Narration in medical care. Selected aspectsof narrative medicine in psychiatry 医疗护理中的叙述。精神病学叙事医学的若干方面

Narrative in medical practice is a way to explore the nature of the disease in the face of the patient’s uniqueness and individuality, regardless of the diagnosed disorder or disease. The narrative approach provides patients with a representation of their own story, a demonstration of interest on the part of their treatment team, and a positive relationship with the highest level of health care provision, always in relation to the patient’s current health and life situation. The use of narrative medicine is important in forming relationships with patients or when professional work becomes a source of burnout and lack of perceived satisfaction. Aim of the study was to present selected issues related to the practice of narrative in medical care, including the care of patients with mental disorders. Thirty-three selected original papers were thoroughly analysed. All works were written in the period 2011-2020 and could be characterized as demonstrative or research papers and case studies. Medline, PubMed, SAGE, and other databases were used. Literature analysis confirms the presence of narrative medicine, including in the field of psychiatry. Researchers emphasize the importance of narrative in creating relationships with patients, shaping both their activity and involvement, as well as with members of the therapeutic team. In practice, narrative involves professionals expressing empa-thy, using narrative techniques, and being accountable when interacting with patients who discover themselves. The importance of narrative in the care of patients with various disorders, including mental disorders, is significant in providing care that is consistent with patients’ expectations and needs.

The process of nursing a child with Haddad syndrome 照顾患有哈达德综合症的孩子的过程

Haddad syndrome is a combination of congenital central hypoventilation and Hirschsprung’s disease. This disease is very rare and causes mainly sleep apnoea and problems with intestinal peristalsis. Haddad syndrome is characterised by hereditary disorders of the autonomic nervous system and the lack of nerve ganglia in the distal part of the large intestine. The purpose of the above work is to illustrate the role of the nurse in caring for a child with Haddad syndrome and to present diagnoses and interventions based on the ICNP terminology, which the staff should include in the treatment process. In this work the method of literature analysis as well as the individual case was used. It was also based on interview, observation, and interpretation of the patient’s medical records. The study was conducted in September 2018 during a home visit as part of nursing care for a mechanically ventilated patient. The patient was diagnosed. Eight nursing diagnoses were formulated. These included: apnoea, impaired airway patency, abdominal pain, weakness, underweight, disturbed sleep, lack of appetite, and fatigue. The International Classification of Nursing Practice ICNP ® is a huge source of terms that facilitate, speedup, and make the nurse’s work more effective. It enables the documenting and describing of specific diagnoses, interventions, and results selected and applied to them [6]. The use of an appropriate diet and pharmacotherapy proved to be effective, which improved the patient’s condition and allowed her to return to normal functioning.

Clostridioides difficile infection in intensive care units in nursing practice 重症监护病房难辨梭菌感染的护理实践

Introduction: Patients hospitalised in intensive care units (ICU) are patients in severe health, who are at risk of serious hospital infections. Despite the development of medicine, developed procedures, management strategies, and high awareness of medical personnel, hospital infections are still a very big problem. One of the serious hospital infections is Clostridioides difficile , especially in ICU patients, which can even be fatal. The severity of the infection and its complications, which hamper treatment and care, are a challenge for the entire therapeutic team. At the same time, this infection is a major epidemiological threat and increases the cost of treatment. That is why the diagnosis of infection, the application of the recommended treatment, as well as the correct prevention are so important. The basis for preventing the spread of C. difficile infection is hand hygiene, the use of personal protective equipment, patient isolation, maintenance of disinfection, infected patient care, education of visitors, and monitoring and recording of infections. Aim of the study: Discussing current data on treatment in the presence of C. difficile infection and describing problems faced by ICU medical personnel. Conclusions: Following current recommendations for diagnosis and treatment and applying appropriate control procedures and methods contributes to preventing the spread of C. difficile infection in intensive care units. The use of board games in healthcare teaching 棋盘游戏在医疗教学中的应用

Modern healthcare education has occurred in an era of great challenges, but also of great opportunities. Learners have become accustomed to being bombarded by multiple stimuli to the point where traditional education often seems boring and monotonous. For this reason, modern teaching methods based on information technology are increasingly being used. Gamification, the use of game elements in a non-entertainment context, is becoming increasingly popular in education. These include board games. The literature on board games highlights many areas in which board games can be effective within healthcare settings. Board games differ from electronic ones primarily because of the element of social interaction, although some electronic games exist in the form of puzzles or competition with the game mechanism itself. The biggest advantages of board games include improving communication and active learning based on interaction with other players. Board games cannot substitute traditional teaching, but they can help students to master particularly difficult concepts. Games help to organise knowledge, engage and motivate students to learn difficult material, but also encourage students to take responsibility for their learning. Games in medical education, when properly used, can be valuable tools for an instructor. They create a non-threat-ening environment that supports mutual learning. A properly prepared game, tailored to the subject, allows mastery of the material in less time than in a traditional lecture.

Board games cannot substitute traditional teaching, but they can help students to master particularly difficult concepts. Games help to organise knowledge, engage and motivate students to learn difficult material, but also encourage students to take responsibility for their learning. Games in medical education, when properly used, can be valuable tools for an instructor. They create a non-threat-ening environment that supports mutual learning. Quality of life of adults with celiac disease 患有乳糜泻的成年人的生活质量

Introduction: Celiac disease is a food intolerance leading to problems with digestion and absorption of nutrients. It modifies the assessment of quality of life, but its overall evaluation should include sociodemographic factors and received health treatment. Aim of the study: To assess the quality of life of patients with celiac disease and to establish its determinants. Material and methods: A total of 165 adults with celiac disease participated in the study. The authors’ questionnaire and the Gastrointestinal Quality of Life Index (GIQLI) questionnaire were used. Statistical analysis was performed using IBM SPSS Statistics 25, and the significance level was p < 0.05. Results: The respondents rated their quality of life in the social dimension the highest, and in the physical dimension the lowest. Age, marital status, and professional activity were not related to the overall assessment of the quality of life. This rating was higher in the group of men, residents of large cities, and people with higher education. The duration of celiac disease did not significantly affect the quality of life. Respondents without symptoms of the disease or with the occurrence of 1 or 2 of them assessed their quality of life better. Comorbidities significantly reduced quality of life. Respondents describing the support received from professionals as bad had lower quality of life in terms of physical and social functioning. Conclusions: Treatment of celiac patients requires professionals providing care to implement interventions in order to improve patients’ assessment of quality of life.
