Although studies on public health and lifestyle diseases have a long history, they are characterized by the selective nature of the consideration of the issue. In modern algorithms of arterial hypertension treatment, the main strategic approach aimed at increasing the therapy effectiveness is the use of fixed combinations (FC) of two antihypertensive drugs at the start of therapy and of three drugs (preferably in one tablet) when it is necessary to intensify pharmacotherapy. Starting therapy with two drugs in one tablet (FC) is recommended for more than 70 % of patients. The article provides up-to-date information on the choice of a fixed combination for the treatment of patients with arterial hypertension (AH) according to modern national and international recommendations. The place of a fixed combination of ramipril and amlodipine among antihypertensive drugs in achieving the goal of treating patients with arterial hypertension was determined. The advantages of the fixed combination of ramipril and amlodipine due to the wide range of indications and its effectiveness for patients with hypertension in combination with chronic kidney disease (CKD) and metabolic syndrome are discussed. The distinctive properties of the fixed combination of ramipril and amlodipine are emphasized: it allows achieving target values of blood pressure (BP), improves adherence to treatment, reduces the frequency of prescribing additional antihypertensive drugs, and increases patient satisfaction with treatment. In patients with diabetes mellitus and metabolic syndrome, the fixed combination of ramipril and amlodipine allows achieving target values of blood pressure, promotes adherence to treatment, reduces heart rate, improves biochemical parameters of lipid and carbohydrate metabolism and functional kidney indicators. Ramipril, along with perindopril, is widely prescribed for patients with arterial hypertension, and its combination with amlodipine can be considered the method of choice in arterial hypertension patients with diabetes mellitus, chronic kidney disease, and metabolic syndrome.

Based on the research by the Institute of Psychology of the Russian Academy of Sciences, the level of anxiety and depression is growing across the country. The greatest blow falls on young people aged 18–24, with about 77 % noting depressive symptoms in themselves. Zurab Kekelidze, the director general of the Serbsky Center, responsible for providing the World Health Organization (WHO) with data on people with depression, revealed that about 8 million people in Russia suffer from this disease. That is why it is essential for a practitioner to pay attention to the effectiveness of the chosen drug therapy, as well as to take into account adverse reactions. The purpose of the review. This article provides an update on the clinical pharmacology, mechanisms of action, and safety of antidepressants, antipsychotics, and the results of their combination in the treatment of depression. We identified potentially relevant studies by searching the electronic databases MEDLINE, SCOPUS, PubMed, Google Scholar, and the Cochrane Database of Systematic Reviews for the last 5 years up to December 2022, using the terms: depression, mental disorders, therapy, safety, efficacy, adverse effects. In the near future, global healthcare will undergo fundamental changes associated with digital transformation of medicine, which is supported by numerous innovative technologies. Digitalization of domestic healthcare is currently a priority for its development. It requires medical professionals to have specialized knowledge and a willingness to make the most of the advances in digital medicine. Content analysis of the information space of digital medicine makes it possible to determine people's attitudes by learning the opinions of representatives of various strata of civil society, expressed in the media, and to draw a conclusion about the state of healthcare both in Russia and around the world. In this regard, the methodological support of computer-based content analysis, which must be carried out at the most modern technological level, is of particular interest. The article presents a clinical case of surgical treatment of a patient with a multimorphic form of nonspecific purulent-inflammatory lesions of the spine. This patient had spinal epiduritis, paraspinal and subdural abscesses, and purulent meningitis; the course was complicated by bilateral lower lobe pneumonia, systemic inflammatory response syndrome, and sepsis. The main stages of surgical correction were laminectomy, sanitation, and drainage of paraspinal, epidural and subdural abscesses, tracheostomy, puncture and drainage of the psoas abscess on the left side under ultrasound navigation. In addition, the patient received targeted antibiotic therapy. After treatment in the neurosurgical department, the patient was transferred to rehabilitation treatment. Based on the results of surgical treatment, a conclusion was made about the clinical validity of using a combination of methods for the treatment of patients with a multimorphic form of nonspecific purulent-inflammatory lesions of the spine.
