Value of ascitic fluid lipids in the differentiation between cirrhotic and malignant ascites 腹水脂在肝硬化与恶性腹水鉴别中的价值

Background: The diagnosis of malignant ascites can be difficult. The overall sensitivity of cytology smears for the detection of malignant ascites is 58%–75%. There is a debate about the role of ascitic fluid lipids and their gradients in the differential diagnosis of ascites. This study aims to evaluate the role of ascitic fluid lipids and their gradients in the differentiation of malignant ascites from cirrhotic ascites. Patients and Methods: Ninety-six patients were enrolled in this study divided into two groups according to the cause of ascites; Group I: included 48 patients with malignant ascites and Group II: included 48 patients with cirrhotic ascites. Results: Ascitic fluid cholesterol, triglycerides, and phospholipids were significantly higher among patients of Group I (77.5 ± 11.5, 82.4 ± 17.04 mg/dl, and 0.92 ± 0.14 mmol/L vs. 27.7 ± 7.5, 56.2 ± 16.2 mg/dl, and 0.33 ± 0.09 mmol/L in succession P < 0.001). The cutoff values of cholesterol, triglycerides, and phospholipids were (41.5, 62.5 mg/dl and 0.45 mmol/L successively); they can predict the presence of malignant ascites with sensitivity of (100, 87, and 100% successively) and with specificity of (97.9, 60.4, and 87.5% successively). Conclusion: Ascitic fluid lipids are valuable markers in the differentiation of malignant ascites from cirrhotic ascites.

Molecular Imaging for Cancer Diagnosis and Surveillance 肿瘤诊断和监测的分子成像

Nowadays, molecular imaging technologies have a pivotal role in the field of clinical oncology. The utilization of imaging methods in the early detection of cancer, assessment of treatment response, and development of new therapies is steadily increasing and has already had a significant impact on the clinical management of cancer. Molecular imaging is indispensable for both the detection and treatment of cancer. It focuses on various biomarkers used in targeted therapy, and nuclear medicine-based molecular imaging is a real-time and non-invasive technique that has the potential to identify tumors at an earlier and more manageable stage, before anatomical imaging methods reveal the presence of the disease. Molecular imaging offers extensive possibilities for visualizing cellular and molecular activities throughout tumor growth, serving as a biomedical imaging technology with remarkable sensitivity in detecting and resolving images. It provides non-invasive methods for observing, characterizing, and quantifying biological processes at the cellular and subcellular levels. The development of molecular imaging biomarkers is aimed at improving the evaluation of the effects of targeted therapy. Examples of molecular imaging techniques include positron emission tomography (PET), single-photon emission computed tomography (SPECT), magnetic resonance imaging (mMRI), magnetic resonance spectroscopy (MRS), optical imaging, photoacoustic imaging, and multimodal imaging. Some modalities require the administration of molecular probes, while mMRI and photoacoustic imaging can track the effectiveness of drugs using either endogenous molecules or exogenous molecular probes.

Maternal and Fetal Outcomes in Pregnancies Affected by Osteosarcoma of the Jaw: A Case Report 颌骨骨肉瘤影响妊娠的母胎结局:1例报告

The occurrence of cancers during pregnancy is very rare. The common malignancies during pregnancy include breast and cervical cancers, however, head and neck malignancies, on the other hand, are particularly rare, including osteosarcoma. The occurrence of osteosarcoma of the jaw during pregnancy is extremely rare, and its diagnosis can be challenging. When osteosarcoma is detected in pregnancy, the prognosis of both the mother and the fetus may be compromised. This article describes a case of osteosarcoma of the jaw in a 29-year-old African woman in her 28 th gestation week. She presented with a complain of a painful facial swelling for nearly 3 months. The swelling was preceded by a toothache in the posterior aspects of the left side of the upper jaw. She underwent a series of surgical procedures but the lesion recurred, and subsequently, chemotherapy was administered to her, however, the disease did not respond well and she subsequently succumbed to her disease. Here we discuss the challenges of diagnosis, management, and outcome of treatment for osteosarcoma of the jaw bone in pregnancy.

Analysis of Interaction between Twenty-Seven Pyrimidinone Derivatives with XIAP Using a Theoretical Model 27种嘧啶类衍生物与XIAP相互作用的理论模型分析

For several years, several drugs have been used to treat different types of cancer; however, some of these drugs can cause side effects, such as high blood pressure, liver damage, and erectile dysfunction. In the search for new therapeutic alternatives, some compounds have been developed for the treatment of this clinical pathology; however, the interaction of these drugs with some biomolecules involved in the development of cancer is very unclear. Analyzing this data, this investigation aimed to evaluate the possible theoretical interaction of some pyrimidinone derivatives (compounds 1 to 27) on the X-linked inhibitor of apoptosis protein (XIAP) involved in cancer using the Docking model. The results showed that some pyrimidinone derivatives (1-6, 10, 11, 14, 15, 22-24, 26, and 27) could interact with the XIAP protein surface. In conclusion, these data suggest that some pyrimidinone derivatives can produce changes in the biological activity of XIAP. Therefore, these pyrimidinone derivatives could be good candidates to treat cancer.

Should PRIMA Prognostic Index Be Widely Applied Instead of FLIPI2 in the Prognosis of Follicular Lymphoma? 是否应广泛应用PRIMA预后指数代替FLIPI2评价滤泡性淋巴瘤的预后?

Recent studies have suggested that PRIMA-PI (Prognostic Index) is a simple tool for predicting patients with follicular lymphoma (FL). With the desire to contribute some experience in the prognosis of FL, we conducted a study to analyze the value of PRIMA-PI versus FLIPI2 (Follicular Lymphoma International Prognostic Index) in the prognosis of FL. Forty-five patients diagnosed with newly follicular lymphoma who received chemotherapy were retrospectively analyzed. All patients underwent bone marrow biopsy to detect bone marrow involvement. Based on histology, the grade was defined according to WHO. The Kapplan-Meyer method and the Cox proportion hazards model were used in the OS (overall survival) analysis to evaluate prognostic indices such as FLIPI2, PRIMA-PI, and the histological classification of FL (grade). According to FLIPI2, the 9-year OS rate of the low-risk group, the intermediate-risk group, and the high-risk group was 100%, 52.9%, and 49.5%; respectively. According to PRIMA-PI, the OS rate for the low-risk group, the intermediate-risk group, and the high-risk group at 9 years was 83.9%, 68.2%, and 43.3%; respectively. According to grade, the 9-year OS rate for grades 1, 2, 3a, and 3b was 83.3%, 73%, 59.3%, and 25%; respectively. Univariate and multivariate analysis showed that only FLIPI2 and grade were truly independent prognostic factors for OS with P =0.03, and 0.012; respectively. On the contrary, we had not yet found the significance of PRIMA-PI in the prognosis of OS, ( P =0.056). Caution should be exercised when planning to apply PRIMA-PI widely instead of FLIPI2 in the prognosis of follicular lymphoma.

Forty-five patients diagnosed with newly follicular lymphoma who received chemotherapy were retrospectively analyzed. All patients underwent bone marrow biopsy to detect bone marrow involvement. Based on histology, the grade was defined according to WHO. The Kapplan-Meyer method and the Cox proportion hazards model were used in the OS (overall survival) analysis to evaluate prognostic indices such as FLIPI2, PRIMA-PI, and the histological classification of FL (grade). According to FLIPI2, the 9-year OS rate of the low-risk group, the intermediate-risk group, and the high-risk group was 100%, 52.9%, and 49.5%; respectively. According to PRIMA-PI, the OS rate for the low-risk group, the intermediate-risk group, and the high-risk group at 9 years was 83.9%, 68.2%, and 43.3%; respectively. According to grade, the 9-year OS rate for grades 1, 2, 3a, and 3b was 83.3%, 73%, 59.3%, and 25%; respectively. Univariate and multivariate analysis showed that only FLIPI2 and grade were truly independent prognostic factors for OS with P =0.03, and 0.012; respectively. On the contrary, we had not yet found the significance of PRIMA-PI in the prognosis of OS, ( P =0.056). Pulmonary Carcinosarcoma is a Rare Cancer with a Poor Prognosis – A Retrospective Study 摘要肺癌肉瘤是一种预后较差的罕见癌症

Pulmonary carcinosarcoma (PCS) is a rare malignant mixed tumor characterized by a biphasic histopathological pattern of both epithelial and sarcomatous components. It is estimated to account for less than 1% of all pulmonary neoplasms, and patients are predominantly elderly, male, and smokers. The biological behavior and clinicopathological characteristics of PCS are not well documented. This study aimed to address this research gap. This retrospective study included eight patients with a pathologically confirmed diagnosis of PCS. All the patients had a large solid lung tumor, ranging in diameter from 3.5 to 21.5 cm. Six patients had central lesions and two had peripheral lesions. The clinical presentations depended on the localization of the tumor and included mainly cough, dyspnea, and hemoptysis. Complete resection was performed in seven patients. PCS has a highly aggressive nature and a propensity for frequent metastasis, resulting in a poor prognosis and short survival times, ranging between 6 months to 1 year. The use of neoadjuvant and adjuvant chemo-radiotherapy is controversial and not considered fully effective. Surgical lung resection is considered the most effective therapy for this tumor, but even then, survival rates are very low. PCS is a rare and aggressive form of cancer that is difficult to treat. More research is needed to develop better treatments for this disease.
