Transcription and Secretion of Interleukin-1β and HMGB1 in Keratinocytes Exposed to Stimulations Mimicking Common Inflammatory Damages 白细胞介素-1β和HMGB1在角质形成细胞中的转录和分泌模拟常见炎症损伤

Objective: Interleukin-1β (IL-1β) and high-mobility group box 1 (HMGB1) are widely known damage-associated molecular patterns (DAMPs). However, their expression and secretion in different skin diseases, especially in inflammatory skin disorders, remain to be further elucidated. This study was performed to explore and compare the transcriptional and secretory levels of IL-1β and HMGB1 in keratinocytes under 3 types of stimulation: ultraviolet B (UVB) irradiation; co-stimulation by tumor necrosis factor-α (TNF-α) and interferon-γ (IFN-γ) (simulation of T helper 1 cell inflammatory challenge); and psoriasis-like stimulation by M5, a mixture of 5 proinflammatory cytokines. Methods: We used quantitative reverse-transcription polymerase chain reaction to determine the transcription levels of IL-1β and HMGB1. Western blotting and enzyme-linked immunosorbent assay were used to detect the secretion levels of IL-1β and HMGB1. The results were statistically analyzed by t test. Results: A rapid transcriptional and secretory response of IL-1β from keratinocytes occurred in all 3 types of stimulation mimicking common inflammatory environments (P < 0.05). Transcription of HMGB1 was inhibited in all 3 types of stimulation (P < 0.05), but secretion was increased after exposure to UVB irradiation and co-stimulation by TNF-α and IFN-γ (P < 0.05). We observed no change in the secretion level of HMGB1 after treatment with M5 (P = 0.196). Conclusion: IL-1β is a critical cytokine for the immunomodulatory functions of keratinocytes in inflammatory responses. In this study, keratinocytes restrained transcription of HMGB1 when the secretion of HMGB1 was induced in certain stimulations (eg, by UVB exposure or stimulation by TNF-α and IFN-γ).

Mental Stress Affects the Occurrence and Development of Psoriasis Through Neuroendocrine-Immune Regulation: A Narrative Review 精神压力通过神经内分泌-免疫调节影响牛皮癣的发生和发展

Psoriasis is a systemic immune-mediated inflammatory disease that presents at any age and is characterized by excessive proliferation and abnormal differentiation of keratinocytes as well as infiltration of multiple inflammatory cells. It has been gradually attracting more attention because of not only the increased risk of cardiovascular, metabolic, and autoimmune-related diseases but also the accompanying depression, anxiety, self-affirmation impairments, dementia, sleep disorders, and other mental disorders that place a substantial psychosocial burden on society and individuals. Mental and psychological disorders are involved in the occurrence, development, outcome, recurrence, and treatment of psoriasis through the neuroendocrine-immune signaling network. This review summarizes the pathogenesis of psychological and psychological disorders in patients with psoriasis with the aim of deepening clinicians’ awareness of the mental and psychiatric comorbidities in patients with psoriasis.

Impact of COVID-19 on Medical Care Conduct and Psychological Burden Among Patients With Psoriasis: Real-World Data From a Tertiary Hospital in China COVID-19对银屑病患者医疗行为和心理负担的影响:来自中国某三级医院的真实数据

Objective: The ongoing coronavirus disease 2019 (COVID-19) epidemic has caused extensive damage worldwide. We explored whether the medical care conduct of patients with psoriasis has changed and whether the COVID-19 epidemic has placed more psychological pressure on these patients. Methods: A questionnaire survey was administered to patients with psoriasis in the Dermatology Department of Peking University Third Hospital in July 2022. Information about the patients’ general demographics, psoriasis condition, mental state of anxiety (Generalized Anxiety Disorder-7 [GAD-7] score), changes in medical care conduct, and Concerns about COVID-19-Related Risk Score for Psoriasis (CCRSP, a self-designed questionnaire) was collected. Univariate analysis with the Mann–Whitney U test for continuous variables was used in the estimation of statistical differences. Results: A total of 112 patients with psoriasis completed the survey. Purchasing medicine was the factor that added difficulty for most participants (32.1%). Fewer participants (26.8%) encountered medical registration difficulties than economic and transportation difficulties. Nearly three-quarters of participants (73.2%) reported at least one medical care conduct change resulting from the COVID-19 epidemic. A small proportion of participants (12.5%) thought that COVID-19 prevented them from seeking medical services for psoriasis. The top three items that caused the most concern were psoriasis aggravation resulting from drug reduction and withdrawal because of COVID-19 (44.7%), psoriasis aggravation after infection with COVID-19 (38.4%), and psoriasis-related side effects of the COVID-19 vaccine (35.7%). The GAD-7 score of patients with a high CCRSP (score of >10) was significantly higher than that of patients with a low CCRSP (score of ≤10) (nonparametric test, P = 0.047). Conclusion: COVID-19 poses challenges and places a mental burden on patients with psoriasis. Doctors should regulate the medical behavior of patients with psoriasis according to their specific COVID-19 situation and help patients ease their anxiety to maintain the stability of their psoriasis condition.

Dermoscopic Assessment of Pityriasis Versicolor: A Cross-Sectional Observational Study 皮肤镜下评估糠疹与西洛的关系:一项横断面观察研究

Objective: Pityriasis versicolor (PV) is usually a clinical diagnosis. In uncertain cases, PV is confirmed by microscopic examination with 10% potassium hydroxide (KOH). However, the KOH test is not 100% sensitive in diagnosing PV. Dermoscopy of PV is still an unexplored area with very little data reported. This study was planned to study the various dermoscopic features and their utility in the diagnosis of PV. Methods: This cross-sectional observational study was carried out over a 1-year period (September 2020–September 2021) among 57 patients with KOH-confirmed PV. All patients underwent dermoscopy using a handheld dermoscope (DermLite DL4; DermLite LLC). The chi-square test or Fisher’s exact test was used to analyze the data. Results: Of the 57 patients, 43 (75.44%) had the hypopigmented type, followed by the hyperpigmented type (n = 12, 21.05%) and the perifollicular type (n = 2, 3.51%). Nonuniform pigmentation was the most common dermoscopic finding observed in both patients with hypopigmented PV (n = 42, 97.67%) and hyperpigmented PV (n = 12, 100%) (P = 0.001). Scaling was the second most commonly observed finding; patchy scaling (n = 25, 58.13%) and perifollicular scaling (n = 13, 30.23%) were commonly seen in hypopigmented PV, while hyperpigmented PV showed more diffuse scaling (n = 6, 50.00%) (P = 0.04) followed by patchy scaling (n = 5, 41.66%). Dermoscopy showed unique “double-edged scales” in all lesions with furrow scaling (n = 11, 19.30%) after eliciting a positive evoked scale sign. Other interesting features seen in hypopigmented PV were hypopigmentation around the hair follicle (n = 24, 55.48%) (P = 0.001) and perilesional hyperpigmentation (the halo sign) (n = 15, 34.88%) (P = 0.04). Conclusion: We observed several dermoscopic findings in PV that can serve as useful clues for differentiating PV from other similar disorders.
