Aim: To create a device for PWPV evaluation which would be cheap and could be easily used not only at the inpatient but also at the restorative outpatient and health-resort stages of rehabilitation. Materials and Methods: The clinical examination included 90 people who were randomized into the group of observation (n=75) with patients of AH various degrees. 15 practically healthy persons were involved into the control group. PWPV was measured using a specially designed device according to the generally accepted carotid-femoral technique. Results: Our results showed that the PWPV of healthy persons measured using original device failed to differ statistically from the literature reference values and indicated the correspondence of the measurements we obtained with the data received using traditional apparatus. Out data of original PWPV measurement obtained from examined and treated patients do not differ from the results obtained in wide range of patients aged from 40 to 70 years with the 1st -2nd stages of AH. Conclusions: We described an efficacy of PWPV determination using an original device. The use of the proposed device does not contradict with the generally accepted method of vascular stiffness measuring and studying. The results obtained on it are comparable with the data of the most used stationary device SphygmoCor for these purposes. The prospects of the device using are without the limitations in patients with vascular diseases at the stage of rehabilitation.

Aim: Determining the effectiveness of the developed comprehensive physical rehabilitation program for athletes with chronic patellar tendinopathy. Materials and Methods: Participants were randomly divided into the main and control groups consisting of 11 people each. The complex rehabilitation program in the main group combined laser therapy and different types of therapeutic exercises, in the control group the laser therapy was excluded. The effectiveness of the intervention was evaluated by the intensity of pain in the patella (Visual Analog Scale) and the functional ability of the knee (VISA-P scale). Results: At the end of the intervention, a significant reduction in pain was observed in both groups. The average scores on the VAS scale were reduced to 2.0 points in the main group and to 3.5 in the control one. The share of patients, whose pain was eased, was 27.27 per cent in the main group, in the control group such people were not detected. After two months of intervention according to the results of the VISA-P scale in both study groups, there was an improvement in the functional capacity of the knee joint. Thus, the average number of points increased by 30.5 points in the main group, and by 12.0 in the control one. Conclusions: The use of high-frequency laser therapy in a comprehensive program of physical rehabilitation of patients with patellar tendinopathy shows better results for reducing pain and improving the functioning of the knee joint than without it. We believe that high-frequency laser therapy can be used as an important adjunct to various types of therapeutic exercises in the comprehensive rehabilitation of patients with patellar tendinopathy.

Aim: To analyse and summarize the data of modern scientific literature for the last 10 years on modern approaches to the prevention of chronic recurrent aphthous stomatitis, etiology, pathogenesis and treatment of this disease. Materials and Methods: Analysis and generalization of data from the modern scientific literature on the etiology, pathogenesis, prevention and treatment of CRAS based on databases such as Scopus, Web of Science, MedLine, PubMed, NCBI, the study of which does not exceed 10 years, including literature reviews and results of clinical trials. Conclusions: The prevention of chronic recurrent aphthous stomatitis requires a comprehensive and individualized approach to solve this issue. Reduction of this disease to one cause becomes serious due to its polyetiology, which makes it more difficult to predict the causes of recurrence and solution of the prevention issue. Trigger mechanisms that provoke the formation of CRAS differ in various patients (stress, mechanical disorders, the nature of nutrition, lack of vitamins or trace elements, infections, etc.). However, the immunopathogenetic mechanism may be a common link for the main triggers of the aphthae recurrence emergence. Due to the imperfect understanding of etiopathogenetic factors, the prevention of chronic recurrent aphthous stomatitis is aimed at increasing the intervals between the appearance of new lesions, reducing the number of aphthae and their size, so in other words, it is secondary.
目的:分析总结近10年来关于慢性复发性口腔炎的现代预防方法、病因、病机及治疗的现代科学文献资料。材料与方法:基于Scopus、Web of Science、MedLine、PubMed、NCBI等数据库,对研究时间不超过10年的CRAS的病因、发病机制、防治等现代科学文献数据进行分析归纳,包括文献综述和临床试验结果。结论:慢性复发性口腔炎的预防需要综合、个体化治疗。由于这种疾病的多变性,将其减少到一个原因变得严重,这使得预测复发的原因和预防问题的解决变得更加困难。不同患者诱发CRAS形成的触发机制不同(压力、机械障碍、营养性质、缺乏维生素或微量元素、感染等)。然而,免疫发病机制可能是引起口疮复发的主要诱因之一。由于对发病因素的认识不完善,预防慢性复发性口疮口炎的目的是增加新病灶出现的间隔时间,减少口疮的数量和大小,所以换句话说,它是继发性的。

Aim: The purpose of this study was to asset the effectiveness of complex manual therapy usage in patients with lumbar flexion dysfunction after discectomy, in comparison to standard physiotherapy based on physical therapy, balneotherapy and sensorimotor exercises. Material and methods: The study was conducted in the participation of 40 patients attending outpatient rehabilitation at ORNR “Krzeszowice” SP ZOZ. Participants were divided into two groups (control and experimental) each one including 20 people. The average patients’ age was 52.75 (± 11.58). The patients had been medically examined before the rehabilitation process implementation and after its completion. The test was carried out using the patient’s card, Laitinen’s scale and mobility tests with a plurimeter. Rehabilitation has taken place for four weeks daily. The exercises of the patients from the experimental group were based on comprehensive manual therapy meanwhile the control group underwent a standard package of physical therapy, balneotherapy and sensorimotor exercises. Results: Two statistically significant phenomena were observed in both groups: decrease of pain in the lower spine and the range motion improvement, nevertheless, both results were significantly greater in the experimental group. Conclusions: Both therapeutic programs indicate an advantageous impact in terms of reducing pain and functioning in everyday life, but the manual therapy is much more effective.

Aim: To substantiate and develop organizational and methodological approaches to providing the population with physical rehabilitation at the level of territorial communities. Materials and Methods: The following methods were used in the course of the study: of expert assessments to conduct an expert evaluation of the proposed innovations; medico-statistical to carry out statistical processing of primary information; of structural-and-logical analysis to analyze and present organizational and methodological approaches to providing the population of territorial communities with physical rehabilitation; bibliosemantic in order to study the existing approaches and the state of solving this issue in Ukraine. The Legislative acts of Ukraine on administrative-and-territorial reform and creation of territorial communities, as well as on the development of a system of physical rehabilitation in the country; scientific information sources on the experience of solving this issue in the countries of the world and Ukraine; results of expert evaluation of the proposed innovations were used as the materials of the study. Among the experts there were: 5 scientists, 5 specialists in physical rehabilitation, 5 heads of territorial communities, 3 family doctors, 3 residents of territorial communities in need of physical rehabilitation. Results: Organizational and methodological approaches to providing the population with physical rehabilitation and physical activity at the level of territorial communities have been substantiated and developed. The approaches are comprehensive and cover both managerial and organizational and financial aspects. Conclusions: Implementation of measures for physical rehabilitation and physical activity at the level of territorial communities will ensure further preservation and strengthening of health of the population.

Aim: To determine the features of the early post-infarction period in patients after acute coronary syndrome with concomitant arterial hypertension, depending on the response to dosed physical activity. Materials and Methods: 120 patients with a reduced response to dosed physical activity, with Q, QS MI and concomitant hypertension, who were at the stage of rehabilitation and recovery treatment, were examined. Results: In the course of the study, during the 6-minute walk test, a decrease in oxygen consumption was found in the group of patients of an adequate tolerance to physical activity by 18.42% compared to 15.21% in the group of patients of low tolerance to activity. During the analysis of ABPM (Ambulatory Blood Pressure Monitoring) indicators, it was found that in patients who made up the group of low tolerance to DPA (Dosed Physical Activity), significantly higher average values of systolic blood pressure (SBP) (159.24}3.4) mm Hg and diastolic blood pressure (DBP) (96.26}2 .49) mm Hg, as well as heart rate. During urgent coronary angiography, it was found that in patients of an adequate response to DPA, one vascular lesion was detected in most cases (73.3%), in the group of patients of low tolerance to DPA, one vascular lesion of CA was detected in (14.5%), in (55.5%) recorded two vascular lesions of the CA, and (30%) patients had three or more vascular lesions of the CA. The level of troponin I in blood serum at the time of hospitalization in the group of patients of low tolerance to DPA was 36.38}6.79 ng/ml, the level of NT-proBNP – 726.4}36.32 pg/ml, the level of endothelin-1-9,37}1.34 pmol/L. Conclusions: During the recovery period of treatment of patients after an acute myocardial infarction with concomitant arterial hypertension, a low tolerance to dosed physical activity occurs (in 63.7% of cases), which is accompanied by the appearance of anginal pain, changes in clinical indicators in the early and late post-infarction period and prevents the conducting a full range of rehabilitation measures.

Aim: Improving the quality of diagnosis and treatment orthodontic pathology in patient with temporomandibular joint disorders. Materials and Methods: Analysis – to determine objectives and study plan; clinical-diagnostic methods that include dental and radiological examination (to establish the clinical diagnosis of patients with temporomandibular disorders), electronic axiography (to determine the features of the movements of the articular heads of the mandible), instrumental study the models of jaws in the articulator (to diagnose the state of functional occlusion), statistical (to calculate averages and assess the probability of the results). Results: After developed and implemented algorithm total dental rehabilitation we have eliminated functional disorders in the masticatory muscles and temporomandibular joints, completely restored the lost function of the masticatory apparatus. Conclusions: Creation of occlusal relations, which are harmoniously combined with the function of masticatory muscles and temporomandibular joints, allowed to achieve the long period of remission, absence of the complications and recurrences of the disease in the near and long term. These points became the criterions that improved effectiveness of scientifically based individual treatment of mandibular disorders.

Aim: Evaluation of the effect of hydrogen sulfide baths on pain, tactile, temperature and vibration sensitivity in patients with diabetic polyneuropathy. Materials and Methods: 125 patients with type 2 diabetes complicated by diabetic polyneuropathy were examined. Patients were divided into 2 groups: the 1st group included 61 patients who received standard treatment in an outpatient setting, 64 patients of the 2nd group in addition to standard treatment received hydrogen sulfide baths in a sanatorium. Sensitivity assessment was performed using a tuning fork, monofilament, atraumatic needle, Tip-Term cylinder. Results: In most patients, after the initial examination, a significant decrease mainly in vibration and tactile sensitivity was observed. Only pain sensitivity was truly restored in patients receiving standard outpatient therapy. The best clinical effect of treatment (reliable recovery of all types of sensitivity) was achieved in patients who were in a sanatorium and in addition to standard therapy received hydrogen sulfide baths. Conclusions: The use of hydrogen sulfide baths in the complex treatment of diabetic polyneuropathy contributes to a more effective restoration of sensitivity disorders in patients with diabetes

Aim: The study aimed to develop an algorithm for organizing the work of a multidisciplinary palliative care team in a territorial community. Materials and Methods: The following methods were employed: analysis of the regulatory framework, statistical data, international and domestic experience, scientific literature, opinion survey, strategic sessions, participant observation. The field research was conducted by the Center for Social Research of Sumy State University together with the NGO “Family Circle of Trostianechchyna (Trostianets district)” in the Trostianets territorial community of Sumy region (Ukraine) during September-October 2021. Eighty respondents took part in the survey. Up to 30 people joined the strategic sessions at different stages. Results: The survey discovered that at the research time in the pilot territorial community, the needs of palliative patients were more restrained than those who rendered assistance. However, in both cases, there is a high demand for social services against the background of medical services. Proceeding from the identified needs and by the results of strategic sessions, we proposed an algorithm for organizing the work of a multidisciplinary palliative care team at the territorial community level. The algorithm distinguishes between the program and operational levels. We put forward the structure of the interdisciplinary palliative care team. Conclusions: As a conclusion, we can specify the following points. The study confirmed the hypotheses that the basics of the identified needs and the use of strategic sessions with multidisciplinary participation of community members facilitate the development of an efficient algorithm. The designed operational level of the interdisciplinary team recorded the practices available in the pilot community. Simultaneously, the collaborative design determined the development of the multidisciplinary team itself. The involvement of management representatives, junior medical staff, and social workers became an essential factor in their inspiration, contributing to the development of human resources for a new level of quality in palliative care.

Aim: To investigate the impact of the amount of students’ motor activity on their health status and psychophysical readiness for future life. Materials and Methods: The research was conducted in 2019-2021. The study involved 420 students and 232 graduates of different years. 57 male students took part in the experiment aimed at studying the impact of the amount of motor activity on the health of students and their psychophysical readiness: 28 students were included to the experimental group, 29 students – to the control group. Research methods: theoretical analysis, questionnaires, testing, pedagogical experiment, statistical methods. Results: It was revealed that an increase in the amount of practical (academic and extracurricular) classes to 12-14 hours per week for one year positively affected the state of health of students and their psychophysical readiness for life. In EG, in contrast to CG, the results of most fitness tests significantly improved. The number of students with a high level of health in EG increased by 28.6%, and in CG – by 3.4%. The number of EG students with low level of neuropsychiatric stability decreased by 32.2%, and in CG – by 6.6%. Conclusions: Increasing the level of motor activity of students requires creation of a modern system of physical education of students, which should take into account their interests, motives and personality-oriented choice of the type classes. Classes should be organized in accordance with the level of physical fitness and health status of students.
