A patient with duodenal and liver metastases was admitted to FAHZU in December, 2019, and was received pancreaticoduodenectomy (PD), autologous small intestine transplantation (SIT), and resection of hepatic nodules. It is notable that he had a history of colon cancer. This article summarizes experience of nursing care, including sufficient preoperative evaluation, professional nursing and psychological support, preventive nursing of postoperative complications, observation of blood supply of transplanted small intestine, nursing of pain, nursing of anticoagulation to prevent thrombosis. The patient recovered well and was discharged smoothly and seamlessly.

Ventilator-associated pneumonia (VAP) refers to the infection of lung parenchyma in patients undergoing endotracheal intubation or tracheotomy after receiving mechanical ventilation for 48 h, which is the most common complication of patients undergoing mechanical ventilation and has a high mortality rate (21.2%-43.2%). At present, the main measures to prevent and control VAP are to strictly implement hand hygiene, strengthen oral care, raise the bedside, effectively remove secretions from the airbag, implement airbag management and ventilator circuit management, carry out enteral nutrition and cluster intervention strategies according to the actual situation of hospitals. This article reviews the characteristics, diagnosis, prevention and control of VAP, aiming to provide reference for better prevention and control of VAP.

Pemphigus is a stubborn and an autoimmune dermatology disease which causes skin erosion, with the characteristics of relapse chances, leading to a long-time suffers of patients. Currently, pemphigus is mainly treated by blood transfusion, and correction of water and electrolyte balance, etc. In treating process, non-expected conditions may occur, such as complications and uncontrollable illness. It would be difficult to achieve satisfactory curative effect only by such treatment. Therefore, it is critical to monitor and nurse patient’s clinical conditions. In view of this, this article explores the nursing methods of patients with severe pemphigus, hoping to improve the treatment effect through clinical observation and nursing intervention.

Background: At normal situation, vaginal delivery is recommended to pregnant females who are at low risk of complications. Recently, there is a widespread discussion about the right of the women in choosing their childbirth methods, specifically cesarean section. Aim: We designated to critically discuss the ethical argument on female’s choice of childbirth method as well as the influential factors involved in their decision-making process. Methods: A comprehensive discussion relying on systematic literature reviews was conducted to address the ethics, policies, safety, and suggestions for the choice of childbirth approach. Findings: Most women would like to have the freedom in selecting the delivery method, whereas the decision-making process is complicated and multifactorial and needs to coordinate with safety issues, opinions from family members, and recommendations from doctors. Discussion: No evidences could define the safest fetal delivery method. However, in consideration of both the equality and ethical principle, obstetricians should respect the women’s autonomy while obligate to refuse their requests to lower the risk of both the mother and infant. The establishment of trust, exchange of information, fully consideration of all factors, and legal protection can ensure doctors helping the mothers to make the best decision for childbirth approach. Conclusion: The decision-making process for determining the childbirth method depends on the balance among females’ autonomy, family’s suggestions, obstetrician’s obligation, and legal protection of the doctor.

Objective: this article aims to explore the effect of comfortable nursing in ICU patients with severe infection. Methods: from January 2018 to January 2019, 44 patients with severe infection were selected as research objects. They were randomly divided into observation group (n=22) and control group (n=22). The control group was given routine nursing, while the observation group was given comfortable nursing on the basis of the control group. Pittsburgh Sleep Quality Index (PSQ1), Self-rating Anxiety Scale (SAS) and the incidence of adverse reactions were observed and compared between the two groups before and after intervention. Results: the PSQI score and SAS score of the patients in the observation group were lower than those in the control group. The incidence of adverse reactions in the observation group was 4.59%, which was significantly lower than that in the control group (27.27%), and the difference between the two groups was statistically significant (P<0.05). Conclusion: comfortable nursing intervention for ICU patients with severe infection can obviously improve their sleep quality and negative emotions, and reduce the incidence of adverse reactions during hospitalization, which has clinical application value.
