 thousand cases per 100,000 children, a decrease of 15.6% compared to 2019, in general, for the period from 2016 to 2020 – by 18.6 %. An analysis of the incidence of children showed that in 2020, the conditions of temporary isolation and a decrease in contacts during the COVID-19 pandemic led to a decrease in the registration of a number of diseases: SARS, acute bronchitis, acute conjunctivitis and diseases of the digestive system, as well as a decrease in childhood injuries in all age groups. A characteristic feature for children of all ages was the decrease in the overall incidence in 2020, however, it is worth noting the different rates of decline in indicators for individual classes of diseases depending on age.
