Ballistic projectiles removal in head and neck region 头部和颈部的弹道弹丸清除

ABSTRACT Injuries caused by firearm projectiles in the head and neck region generally imply severe morbidity and mortality, representing an important public health problem. These injuries can be classified as: penetrating, perforating and avulsive, and the latter is the most worrisome, because they are caused by high-velocity projectiles. Low-velocity projectiles commonly cause penetrating injuries, sometimes remaining lodged in tissues, and their removal may or may not be indicated, depending on factors such as location and severity of the lesion. This paper presents the report of three clinical cases that address injuries caused by low-velocity firearm projectiles, in which they had the indication of this artifact removal, addressing the pre, trans and post-surgery procedures. And we point out that full tactical, technical and theoretical domains are required from the Oral-Maxillofacial Surgeons for being successful in the planning and success in the treatment of these lesions.

Oral health: knowledge of those responsible for hospitalized children with cancer 口腔健康:了解对住院癌症儿童负责的人员

ABSTRACT Cancer is a disease that has disordered cell growth with the formation of tumors. The most common types of childhood and adolescent childhood cancer are leukemias, tumors of the central nervous system, and lymphomas. Objective: To evaluate the knowledge of those responsible for oral health practices and care of children assisted in the Polyclinic of Natal, who underwent antineoplastic treatment. In addition to knowing the main oral health practices performed by those responsible. Methods: A quantitative and descriptive cross-sectional study, carried out in the Natal Polyclinic Unit (Liga Norte Riograndense Contra o Câncer), located in Rio Grande do Norte. The sample consisted of 41 questionnaires applied individually to those responsible for the children aged 0 to 12 years who underwent cancer treatment, during March and April 2018. Results: It was found that 51% of the responsible identified some oral health problem in their children, and 43.9% of the children had already been affected by mucositis. Of the 41 hospitalized patients, 90.2% were accompanied by a dental surgeon during the antineoplastic treatment. Conclusion: The responsible ones had a great perception of the children’s oral health problems and knew the importance of the participation of the dentist during the are treatment. There is a need for more research on the subject, to establish a care protocol with oral hygiene of children hospitalized with cancer.

The possible impact of COVID-19 pandemic on dental implant therapy: narrative review COVID-19大流行对种植牙治疗可能产生的影响:叙述性综述

ABSTRACT The COVID-19 has challenged the professions and Implantology also has its challenges in the pandemic. Several factors can interfere with the osseointegration process and more associated factors, greater the interference risks. Risk assessment in the therapy indication is the main decision to define the best technique, the best biomaterial, the best surgical access and the best moment to intervene. The purpose of this review was to investigate and synthesize the scientific evidence on the factors that may interfere with dental implant therapy in the midst of pandemic. The literature was reviewed in databases such as PubMed, Web of Science, Sciello and Google Scholar using the keywords “COVID-19”, “pandemic”, “risk factors”, “impact factor”, “dental implants”, “dentistry”, “oral health”, “osseointegration”, “bone metabolism”, “drug risk factors”, “chronic stress”, “antidepressants”, “zinc”, “hydroxychloroquine”, “ivermectin”, “vitamins”, “corticosteroids”, “surgical risks” and “disinfection”. The present review showed that chronic stress and depression caused by the pandemic, the consequent use of antidepressants, the use of prophylactic and therapeutic drugs such zinc, vitamin D, hydroxychloroquine and corticosteroids, can interfere with bone metabolism and consequently in osseointegration establishment and/or maintenance. Any osseointegrable biomaterial can be influenced by systemic factors and drugs’ actions that can affect the homeostasis of the inflammatory process, cell proliferation and bone remodeling. These factors’ influence on dental implant therapy should be investigated through new reviews, observational studies and randomized clinical trial.

Endodontic treatment during pregnancy: case series and literature review 妊娠期牙髓治疗:病例系列和文献回顾

ABSTRACT The aim of this paper is to provide a scientific basis for conducting safe root canal treatment in pregnant women, illustrating with case reports treated in the extension project Maternal-Child Dental Care at the Federal University of Pelotas. The scientific bases address the main doubts of professionals when performing prenatal dental care: use of radiographic examination, local anesthetics, prescription of medications, among other essential factors in the care of pregnant women. The assisted pregnant women sought care with dental pain. In two cases symptomatic irreversible pulpitis was diagnosed, while a third was experiencing acute apical periodontitis. Endodontic procedures were performed under controlled and standardized conditions. The root canals were prepared in a single session, using mechanical instrumentation with reciprocating kinematics (Wave One® Gold Primary - Dentsply Sirona), and the root canal length was determined by an electronic apex locator FinePex (Schuster). Root canal filling was performed by the single cone technique with Wave One® gutta-percha points. After endodontic treatment, coronal sealing was performed with composite resin A2 Z350 XT (3M ESPE). The care related to the gestational condition is highlighted, and the patients remain under monitoring. It is concluded that the use of apical locators and mechanical instrumentation, using reciprocating files associated with the single cone obturation technique with a standardized cone for each system, represents advances for the accomplishment of endodontic treatment in pregnant patients due to the shorter clinical time and greater comfort of the procedure.

Endocrown as a restorative strategy in endodontically treated teeth: an integrative literature review 内冠作为根管治疗牙齿的修复策略:综合文献综述

ABSTRACT With the consolidation of Adhesive Dentistry and the emergence of new aesthetic restorative materials, Endocrown has been studied as a rehabilitative option for endodontically treated teeth. This is an Integrative Literature Review, based on the PICO strategy, with articles selected from the databases of PubMed and BVS, with a period of time from 2011-2021. The database search strategy included the following keywords: (“Endocrown” OR “Endocrowns” OR “Endocrown Restoration” OR “no-post buildup” OR “adhesive endodontic crowns”). Inclusion criteria for this review were laboratory studies and observational clinical studies. Exclusion criteria were clinical case studies, literature review, systematic review and meta-analysis. Furthermore, articles that addressed only intraradicular retainers or other restorative strategies were also excluded. The selection of articles was based on abstracts. 5 articles were selected for analysis, obtaining data on the endocrown in relation to the stress on the tooth structure, aging, marginal adaptation, fracture resistance, endocrown versus conventional crown and integrity of the tooth structure. The findings in the literature are not yet established regarding the integrity of the dental structure, as the supporting evidence is limited. Only 1 in vitro article showed that dental failures in endocrowns are more favorable to repair and another 2 in silico studies show lower stress distribution on enamel, dentin and cement in the same restoration. Moreover, the restorative material seems to influence the possibility of fracture for both conventional crowns and endocrown restorations.

ABSTRACT Stevens-Johnson syndrome (SJS) and Toxic Epidermal Necrolysis (TEN) are adverse reactions, usually medicine-related, in which extensive and painful mucocutaneous lesions are formed. Oral lesions are characterized by the presence of erosions and ulcers. The treatment usually requires intensive care in a hospital setting, where the Dentist is essential in the multidisciplinary team, as he will act in the treatment of oral lesions. In this article, a 26-year-old female patient was reported, who presented TEN-compatible mucocutaneous lesions associated with the drug Phenytoin and required admission to a Teaching Hospital. The treatment consisted of the immediate suspension of the Phenytoin use and systemic therapy with corticosteroids and immunosuppressants. Oral lesions were monitored by the hospital dentistry team. The procedures adopted were: oral hygiene with 0.12% non-alcoholic Chlorhexidine Digluconate solution, hydration of the oral tissue with an essential fatty acid, application of frozen chamomile tea, and prescription of topical Triamcinolone Acetonide. Also, low-level laser therapy sessions (red and infrared) were performed. Oral lesions evolved with significant improvement. In this case, the importance of the Dentist in the multi-professional team is demonstrated. Although there is no protocol, the procedures used in the present case can be an alternative to therapies used in SSJ and TEN oral lesions.

ABSTRACT Objective: Evaluate the biomechanical behavior of the influence of different internal angles of conical prosthetic connections in fixed partial prostheses of three elements, through the tridimensional finite element method. Methods: Two different models of segment of the posterior portion of the mandible, with two implants with conical connections at angulations of 11.5 and 16 degrees. Afterwards, and occlusal load of 180N was used, and divided into 2 points of application on the premolar, and 400N divided into 5 points of application on the molar. Data were acquired as the tensile, compressive and shear stresses on cortical and medullary bone, and von Mises stresses on implants and prosthetic components of two implants in both in three-unit fixed partial prosthesis. In the quantitative analysis, both in cortical and medullary bone, the tension peaks in tensile, compression, and shear forces were higher for the CM16 group. Results: When analyzing qualitatively, the cortical and medullary bones presented a greater stress around the implant platform with a higher incidence in the implant in the molar region. In the von Misses analysis in both groups, the tensions were concentrated in the cervical region of the implants, in the first threads, in the premolar and molar regions. Regarding prosthetic pillars, the stress concentration was located in the region of contact with the implant. Conclusion: The increased angulation of conicity of the internal conical connections represents a determinant factor to increase in stress concentration in the implants, prosthetic components and adjacent structures, such as the cortical and medullary bone.

Prosthetic rehabilitation of a child with Rubinstein-Taybi Syndrome after dental trauma: case report 儿童鲁宾斯坦-泰比综合征牙外伤后假肢康复一例报告

ABSTRACT Rubinstein Taybi syndrome has a genetic origin in the chromosome 16. It has physical characteristics, delay in physical and mental development, and may present oral alterations. This paper aims to report a case of prosthetic rehabilitation of a patient with syndrome who, due to dental trauma, lost dental elements early. Female patient, 4 years and 7 months old, with loss of the deciduous maxillary central incisors. The data collection was carried out through anamnesis, clinical and complementary exams. After the diagnosis and the establishment of the treatment plan, we made a Denari type prosthesis. It has a tube-bar mechanism that allows maxillary growth and development. At the end of treatment, the caregiver was instructed to make periodic visits to the dentist for clinical and radiographic follow-up of the prosthesis, as well as the need for treatment with the speech therapist. The loss of anterior deciduous teeth can be associated with trauma in this region and the dental absence can affect the development and behavior of children, promoting a change in daily life and impacting the quality of life of the whole family. The Denari prosthesis is a viable treatment option for children with this syndrome, as it accompanies the maxillary growth, restores the functions of the stomatognathic system, prevents deleterious oral habits and helps with phonetics. It is necessary for the dentist to be able to offer the best treatment for these patients, as the syndrome makes it difficult, but does not impede its execution.

Evaluation of temporary cement to prevent bacterial contamination of the root canal after pulpectomy 临时骨水泥预防牙髓切除术后根管细菌污染的评价

ABSTRACT To evaluate the ability of temporary cement (TC) and gutta-percha sticks (GP) to prevent bacterial contamination of the root canal through the coronal seal after pulpectomy. Eighty artificial primary maxillary central incisors were selected and randomly divided into 2 groups: TC (n = 40) and GP (n = 40). Endodontic access, rotary instrumentation, root canal filling, and coronal sealing were performed according to group allocation. The root canal opening was seeded with S. mutans and E. faecalis. Both groups were subdivided into 5 experimental time points (24, 48, 72, 96, and 120 hours), with 8 specimens per time-point group: 5 in which both root canal filling and coronal sealing were performed (with either TC or GP) and 3 controls (coronal sealing alone, without root canal filling). All specimens were incubated in an anaerobic jar at 37°C, and bacterial contamination was assessed in a spectrophotometer. ANOVA (t-test) was used to compare contamination and the Kruskal-Wallis test to compare filling scores between the experimental groups. A significant difference was observed in sealing in the first 24 hours between GP and controls (p = 0.046). There was no significant difference in the filling pattern between canals sealed with TC versus GP. Specimens sealed with GP showed less contamination than controls in the first 24 hours. At later time points, neither GP nor TC were effective at controlling bacterial contamination; both failed to provide adequate coronal sealing.

ABSTRACT Objective: Civatte bodies are colloid bodies of apoptotic keratinocytes located at the dermoepidermal junction and are common in several dermatoses, including lichen planus lesions. The present study aimed to determine the presence of Civatte bodies in a sample of incisional biopsies obtained from patients diagnosed with oral lichen planus. Methods: A retrospective cross-sectional study, carried out with 34 slides stained with hematoxylin-eosin from the archive of an Oral Pathology Laboratory of a Higher Education Institution. The sample obtained was classified into white and red lesions from the available clinical data. Histological analyses were performed under a light microscope, with 10x and 40x objectives, identifying the presence or absence of Civatte bodies. Results: Colloid bodies were present in 73.5% the total sample studied and in 74.2% white lesions. Conclusion: The Civatte bodies were frequent in all samples analysed and should thus be considered a criterion for histological diagnosis in lesions of Oral Lichen Planus.
