Objective: the main objective was to identify the ethical conflicts faced by Nursing professionals during the COVID-19 pandemic in Spain in their professional setting.Method: a qualitative study was conducted through ethnography in order to approach the experience lived by Nursing professionals working in Spain during the first wave of the COVID-19 pandemic. Intentional sampling was used to select 33 participants, who were contacted to participate in non-structured online interviews. Data analysis was conducted with a thematic approach which involved the identification of units of meaning and the generation of codes.Results: the study included 29 Nursing professionals, mostly women, working at a hospital. Three main ethical conflicts were identified. The first was related to “Prioritization by age: conflict in equitable treatment”, which came up due to limited resources and difficult decision making. The second conflict was called “Dying alone: a conflict with the humane treatment of patients”, because relatives could not accompany patients at the end of their lives due to visiting restrictions. The third conflict was: “Care with (in)security: the conflict of putting patients, nurses and relatives at risk”, associated with the lack of protection materials, which generated fear of potential contagion both in the professional and the family setting.Conclusion: the identification of these ethical conflicts underlines the need to redirect the ethics of healthcare for future pandemics by health center managers.

Objectives: to understand the differences in surgical results in patients with preoperative anemia treated with Ferric carboxymaltose (FCM) or without iron before surgery.Methods: a retrospective observational study of two patient cohorts: a Group without preoperative FCM (before the implementation of the Surgical Prehabilitation program), and a Group with FCM (after the implementation of the program). A review of electronic clinical records; the outcome variables were the number of transfusions of blood products required after surgery, postsurgical complications, and days of hospital stay. There was a comparison of study variables through Chi square and Student’s t tests; the SPSS statistical program, v. 26, was used.Results: the study included 152 patients, 92 in the FCM Group (2020-2022) and 56 in the Group without FCM (2019); 60.5% (n= 92) were female. Their mean age (standard deviation) was 63.91 (13) years. The surgical procedures included were: colectomy (n= 77; 64.5%), mastectomy (n= 55; 23%) and hysterectomy (n= 20; 12.5%). Statistically significant outcomes were found between the need for blood transfusions (p< 0.001; OR 15.91 (4.44-57.01)), postsurgical complications (p< 0.001; OR 7.36 (3.35-16.16)), and the mean hospital stay (p< 0.001), which was 4.14 days in the FCM group vs. nine days in the control group without FCM.Conclusions: treatment with FCM reduced the number of blood transfusions, postsurgical complications, and days of hospital stay.

Objective: to analyse the usefulness in clinical practice of the CUDOS scale (Clinically Useful Depression Outcome Scale) to assess depressive symptomatology and identify early remission symptoms. Methods: a descriptive longitudinal prospective multicentre study in >18-year-old persons diagnosed with “mood disorder”. The validated versions of CUDOS and EQ-Decentering (the higher the decentering, the lower the depressive symptomatology) were used for data collection; age, gender, level of education, occupational status, civil status, clinical diagnosis and comorbidities were also collected. Multiple regression analysis was conducted with those variables which appeared clinically significant. Finally, the Area under the ROC Curve was calculated, in order to understand the overall performance of the CUDOS scale. Results: the study included 175 patients; almost 60% of the sample suffered Major Depressive Disorder. The initial mean score in CUDOS was 44.5. The factors predicting early remission identified by Logistic Regression Analysis at Day 4 after admission were: “I thought of a hopeless future” and “I can separate my thoughts and feelings from myself” (EQ-Decentering). The ROC curve analysis showed an Area under the ROC Curve of 0.84. Conclusions: CUDOS is a tool sensitive to change within short periods of time. The cognitive area items are the first to change during depression improvement. Pessimism regarding the future and the ability for defusion regarding thought and feelings allow to predict who will improve in their next assessment.

Objective: to assess the psychological impact experienced by the elderly population of a basic health area in Gran Canaria (Spain), associated with the lockdown implemented on the occasion of the COVID-19 pandemic.Methods: a cross-sectional study (November 2021-March 2022). Simple random sample of adults who were 65 years old or older, users of the Telde-San Gregorio basic health area (Gran Canaria, Spain). Telephone surveys were conducted with the Questionnaire on the Psychological Impact of Coronavirus adapted by Sandín et al. (2020). Descriptive analysis with the SPSS statistical program, version 18.Results: the survey included 178 elderly persons (mean age: 74.62 years). Of these, 62.9% (n= 112) was autonomous; 55.1% (n= 98) was in a stable situation according to chronicity classification; 28.1% (n= 50) lived alone; 10.7% (n= 19) had presented COVID-19 symptoms; 84.8% (n= 151) declared that they had not experienced major changes in their routine. The fear to infect a relative/loved one prevailed in 50.6% (n= 90) of elderly persons; 57.9% (n= 103) did not feel any fear/anxiety regarding coronavirus; 55.6% (n= 99) reported not feeling sadness / depression; 6.2% (n= 11) of elderly persons felt lonely / isolated, and 51.1% (n= 91) reported that they had felt upset for not being able to see some of the relatives they usually saw.Conclusions: lockdown did not cause major changes in the routines of participants. There was a high level of compliance with preventive behaviours. More than half of the respondents expressed being very afraid that a relative would get infected. Lockdown due to the COVID-19 pandemic had a certain degree of psychological impact on the health of this age group

Objective: to evaluate the level of learning and the methodology of an academic paper on the design of a local community activity.Method: a mixed evaluative study conducted on Nursing students. After a theoretical introduction on community action, 144 students formed teams and, during two months, conducted a partially real population analysis, article mapping and bibliographic search in order to understand the lifestyles, search and critical analysis of previous community experiences, and design a proposal of activity. Professors graded it through signature (0 to 10 scores), and students were offered an evaluation through four open questions and two closed-ended questions (interest generated and the need to maintain the training activity, Likert scales with min. 1 to max. 5). Descriptive statistical analyses were conducted, as well as content analysis.Results: the activity was conducted by 27 groups, who achieved a median score of 8.17. It was then evaluated by 96 students, who considered that it was interesting (x–= 4.29) and should be maintained (x–= 4.17). They identified as “necessary” and important the health promotion projects in the local community and the work shared with it. They showed that they had discovered another area of professional action. They scarcely showed salutogenic vision and had difficulties to focus the task on a search for bibliography and previous experiences.Conclusion: the activity was successfully conducted by students, and the desired familiarity with the community work was achieved at short term. Students met difficulties to develop the methodology but perceived the activity as interesting and relevant.

Objective: to analyse patient satisfaction regarding care received from the Nursing staff during their admission at a hospitalization unit for acute mental health patients.Methods: an analytical cross-sectional study conducted in 2021 at the hospitalization unit for acute Psychiatry patients at a Vallés Occidental hospital in Catalonia (Spain). The study population were those persons hospitalized for over a week who completed the Caring Assessment Tool CAT-V (range 27-135), in its version translated into Spanish. Besides, the clinical variables of participants were collected: a) type of admission: voluntary / involuntary, b) diagnosis at admission, c) use of mechanical restraint, d) use of toxic substances. There was univariate and comparative analysis of the variables collected.Results: the study included 100 patients, with 42.5 years as mean age (19-72); of these, 55% (p= 55) was female. Satisfaction obtained a median score (P25-P75) of 114 (103-125), and was placed in the fourth quartile. Satisfaction values stayed in the fourth quartile, without showing any statistically significant differences in any of the factors considered, except for those values associated with the use of mechanical restraint, which showed statistical differences (p= 0,023), with a worse perception of satisfaction with care in those for whom it was used, median (P25-P75) 96.5 (87-114), vs. those for whom it was not, 116 (108-125). Even though no significant differences were observed for the rest of variables studied, an increase in satisfaction stood out among voluntary admissions and older persons.Conclusions: patients presented high satisfaction with care received from Nursing staff during their admission at a hospitalization unit for acute mental health patients. The prevalence of the use of mechanical restraints had a negative impact on satisfaction with care received.

Objective: to assess the feasibility of implementing the diary as a humanization tool at a clinical Intensive Care Unit (ICU) in Catalonia, without previous experience, in terms of acceptance, satisfaction and continuity of the measure by Nursing professionals, patients and relatives.Method: a pilot study conducted during 15 weeks with patients hospitalized >72 hours at the ICU, under sedation and with invasive mechanical ventilation for ≥ 48 hours. There were three stages: 1) Training for Nursing staff, 2) Implementation of the diary, and 3) Evaluation of the perception of the tool by the three groups, with anonymous questionnaires designed ad hoc.Results: a printed diary was designed, and Nursing professionals received seven training sessions previously. Eight diaries were written (one per patient), and photographs were inserted in six of them. Questionnaires were handed out to 35 nurses, nine relatives and six patients. 83% of the nurses considered that the diary could be implemented in the future, 83% of patients enjoyed reading a diary about the day to day of their hospital stay, and it helped 89% of relatives to express their thoughts and emotions. 40% of nurses considered that photographs could be upsetting for patients, while the majority of relatives and patients thought otherwise.Conclusions: the conclusion of this pilot study is that it is feasible to implement the diary at the ICU. All participants considered that this is a tool that humanizes care, and improves communication, understanding and information.

espanolObjetivo: conocer que piensan y sienten las personas con esquizofrenia ante la autopercepcion del estigma de su enfermedad. Metodo: se realizo un estudio cualitativo con enfoque fenomenologico. La poblacion de estudio fueron los usuarios del Centro de Dia de la Asociacion Morea (Asociacion de familiares y enfermos mentales) de Ourense. El muestreo fue intencional, seleccionandose siete personas mayores de edad con diagnostico de esquizofrenia (DMS-5), con una evolucion minima de dos anos. Para la recogida de los datos se utilizo entrevista semiestructurada individual (dos entrevistas consecutivas al mismo sujeto), efecuada por dos entrevistadores, que fueron grabadas y transcritas. Se llevo a cabo analisis tematico (identificacion y categorizacion de los principales ejes de significado subyacentes en los datos). Resultados: tras el analisis de las entrevistas se detectaron cinco categorias tematicas: “culpabilidad”, “roles sociales”, “miedo al rechazo y peligrosidad”, “responsabilidad” y “perdida de capacidades y control”. Todos los participantes son conscientes de su enfermedad, de su cronicidad y de que tendran que tomar tratamiento de por vida. Manifiestan haber perdido relaciones con amigos e incluso familia a raiz de su enfermedad. La mayoria percibe que el resto de la poblacion les tiene miedo. Todos consideran que la enfermedad les ha hecho perder capacidades como la memoria, la atencion y la autonomia para algunas facetas de la vida. Conclusiones: las personas con esquizofrenia se sienten estigmatizadas por la sociedad. A colacion de los resultados obtenidos parece necesario aumentar la informacion y educacion en la poblacion para disminuir el estigma y los prejuicios hacia las personas que padecen esquizofrenia y poder asi facilitar su plena integracion en la sociedad. Individual semi-structured interview was used for data collection (two consecutive interviews with the same subject), conducted by two interviewers, which were recorded and transcribed. Thematic analysis was conducted (identification and classification of the main axes of meaning underlying in data). Results: after the analysis of the interviews, five thematic categories were detected: “guilt”, “social roles”, “fear of rejection and danger”, “responsibility” and “loss of abilities and control”.
