A case of polytrauma caused by car accident complicated by the development of tracheoesophageal fistula (TPS) with prolonged ventilation support is presented. The article describes clinical manifestations, diagnostic methods, and palliative therapy of this pathology. Despite complex treatment, the presence of TEF in the patient led to worsening of the condition, which in complex with number of other injuries and various complications led to death.

Drug-induced liver damage (DILD) is a common condition that can lead in some cases to acute liver failure and unfavorable patient outcomes. DILD development mechanisms are of two main types. The first is a direct dose-dependent type and the second is dose-independent idiosyncratic one. Cytochrome P450 system is of great importance in the genesis of direct DILD, since during drugs metabolism highly toxic metabolites can be formed in this liver enzymatic system. As a result, hepatocytes die destructing mitochondria and thus blocking the production of cell energy substrates, triggering oxidative stress and lipid peroxidation. It also leads to direct DNA damage and activation of signaling pathways for apoptosis. In most cases idiosyncratic DILD is associated with individual genetically determined predisposition with a wide range of clinical manifestations. In case of this type of DILD, the immune cells are sensitized to hepatocytes activating humoral and cellular immune response and rather often forming antibodies to some structural element of hepatocytes. The structure of antigens of the main histocompatibility complex as well as receptor-mediated interactions with Fas-ligand participation, interferon gamma and tumor necrosis factor play a certain role in the development of idiosyncratic DILD. By present, hypotheses of immune system activation in idiosyncratic DILD have been proposed, including haptenization hypothesis; the hypothesis of direct interaction of drugs with the molecules of major histocompatibility complex and subsequent activation of the immune system; the hypothesis of changes in structural sequences of endogenous biologically active peptides with impaired interaction in the backbones of major histocompatibility complex and occurrence of autoimmune reactions. Finally, there is the hypothesis of multiple determinants, which postulates that there are various risk factors (for example, genetic polymorphisms, gender, age, etc.) that contribute to DILD onset in case of comorbidity. DILD mechanisms can also be considered according to clinical and morphological type of liver damage. One can distinguish cholestatic DILD type, vascular type, steatosis, and the type when liver tumor develops. The cholestatic DILD type is clinically expressed by cholestasia or inability of bile to enter the lumen of small intestine as a result of abnormal secretion of bile acids by hepatocytes or obstruction of biliary tract. Hepatic steatosis is histologically determined as deposition of triglycerides within hepatocytes. Drug-induced steatosis is reversible unless steatohepatitis or cirrhosis has developed. Vascular DILD type is associated with damage of stellate and endothelial liver cells, lining sinusoidal capillaries. Ultrasound lungs examination has been considered uninformative for decades due to the high acoustic resistance between soft tissues and airy lungs. Recently, the work in emergency conditions has proved the usefulness of ultrasound technologies, especially while diagnosing acute respiratory failures. Introduction of ultrasound technologies into the practice of intensive care units made it possible to change the ‘philosophy’ of diagnosing critical conditions, promoting ultrasound as primary technique, even in the era of widespread use of computed tomography. In reconnecting marketing to more plastic and malleable markets, we need more understanding about market evolution. In this research we explore how to assess the state of a market, and how the roles of a market-shaping actor vary depending on this state. We view markets as configurations of 25 interdependent elements and argue that well-functioning markets have a high degree of configurational fit between elements. The level of configurational fit describes the state of a market as a continuum from low to high marketness. The clout of a market actor to influence a market configuration is an amalgamation of the actor’s capabilities, network position and relative power. By exploring marketness and clout as contextual contingencies, we identify four market-shaping roles: market maker, market activist, market champion, and market complementor. The focus of a market-shaping actor, in terms of which elements to influence and in which order, vary significantly between roles.

Nowadays, there is an increasing necessity to use a lot of medical products (MP) in one patient. This can lead to potential nephrotoxic adverse reactions (ADRs) among drugs. One of drug-induced kidney damage manifestations is acute tubular necrosis (ATN), which causes the development of severe complications associated with the use of hemodialysis and a high risk of mortality. The purpose of this review is to analyze literature data concerning groups of drugs and individual drugs, the intake of which is associated with the development of drug-induced ATN, risk factors, prevention and treatment methods. Among drug-induced ATN development risk factors are both general, associated with the development of drug-induced acute kidney damage (for example, age, history of chronic kidney disease, etc.), and specific ones, typical for certain groups of drugs. Most often, drug-induced ATN develops while taking antibacterial, antiviral and anticancer drugs. The main method of treatment for drug-induced ATN is abolition of nephrotoxic drugs, but if it is impossible, then there are certain patient management schemes. To prevent and to diagnose drug-induced ATN at early stages, that is the key for favorable prognosis, it is necessary to aware of doctors of different specialties about such pharmacotherapy complications.

Over the last 40 years, the impact of stereotypes in a service context has been investigated repeatedly, as stereotypes can have a strong influence on interactions during the service encounter. The many academic studies analysed various stereotypes, took a customer or employee perspective, investigated attitudinal or behavioural outcomes before and after an interaction and found both positive and negative effects of stereotypes. Thus, a synthesis of research is needed that integrates existing knowledge to clarify what researchers have learnt about stereotypes in services. The main contribution of our research is to aggregate and categorise the highly specialized findings that exist on specific stereotypes and thus make the current knowledge more generalisable. The results of our study reveal that a strong focus on customer stereotypes regarding employees exists, but other stereotype constellations are less often investigated. Similarly, the investigation of more subtle stereotype triggers and the consideration of contextual factors should receive more attention. Finally, even as we identified meaningful managerial implications to address the consequences of stereotypes, academic papers need to include a practitioner’s perspective more consequently.
