Background: Adductor spasmodic dysphonia (ASD) is caused by the involuntary contraction of laryngeal muscles due to dystonia localized to the larynx. In the case of ASD, conventional treatment is mainly performed with a botulinum toxin injection. However, the botulinum toxin injection has a short-lasting effect and requires repeated injections. Alternatives are needed due to concerns over adverse effects, such as general weakness and airway aspiration caused by the botulinum toxin injection.Case report: A 46-year-old female patient with ASD complained of voice tremor and hoarseness. The combined Korean medical treatments-Ukgan-san-gami, Jakyakgamcho-tang, acupuncture, and transcutaneous electrical nerve stimulation (TENS)-were administered on the first day the patient was hospitalized. The Voice Handicap Index (VHI) was evaluated during the treatment. The VHI taken on the second day totaled 92 points. On the ninth day, 81 points were recorded. Total score gradually improved, and on the 16th day, 62 points were recorded. Combined Korean medical treatment lasted 19 days.Conclusion: The present case report suggests that a combined Korean medical treatment approach with Ukgan-san-gami, Jakyakgamcho-tang, acupuncture, and TENS might be effective for symptoms such as voice tremors and hoarseness. Combined Korean medical treatment can be a therapeutic option for patients with ASD.

Objective: This study reports the effectiveness of the Korean medicine treatment of patients with intracerebral and intraventricular hemorrhages suffering unilateral neglect symptoms.Case presentation: The patient was treated using Korean medicine, including acupuncture, herbal medicine, moxibustion, and a limb activation exercise. We measured Unilateral Neglect Symptom progress with a numeric rating scale, Albert’s test, Line bisection test, Clock drawing test, and Catherine Bergego Scale. After eight days of combined Korean medicine and rehabilitation exercise, the Unilateral Neglect Symptom intensity reduced from NRS 8 to 6. In addition, Albert’s test score decreased from 2.5% to 0%. The Line bisection test score decreased from 19 mm to 12 mm, and the Clock drawing test score improved from 8 to 10. Last, the Catherine Bergego Scale decreased from 12 to 9. No adverse events were observed during treatment.Conclusion: This study suggested that a complex treatment including Jaeumkunbi-tang, acupuncture, moxibustion, and limb activation exercise can be effective for an intracerebral hemorrhage with intraventricular hemorrhage patient suffering from Unilateral Neglect Symptoms. Consequently, more research is needed in the future.

Objective: This study identified the effects of Korean medicine treatment on a patient with metabolic dysfunction-associated fatty liver disease (MAFLD).Methods: A 43-year-old man with MAFLD was treated with Saenggangunbi-tang with regular exercise from August 13, 2022, to December 24, 2022, to reduce fatigue and dyspepsia and to improve laboratory findings, such as liver enzymes and lipid profiles. We observed changes in symptoms and laboratory findings during the approximately four-month treatment.Results: Treatment with Saenggangunbi-tang resulted in decreased serum levels of liver enzymes, triglycerides, hepatic steatosis index scores, and clinical symptoms. During the treatment, the patient performed regular exercise; however, there was no significant change in body weight until the end of the study.Conclusion: This study suggests the availability of Saenggangunbi-tang as a therapeutic option for managing MAFLD patients.

Objective: This study addressed the case of a cerebral infarction patient with recurring aspiration pneumonia.Methods: A patient diagnosed with cerebral infarction with recurring aspiration pneumonia was treated with Korean medicine therapies, including herbal medication (Shigyungbanha-tang-gami), acupuncture, electro-acupuncture, and moxibustion combined with antibiotics for 31 days. To evaluate the therapeutic effect, we checked chest X-rays, lab evaluations, the number of night suctions, and body temperature.Results: After treatment, chest X-rays and inflammation markers improved. In addition, the number of night suctions decreased, and the body temperature was normalized. Aspiration pneumonia recurred. However, the intensity of the symptoms gradually weakened, and the recovery speed increased.Conclusion: These results suggest that Korean medicine therapies, including Shigyungbanha-tang-gami combined with antibiotics, have a beneficial effect on recurring aspiration pneumonia, an additional disease related to cerebral infarction.

Restless leg syndrome is a nervous system disorder that causes an overpowering urge to move one’s legs. Symptoms of restless leg syndrome usually worsen when one tries to fall asleep and can prevent sufficient sleep. Restless leg syndrome is common in patients with chronic kidney failure and can be caused or worsened by chronic kidney failure and hemodialysis. Various medications can treat restless leg syndrome, though the long-term use of medications can cause augmentation and adverse effects. In addition, the use of dopamine agonists is limited in patients with chronic kidney failure. This is because the dose of administration should be controlled for patients with chronic kidney failure, and the treatment effect has not been clearly proven. This study reports the case of a 56-year-old male diagnosed with chronic kidney failure complaining of uncomfortable leg sensations. The patient underwent Korean medicine treatment using Jakyakgamcho-tang. The IRLS, NRS, and AIS scores were evaluation tools during treatment. This study suggested significantly improved symptoms through the individual interventions of Jakyakgamcho-tang in a restless leg syndrome patient with chronic kidney failure.

Background: Pressure injuries are localized areas of damage to the skin and/or underlying tissue, usually over a bony prominence due to pressure. Cerebrovascular disease increases the risk of pressure injuries due to the immobility caused by physical paralysis. The general approach to managing a patient with pressure injuries should include pain relief, the treatment of the infection, optimizing nutritional intake, proper positioning, and contamination prevention. Nonetheless, the duration of treatment for pressure injuries varies from person to person.Case report: An 80-year-old female intracerebral hemorrhage patient developed a pressure injury. To improve the injury faster, a Radix Astragali pharmacoacupuncture solution was applied to the pressure sore. The pressure injury’s width, length, and depth was assessed using a ruler, and the exudate amount and tissue types were assessed. The treatment was performed for 35 days. The rate at which the size of the pressure sore lessened increased since the Radix Astragali pharmacoacupuncture solution was applied to the pressure sore. In addition, the tissue type of the pressure injury improved, and the exudates decreased. There was no significant difference in the Pressure Ulcer Scale for Healing Tool 3.0, since the Radix Astragali pharmacoacupuncture solution was applied to the pressure injury.Conclusion: This clinical case study suggests that the Radix Astragali pharmacoacupuncture solution might be effective in speeding up the healing of pressure injuries.

We observed changes in the symptoms and improvement of laboratory findings during the four-month treatment.Results: The approximately four-month treatment with Saenggangunbi-tang showed considerable improvement in laboratory findings and fatigue. In addition, no adverse effects, such as liver injury, were observed during Korean medicine treatment.Conclusion: This study suggests the availability of Saenggangunbi-tang as a therapeutic option for managing patients with iron overload.

Objective: This case study reported the effectiveness of adjuvant Korean therapy on gait disturbances induced by drug-induced Parkinsonism.Method: A patient suffering from frontotemporal lobe dementia was diagnosed with drug-induced Parkinsonism and treated with adjuvant Korean therapy, including herbal medicine and pharmaco-acupuncture. The evaluation was performed by monitoring the length of time and number of steps during an 8 m gait, using the Unified Parkinson’s Disease Rating Scale (UPDRS).Results: After 17 days of adjuvant Korean therapy, the UPDRS score improved from 32 to 16. The length of time for the 8 m gait improved from 20 seconds to 14 seconds. The patient also showed a decrease in the number of steps during the 8 m gait from 43 to 22.Conclusion: This case suggests that adjuvant Korean therapy can be effective for drug-induced Parkinsonism.

Ko, Seong-Uk Park","doi":"10.22246/jikm.2023.44.2.187","DOIUrl":"https://doi.org/10.22246/jikm.2023.44.2.187","url":null,"abstract":"Objective: This case study reported the effectiveness of adjuvant Korean therapy on gait disturbances induced by drug-induced Parkinsonism.Method: A patient suffering from frontotemporal lobe dementia was diagnosed with drug-induced Parkinsonism and treated with adjuvant Korean therapy, including herbal medicine and pharmaco-acupuncture. The evaluation was performed by monitoring the length of time and number of steps during an 8 m gait, using the Unified Parkinson’s Disease Rating Scale (UPDRS).Results: After 17 days of adjuvant Korean therapy, the UPDRS score improved from 32 to 16. The length of time for the 8 m gait improved from 20 seconds to 14 seconds. Objective: This study reported the effectiveness of Korean medicine for a patient with Type-1 diabetes mellitus after pancreatectomy with a compression fracture of the thoracic spine.Methods: A patient diagnosed with Type-1 diabetes mellitus after a pancreatectomy was treated with acupuncture, pharmacopuncture, Gami-hwanggigyejiomul-tang (modified Huangqi Guizhi Wuwu decotion), Jindang-won, etc. The conventional treatment was maintained using an insulin pump and insulin injection.Results: Blood glucose (FBS/PP2hrs), ESR and CRP decreased after one month of treatment.Conclusion: Gami-hwanggigyejiomul-tang and Jindang-won may be effective treatments for Type-1 diabetes mellitus. Further studies are needed to confirm the findings of this case.

Objective: This study reports a clinical case of headache accompanied by epilepsy that showed improvement with Korean medicine treatments.Methods: A 67-year-old woman with headache was treated with Korean medicine therapies, including Cheongshimondam-tang, acupuncture, electroacupuncture, and cupping, for 14 days. The response before and after treatment was evaluated using the Numerical Rating Scale, Headache Impact Test-6 (HIT-6), and Quality of Life in Epilepsy-10 (QOLIE-10).Results: After the treatments, the headache was relieved from NRS 8 to NRS 1, and anxiety decreased from NRS 7 to NRS 2. The HIT-6 changed from 66 to 48, and the QOLIE-10 dropped from 44 to 30.Conclusion: This study suggests that combined Korean medicine treatments might be a therapeutic option for relieving headache with epilepsy.
