Caffeine Consumption & Perception of Its Effects Amongst University Students 大学生咖啡因摄入及其影响的认知

Introduction: A variety of research studies have shown that caffeine usage is highly prevalent among university students and many of them have different perception of its effect on their mental and physical health. These perceptions can influence the intake of caffeine which may lead to its addiction, resulting in serious public health consequences. Aims & Objectives: The objective of the study is to determine the frequency of caffeine consumption and the perception of its effects among university students. Place and duration of study: This study was conducted within 6 months duration, from May 2018 to October 2019 among students of three private universities in Lahore, Pakistan. Material & Methods: This was a descriptive cross-sectional study. A sample size of 670 university students was taken and the data were collected from three private universities of Lahore through convenience sampling technique. The perception of effects of caffeine was determined through a pre-validated questionnaire, Caffeine Expectancy Questionnaire (CaffEQ) from consumers and nonconsumers of caffeine. Results: A total of 670 university students participated in the study, out of which, 278 (41.5%) were males and 392 (58.5%) were females. 506 (75.5%) participants were consumer of caffeine, while 164 (24.5%) were non consumer. The occurrence of caffeine consumption was more in males (79.9%) than in females (72.4%). Participants reported that they consume caffeine because it makes them feel more alert (83.4%), they experience caffeine withdrawal without caffeine (83%) and caffeine makes them feel more energetic (80.6%). The perceptions due to which participants don't consume caffeine were that they expect caffeine makes their heartbeat irregular (81.7%), they don't like the way caffeine makes them feel (80.5%) and caffeine makes them irritable (76.8%). Conclusion: The study concludes that male students had more tendencies towards caffeine consumption than females. Perceptions for consuming caffeine were feeling of alertness, experiencing caffeine withdrawal symptoms, feeling of energy and ability to work over long periods of time after having caffeine. The perceptions for not consuming caffeine were irregular heartbeat, bad feelings, irritability, and sleep disturbances.

Medical Student’s Perception of Educational Environment 医学生对教育环境的感知

Introduction: The importance of educational environment in any medical institute cannot be ignored. Medical students should be provided a healthy environment so as to maximize their learning. Also understanding their perception about the educational environment can be used tactfully to evaluate and address the deficiencies and to make necessary changes required accordingly.Aims & Objectives: To assess students’ perception regarding educational environment in a medical college of Lahore.Place and duration of study: This cross-sectional study was conducted from September 2019- December 2019 in CMH Lahore Medical College.Material & Methods: 450 students of M.B.B.S from first year to final year (90 students / year) were included by simple random method after taking written informed consent. DREEM questionnaire was used having 5 subscales including students’ perception of learning (PoL), perception of Teachers (PoT), students’ academic self-perception (ASP), students’ perception of atmosphere (PoA) and students ‘self-perceptions (SSP). Statistical analysis was performed using SPSS version 22 .Data was presented as mean ±s.d, normality assessed with Shapiro -Wilk’s test followed by Spearman’s correlation analysis.Results: Reliability of the scale was found to be 96%, indicative of excellent consistency of the responses provided by students. Mean scores of subscales from both male and female students; PoL, PoT, ASP, PoA and SSP were found as 29.89±7.36, 28.07±7.21, 19.50±5.17, 28.78±7.36 & 16.97±4.55 respectively. Total DREEM score came out to be 123.21+29.48.Conclusion: It is concluded that students perceived a positive learning environment in this medical college’s experience.

Self Medication and Associated Health Care Seeking Amongst Mothers of Children Aged Under 5 with Diarrhea and Respiratory Tract Infections in an Urban Slum 城市贫民窟中患有腹泻和呼吸道感染的5岁以下儿童的母亲自我用药和相关的保健寻求

Introduction: Self-medication practice in childhood illnesses is quite common in developing countries. Self-medication practices are found to be influencing healthcare seeking behavior in developing countries.Aims & Objectives: To assess self-medication practices and its association with health care seeking in mothers of children aged under 5 with diarrheal and respiratory illness episodes residing in an urban slum.Place and duration of study: A cross-sectional analytical study conducted in Samsani-khui, an urban slum in district Lahore, from September 2016- February 2017.Material & Methods: 422 mother-child units (only one child aged under 5 years) were recruited through systematic random sampling. Responses were recorded on a structured, self-constructed questionnaire about self-medication practices of mothers for the selected child during last 6 months in acute episodes of diarrhea or respiratory illness. Data was entered and analyzed on SPSS version 21. Fisher’s exact test was applied.Results: Mean age of mothers was 26.81 ± 4.744 years, 16% were illiterate, 44.3% had attained primary education and 92.65% were housewives. 70.62% children included in study were boys. 61.8% mothers never practiced self-medication in their child whereas 38.2% administered medicines occasionally, frequently or every time the child got ill. 92.5% of the mothers practicing self-medication admitted that self-medication is responsible for delay in health care seeking. Out of these mothers, 59% sought formal health care without delay for their child suffering from diarrhea or RTI during last 6 months. 70.8% children recovered completely after practicing self-medication, as reported by mothers. While 23.0% reported late recovery, 3.1% reported complications and 3.1% reported hospitalization after self-medication. A highly significant association was found between practice of self-medication and health care seeking behavior (p=.001).Conclusion: Practice of self-medication is present in one third of mothers of low income, literacy poor families. Mothers of this stratum showed poor perception about self-medication and as well as treatment delay. Self-medication practice strongly affects health care seeking behavior.

Initial Experience with Laparoscopic Nephrectomy at Services Hospital Lahore 拉合尔服务医院腹腔镜肾切除术的初步经验

Introduction: Nephrectomy means removal of kidney. Laparoscopic nephrectomy is an effective way to remove a diseased or cancerous kidney. Nephrectomy is performed to treat malignant or benign tumors of the kidney and symptomatic non functioning kidney. Laparoscopic nephrectomy is performed through transperitoneal or retroperitoneal approach.Aims & Objectives: To determine the initial experience of laparoscopic nephrectomy.Place and duration of study: Department of Urology, Services Hospital, Lahore from 1st January 2018 to 30th June 2019.Material & Methods: A total of 50 patients who underwent laparoscopic nephrectomy were included. The demographic data like age, gender, site of operation, operative time, conversion rates, postoperative complications and postoperative hospital stay were recorded. Data was analyzed using SPSS 20.0 and reported as mean± s.d, frequencies and percentages.Results: There were 28 (56%) female patients and 22 (44%) male patients. Six (12%) patients were ages <25 years, 20 (40%) were ages 25 to 40 years, 18 (36%) were ages 41 to 55 years and 6 (12%) were ages above 55 years. Non-functional kidney due to stone disease was the most common indication 35 (70%). Mean operative time was 170.43+8.45 minutes. Conversion rate was in 1 (2%) patients. Mean hospital stay was 4.32+1.45 days.Conclusion: It is concluded from this study that laparoscopic nephrectomy is safe and better technique with very low rate of complications.
介绍:肾切除术是指切除肾脏。腹腔镜肾切除术是切除病变或癌变肾脏的有效方法。肾切除术用于治疗肾的恶性或良性肿瘤以及有症状的肾功能不全。腹腔镜肾切除术通过经腹膜或后腹膜入路进行。目的:探讨腹腔镜肾切除术的初步经验。研究地点和时间:2018年1月1日至2019年6月30日,拉合尔服务医院泌尿外科。材料与方法:共纳入50例行腹腔镜肾切除术的患者。记录年龄、性别、手术部位、手术时间、转换率、术后并发症及术后住院时间等人口统计学资料。数据采用SPSS 20.0进行分析,以mean±s.d、频率和百分比报告。结果:女性28例(56%),男性22例(44%)。年龄<25岁6例(12%),25 ~ 40岁20例(40%),41 ~ 55岁18例(36%),55岁以上6例(12%)。肾结石所致的无功能肾是最常见的适应症35(70%)。平均手术时间170.43+8.45分钟。转换率为1例(2%)。平均住院时间为4.32+1.45天。结论:腹腔镜肾切除术安全可靠,并发症发生率低。

Laparoscopic nephrectomy is performed through transperitoneal or retroperitoneal approach.\u0000\u0000Aims & Objectives: To determine the initial experience of laparoscopic nephrectomy.\u0000\u0000Place and duration of study: Department of Urology, Services Hospital, Lahore from 1st January 2018 to 30th June 2019.\u0000\u0000Material & Methods: A total of 50 patients who underwent laparoscopic nephrectomy were included. The demographic data like age, gender, site of operation, operative time, conversion rates, postoperative complications and postoperative hospital stay were recorded. Data was analyzed using SPSS 20.0 and reported as mean± s.d, frequencies and percentages.\u0000\u0000Results: There were 28 (56%) female patients and 22 (44%) male patients. Six (12%) patients were ages <25 years, 20 (40%) were ages 25 to 40 years, 18 (36%) were ages 41 to 55 years and 6 (12%) were ages above 55 years. Non-functional kidney due to stone disease was the most common indication 35 (70%). Mean operative time was 170.43+8.45 minutes. Conversion rate was in 1 (2%) patients. Small Group Discussion; Medical Students Perspective 小组讨论;医学生视角

Introduction: Small group discussion teaching is a key instructional strategy being incorporated in medical education and is aimed at enhancing the personal, social and cognitive skills of students. Learning in small groups helps the students to enhance the acquisition, processing and retention of the medical knowledge.Aims & Objectives: To assess medical student’s perception about small group discussion at a medical college.Place and duration of study: September 2019 at the University College of Medicine and Dentistry, University of Lahore.Material & Methods: This cross- sectional survey was conducted during September 2019 at University College of Medicine and Dentistry, University of Lahore. Convenient sampling technique was used to obtain a sample of 322 students from the first three years of MBBS. A pre validated questionnaire was distributed and the students were asked to record their experience about small group discussion using a 5-point Likert scale. Data was analyzed by using SPSS22 and reported as percentage distribution and mean± s.d.Results: Regarding quality of these small group discussion sessions, 25.8% of the students appeared satisfied, 24.8% of the students were neutral, whereas, 49.4% of the students remained unsatisfied. When asked about being clear of their role in a SGD session, 55.9% of the students agreed, 22.7% were unclear and 21.4% neutral. As regards the benefits from small group sessions, majority (35.1%) agreed that activities taught lifelong learning, 8.7% strongly agreed. 27% remained neutral with 16.5% disagreeing and 12.75% strongly disagreeing. With reference to assessment of tutors, majority of students found SGD facilitators enthusiastic about process with 13.4% strongly agreeing and 36.3% agreeing, 28% stayed neutral while 9.6% disagreed and 12.75% strongly disagreed. Majority of students, (35.1% agreed, 13.4% strongly agreed) to that tutors just gave mini lecture, 27.6% responded neutral to statement while 11.2% disagreeing and 12.7% strongly disagreeing to the enquiry.Conclusion: The study reveals that majority of the students are clear about their role in small group discussion sessions and majority (35.1%) also agreed that activities taught them lifelong learning. Tutors are helpful and enthusiastic, however, they provide plenty of information and need retraining for conducting a small group learning session.

The Potential Role of ACEi and ARBs in COVID-19; A Perspective ACEi和arb在COVID-19中的潜在作用一个透视图

ABSTRACT. COVID-19 pandemic has caused significant morbidity and mortality around the world. The disease severity ranges from mild upper respiratory infection to severe lower respiratory and cardiac illness. Acute respiratory distress syndrome (ARDS) is the most serious complication and results in diffuse inflammatory alveolar damage, respiratory failure, and death. Components of the Renin -Angiotensin-Aldosterone -System (RAAS) are involved in an inflammatory reaction in the lungs. Various studies have shown that blocking RAAS peptides in the lungs especially angiotensin-converting enzyme (ACE) and type-1 angiotensin receptor (ATR1) reduces lung injury, improves respiratory function and is associated with better clinical outcomes in the COVID-19 patients. We suggest that angiotensin-converting enzyme inhibitors (ACEi) and angiotensin receptor blockers (ARBs) – drugs that block RAAS peptides – be considered for a repurposed use in COVID-19 induced lung injury.
