Background and objectives. The incidence of brain metastases is higher than primary brain tumors, with lung cancer as a common etiology at “Dr. Cipto Mangunkusumo” General Hospital. Delay in diagnosis can cause brain herniation, resulting in disability and death. We aimed to investigate the compatibility between daily clinical practice and clinical practice guidelines for diagnosing brain metastases due to lung cancer at “Dr. Cipto Mangunkusumo” General Hospital while also examining factors that might influence the duration of the diagnosis. Materials and methods. We conducted a retrospective cohort study to determine the conformity between the duration of diagnosis of metastatic brain tumors due to lung cancer in daily clinical practice with clinical practice guidelines. Secondary data was obtained from medical records from November 2021 until June 2022. Results. Twelve subjects (30%) were diagnosed with brain metastases from lung cancer within two weeks with a median duration of 18.5 days (IQR 12-34 days). The duration of 7 days (IQR 4-11 days) was required to obtain a lung mass, 8 days (IQR 4.5-13 days) to perform a biopsy, and 6 days (IQR 3.5-7 days) to obtain t pathology results. No statistically significant relationship exists between the variables assessed and the duration of diagnosis. Conclusions. The clinical practice guidelines at “Dr. Cipto Mangunkusumo” General Hospital for diagnosing brain metastases from lung cancer within two weeks could only be carried out in 30% of subjects in this study. Collaboration between departments is needed to make the diagnosis faster.

Background and aim. One of the most prevalent chronic joint illnesses is osteoarthritis (OA), which can impair a patient's quality of life by causing joint pain, stiffness, deformity, and narrowed joint spaces. Many factors influence the pathogenesis of Osteoarthritis such as age and body weight, as well as genetic factors that are reported to play a role in the occurrence and progression of Osteoarthritis. Recently, many studies have demonstrated that inflammatory variables contribute to osteoarthritis pathomechanisms. It is currently thought that inflammatory variables alone are an independent risk factor for osteoarthritis. Methods. Observational sub-analysis with a cross-sectional approach to examine the relationship between Angiotensin Converting Enzyme 1 (ACE 1) levels and the grading of osteoarthritis at “Dr. Wahidin Sudirohusodo” Hospital from October 2020 to February 2021. ACE 1 levels were measured using the Elisa KIT and grading of osteoarthritis using the Kellgren and Lawrence criteria. Results. Analysis of the relationship between ACE 1 levels and osteoarthritis grading using the ANOVA test and Post Hoc Bonferonni Test showed a significant relationship between ACE 1 levels and osteoarthritis grading, where high ACE 1 levels were related to osteoarthritis grading. Deficiency and insufficiency (p<0.05). The results of this study are in line with previous research. Conclusion. The higher the ACE 1 level, the more severe the degree of knee osteoarthritis.

Background. Emergency groin hernia is an understudied research field despite its severity requiring emergency surgery associated with high postoperative morbidity and mortality rates. Therefore, the purpose of this study was to investigate the efficacy, critical technical elements, and management of problems associated with the transabdominal preperitoneal (TAPP) laparoscopic technique for adult patients in need of an emergency groin hernia repair. Methods. We analyzed adult patients who were treated between January 2021 and March 2023 for emergency groin hernia repair. These patients' preoperative data were examined. Results. The surgery was successfully performed on all 21 patients. The duration of the surgery was 71.6 ± 19.7 minutes (range, 45–120 minutes). The hospitalization time was 3.2 ± 2.2 days (range, 2–5 days). Conclusions. The TAPP approach is a secure and efficient method for performing emergency groin hernia repair in adults due to its ability to evaluate hernia content and reduce the incidence of incisional infection.

Atrioventricular conduction disorders represent an alteration in the normal function of the heart’s electrical conduction system, a system that connects the atria and ventricles through the atrio-ventricular node and the His-Purkinje system. Consequently, bradycardia can develop and lead to the occurrence of symptoms (dizziness, loss of consciousness, tachyarrhythmias, heart failure phenomena). Identifying patients at risk of developing atrioventricular conduction disorders will allow early interventions on risk factors and improvements in prognosis. This narrative review explores the risk factors that determine the development and progression of atrioventricular conduction disorders. We will discuss traditional risk factors (age, sex, arterial hypertension, ischemic cardiac disease, diabetes, chronic kidney disease), but also risk factors that are currently being researched (genetic predisposition and inflammatory markers). Finally, we will discuss the limitations and challenges of predicting risk factors for atrioventricular conduction disorders.

Objectives. We present a case report of a patient with malaise, abdominal pain, and vomiting, later underwent bone marrow biopsy and found to have CNL with no genetic mutation. Case presentation. A 40 years old male came into hospital with malaise, abdominal pain, nausea, and vomiting. Furthermore there were bilateral lymphadenopathy in the neck, hepatomegaly and moderate splenomegaly in abdominal examination. Anemia, leukocytosis and increase of Lactat Dehydrogenase (LDH) were found. Bone marrow aspiration and biopsy was performed and showed predominant neutrophil and the bone marrow biopsy showed hypercellular bone marrow with increased myeloid-neutrophil, increased myeloid erythroid ratio. Outcome. After further investigation this patient was diagnosed with chronic neutrophilic leukemia based on bone marrow aspiration and biopsy result.

Introduction. Type 2 diabetes mellitus (T2DM) is currently an important global health disease. At the same time vitamin D deficiency is considered the other public health problem around the world. Some studies suggest involving insufficient or deficiency of vitamin Din the etiopathogenesis of T2DM. Materials and methods. 58 patients, 33 women (56.89%) and 25 men (43.10%) were included in the observational study. Demographics (age, gender), personal anamnestic data (duration of diabetes, micro-and macrovascular complications, therapy with vitamin D supplements), anthropometric and biochemical parameters were assessed. The anthropometric measurement included height (meters), weight (kg) and body mass index (BMI) defined as a ratio of weight to the square of height (kg/m2). Biochemical parameters included the fasting blood glucose, glycated hemoglobin (HbA1c), serum total cholesterol (TC), high-density lipoprotein (HDL) cholesterol, low-density lipoprotein (LDL) cholesterol, very-low density lipoprotein (VLDL) cholesterol, triglycerides (TG), as well the level 25(OH)D. We followed the correlation between vitamin D levels and the duration of diabetes, BMI, blood glucose levels, HbA1c, TC, LDL-cholesterol, HDL-cholesterol, VLDL-cholesterol, TG. Results. The average values recorded were 11.72 years for the duration of T2DM, 31.77 kg/m2 for BMI, 169.00 mg/dL for blood glucose levels, 8.01% for HbA1c, 200.40 mg/dL for TC, 129.04 mg/dL for LDL-cholesterol, 47.67 mg/dL for HDL-cholesterol, 26.69 mg/dL for VLDL-cholesterol and 160.34 mg/dL for TG. Conclusion. Following statistical analysis, the observational study highlight statistically negative correlation recorded between the level of vitamin D and BMI, glycemic balance (highlighted by the level of fasting blood glucose and HbA1c). The results found nonsignificant correlation between the levels of vitamin D: duration of diabetes and lipid profile.

Pregnancy is an important event in women’s life as it involves various physiological and anatomical alterations to match and accommodate with the development of the fetus. Due to c-morbidities and pregnancy related complications, pregnant women often requires medical management which includes medication use. We reviewed the literature for studies that assessed medication use among pregnant women in the Arabian countries. Prevalence of medication use among pregnant women from Arabian countries varied from 40% to 98.2%. The most commonly used medications were vitamin and supplements, analgesics, antibiotics, and herbal medicines. The most frequently reported justification for self-medication were: inconvenience, less expensive, previous experience with disease, and that the diseases are not serious. In conclusion, the use of medications during pregnancy were found to be prevalent among pregnant women in the Arabian countries.

Duplication cysts are congenital anomalies that typically have some connection to the gastrointestinal (GI) tract, but in rare cases the cysts can be completed isolated from the GI tract. We present the case of an appropriate for gestational age male infant born at 40 weeks gestational age with neonatal intestinal duplication cyst isolated from the GI tract. Postnatal diagnostic methods included ultrasound (US) and magnetic resonance imaging (MRI) based on abnormal antenatal findings. The positive diagnosis was based on the imaging results. Duplication cysts may remain asymptomatic and that is the reason why antenatal and postnatal US screening plays an important key role in the diagnosis. Series of imaging may confirm the diagnosis, prevent future complication and help the clinician to determine the optimal operative moment.

Background. Cancer is a significant health problem worldwide. Globally, cancer is one of the most common causes of morbidity and mortality and this is predicted to increase by at least 70% by 2030. The aim of this systematic review was to identify the state of the scientific evidence regarding the quality of life in patients. with cancer. Methods. The literature was searched in the Scopus, Web of Science and Pub Med databases, the literature related to the quality of life in cancer patients. Keywords are combined with Boolean operators (AND/OR). There are sixty studies were included in the current review. Results. The social and physical domains were the most endangered, while the environment was the most preserved. Comparison between scores showed a significant difference only in terms of self-rated health. The highest scores were found in the group with the best assessment of their health. The results suggest that self-rated health can be a reliable predictor of quality of life in these patients, being important for further studies on this topic. Discussion. The findings of this review indicate low QoL among cancer patients on anticancer therapy. Functional well-being was most affected among cancer patients, followed by emotional well-being among cancer patients on cancer therapy. From our study, it was found that the overall quality of life of the patients was influenced by the education and professional status of the patient. Unemployed and illiterate patients have worse QoL than employed and educated patients.
