Stéphanie Larchanché is a medical anthropologist and therapist who specializes in the study of cultural representations of illness and care strategies in a migratory context, particularly in mental health. Currently a consultant, independent trainer and head of research at Sigmund-Freud University-Paris, she has, for twelve years, directed training, teaching and research activities at the Françoise-Minkowska medical-psychological center in Paris. She is also an administrator of the ISM-Interpretariat association, and has a pertinent view of the role played by the interpreter with the health professional and his patient. The following interview took place before she left the Minkowska Center.

The care of unaccompanied young exiles in public health care facilities for adolescents often requires teams to adapt their capacities for institutional containment. We describe the case of a 16 year old female migrant adolescent, whose follow-up occurred at the Maison des adolescents of the Cochin Hospital, with several healthcare workers involved. The healthcare team had to reflect on the meaning of her somatic symptoms and why the referring adults were so worried (such as fear of death). We describe how we articulated somatic and psychological care for this adolescent girl living in a precarious situation.

The context of containment due to the Covid-19 epidemic forced professionals to suspend their face-to-face therapeutic education programs. For young patients with asthma, the situation was made even more complex by anxiety-provoking communications (which turned out to be inaccurate) about the possible aggravating role of corticosteroids in the event of Covid-19, which led to untimely discontinuations and sometimes to a decrease in their therapeutic adherence, exposing them to an increased risk of poor control of their disease. Faced with the feeling of abandonment felt by some families in this singular context, a team at Trousseau Hospital in Paris decided to rethink and adapt its distance therapeutic education workshops.

The majority of psychoanalysts today admit that the body and the mind are not separate, but are on the contrary deeply intertwined with each other. Thus, it is possible to consider the psychic life as stemming from the vital necessity to process sensory information. Based on the recent discoveries of embryology, it becomes possible to rethink the consequences of the first inscriptions in the construction of the psyche, in connection with sensory experiences. This new knowledge makes it necessary to revise the metapsychology, making the first impulse appear as "tact-pulsion".
