Investigation of Anxiety and Depression Level in Patients who Applied with a Request of Pregnancy Termination During The COVID-19 Pandemic 新冠肺炎大流行期间申请终止妊娠患者焦虑抑郁水平调查

Objective: At a time when the spread of the COVID-19 virus is very rapid in Turkey, there is not enough information about the course of the pandemic and the effects of the COVID-19 infection in pregnancy, the family planning clinic of our hospital evaluates the levels of anxiety and depression in patients with demand for termination of pregnancy and the desire to terminate the pregnancy for COVID-19. We planned to investigate whether it was associated with concerns related to the COVID-19 pandemic. Methods: 132 patients with pregnancies below 10 weeks who applied to Etlik Zübeyde Hanım Training and Research Hospital Family Planning Outpatient Clinic with a request for termination of pregnancy in April 2020 and July 2020 were included. Results: The reasons for termination of pregnancy were stated as 60% not wanting another child, 23.5% economic reasons, 9.8% risky drug use, and 6.8% other reasons. None of the patients included in the study decided to terminate their pregnancy due to the COVID-19 pandemic. The mean total score of the patients on the Beck Depression Inventory was 14 (0-43). Forty-six patients (34.8%) were evaluated as minimally depressed, 30 (22.7%) mild depression, 38 (28.8%) moderate depression, and 18 (13.6%) severe depression. The mean total score of the Beck Anxiety Inventory was 5 (0-44). One hundred and fifteen (87.1%) patients were classified as mild, 14 (10.6%) patients as moderate, and 3 (2.3%) patients as severe anxiety. Conclusion: During the COVID-19 pandemic, there is an increase in the frequency of anxiety and depression in patients who have undergone pregnancy termination and especially who live with an elderly relative. It was observed that the COVID-19 pandemic was not effective in the decision to terminate the pregnancy and, similar to the previous data, not wanting another child was the most common reason.

Comparison of Maternal and Neonatal Outcomes in Elective and Emergent Placenta Previa Operations 择期和紧急前置胎盘手术产妇和新生儿结局的比较

Objectives: The purpose of this study was to compare maternal and fetal outcomes between planned and emergent placenta previa operations. Methods: Patients who underwent cesarean section for placenta previa were included in the study between January 2018 and January 2019 at tertiary center, retrospectively. Patients were divided into two groups as planned cesarean delivery and emergent cesarean delivery. Maternal characteristics, maternal and neonatal outcomes were compared. Results: Of the 84 patients with placenta previa, 36 (43%) were in the planned group, and 48 (57%) were in the emergent cesarean delivery group. There were no significant differences in the transfusion rates, operation time, length of hospital stay, hemostasis procedures, intraoperative complications between planned and emergency deliveries. Lower birth weight and higher neonatal intensive care unit acceptance were detected in the emergent cesarean delivery group due to earlier gestational week of delivery ( 34±2.4 vs. 36±2.3 p < 0.001). There was no statistically significant difference in hysterectomy rates performed due to the placental accreata spectrum between the two groups. Conclusions: In conclusion, emergency cesarean delivery for women with placenta previa by an experienced multidisciplinary team in a tertiary center with adequate resources is not associated with increased maternal morbidity in terms of transfusion rate, hospital stay, and intraoperative complications. Planned cesarean delivery at 37 0/7 -37 6/7 gestational weeks may be considered in the absence of significant antenatal bleeding or other risk factors.

Evaluation of Infection Factor and Antibiotic Resistance Distributions in Palliative Care Patients Developed Urinary Tract Infection 姑息治疗患者尿路感染感染因素及抗生素耐药分布的评估

Objectives: In this study, it was aimed to identify the causative bacteria of urinary tract infections, to determine antibiotic susceptibility, and to examine acute phase markers in patients hospitalized in the palliative care service of Ankara Polatlı Duatepe State Hospital between January 2019 and December 2020. Materials and Methods: These sex, age and, detected diseases of a total of 72 palliative care patients included in our study were analyzed retrospectively. To determine the causative agents of urinary tract infections in these patients, Gram staining was performed on the bacterial cultures that developed in the urine samples, and the Vitek-2 (bioMérieux, France) automatic test device was used to identify these cultures and determine their antibiotic susceptibility. In the blood sample staken from these patients, leukocyte (WBC), C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR) measurements were also made. Results: The causative agents of urinary tract infections of these patients are Gram-negative bacteria Escherichia coli , Klebsiella pneumoniae , Pseudomonas aeruginosa , Proteus mirabilis , Acinetobacter baumannii , Citrobacter koseri , Morganella morganii ssp. morganii , Enterobacter cloacae complex , and Gram-positive bacteria Enterococcus faecium , Enterococcus spp ., and Staphylococcus epidermidis were found. Various antibiotics were determined to have different levels of sensitivity. Conclusions: It is important to determine the causative agents of urinary tract infections, which are among the most frequently detected infections in palliative care patients, and to determine antibiotic susceptibility and acute phase response markers fo these agents. Mortality and morbidity will be reduced by determining the causative agents of these infections and applying appropriate antibiotic treatments.

Correlation of Culture Results and Hematological Parameters with Tissue PCR Test in the Diagnosis of Periprosthetic Joint Infection 组织PCR检测与培养结果及血液学参数的相关性研究

Objective. Detection of the pathogen and appropriate early treatment are very important in the treatment of periprosthetic joint infection (PJI). In our study, we evaluated the correlation of culture results and hematologic parameters with tissue polymerase chain reaction (PCR) test in the diagnosis of PJI. Methods. The study included 26 patients whose culture and tissue PCR samples were examined with suspicion of PJI in our clinic. Tissue samples were obtained for PCR and culture during revision surgery. All data were statistically analyzed. Cut-off values of acute phase reactants were tried to be determined according to the culture results. Pathogens were detected in only one patient in culture, whereas pathogens were detected in 5 patients in PCR test. Results . C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) values of patients with positive PCR test results were significantly higher than those with negative PCR test results. As a result of the ROC analysis performed to determine the cut-off values, the cut-off value of CRP was 24.5 with 100.0% sensitivity and 90.0% specificity (p<0.05), and the cut-off value of ESR was 64.5 with 80.0% sensitivity and 85.7% specificity (p<0.05). Conclusions. Diagnosing infection with PCR test reduces the possibility of making mistakes. In addition, we tried to find the cut-off values for CRP and ESR in the diagnosis of PJI in our study. To confirm these cut-off values we need prospective studies with larger number of patients.

Presbyphagia; is the physiological, structural and sensory changes that occur in the structures related to swallowing with age. Elderly patients adapt to all these changes by making various alteration in their eating habits to maintain normal swallowing function; however, an intervening stressor, comorbid disease or sarcopenic changes can make an asymptomatic swallowing disorder "symptomatic". This situation may be overlooked by the patient himself or the caregivers and may be accepted as a natural consequence of aging. Dysphagia in elderly can lead to serious complications such as malnutrition, dehydration and aspiration pneumonia. Recognizing the situation and not accept it as a natural consequence of old age plays a key role in this problem
