Abstract: Objective: Covid-19 pandemic has raised concerns about how to deal with the psychiatric aspects of the pandemic and it was a matter of curiosity how the perceptions of individuals would be who already had a psychiatric diagnosis, especially a vulnerable diagnosis of Generalized Anxiety Disorder (GAD). We aimed to explore the perceptions about Covid-19 pandemic in a sample of individuals with the diagnosis of GAD. Methods: Qualitative data obtained using a semi-structured interview questionnaire by a psychiatrist with 33 individuals diagnosed with GAD. Results: 14(42.4%) of the participants were male, 19(57.6%) were female. Average age was 34.7. 7(21.2%) participants had Covid-19 infection. Three main themes were determined as " learning from the pandemic process", "impact of the pandemic process on changes in personal development", "effects on the psychiatric treatment process". Background: Mean platelet volume to platelet count ratio can be a new marker of mortality in critical COVID-19 cases in intensive care unit to retrospectively examine the factors predicting death rate in COVID-19 cases. Methods: 106 patients infected with COVID-19 in intensive care unit were enrolled in this research. The patients’ hospital records and the patient management devices were thoroughly examined. Results: The Interleukin-6, C-reactive protein, procalcitonin, leukocyte count, neutrophil count, neutrophil %, neutrophil/lymphocyte ratio, and neutrophil/albumin ratio were significantly higher among nonsurvivors (respective results are as follows: p=0.0001, p=0.004, p=0.003, p=0.049, p=0.007, p=0.009, p=0.007 and p=0.0001). While the survivors had lower widths of platelet distribution and red blood cell distribution, as well as mean platelet volume and mean platelet volume to platelet count ratio (respective results are as follows: p=0.016, p=0.03, p=0.005 and p=0.049), hemoglobin, platelet, mean corpuscular hemoglobin concentration, lymphocyte % ve monocyte % were higher (respective results are as follows: p=0.022, p=0.033, p=0.042, p=0.008 and p=0.04). In the logistic regression, five features -including high levels of C-reactive protein, procalcitonin, pro-brain natriuretic peptide, mean platelet volum to platelet count ratio, and low level of platelet were shown as mortality predictors for COVID-19 patients in intensive care unit (respective results are as follows: p=0.045, p=0.025, p=0.017, p=0.027 and p=0.041). ABSTRACT Aim: Loss of balance can be seen in idiopathic Parkinson's disease (IPD) There are only a few studies in the literature in which brainstem involvement in IPD has been researched with neurophysiological tests such as vestibular evoked myogenic potential (VEMP). In this study, it was investigated whether there is a difference in the results of VEMP testing in early or late stage of IPD. Material and method: The IPD cases were classified as early stage and late stage according to the Hoehn-Yahr scale. The presence of a positive wave with a latency of P13 and a negative wave with a latency of N23 was investigated as the first reflex response The latencies of these potentials and the absolute amplitude of the P13-N23 component were measured. The VEMP results of the patients with early and late stage IPD were compared with those of the control group. Results: The right P13 latency mean value in the late stage patient group was significantly prolonged than in the early stage patient group and the control group. The right P13-N23 amplitude mean value of the late and early stage patient groups was significantly smaller than that of the control group (p < 0.002 and p < 0.001, respectively). Among the patients with IPD, the P13 latency was statistically increased in those with a fall history than in those without a fall history. In conclusion, this study indicates that the VEMP pathway is affected over time especially in patients with late stage Parkinson’s disease.

It is known that especially glomerular side effects of lithium lead to serious consequences such as end-stage renal disease. Therefore, it is critical to evaluate patients on long-term lithium therapy for glomerular pathologies. The present study investigated the changes in renal functions, prevalence of renal failure and progress in patients have been followed up for at least six years with bipolar disorders (BD) and on regular lithium treatment. 51 patients with BD and 38 age and sex matched healthy controls were enrolled for the study. The serum blood urea nitrogen (BUN), creatinine, uric acid, electrolytes, calcium (Ca), phosphorus (P), vitamin D (25-OH D3), parathyroid hormone (PTH) and eGFR levels were measured to compare the kidney functions of patients and control group. The relation between the renal functions and mean serum lithium levels and duration of lithium treatment were also investigated. Mean eGFR level, 25-OH D3 and urine density of patients with were significantly lower whereas creatinine, uric acid, Ca and PTH were significantly higher than that of controls.. The duration of lithium treatment and mean lithium levels were negatively correlated with eGFR level. Eight of 51 patients have critical eGFR level as lover than 60ml/minute thus further nephrological investigation was needed. The study revealed that the renal functions of the patient group was significantly lower than controls. The findings suggested that both duration of lithium treatment and high serum lithium levels may have a negative impact on renal functions. These findings suggest that it is important to clarify the response type to lithium in patients who are on long term treatment with lithium and maintain the treatment with the lowest possible therapeutic serum levels and carefully monitoring the renal functions in patients with good response to lithium.

Aim: Cystatin C is a cysteine protease inhibitor that has been shown to have antiviral, antibacterial and neuroprotective efficacy. The aim of this study is to assess the effect of levetiracetam monotherapy on serum cystatin C levels and the neuroprotective efficacy thereof in patients with epilepsy. Material and Methods: 30 patients, who were diagnosed with epilepsy for the first time and subsequently started on levetiracetam monotherapy, were included in the study as the study group, whereas 30 healthy volunteers were included in the study as the control group. Serum cystatin C and creatinine levels of patients were measured twice, once before they were started on the levetiracetam treatment and once after the completion of six months of treatment levetiracetam, whereas the serum cystatin C and creatinine levels of the healthy control subjects were measured once. Results: Both the pre-treatment and post-treatment creatinine levels of epilepsy patients were found to be statistically significantly higher compared to the creatinine levels of the healthy control subjects. The pre-treatment and post-treatment serum cystatin C levels of epilepsy patients were found to be lower compared to the serum cystatin C levels of the healthy control subjects, albeit not statistically significantly. Additionally, serum cystatin C levels of epilepsy patients were found to have increased after the completion of the levetiracetam treatment, even though not statistically significantly. Conclusion: The increase observed in the cystatin C levels, which is a neuroprotective agent, in epilepsy patients following the completion of the levetiracetam treatment indicates that levetiracetam has neuroprotective activity.

The pre-treatment and post-treatment serum cystatin C levels of epilepsy patients were found to be lower compared to the serum cystatin C levels of the healthy control subjects, albeit not statistically significantly. When the Mediterranean diet is set in focus, scientific studies report a strong statistical correlation between human nutrition, diet, and cancer incidence. Considering the anticancer effects of a fiber-rich diet, it is understood that the anticancer effect is not only due to the bulk cellulosic material load but also related to the increased bioavailability of cellulose-bound bioactive (anticancer) compounds released due to intestinal microfloral activities. Ferulic acid (FA) is one of the components found ubiquitously in the fiber fraction of plant food. Because of its effects on cancer cell viability and its association with a low incidence of cancer concerning a high-fiber diet, FA can be considered an anticancer agent. Here in this work, it was investigated whether FA can potentiate the effects of anticancer drugs at lower doses. For this, a general anticancer drug named 5-Fluorouracil (5-FU) was used, and potentiation tests were performed on two cancer cell lines, namely A2780 besides HT-29, which has the homozygous mutation for p53. The results are interpreted as follows: anticancer effect of 5-FU was readily potentiated with 200 µM of FA in both cancer cell lines, and DNA damage-induced with 5-FU was enhanced with co-administration of FA. When cell viability and DNA damage of A2780 and HT-29 lines are evaluated together, we think it is most probable that 5-FU and FA administered jointly show its anticancer effect, especially by strengthening the apoptosis pathway triggered by DNA damage. If it might be possible to uncover the mechanism that drove DNA damage mediated apoptosis in p53 mutant HT-29 cells in our work we may shed light on the treatment of chemotherapy-resistant cancer incidences.

Objectives The use of liquid chromatography with tandem mass spectrometry (LC-MS/MS) device has increased significantly in toxicology validation laboratories in recent years. The maintenance cost of this expensive equipment is high as well as the cost of consumables. Reduction of the matrix effect and preparation more clear samples are very important for the validation of methods in clinical laboratories. The primary goal is to create a cleaner urine sample preparation technique to reduce the cost of maintenance of the LC-MS/MS device without affecting test results. Methods We prepared the patients’ urine in two different ways; routine urine preparation method and used our centrifuged method (14000 rpm, 10 minutes) for routine illicit substance use. The standard material used to determine whether there was a statistical difference in the urine sample with both different methods was added to both urine samples. Results Our findings showed that there was no statistical difference between the results of both methods for detection of illicit substance use. SARS-CoV-2 virus is a new and highly contagious respiratory virus that transmission by droplet route. Here we reported international travel-related SARS CoV-2 transmission, and then cause an outbreak by the wedding ceremony. Two index passenger groups include three families who come from Paris to İstanbul to attend the wedding ceremony transmitted COVID-19 infection to Turkey. During the wedding ceremony and later period, they were in close contact with about 350 persons. Clinical pictures COVID-19 infection occurred in 53 relatives. PCR positivity was found in 35 hospitalized cases. The infection spread to four cities in Turkey. The most frequent symptoms of 35 confirmed cases were dizziness (77%), fever (57.1%), joint and muscle pain (57.1%), loss of smell (45.7%) and loss of taste (42.9%), and shore throat (37.1%), dry cough (34.3%), diarrhea (25.7%), rhinorrhea (14.3%) and dyspnea (8.5%). 48 cases (90.5%) were mild-moderate. Severe pneumonia was developed, intensive care and intubation need in five cases (9.4%), and four of them were died (7.5%). COVID-19 virus can be easily acquired during plane travel and transmitted to other persons with unprotected contact. The infection prevention rules should be strictly applied for the protection from disease. Persons should avoid attend gatherings, even with their family or relatives, and should stay at home. The wedding and other social activities should be postponed after the pandemic.
