Clinical Characteristics of Human Parvovirus B19 Infection in Children 儿童人细小病毒B19感染的临床特点

Purpose: Human parvovirus B19 infection is widespread and has a heterogeneous clinical spectrum, ranging from asymptomatic infection to potentially life-threatening complications. We investigated the various clinical features of human parvovirus B19 infection during an outbreak of the virus in our community. Methods: A retrospective chart review study was conducted at the Pusan National University Children's Hospital from December 2017 to April 2019. We investigated the clinical features of children with parvovirus B19 immunoglobulin M or parvovirus B19 DNA detected using polymerase chain reaction. Results: A total of 24 children were diagnosed with parvovirus B19 infection. Twelve (50%) had lace form rashes, and four (16.7%) had petechial rashes. Two (8.3%) were diagnosed with fever without a focus. Six (25%) developed aplastic crisis as a complication of infection, of whom three were previously diagnosed with hereditary spherocytosis and three with acute lymphoblastic leukemia. Conclusions: In addition to erythema infectiosum, the parvovirus B19 infection can present clinically with various types of rashes and fever without a focus. Furthermore, hematologic manifestations such as neutropenia and aplastic crisis can occur during infection.

Human Parechovirus: an Emerging Cause of Sepsis-Like Syndrome in Infants Aged under 3 Months 人类Parechovirus: 3个月以下婴儿败血症样综合征的新病因

Purpose: This study aimed to investigate the clinical characteristics of human parechovirus (HPeV) infection in sepsis-like syndrome in infants aged under 3 months. Methods: Medical records of infants aged under 3 months with sepsis-like symptoms who were admitted between July 1, 2018 and August 31, 2018 were reviewed. A multiplex reverse transcription-polymerase chain reaction panel test was performed on the cerebrospinal fluid (CSF). Thirty-nine enrolled infants were categorized into three groups: 11 in group 1 (HPeV detected in the CSF), 13 in group 2 (enterovirus detected in the CSF), and 15 in group 3 (no virus detected in the CSF). Results: Compared with groups 2 and 3, a higher proportion of group 1 had tachycardia, tachypnea, apnea, and hypotension (P<0.05). A significantly lower white blood cell (WBC) count was noted in group 1 than in groups 2 and 3 (5,622±2,355/μL, 9,397±2,282/μL, and 12,312±7,452/μL, respectively; P=0.005). The CSF WBC count was lower in group 1 than in groups 2 and 3 (0.9±1.7/μL, 85.1±163.6/μL, and 3.7±6.9/μL, respectively; P=0.068). The proportion of patients requiring inotrope support (36.6% vs. 0% and 6.6%), mechanical ventilation (18.1% vs. 0% and 0%), and high flow nasal cannula (45.4% vs. 15.3% and 6.6%) was higher in group 1 than in groups 2 and 3. All patients recovered completely without complications. Conclusions: HPeV infection shows a severe clinical course and can cause a severe sepsis-like syndrome in infants aged under 3 months. Early diagnosis and proper treatment of HPeV infection are required.

Epidemiological Characteristics of Influenza in Children during the 2017–2018 and 2018–2019 Influenza Seasons in Jeju, Korea 韩国济州2017-2018年和2018-2019年流感季节儿童流感流行病学特征

목적: 제주 지역에서 2017-2018 절기와 2018-2019 절기에 소아 연령에서 인플루엔자의 역학에 대해 알아보고자 하였다. 이에 더하여 국내 인플루엔자 표본감시체계와 비교하여 그 대표성을 확인해보고자 하였다.방법: 2017-2018, 2018-2019 각각의 절기에 제주대학교 병원에 방문하여 인플루엔자 검사를 시행 받은 13세 미만의 소아를 대상으로 하였다. 의무기록을 통하여 대상 환자들의 인구학적 자료, 인플루엔자 검사 결과를 후향적으로 분석하였다.결과: 총 5,219명의 인플루엔자 의사환자가 연구 대상자로 포함되었다. 전체적으로 연구 대상자의 평균 나이는 2.85±2.79세였고, 두 절기 모두 인플루엔자 의사환자는 1세 연령 그룹이 가장 많았다. 인플루엔자 감염이 확인된 소아는 총 902 (17.3%)명이었다. 2017-2018 절기에는 인플루엔자 A형의 양성률은 10.4% (236/2,279), 인플루엔자 B형의 양성률은 9.1% (208/2,279)이었다. 2018-2019 절기에는 인플루엔자 A형의 양성률은 10.3% (303/2,940), 인플루엔자 B형의 양성률은 5.3% (155/2,940)이었다. 인플루엔자 환자의 평균 연령은 2017-2018 절기에는 4.09세, 2018-2019 절기에는 5.05세로 통계적으로 유의한 차이를 보였다 (P<0.05). 인플루엔자의 주별 분포는 국내 임상감시시스템의 인플루엔자 의사환자와 유사한 형태로 나타났다.결론: 제주 지역에서 2017-2018 절기와 2018-2019 절기 사이에 인플루엔자 유행 양상과 연령 분포의 뚜렷한 차이를 보였다. 국내 다른 지역과의 역학적 특성과 비교하여 제주 지역 고유의 인플루엔자 역학에 대한 지속적인 연구가 필요하다.

Aspergillus Laryngotracheobronchitis in a Child with Primary Immunodeficiency 原发性免疫缺陷儿童的曲霉菌性喉气管支气管炎

Laryngotracheobronchitis (LTB) is a common disease in the pediatric population, and it is rarely caused by a fungal infection. Acute respiratory failure caused by fungal LTB mainly occurs in immunocompromised patients, and early diagnosis is closely associated with morbidity and mortality. However, an appropriate diagnosis is challenging for pediatricians because symptoms and signs of LTB caused by Aspergillus spp. are nonspecific. Here, we report a case of progressive respiratory failure caused by pseudomembranous LTB in a child with a suspicion of primary immunodeficiency and highlight the importance of an early investigation, especially in immunocompromised patients.

Children with COVID-19 after Reopening of Schools, South Korea 韩国,学校开学后感染COVID-19的儿童

Purpose: To describe pediatric coronavirus disease 2019 (COVID-19) cases after the reopening of schools in the Republic of Korea and their transmission routes Methods: All case report forms and epidemiologic investigation forms for children aged 3–18 years reported as COVID-19 cases to the National Notifiable Disease Surveillance System from May 1 to July 12, 2020, were reviewed Results: After the schools were reopened in May 2020, a total of 127 pediatric COVID-19 cases were confirmed until July 12 Of these, 59 children (46%) were exposed to severe acute respiratory syndrome coronavirus 2 through family and relatives, followed by 18 children (14%) through cram schools or private lessons, 8 children (6%) through multi-use facilities, and 3 children (2%) through school Conclusions: The present data do not suggest an increased risk of COVID-19 transmission in the context of stringent school-based infection prevention measures introduced across the country © 2020 The Korean Society of Pediatric Infectious Diseases

Purpose: This pilot study aimed to evaluate the efficacy of high-dose ampicillin/-sulbactam and colistin combination therapy for treating ventilator-associated pneumonia (VAP) caused by carbapenem-resistant Acinetobacter baumannii (CRAB) in the pediatric intensive care unit of Pusan National University Children's Hospital. Methods: We retrospectively reviewed 17 pediatric patients VAP with caused by CRAB from June 2017 to August 2018. 10 (58.8%) patients were treated with high dose ampicillinsulbactam and colistin combination (combination therapy group), and 7 were treated with colistin only, or various combination of with or without colistin (other antibiotics group). Clinical and bacteriological outcome were compared between groups. Results: The mean duration of fever after antibiotic use was 1.30±1.70 days in the combination therapy group and 1.71±1.49 days in the other antibiotics group. 목적: 부모의 소아 자녀에 대한 인플루엔자 및 인플루엔자 백신 관련한 인식도 차이 및 변화를 홍보물 교육 유무에 따라 비교 분석하였다.방법: 2015년 서울, 경기 북부 5개 지역 거주자 중 생후 6개월부터 59개월까지의 자녀가 있는 보호자 1,034명을 대상으로, 설문지 조사를 시행하였다. 설문내용은 인플루엔자 및 인플루엔자 백신에 대한 인식 및 변화 및 인플루엔자 예방접종 의향 및 이유 등을 포함하였다. 부모는 3군으로 나누어 교육 유무(A군: 교육받지 않음, B군: 교육받음)에 따른 인식도 차이와 교육 전후(C군)의 인식도 변화를 분석하였다.결과: A군과 B군 간 교육에 따른 인식도 차이 중 의미 있는 항목은 ‘자녀가 고위험군임을 인식함’, ‘독감 예방접종은 효과적’, ‘독감 예방접종은 비쌈’ 등이었다. C군에서 교육 전후에 따른 인식도 변화 중 의미 있는 항목은 ‘자녀가 고위험군임을 인식함’, ‘독감 예방접종은 안전함’, ‘독감 예방접종은 비쌈’, ‘독감 예방접종을 부정적으로 인식함’ 등이었다.결론: 인플루엔자와 인플루엔자 예방접종에 대한 부모의 인식은 홍보물 교육 이후 긍정적인 효과를 보였다.
目的:根据有无宣传物教育,比较分析父母对小儿子女流感及流感疫苗认识的差异及变化。方法:2015年在首尔、京畿北部5个地区居住者中,以1034名有出生6个月到59个月子女的保护者为对象,进行了问卷调查。问卷内容包括对流感及流感疫苗的认识及变化,流感预防接种意向及理由等。父母分为3群,分析有无教育(A群:不受教育,B群:受教育)的认识差异和教育前后(C群)的认识变化。结果:A君和B君之间根据教育的认识差异中有意义的项目是“认识到子女是高危险群”,“流感预防接种是有效的”,“流感预防接种是预防接种”等。在C君教育前后的认识变化中,有意义的项目有“认识到子女是高危险群”、“流感预防接种是安全的”、“流感预防接种是预防接种”、“对流感预防接种是否定的”等。结论:父母对甲型h1n1流感预防接种的认识在宣传物教育之后取得了积极的效果。

2019년 12월 발견되어 2020년 3월 11일 전 세계 대유행이 선포된 코로나바이러스감염증-2019 (coronavirus disease-2019, COVID-19)는 신종 감염병으로서 수많은 감염사례와 사망을 초래하여 사회 전반적으로 영향을 끼치고 있다. 소아청소년 COVID-19는 발병 환자 중 차지하는 비율이 적으며 성인보다는 경증인 경우가 많으나, 일부에서는 중증으로 진행할 수 있다. 신생아 시기부터 감염될 수 있으며, 지역사회 감염의 전파자로서 역할을 할 수 있을 것으로 보인다. 본 종설에서는 현재까지 소아청소년에서의 COVID-19에 대해 알려진 내용을 정리하여 소아청소년 환자들의 진료에 도움을 주고자 한다.
2019年12月被发现,并于2020年3月11日宣布全球大流行的冠状病毒感染症-2019 (coronavirus disease-2019, COVID-19)是一种新型传染病,造成大量病例和死亡,对整个社会产生影响。少儿和青少年COVID-19在发病患者中所占比率较少,比成人轻微的情况较多,但也有可能发展成重症。从新生儿时期开始就有可能被感染,预计将起到地区社会感染的传播者的作用。本综述将整理目前为止对少儿青少年COVID-19的相关内容,以帮助少儿青少年患者的治疗。
