Targeting Caregiver Psychopathology in Parent Management Training for Adolescents: A Scoping Review of Commercially Available Treatment Resources. 在青少年家长管理培训中针对照顾者的心理病理学:对市售治疗资源的范围审查。

Background: While parent management training (PMT) has been shown to be an effective treatment for adolescents with externalizing concerns, evidence suggests that effectiveness is not equitable across all types of families. Research suggests that caregiver psychopathology may adversely affect PMT success for adolescents. However, it remains unclear whether research on caregiver psychopathology is integrated within adolescent PMT resources (e.g., treatment manuals).

Objective: A scoping review of commercially available, clinician focused PMT resources was conducted to assess for information on caregiver psychopathology and clinical guidance for addressing caregiver psychopathology.

Methods: A search for commercially available resources was conducted among national treatment databases and book resource websites. Information on caregiver psychopathology was extracted from individual sessions of the PMT resources.

Results: Seven treatment resources met inclusion criteria. The majority of the treatment resources made at least one explicit comment that caregiver psychopathology may impact the course of treatment. There was very limited mention of caregiver psychopathology in skill building sections of the resources.

Conclusions: While most resources mentioned caregiver psychopathology, these comments lacked breadth and depth in providing clinicians with recommendations on how to tailor treatment to caregivers' needs. Recommendations are provided for how PMT programs might be improved in the future to support clinicians when faced with caregiver psychopathology by recognizing inequities and lack of diversity in resource development, using a transdiagnostic perspective (including a transdiagnostic approach to assessment of care-giver psychopathology), and integrating caregiver skill development.

Influencing factors and clinical significance of liver function damage in patients diagnosed with COVID-19 新冠肺炎患者肝功能损害的影响因素及临床意义

Objective: To invetigate the influencing factors and clinical significance of liver function damage (LFD) in patients diagnosed with Corona Virus Disease 2019 (COVID-19) Methods: The retrospective case-control study was conducted The clinicopathological data of 51 patients with COVID-19 who were admitted to the Sino-French New City Branch of Tongji Hospital Affiliated to Huazhong University of Science and Technology by the 5th group assisting team from the First Hospital of Jilin University from February 9th to 27th in 2020 were collected There were 27 males and 24 females, aged from 36 to 86 years, with an average age of 68 years The treatment modality was according to the diagnostic and therapeutic guideline for COVID-19 (Trial 6th edition) issued by National Health Commission Observation indicators: (1) clinical data of patients;(2) analysis of liver function index and treatment of LFD;(3) analysis of influencing factors for LFD Measurement data with normal distribution were represented as Mean±SD, and measurement data with skewed distribution were described as M (range) Count data were described as absolute numbers or percentages, and comparison between groups was analyzed using the chi-square test The Logistic regression method was used for univariate analysis Results: (1) Clinical data of patients: of the 51 patients, 21 were classified as ordinary type of COVID-19, 19 as severe type and 11 as critical type In terms of medical history, 31 patients suffered from more than or equal to one kind of chronic disease, 20 had no history of chronic disease Thirteen patients had the drinking history and 38 had no drinking history Seven patients were hepatitis positive and 44 were hepatitis negative Five patients had septic shock at admission, 5 had systemic inflammatory response syndrome (SIRS), and 41 had neither shock nor SIRS The body mass index (BMI), time from onset to admission, temperature, heart rate, respiratory rate of the 51 patients were (24±3)kg/m2, (13±5)days, 36 5 ℃ (range, 36 0-38 1 ℃), 82 times/minutes (range, 50-133 times/minutes), 20 times/minutes (range, 12-40 times/minutes) The white blood cell count, level of creatinine, and level of b-type natriuretic peptide within 24 hours after admission were 6 3×109/L [range, (2 2-21 7)×109/L], 75 μmol/L (range, 44-342 μmol/L), 214 ng/L (range, 5-32 407 ng/L) (2) Analysis of liver function index and treatment of LFD: the level of alanine aminotransferase (ALT), aspartate aminotransferase (AST), glutamyl transpeptidase (GGT), alkaline phosphatase (ALP), direct bilirubin (DBil), indirect bilirubin (IBil), activated partial thromboplastin time (APTT) and prothrombin time (PT) were 31 U/L (range, 7-421 U/L), 29 U/L (range, 15-783 U/L), 36 U/L (range, 13-936 U/L), 76 U/L (range, 41-321 U/L), 4 9 μmol/L (range, 2 6-14 3 μmol/L), 5 8 μmol/L (range, 2 6-23 9 μmol/L), 37 2 s (range, 30 9-77 1 s), 13 9 s (range, 12 5-26 7 s), respectively The percentages of cases with abnormal ALT, AST, GGT, ALP, DBi

Objective: To investigate the clinical value of outpatient screening in department of general surgery during the Corona Virus Disease 2019 (COVID-19) outbreak Methods: The retrospective and descriptive study was conducted The clinical data of 57 patients who visited surgery clinic and emergency department of Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology between February 1st and 26th in 2020 were collected There were 30 males and 27 females, aged (53±16)years, with a range from 17 to 87 years All the 57 patients were measured score of outpatient screening in department of general surgery The score ≥3 indicated high risk and the score 0 05) The 26 confirmed or suspected cases of COVID-19 had an age of (57±16)years, and 31 non-infection cases had an age of (50±16) years, with no significant difference between the two groups (t=-1 646, P>0 05) (2) Score of outpatient screening for COVID-19 of patients: the score of outpatient screening for COVID-19 of the 26 confirmed or suspected cases was 3 0(4 0), versus 1 0(1 0) of the 31 non-infection cases, showing a significant difference between the two groups (Z=-3 695, P<0 05) There were 17 and 9 of the 26 confirmed or suspected cases with high risks and low risks, respectively, versus 3 and 28 of the 31 non-infection cases, with a significant difference between the two groups (χ2=19 266, P<0 05) Conclusion: During the COVID-19 outbreak, outpatient screening in department of general surgery can effectively screen out high-risk patients Copyright © 2020 by the Chinese Medical Association

Emergency surgical strategies for patients with acute abdomen during the COVID-19 outbreak 新冠肺炎爆发期间急腹症患者的紧急手术策略

Objective: To investigate the emergency surgical strategies for patients with acute abdomen during the Corona Virus Disease 2019 (COVID-19) outbreak Methods: The retrospective and descriptive study was conducted The clinical data of 20 patients with acute abdomen who were admitted to the Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology between January 18, 2020 and February 10, 2020 were collected There were 13 males and 7 females, aged from 25 to 82 years, with an average age of 57 years All the patients with emergency surgeries received pulmonary computed tomography (CT) examination before surgery, and completed nucleic acid detection in throat swab if necessary Patients excluded from COVID-19 underwent regular anesthesia, suspected and confirmed cases were selected a proper anesthesia based on their medical condition and surgical procedure Patients excluded from COVID-19 underwent emergency surgeries following the regular procedure, suspected and confirmed cases underwent emergency surgeries following the three-grade protection Observation indicators: (1) surgical situations;(2) postoperative situations Measurement data with normal distribution were represented as average (range) Count data were described as absolute numbers Results: (1) Surgical situations: of the 20 patients with acute abdomen, 16 patients were excluded from COVID-19, and 4 were not excluded All the 20 patients underwent emergency abdominal surgeries successfully, of whom 2 received surgeries under epidural anesthesia (including 1 with open appendectomy, 1 with open repair of duodenal bulbar perforation), 18 received surgeries under general anesthesia (including 9 with laparoscopic repair of duodenal bulbar perforation, 3 with open partial enterectomy, 3 with laparoscopic appendectomy, 1 with laparoscopic left hemicolectomy, 1 with laparoscopic right hemicolectomy, 1 with cholecystostomy) The operation time of patients was 32-194 minutes, with an average time of 85 minutes The volume of intraoperative blood loss was 50-400 mL, with an average volume of 68 mL (2) Postoperative situations: 16 patients excluded from COVID-19 preopratively were treated in the private general ward postoperatively One of the 16 patients had fever at the postoperative 5th day and was highly suspected of COVID-19 after an emergency follow-up of pulmonary CT showing multiple ground-glass changes in the lungs The patient was promptly transferred to the isolation ward for treatment, and results of nucleic acid detection in throat swab showed double positive Medical history described by the patient showed that the patient and family members were residents of Wuhan who were not isolated at home during the epidemic There was no way to confirm whether they had a history of exposure to patients with COVID-19 Medical staffs involved in this case did not show COVID-19 related symptoms during 14 days of medical observation The other 15 patients recovered well
目的:探讨2019冠状病毒病(新冠肺炎)暴发期间急腹症患者的急诊手术策略。方法:采用回顾性和描述性研究方法,对华中科技大学同济医学院附属协和医院收治的20例急腹症病人的临床资料进行回顾性分析收集2020年1月18日和2020年2月10日,男13例,女7例,年龄25~82岁,平均57岁。所有急诊手术患者术前均接受肺部计算机断层扫描(CT)检查,必要时完成咽拭子核酸检测,根据疑似和确诊病例的医疗状况和手术程序选择合适的麻醉方法排除新冠肺炎的患者按照常规程序进行紧急手术,疑似和确诊患者按照三级保护进行紧急手术观察指标:(1)手术情况;(2) 术后情况测量数据以平均值(范围)表示,呈正态分布。计数数据以绝对数表示。结果:(1)手术情况:20例急腹症患者中,16例排除新冠肺炎,4例未排除,其中硬膜外麻醉手术2例(其中阑尾切除术1例、十二指肠球部穿孔开放修补术1例),18例患者在全麻下接受手术(其中腹腔镜十二指肠球部穿孔修补术9例,开放性肠部分切除术3例,腹腔镜阑尾切除术3例行,腹腔镜左半结肠切除术1例,腹腔镜右半结肠切除手术1例,胆囊造口术1例),手术时间32-194分钟,平均85分钟。术中失血量为50-400mL,平均容量为68 mL(2)术后情况:16名术前排除在新冠肺炎之外的患者术后在私人普通病房接受治疗。16名患者中的一名在术后第5天发烧,在肺部CT紧急随访显示肺部多处地面变化后,高度怀疑为COVID-19转入隔离病房治疗,咽拭子核酸检测结果为双阳性患者所述病史显示患者及其家属为疫情期间未在家隔离的武汉居民无法确认其是否有新冠肺炎患者接触史该病例涉及的医务人员未显示新冠肺炎相关14天医学观察期间的症状其他15例患者术后恢复良好根据病史和肺部CT检查结果,术前未排除新冠肺炎的4例患者,术后直接转入隔离病房治疗,连续两次核酸阴性结果排除新冠肺炎20例急腹症患者中有2例有术后并发症,1例有手术切口感染,经切开、消毒、包扎二次闭合后痊愈,另一例出现肠漏,经腹部引流保守治疗后病情好转。20例急腹症患者无死亡。结论:急腹症需要通过急诊筛查排除在新冠肺炎之外的患者按照常规程序进行急诊手术,未排除在新冠肺炎之外的患者在三级防护后接受紧急手术。术后进行体温、血常规等实验室检查以监测患者,必要时应进行肺部CT和咽喉核酸检测。术前排除在新冠肺炎之外的患者术后在私人普通病房接受治疗,确诊后应及时转入隔离病房治疗术前根据病史未排除新冠肺炎的患者,术后应直接转入隔离病房进行治疗版权所有©2020中华医学会
