Comparison of Mortality Rates and Functional Scores of Proximal Femur Nail and Partial Hip Arthroplasty in Intertrochanteric Femur Fractures: A Retrospective Study 股骨转子间骨折股骨近端钉和部分髋关节置换术死亡率和功能评分的比较:回顾性研究

Objective: Intertrochanteric femur fractures are common fractures with increasing life expectancy. The aim of treatment is to provide early mobilization and pre-fracture function in patients. In this study, we aimed to compare the mortality and function scores of two treatment modalities (proximal femoral nail and bipolar hemiarthroplasty) commonly used in the treatment of intertrochanteric femur fractures in a secondary care hospital. Materials and Methods: The study was planned for patients who underwent proximal femoral nail (PFNA) and bipolar hemiarthroplasty (BHA) due to femoral intertrochanteric fracture in a second-care hospital between 2017 and 2020. The data from a total of 199 patients were analyzed retrospectively. The operation method, age, sex, vitality, operation time, and hospitalization were recorded. The short form-36 (SF-36) questionnaire, recorded at the last visit, was evaluated in the surviving patients. Results: No significant difference has been found for gender and age compared to the operation method. However, the mortality rate of BHA is found to be higher than the rate of PFNA (p<0.001). Compared to the time of death, no significant difference is found between operation methods. The operative time and hospitalization were significantly lower in the PFNA (p<0.01, p<0.05; respectively). At postoperative measurements, SF-36 physical functioning, energy/fatigue, emotional well-being, social functioning, pain, general health, and total scores were higher in the PFNA. Conclusion: In this study, PFNA has low mortality and high functional scores in treating femoral intertrochanteric fractures. Therefore, PFNA is safer in a secondary hospital treating femoral intertrochanteric fractures and is recommended as a primary treatment option.

Ruptured Common Hepatic Artery Aneurysm Masquerading as an Intestinal Obstruction 肝总动脉破裂动脉瘤伪装成肠梗阻

Visceral artery aneurysms are uncommon but can present as life-threatening often fatal emergencies when ruptured. Hepatic artery aneurysms account for 20% of splanchnic artery aneurysms and are associated with a 25% rupture rate with 70%-100% mortality. We report a case of a 33-year-old male who presented with a 1-day history of severe epigastric pain associated with hypotension and anaemia with no clinical signs of overt bleeding nor history of trauma. Computed tomography abdomen and pelvis showed a ruptured saccular common hepatic aneurysm which was successfully embolized by our interventional radiologist.

RESPINOS Remote Monitoring Device to Observe the Vital Signs of Mild – Moderate COVID-19 Patients RESPINOS远程监测设备用于观察轻度至中度新冠肺炎患者的生命体征

Objective : To assess the validity of RESPINOS, a remote monitoring device, to measure the vital signs of patients with mild – moderate COVID-19. Methods : A cross sectional study was conducted in Dr. Soetomo Hospital, Surabaya, Indonesia. All patients with mild – moderate COVID-19 were enrolled to our study, and patients with unable to cooperate in operating the tool were excluded. The implementation of RESPINOS was considered the predictor covariate, and the vital signs were considered the outcome, compared to the measurement of standard device. A t test and kappa agreement were used to assess the similarity between the measurement of RESPINOS and the standard device. Results : A total of 180 participants was employed to our study. Of them, we found that the measurements of respiratory rate, heart rate, and oxygen saturation had similarity between RESPINOS and the standard device (p > 0.05). Moreover, our analysis in kappa agreement also found that the coefficient of kappa agreement was higher in respiratory rate, heart rate, and oxygen saturation, suggesting that the measurement either by using RESPINOS or standard device provided similar findings. Conclusion : RESPINOS offers the benefits for a remote monitoring in patients with mild – moderate COVID-19.

The Frequency of COVID 19 in Children with Fever and/or Respiratory Symptoms in 2020-2021 Winter: Preliminary Report from Cyprus 2020-2021年冬季有发烧和/或呼吸道症状的儿童感染新冠肺炎的频率:塞浦路斯的初步报告

Objective. The data regarding Covid-19 in children is still limited. Although the course of Covid-19 disease in children seems milder than adults, the co-morbid conditions that may cause severe infection or death have not been fully clarified and this creates anxiety. This study aims to evaluate the frequency of Covid-19 in children admitting to hospital with symptoms. Methods. A retrospective study during 7 months of the pandemic period was conducted in two tertiary care academic hospitals in North Cyprus. All patients admitted into the pediatric outpatient clinics with fever and/or respiratory symptoms and tested for Covid-19 with real-time polymerase chain reaction (RT-PCR) were involved in the study. Clinical characteristics including symptoms, physical examination findings, laboratory results, hospitalization status, and diagnosis were all recorded from hospital database retrospectively, and the frequency of positive Covid-19 testing in children was evaluated. Results. A total of 111 patients with fever and/or respiratory symptoms without a contact history with Covid-19 were assessed. The age of the children ranged from 2 -172 months, with a median age of 32 months, and 60.4% were male. A total of 19 (17.3%) were hospitalized and the Covid-19 positivity rate was 1.8%. Conclusion. There is a certain need for guidelines to be followed for Covid-19 diagnosis in symptomatic children without a contact history. Test timing, the need for consecutive testing or isolation and recommendations for symptomatic child caregivers about how to protect themselves should be considered and seriously discussed.

Evaluation of the Potential Drug-Drug Interactions among Hospitalized Patients with COVID-19: A Cross-Sectional Study 新冠肺炎住院患者潜在药物相互作用的评估:一项跨部门研究

Objective: COVID-19 related drugs and concomitantly used drugs of the patients for their co-morbid diseases may enhance the risk for having potential drug-drug interactions (pDDIs) during hospitalization. The aim of the present study was to identify the frequency and severity of pDDIs between the COVID-19 treatment options and concomitantly used drugs at COVID-19 inpatient clinics of a public hospital. Methods: A total of 321 files of the patients with COVID-19 who were hospitalized at inpatient clinics of a public hospital were assessed retrospectively. The frequency, risk category and severity of the pDDIs were assessed according to the Lexi-comp® online drug interaction database. Results: A vast majority of the patients (99.1%) had at least one pDDI with a maximum of 25 pDDIs. The most commonly detected pDDIs were between Favipiravir and Paracetamol (B risk category), (24.4%), followed by the pDDIs between Enoxaparin-Acetylsalicylic acid (D risk category), (10.1%) drug pairs. Hyper-polypharmacy (≥10 drugs) was significantly and positively associated with male gender (p=0.023), adult age group (p=0.002) and co-morbid diabetes mellitus (p<0.001) and essential hypertension (p<0.001). The presence of moderate to major pDDIs was significantly and positively associated with male gender (p=0.012) and adult age group (p=0.019). Conclusion: The present study revealed that the frequency of pDDIs among hospitalized patients with COVID-19 are quite high. Although the severity of pDDIs at COVID-19 inpatient clinics in general, are moderate and preventable, physicians should pay a particular attention regarding the pDDIs which may have life-threatening consequences such as the risk of bleeding.

Hyper-polypharmacy (≥10 drugs) was significantly and positively associated with male gender (p=0.023), adult age group (p=0.002) and co-morbid diabetes mellitus (p<0.001) and essential hypertension (p<0.001). The presence of moderate to major pDDIs was significantly and positively associated with male gender (p=0.012) and adult age group (p=0.019). Conclusion: The present study revealed that the frequency of pDDIs among hospitalized patients with COVID-19 are quite high.
