Since 2009, the law has reinforced the role of the general practitioner in coordinating care in France. Various structures, such as coordination support systems and specific regional systems, have been set up to improve the efficiency of care pathways. Primary care teams and specialized care teams organize care around patients. Multi-professional health centers and health centers facilitate local collaboration between professionals.

In recent years, our healthcare system has been changing, with medicine becoming increasingly modern and technical. As a student nurse, you begin to build your professional identity. Like all individuals, they are nourished by different representations, which will play a part in this development. Professional identity is in a constant state of flux, influenced as it is by the vagaries of personal and professional life. The study presented below sought to identify the representations of the profession and the founding elements of identity construction among nurses at the start of their studies.

In a weakened healthcare system, the mental health of healthcare workers has become a priority issue. The aim of this longitudinal study was to assess the mental health of primary care nurses during the Covid-19 pandemic, using a postal survey. Four validated self-report questionnaires were used. The nurses reported mainly burnout symptoms and around 20% reported a post-traumatic stress.

Since 2021, the Pévèle Douaisis territorial professional health community has set up an assessment and support pathway for learning disorders in children and adolescents. The aim is to support young people with dyslexia in developing their care pathway, in order to facilitate their care and schooling via the implementation of educational support. The program is managed by an advanced practice nurse, whose role is to carry out a holistic assessment of the situation.

Interconnection between hospital and outpatient care is crucial to enhancing continuity of care for patients, particularly those with chronic illnesses. The systems currently in place in France, such as territorial hospital groups, the return-to-home program, the mobile palliative care teams, the shared medical record and territorial healthcare professional communities, are meeting the main challenges associated with this coordination. Their aim is to optimize care trajectories, reduce re-hospitalization and improve the quality of care.

This can be achieved by: allowing longer consultation times; implementing non-drug interventions; prescribing only drugs with proven benefits; replacing inhalers with dry powders; and anchoring action in a circumscribed territory.

This care-integrated practice requires professionals to adapt their practices and adopt an educational posture, defined as the cornerstone of TPE. The notion of an educational posture can be complex to implement, and needs to be clarified in its various dimensions.
