Objectives: This study reports on the efficacy of Korean medicine treatments for peripheral facial nerve palsy and sleep disorders that occur after microvascular decompression.Methods: A 57-year-old female patient with right facial palsy was treated with herbal medicines and acupuncture for 36 days. The treatment effect was evaluated using the House Brackmann Grading System (HBGS), Yanagihara’s Unweighed Grading System (Yanagihara’s score), and the Korean Modified Leeds Sleep Evaluation Questionnaire (KMLSEQ).Results: Following treatment, the patient showed a decrease in HBGS and an improvement in Yanagihara’s score and KMLSEQ score.Conclusions: Korean medicine treatments appeared to be effective in reducing facial nerve palsy. Further clinical research on patients with facial nerve palsy is needed.

Objective: This study examined the effectiveness of Korean medicine treatments in two patients with Parkinson’s disease complaining of discomfort stemming from postural instability and gait disturbance (PIGD).Methods: Two patients were treated for 3 months. They visited the clinic once a week for the first month and thereafter once every 2 weeks. The Unified Parkinson’s Disease Rating Scale (UPDRS) and a three-dimensional gait analysis were performed at the first visit and at 1, 2, and 3 months thereafter.Results: In Case 1, gait speed, stride length, and swing phase increased. Double support decreased until 2 months after treatment but increased slightly after 3 months. Among the kinematic parameters, tilt and rotation increased. The total UPDRS Part III score decreased from 51 points to 29 points after 3 months of treatment. In Case 2, gait speed, stride length, and swing phase increased, but double support decreased. Among the kinematic parameters, tilt, rotation, and obliquity decreased. The total UPDRS Part III score decreased from 11 points to 7 points after 3 months of treatment.Conclusions: This study suggests that Korean medicine can be an effective treatment for patients with Parkinson’s disease who experience discomfort due to PIGD.

Objectives: This study summarizes current trends and results in clinical studies on herbal medicine treatments for chronic pancreatitis.Methods: Studies published from 2014 to 2023 were searched for on the China National Knowledge Infrastructure (CNKI), PubMed, and KMbase using the keywords “chronic pancreatitis”, “herbal medicine”, “herb”, “만성 췌장염”, and “한약”. The studies were analyzed according to year, study design, characteristics of the patients, and traditional Chinese medicine (TCM) intervention.Results: In total, 23 studies were selected. There were 21 randomized controlled trials and 2 case-control studies. The most common herbal prescriptions were Qingyi-tang (淸胰湯), Radix Bupleuri (柴胡), Radix Paeoniae Alba (白芍藥), Radix Glycyrrhizae (甘草), Radix Scutellariae (黃芩), and Rhei Rhizoma (大黃). In all the studies, herbal medicine treatments were effective in the treatment of chronic pancreatitis, and efficacy was assessed using clinical symptoms, physicochemical examination, radiological examination, and relapse.Conclusions: Herbal medicine treatment can be an effective treatment for chronic pancreatitis and has potential in clinical practice.

Objective: This study assessed the effectiveness of using Yijung-tang to treat chronic gastritis through a systematic review and meta-analysis of randomized controlled trials (RCTs).Methods: The search was conducted using keywords such as “gastritis”, “Lizhong”, “Yijung”, and “Chronic gastritis” on July 31, 2023. A meta-analysis was conducted according to outcome measurements, such as total effective rate (TER), Traditional chinese medicine symptom score (TSS), gastric hormones, and symptom relief time, using the Review Manager website.Results: A total of 11 RCT studies were selected. The treatment group (Yijung-tang-gagam or Yijung-tang-gagam+ Western medicine) showed significant improvement effects in terms of TER, TSS, serum gastrointestinal hormones (motilin (MTL), gastrin (GAS), and somatostatin (SS)), and symptom relief time compared to the control group (Western medicine). TER-RR : 1.20, 95% CI : 1.11 to 1.30, P≤0.00001, RR : 1.21, 95% CI : 1.14 to 1.28, P≤0.00001; MTL-MD : 35.99, 95% CI : 30.79 to 41.20, P≤0.00001); GAS-MD : 103.33, 95% CI : 97.62 to 109.04, P≤0.00001); SS : MD : -37.19, 95% CI : -41.75 to -32.64, P≤0.00001; symptom relief time (nausea)-MD : -1.96, 95% CI : -2.47 to -1.45, P≤0.00001; symptom relief time (colic)-MD : -1.75, 95% CI : -2.31 to -1.18, P≤0.00001.Conclusion: This study suggests that Yijung-tang is effective against chronic gastritis. However, the number of studies was insufficient, and the diagnosis and evaluation criteria were not standardized. To evaluate the effect on clinical symptoms, a manual muscle test, the Modified Functional Ambulation Classification, and the Korean version of the Modified Barthel Index were used. After 84 days of treatment, the MMT grade of the right upper and lower limbs improved from Gr. 2+/2- to Gr. 4/3~4 on average, and personality change and aphasia were gone.Conclusion: A complex treatment that includes Jaeumkunbi-tang, acupuncture, moxibustion, and limb activation exercises can be effective following an intracerebral hemorrhage resulting in hemiplegia and personality change. However, more research is needed.

Objective: This long-term case report details a case of pancreatic cancer with lung metastasis suppressed by integrative cancer medicine (IMT).Methods: A 64-year-old woman diagnosed with pancreatic cancer visited a Korean medicine hospital complaining about the side effects of chemotherapy for lung metastasis. She received IMT involving Korean traditional medicine and Western immunotherapy from May 2017 to June 2023. Tumor dimensions were assessed through computed tomography (CT) and positron-emission tomography/CT scans, while tumor markers and safety were monitored by laboratory tests.Results: IMT suppressed the progression of cancer, as observed by imaging and laboratory tests. The patient achieved 5-year survival, even after discontinuing chemotherapy.Conclusion: This case presents a potential therapeutic alternative for patients who are ineligible for chemotherapy or surgical interventions.

Objective: This study attempts to increase awareness of hepatotoxicity caused by antipsychotic drugs and to provide updated information on drug-induced liver injury (DILI) to physicians in Korean medicine (KM) clinics.Methods: This study presents a detailed case of a female patient diagnosed with DILI attributed to antipsychotic drugs, highlighting the improvement observed through laboratory findings.Results: A 56-year-old female patient with underlying disorders, including mixed connective tissue disease and depression, was under medical care. One day, she reported experiencing intense fatigue and distressing sensations, prompting the author to order blood tests. The levels of AST and ALT were significantly elevated by more than 2.5-fold, indicating hepatocellular DILI. The RUCAM score for antipsychotic drugs was 9, as no other medications, including herbal medicine, were being taken. Upon discontinuation of the antipsychotic drugs, the patient’s laboratory findings returned to normal levels within 2 weeks, accompanied by a recovery of subjective symptoms.Conclusion: This study presents a noteworthy case of hepatotoxicity caused by antipsychotic drugs, serving as an illustrative example that highlights the crucial need for awareness among doctors of KM in clinical settings.

Objectives: This review investigated the research on warm herbal foot bath therapy (WHFT) for insomnia.Methods: A search was conducted on the China National Knowledge Infrastructure (CNKI) database to collect relevant studies published up to August 29, 2023. Randomized controlled trials (RCTs) comparing WHFT and sleeping pills in patients with insomnia were included. The methodological quality of the included studies was assessed using the Cochrane risk-of-bias assessment tool. The results of the meta-analysis were presented as risk ratios (RRs) or mean differences (MDs) and their 95% confidence intervals (CIs).Results: A total of 11 RCTs were included. WHFT as monotherapy resulted in a significantly higher total effective rate (TER) (RR, 1.25; 95% CI, 1.15 to 1.36; I2=25%) and an improved Pittsburgh Sleep Quality Index (PSQI) global sore (MD, -3.10; 95% CI, -4.24 to -1.95; I2=73%) compared to benzodiazepines. Additionally, WHFT as a combined therapy with benzodiazepines resulted in a significantly higher TER (RR, 1.15; 95% CI, 1.04 to 1.27; I2=0%) and an improved PSQI global score (MD, -2.23; 95% CI, -4.09 to -0.38; I2=80%) compared to benzodiazepines alone. In network analysis visualizing the components of HWFT, four clusters were discovered, and Polygoni Multiflori Ramuls and Ziziphi Spinosae Semen were the key herbs used in WHFT. Overall, the methodological quality of the included studies was poor.Conclusions: There was limited evidence that WHFT as a monotherapy or combined therapy was effective in improving insomnia. The findings can be used as basic data for future WHFT research in South Korea.

Objectives: This study investigated the effects of Korean medicine treatments on Buerger’s disease.Methods: From December 27, 2022, to January 16, 2023, we treated Soeumin patients with Buerger’s disease admitted to a hospital specializing in Korean medicine. The patient’s symptoms included feelings of coldness in the lower extremities, feelings of dullness, and stabbing pain. For treatment, Korean medicine treatments, such as acupuncture, moxibustion, and herbal medications (Sogyeonghwalhyeol-tang granules and Gwangyebujaijung-tang-gami) were administered. The patient’s improvement was evaluated using the numerical rating scale (NRS) every day. Lower extremity and foot temperatures were evaluated using digital infrared thermal imaging (DITI).Results: Through the treatment regimen, the patient’s NRS scores decreased, and lower extremity and foot temperatures increased.Conclusions: Korean medicine treatments for cold lower extremities, feelings of dullness, and stabbing pain caused by Buerger’s disease are potentially effective.

Objectives: This study analyzed the clinical characteristics of patients with functional dyspepsia (FD) who received Korean medicine treatment.Methods: A retrospective chart review was conducted to investigate sociodemographic characteristics, clinical characteristics, and prescribed treatments. The clinical records of 192 patients who visited Kyung Hee University Korean Medicine Hospital for FD from May 1, 2022, to May 31, 2023, were analyzed. In addition, the distribution of pattern identification and symptom type according to body mass index (BMI), as well as prescription history, were analyzed.Results: As the degree of obesity increased, the proportion of Spleen-Qi deficiency pattern patients decreased (p=0.012), and the proportion of damp-phlegm pattern patients increased (p=0.000). Additionally, as the degree of obesity increased, the proportion of patients with excess differentiation increased (p=0.002). The PDS (Postprandial distress syndrome) symptom type was significantly more frequent in the underweight and normal groups than in the overweight and obese groups, and the EPS (Epigastric pain syndrome) symptom type was more frequent in the overweight and obese groups. Regardless of the type of pattern identification, the most frequently used prescriptions were Naesowhajung-tang, Hanshin Naeso-san, and Sojeokgunbi-hwan granule.Conclusion: This study analyzed the medical records of patients with FD to elucidate the use of Korean medicine treatments. Our study is meaningful in that we found that the distribution of pattern identification and symptom patterns are linked to the degree of obesity in FD patients and identified the tendency for herbal medicine treatments to be prescribed in clinical practice.
