Purpose. Demonstration of a clinical case of macular edema treatment associated with non-infectious uveitis by intravitreal injection of a dexamethasone implant (Ozurdex). Material and methods. A clinical case of uveitis associated with rheumatoid arthritis in a 34-year-old female patient is presented. She underwent sequential two intravitreal injections (IVI) of long-acting dexamethasone (Ozurdex implants), with an interval of 3 months. Results. 3 days after Ozurdex IVI Vis OD = 1.0, Vis OS = 0.8 (initially 0.2), intraocular pressure (IOP) OD = 18 mmHg, IOP OS = 19 mmHg. A single cell suspension remained in the vitreous body. The retinal thickness in the macula decreased to 314.5 µm (initial 725.2 µm), and macula volume decreased to 14.1 mm³(initially 18.5 mm³ ). 3 months after IVI of Ozurdex in the vitreous: opacification +2 points, the implant was not detected due to its rapid biodegradation. OCT of the macular area again revealed edema with cystic cavities. Ozurdex was reinjected intravitreally. 3 days after the operation, all signs of the inflammatory process were stopped, Vis OD = 1.0, Vis OS = 1.0, IOP OD = 18 mmHg, IOP OS = 18 mmHg. Conclusion. The Ozurdex implant made it possible to achieve complete regression of macular edema and resorption of cell suspension in the vitreous body. The article presents a description of two clinical cases of late diagnosis of penetrating wounds of the orbit with the introduction of wooden foreign bodies in children. Late diagnosis was associated with the inattention to the details of the anamnesis, untimely surveys or misinterpretation of their results. Longtime stay of foreign bodies in the orbit is dangerous as it may contribute to the development of severe complications both in the early and late periods after injury. The slightest suspicion of penetrating wound of the orbit is an indication for additional imaging techniques (CT, MRI) followed by a thorough revision of the wound channel. 94 patients were treated with recurrent corneal erosion of herpes genesis on the background of antiviral, reparative therapy. Physical therapies were added to the baseline therapy to stimulate reparative processes. The following methods of treatment were used: exposure to local reduced pressure, magnetic therapy, vitamin-protein baths. In a comparative analysis with a control group, results were obtained that allow reducing the treatment time, restoring corneal transparency «to achieve a high optical effect. Purpose. Optimization of treatment of chronic conjunctivitis in the Southern Aral Sea region. Material and Methods. The research included 62 patients (120 eyes) with chronic conjunctivitis, aged 35–62 years, who had been observed in a polyclinic for the last 3 years, with signs of DES. All studied patients were divided into 2 groups by random sampling after preliminary adequate antibacterial and anti-inflammatory therapy; The control group (20 patients, 40 eyes) was instilled with an antiseptic - Tsinosol (composition: zinc sulphate, boric acid, Aseptic), 2 drops 3 times a day, for 30 days. In the main group (40 patients, 80 eyes), patients were instilled with an antiseptic - Tsinosol (2 drops 3 times a day) and an artificial tear (AT) medicine - Keratrop (composition: sodium carmellose, glycerin, levocarnitine, erythrolfirma Aseptica), 2 drops 3 times a day for a month. All patients underwent standard ophthalmological tests, and special studies were carried out to verify DES, including tests by Schirmer, Norn, Jones, the height of the lower lacrimal meniscus and the severity of the conjunctival fold were determined. Results. Studies have shown that in the main group, both subjective and objective signs of the disease significantly decreased compared with the baseline and with the data of the control group, and a significant improvement in the stability of the tear film was noted. Conclusion. To optimize the therapy of secondary DES, it is necessary to include in the treatment regimen AT medicines in general and the domestic medicine Keratrop in particular, as the medicine of choice in this region. One of the manifestations of scleritis is perforating scleromalacia (necrotizing anterior scleritis without inflammation), characterized by an almost complete absence of clinical manifestations and usually associated with systemic diseases or postoperative complications. Scleromalacia is a serious, progressive disease that is difficult to treat and can lead to vision loss, perforation and loss of the eye as an organ. Treatment of such patients requires an integrated approach together with rheumatologists and is difficult, despite the many already proposed methods of surgical treatment using various grafts and implants. External dacryocystorhinostomy is a highly effective surgery using for treatment the obstruction of the lacrimal ducts of various etiologies and is still relevant from the last century to the present. However, this method is quite labor-intensive for the surgeon, traumatic for the patient, and the result is not always cosmetically satisfactory, since one of the significant disadvantages of external dacryocystorhinostomy is the formation of a scar at the point of the skin incision. In modern conditions, the presence of a postoperative scar on the face brings some doubts among young patients, especially women, which inclines this group of patients towards choosing the endonasal approach, which in turn does not always lead to a positive result in restoring the patency of the lacrimal ducts. Various techniques for applying dacryocystorinoanastomosis and suturing the surgical wound present an extra complexity, and the formation of a postoperative skin scar that is cosmetically unsatisfactory for the patient is one of its main drawbacks in the modern terms of idea of an aesthetic result. The use of microsurgical technology, modern suture material is aimed to prevent the development of cicatricial deformation of the postoperative scar. The purpose of this work was to evaluate the cosmetic results of external dacryocystorhinostomy using the microsurgical technique of layer-by-layer tissue suturing. In order to investigate frequency of occurrence and features of the clinical course of dry eye syndrome (DES) in children of various ages with certain types of chronic dermatoses, 80 patients aged 4 to 17 years were examined. The criteria for their inclusion in the study were the presence of a verified diagnosis of atopic dermatitis (in the acute stage) (60 children) or ichthyosis (vulgar and congenital) (20 children). All were examined for the severity of subjective discomfort, the condition of the ocular surface, the production and stability of the pre-corneal tear film. The results obtained indicate the presence of clinical and functional signs of DES (mainly due to dysfunction of the meibomian glands) in 41 children (68.3 %) with atopic dermatitis and 15 (75.0 %) with ichthyosis. At the same time, in children with the studied types of chronic dermatoses, DES occurred mainly in mild and moderate forms. It is advisable to conduct an examination of patients with other forms of chronic dermatoses in order to identify and treat their meibomian gland dysfunction and the associated dry eye syndrome. Zaslavsky","doi":"10.25276/2312-4911-2022-6-33-39","DOIUrl":"https://doi.org/10.25276/2312-4911-2022-6-33-39","url":null,"abstract":"In order to investigate frequency of occurrence and features of the clinical course of dry eye syndrome (DES) in children of various ages with certain types of chronic dermatoses, 80 patients aged 4 to 17 years were examined. The criteria for their inclusion in the study were the presence of a verified diagnosis of atopic dermatitis (in the acute stage) (60 children) or ichthyosis (vulgar and congenital) (20 children). All were examined for the severity of subjective discomfort, the condition of the ocular surface, the production and stability of the pre-corneal tear film. The results obtained indicate the presence of clinical and functional signs of DES (mainly due to dysfunction of the meibomian glands) in 41 children (68.3 %) with atopic dermatitis and 15 (75.0 %) with ichthyosis. At the same time, in children with the studied types of chronic dermatoses, DES occurred mainly in mild and moderate forms. It is advisable to conduct an examination of patients with other forms of chronic dermatoses in order to identify and treat their meibomian gland dysfunction and the associated dry eye syndrome. Increasing the number of patients at an ophthalmic appointment with chronic eye diseases requires a more detailed analysis of diagnosis and treatment. When analyzing the incidence of patients with chronic blepharitis, the vast majority with demodecous etiology of ocular lesions were revealed. The treatment of eye demodecosis has been sufficiently studied. However, even if patients comply with the recommendations of doctors, if there is a positive trend, there is a high risk of recurrence of this disease. Under our supervision were 326 patients diagnosed with chronic blepharitis of both eyes. All patients were divided into two groups. In the first group, the treatment was carried out according to clinical recommendations, in the second, the immunomodulatory drug Proteflazide was added to the treatment, as well as the wide-spectrum antihelminth agent Albendazole. Background. Bullous keratopathy is a chronic steadily progressive disease that leads to a significant decrease in vision and is accompanied by the appearance of a pronounced pain syndrome. Currently, the role of cytokines in the pathogenesis of the disease remains insufficiently studied. The aim of the study was to study the level of proinflammatory cytokines of the lacrimal fluid in bullous keratopathy. Results. There was a statistically significant increase of the level of proinflammatory cytokines in the lacrimal fluid of patients with bullous keratopathy: interleukin (IL) 1β by 3.6 times (p< 0.05) compared with the group of conditionally healthy volunteers (comparison group), IL-6 by 3.8 times (p< 0.05), the level of tumor necrosis factor (TNF) α– 2.0 times (p< 0.05). Conclusion. The bullous keratopathy is accompanied by a statistically significant increase in the concentration of proinflammatory cytokines in the lacrimal fluid of the diseased eye. The revealed feature of the local immune response in this pathology opens up prospects for new conceptual constructions of the pathogenesis of the bullous keratopathy with the subsequent development of effective methods of treating the disease.
