How Do Counsellors Make Sense of the Online Disinhibition Effect When Counselling Clients via Video-Conferencing? An Interpretative Phenomenological Analysis Study 当咨询师通过视频会议咨询客户时,如何理解在线去抑制效果?解释性现象学分析研究

Objectives

This qualitative study focuses on how counsellors make sense of the online disinhibition effect via video conferencing. Whilst online disinhibition is recognised within the profession as the unregulated release of emotions when online, there is a lack of research exploring how this is experienced and understood by counsellors using video conferencing.

Method

Six counsellors participated in semi-structured interviews, ‘in person’ or via video-conferencing. Data was analysed using interpretative phenomenological analysis to provide a nuanced exploration of participants' rich data on their lived experiences and meaning-making processes of the online disinhibition effect via video-conferencing.

Results

Three group experiential themes were generated from the data: the phenomenology of disinhibition, adapting to technology in the post-COVID world and the therapeutic relationship via video-conferencing. Results suggest the experience of disinhibition is highly individualised with participants making sense of this concept through unique perspectives. Their understanding of online disinhibition was influenced by their transition into a post-COVID-19 world and the need to adapt to technological changes. Consequently, participants made sense of the online disinhibition effect within the video-conferencing therapeutic relationship.

Conclusion

Findings indicate counsellor and client co-experience each other, and the mediating factors of disinhibition, within the context of the technological relationship. As such, online disinhibition is seen here as a complex process of co-creation within the relationship, as opposed to something that ‘happens’ to the client. Training providers, supervisors and practitioners could facilitate more effective and ethical practice by increasing their awareness of the element of co-creation and highly individualised experience of online disinhibition.

Perceptions of Psychotherapy After Therapists Repair Ruptures With Immediate or Expressive Strategies 治疗师用即时或表达策略修复破裂后对心理治疗的看法

Objective

Researchers have long recognised that disruptions in the psychotherapy relationship can lead to poorer treatment outcomes or dropout, with several strategies proposed to repair such therapeutic ruptures. The aim of the study was to compare two recommended repair strategies: (a) immediate repair, in which the therapist tries to resolve an obstacle by immediate problem-solving, or (b) expressive repair, in which the therapist addresses the disruption by focussing on emotional issues that may be the basis for the rupture. Each strategy was also compared with a condition consisting of minimal encouragers, representing no attempt at repair other than adopting a listening stance.

Method

In the study, participants watched two brief audiovisual recordings of stimuli presented as coming from two different psychotherapy cases in which a disruption had occurred. In one recording, the therapist uses either a repair strategy or the listening stance, and in the other recording, the therapist uses either the second repair strategy or the listening stance. For half the participants, therapist and client in the therapy recordings were male; for the other half, therapist and client were female. After each viewing, participants rated therapeutic bond and treatment credibility.

Results

Both repair strategies were superior to minimal encouragers for treatment credibility, and problem-solving was also superior to minimal encouragers for therapeutic bond. Problem solving was superior to emotion focussing for both types of perceptions. The effect of both strategies further depended on the race, sex and/or therapy experience of participants.

Conclusion

The findings suggest that therapists should attempt repair of a rupture rather than avoid it, with problem-solving appearing generally as a safer strategy than emotion focussing.

Background

Pet loss grief remains insufficiently understood and often underestimated, despite its profound emotional and existential impact. Exploring this experience in supportive contexts can deepen the understanding of how individuals navigate this form of bereavement.

Aims

This study aimed to explore the lived experience of pet loss grief within an open online support group, adopting a phenomenological–existential perspective.

Materials and Methods

Seven online group meetings were conducted, each lasting two hours, comprising a total of 28 participants. The analysis followed a phenomenological–existential approach to describe and interpret the participants’ experiences of pet loss grief.

Results

Pet loss grief involved a deep reconfiguration of caregivers’ being-in-the-world, revealing emotional, relational and social complexities that challenge superficial understandings of this type of loss. Participants expressed profound shifts in daily routines, identity and meaning-making following the death of their pets.

Discussion

The findings illuminate how pet loss grief extends beyond emotional distress, involving existential disruption and a need for recognition in social contexts. This underscores the importance of considering the relational and existential dimensions of this experience in both research and clinical practice.

Conclusion

Pet loss grief should be acknowledged as an existentially significant experience. Interventions that validate and meaningfully integrate this loss into individuals’ lives are essential for enhancing support and clinical care in this domain.

Cognitive Emotion Regulation Strategies as Mediators Between Childhood Maltreatment and Eating Disorders: A Cross-Sectional Study 认知情绪调节策略在儿童虐待与饮食失调之间的中介作用:一项横断面研究

Background

Eating disorder (ED) symptoms are common in the general population and are often associated with childhood maltreatment (CM). Emotion regulation difficulties are likewise linked to both CM and ED symptoms, yet the role of cognitive emotion regulation strategies (CERS) in this relationship remains underexplored. Culturally specific forms of adversity, such as parental overprotection and overcontrol (OP–OC), have also been largely overlooked.

Method

This cross-sectional study investigated whether adaptive and maladaptive CERS mediate the associations between six forms of CM and ED symptoms in a non-clinical sample of 352 Turkish adults. Participants completed the Childhood Trauma Questionnaire–Expanded, the Eating Disorder Examination Questionnaire–Short Form, and the Cognitive Emotion Regulation Questionnaire. Mediation analyses were conducted using the PROCESS macro (Model 4) in SPSS v29.

Results

The results revealed that, except for physical abuse, maladaptive CERS significantly mediated the associations between all CM types and ED symptoms, with full mediation found for emotional neglect and sexual abuse. Although adaptive CERS did not mediate these relationships, their use was significantly lower among individuals exposed to emotion-related maltreatment, including OP–OC.

Discussion

These findings underscore the potential of maladaptive CERS as early indicators and modifiable risk factors in subclinical populations. Tailoring emotion regulation components within interventions like Cognitive Behavioural Therapy–Enhanced (CBT-E) and Dialectical Behaviour Therapy (DBT) may enhance efficacy, especially for trauma-exposed individuals. Preventive psychoeducational programmes in youth and community settings could offer timely support before clinical symptoms emerge.

This paper explores the journey of an Indian therapist-researcher as she delves into the complex realm of the therapists' use of self in India. It examines the reflection of an experienced Indian therapist-researcher investigating the complex phenomenon of the use of self.

Methods

Using qualitative phenomenological interviews, the present study explores the dilemmas that therapists as researchers face during the interview process. This paper draws from a study with eight participants on the topic ‘Use of Self in Therapy: An Exploratory Study Among Indian Counsellors and Therapists’ using interpretative phenomenological analysis.

Results and Discussion

The dynamic interplay between researcher and participant identities underscores the necessity of carefully navigating boundaries and tailoring the inquiry process to participant needs and research objectives. This paper illustrates possible pathways of responding to these dilemmas of identity by transforming the interview process into a more participant-led conversation and using therapeutic skills to ethically navigate research interviews. As the researcher grapples with the question of whether the therapist self should be held back or freely expressed, the paper offers insights into the delicate balance between knowledge-gathering as part of research and maintaining participant safety. Finally, this study emphasises the importance of reflexivity in understanding the self of the therapist, particularly in the context of India's evolving therapy landscape.

Conclusion

This paper highlights the questions and dilemmas that arise in qualitative research and the need for ethical reflexivity, responsiveness and sensitivity while working with people.

Artificial intelligence (AI) has the potential to reshape the therapeutic landscape of the future, offering new possibilities for access, efficiency and insight, while simultaneously challenging foundational principles of human connection, ethics and professional identity. This editorial introduces the growing interface between AI and therapy, highlighting how conversational agents, analytic tools and generative systems are beginning to influence assessment, supervision and clinical decision-making. It also emphasises the need for critical, ethically informed engagement, integrating AI within ecosystems of support and learning that respect the relational and contextual nature of therapeutic work. The editorial introduces a collection of papers in Counselling and Psychotherapy Research exploring these developments. Together, the articles address the integration of AI into training, ethics and multicultural practice; examine novel uses of AI for outcome monitoring and process analysis; and offer both advocacy and critique of AI's emerging role in mental health. Collectively, they reflect a profession in dialogue, one that is curious, cautious, and committed to ensuring that technology serves, rather than shapes, the values of therapy.

Introduction

Approximately 41% of NHS staff experience work-related stress. Previous reviews on mental health professionals' (MHPs) burnout have suggested that up to 40% experience emotional exhaustion, 22% depersonalisation and 19% low personal accomplishment. The present review aimed to explore the prevalence of burnout among MHPs in NHS mental health services and the associated risk factors.

Methods

A systematic review and meta-analysis on the prevalence and risk factors of burnout was conducted. Synonyms and related terms with ‘mental health professionals’ (population), ‘burnout’ (outcome) and ‘NHS’ (context) were searched on CINAHL, MEDLINE, EMBASE, PsycINFO and Web of Science in March 2025. The meta-analysis on burnout prevalence was conducted using the ‘metafor’ package on R. Cochran's Q and I2 were used to assess heterogeneity. A narrative synthesis was employed to explore risk factors.

Results

A total of 14 studies were included, seven of which were used for the meta-analysis due to the available data, replicating previous meta-analysis studies on burnout. Pooled prevalence for emotional exhaustion was 42%, for depersonalisation it was 22% and for personal accomplishment it was 19%. Risk factors are conceptualised at an individual and organisational level.

Discussion

Future research is needed to explore NHS MHPs' experiences of burnout, particularly clinical psychologists, who were the least represented group across the included studies. Clinical implications and limitations are also discussed.

Background

Adolescent mental health has become an increasing concern globally, particularly within under-resourced urban areas where poverty, violence and limited educational resources intersect. In South Africa, adolescents in disadvantaged schools face heightened exposure to psychosocial and environmental risks that contribute to poor mental health outcomes. Understanding both the risk and resilience factors affecting this population is essential for developing culturally relevant, community-based mental health interventions.

Aims

This study aimed to explore the mental health risk and resilience factors among adolescents in under-resourced schools in Johannesburg South. It further sought to examine how these factors interact within the biopsychosocial and Nsamenang's social ontogenesis theories to shape adolescent well-being.

Materials and Methods

A qualitative research design was employed using semi-structured interviews, focus groups and the Incomplete Sentences projective technique. Thirty adolescents aged 13 to 18 years participated. Data were analysed using Braun and Clarke's thematic analysis.

Results

Findings revealed multiple interacting risk factors across biological, psychological and social domains. Reported mental health symptoms included anxiety, sadness, sleep disturbances, intrusive thoughts and post-traumatic stress. Key risk factors were academic pressure, financial hardship, exposure to violence, substance abuse, physical health difficulties and lack of emotional support. Despite these adversities, participants demonstrated resilience through intrinsic motivation, self-care strategies, adult and peer support, and religion.

Discussion

The findings underscore the biopsychosocial nature of adolescent mental health in low resource setting. These results align with mulitsystemic resilience models emphasising that resilience emerges through dynamic interactions between individual, familial, social and cultural systems. Interpersonal connectedness, particularly with trusted adults and peers, played a pivotal role in buffering the effects of chronic stress and socioeconomic disadvantage.

Objectives

The United Kingdom's NHS Talking Therapies programme currently provides talking therapy free at the point of use, namely by high-intensity cognitive behavioural therapy practitioners. However, the programme suffers from chronic workforce shortages and limited treatment options. This study explores the experiences of participants in a pilot programme designed to address these issues by introducing psychotherapeutic counsellors from alternate underrepresented therapeutic modalities into the NHS Talking Therapies programme. The research aimed to understand the positive and negative aspects of working within the Talking Therapies programme and fit with similar research on therapeutic practice in the NHS.

Methods

Two focus groups were conducted with participants of the pilot programme. The data were analysed via template analysis.

Results

The results touched on a number of themes, revealing both the positives of working within the talking therapies programme (wide client base, collegiality, job stability and opportunity) as well as pressures from service delivery standards, long waiting lists and the reality of working in a structure that was designed for a different therapy. Participants navigated these pressures by adapting their therapeutic practice, sometimes in ways that they found unethical.

Conclusion

The ongoing ethical concerns suggest that accommodations on the part of the NHS would be fruitful alongside adaptations on the part of practitioners.

However, the programme suffers from chronic workforce shortages and limited treatment options. This study explores the experiences of participants in a pilot programme designed to address these issues by introducing psychotherapeutic counsellors from alternate underrepresented therapeutic modalities into the NHS Talking Therapies programme. The research aimed to understand the positive and negative aspects of working within the Talking Therapies programme and fit with similar research on therapeutic practice in the NHS.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Methods</h3>\u0000 \u0000 <p>Two focus groups were conducted with participants of the pilot programme. A Duoethnographic Account of Anti-Racism Training in Counselling Pedagogy 反种族主义训练在辅导教育学中的多民族志叙述

Introduction

As authors, we co-facilitate a teaching session on anti-racism as part of a module on ethical and cultural issues on an MA in counselling and psychotherapy in the North of England to a cohort of between 14 and 24 students. We have been ongoingly evaluating and conducting research with the student groups involved to try and improve the teaching session.

Methodology

This article reports on our duoethnographic approach to consider our learning from our pedagogical experience.

Findings

Themes emerged from our conversations using Colaizzi's phenomenological method which were: defences, pedagogies of discomfort, the work, and transformation. We also present an exhaustive description of the anti-racist work involved.

Discussion and Recommendations

We draw out implications and recommendations for an anti-racist pedagogy in counselling education, focusing on each of our responsibility to undertake the deep self-examination and listening to others that this requires.
