Research-informed counselling and psychotherapy: A training and accreditation agenda 以研究为基础的咨询和心理治疗:培训和认证议程

Training and accreditation standards play a critical role in the development of a research-informed agenda. This commentary on Barkham et al. (Counselling and Psychotherapy Research, 2024) discusses obstacles to fully integrating research into the training and standard agendas, and the potential role that the British Association for Counselling and Psychotherapy (BACP) can play in helping to overcome such obstacles. Knowledge of research evidence is currently limited in training and accreditation standards for counselling and psychotherapy, and course tutors are often not familiar—or engaged—with research findings. In the development of professional standards (for instance, within SCoPEd), the BACP should work to ensure that research competencies are comprehensive, contemporary and explained in a granular manner. Further helpful developments might include an annual research-oriented conference for trainers and clinical supervisors, a journal dedicated to disseminating research and good practice in training and clinical supervision and/or a training/clinical supervision research network to coordinate activities in these area. BACP events—across all elements of the profession—should strive to address issues of research awareness and participation, and the use of research to inform practice. Critically, across all of these possibilities, BACP's Research Department, alone, cannot be left to support moves to a research-informed profession. Rather, an organisation-wide initiative is needed in which appreciation of the research evidence is at the heart of all aspects of the Association's work.

A perspective on neurodivergent-affirming relational practice: Exploration of identity and the healing process 从神经变异体肯定关系实践的角度:探索身份和治疗过程

Background

This commentary examines the historic primacy of behavioural interventions in psychotherapeutic work with neurodivergent people. Such interventions have been increasingly criticised as failing to affirm aspects of neurodivergence as significant for one's identity and positive sense of self, instead emphasising conformity to socially and academically determined norms, which may not always be the goals of the individual engaging with therapy.

Findings

Relational therapeutic interventions, which have not traditionally been centred in clinical approaches with this group of people, have great potential to address the inadequacies of more behaviourally oriented modalities as they were designed to address experiences of internalised stigma and invalidation, enhance a cohesive sense of self, affirm intersectional identity, tune into emotional experiences, and engage in healing through relationships and connection.

Discussion

While further research is needed to support the efficacy of relational approaches in work alongside neurodivergent people, their application is promising and aligned with the expressed needs of this community.

The relationship between lifelong learning and professional development activities in a Portuguese sample 葡萄牙样本中终身学习与专业发展活动之间的关系

Objective

A dispositional commitment to lifelong learning (LL) is an important aspect of continuing professional development for practicing psychotherapists. Equally important is the dedication of psychotherapists to engaging in professional development activities to foster growth and maintain competence. However, there are limited available data on the range of practices and factors influencing psychotherapists' commitment to these activities. The primary objective of this study was to examine whether a higher commitment to LL would significantly predict greater participation in a range of different professional development activities.

Method

A sample of 166 Portuguese psychotherapists completed an online survey on professional development. The survey included two key measures: the Jefferson Scale of Psychotherapist Lifelong Learning and a list of professional development activities based on the Retrospective Analysis of Psychotherapists' Involvement in Deliberate Practice. Exploratory factorial analysis was conducted to determine an alternative and suitable grouping for the list of activities, and structural equation modelling was utilised to assess the relationship between the commitment to LL and engagement in these activities.

Findings

Results provided valuable insight into the diverse levels of participation among therapists across five categories of activities: (1) supervision and case discussion; (2) recording-related activities; (3) reading and focussed learning; (4) reflecting; and (5) workshops supporting ongoing professional competence. Findings indicated that a stronger commitment to LL significantly predicted increased dedication to reading, focussed learning activities and attendance to training workshops. The implications and limitations of the study are discussed concerning their practical application for professional psychologists, counsellors and psychotherapists.

Background

The rising demand for psychotherapy exceeds therapist availability, often due to financial or physical proximity constraints. Artificial intelligence (AI), particularly through chatbots, presents a promising solution, having shown efficacy in enhancing mental health.

Aims

This study investigated the psychotherapeutic relationship in the context of using chatbots for psychotherapy, in which the intimate and emotionally involving nature of the relationship might render the need for human interaction a strong deterrent to people's acceptance of anthropomorphised chatbots.

Methods

This study included four hundred ninety-four participants, aged 15 to 69 years (M = 22.77; SD = 8.43); 75.1% of them were females. They completed scales that measure attitudes towards the use of chatbots in psychotherapy, AI and seeking professional psychological help, need for human interaction in psychotherapy, importance of anthropomorphic qualities and intentions to use chatbots in psychotherapy.

Results

Individuals' engagement in using chatbots in psychotherapy is related to their attitudes towards chatbots and towards AI in general, not only to the importance that they place on chatbots having anthropomorphic traits, but also to the strength of their needs for human interaction in the psychotherapeutic relationship.

Conclusion

Technology can enhance the psychotherapeutic process, but should not replace therapists entirely for people who seek psychological assistance.

Developing a minimum data standard for student mental health services in the UK: A qualitative study with staff from university support services 为英国学生心理健康服务制定最低数据标准:对大学支持服务人员的定性研究

Objective

This study is part of the Student Counselling Outcomes Research and Evaluation (SCORE) initiative that is pooling university counselling service data to improve datasets and evidence for counselling in this sector. This study aimed to explore university counselling staff views about which data should be included in a national minimum data standard (MDS) to provide evidence for counselling in the sector.

Method

Semi-structured focus groups were conducted with university counselling staff including counsellors, service leaders and mental health advisors. They explored data to include in an MDS, current data collection practices, service and sector level data collection barriers. Two researchers performed content analysis involving identifying fields for inclusion. Thematic analysis was used to identify contextual issues surrounding data collection. Ten focus groups were conducted with 41 participants across 26 university counselling services.

Results

Content analysis identified fields of information for the MDS within the categories of student characteristics, demographics, clinical assessment and service evaluation. Thematic analysis explored context and culture surrounding data collection, barriers and facilitators, institutional influence and ideal data collection practices. This highlights the need for service support and training around data collection, clear processes, and adequate IT infrastructure and staff resources.

Discussion

These findings will inform the development of an MDS as part of the SCORE study. The next stage will involve further development of the MDS and piloting in services. The longer term aim is to implement an MDS across university counselling services, enabling the growth of evidence for the sector which may allow increased funding and service provision.

This study aimed to explore university counselling staff views about which data should be included in a national minimum data standard (MDS) to provide evidence for counselling in the sector.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Method</h3>\u0000 \u0000 <p>Semi-structured focus groups were conducted with university counselling staff including counsellors, service leaders and mental health advisors. They explored data to include in an MDS, current data collection practices, service and sector level data collection barriers. Two researchers performed content analysis involving identifying fields for inclusion. Thematic analysis was used to identify contextual issues surrounding data collection. Ten focus groups were conducted with 41 participants across 26 university counselling services.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Results</h3>\u0000 \u0000 <p>Content analysis identified fields of information for the MDS within the categories of student characteristics, demographics, clinical assessment and service evaluation. Thematic analysis explored context and culture surrounding data collection, barriers and facilitators, institutional influence and ideal data collection practices. This highlights the need for service support and training around data collection, clear processes, and adequate IT infrastructure and staff resources.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Discussion</h3>\u0000 \u0000 <p>These findings will inform the development of an MDS as part of the SCORE study. The next stage will involve further development of the MDS and piloting in services. Background

Breathing practices are increasingly being utilised in therapeutic interventions for mental health problems, including depression. One of the challenges of evaluating the impact of breathing exercises on the treatment of depression is the variability in the type of exercises used.

Aims

To bring greater clarity in this area, this scoping review aimed to identify, categorise and synthesise breathing interventions that have been evaluated for depression, in order to inform further research and reviews.

Method

A thematic synthesis approach was used to analyse the descriptions of breathing interventions in order to categorise these via their common features.

Results

Thirty papers were included in the final synthesis. Main themes developed from thematic synthesis were related to procedural aspects of breathing interventions, namely: attention and awareness, using the body and senses, and paced breathing. Clinical implications and suggestions for further research are discussed.

Aim

This perspective piece will consider utilising the concepts of positive psychology, specifically in terms of relationships through the application of artificial intelligence (AI) for therapeutic intervention. We will provide an overview of positive psychology, consider how this can be applied in the therapeutic setting and how AI, and more currently artificial wisdom, can support this application.

Findings

Through an overview of existing research concerning the use of AI in therapeutic environments, it is evident that research is looking into this area with the prospect of supporting a more diverse client base, and it is an area therapists need to be aware of, and potential training in.

Discussion

The paper considers the future use of AI and artificial wisdom, providing potential insights into the direction of research in this area.

Objective

Dropout from psychotherapy remains an issue across various treatment modalities and psychological disorders, with roughly 20% of clients failing to complete treatment. Dropping out of psychotherapy is associated with worse psychological and physical health outcomes. This study aimed to use a clinically generalisable definition of dropout to identify risk factors for dropping out of psychotherapy.

Method

Retrospective chart review methods were used to collect data on 203 clients seen at a community-based clinical psychology doctoral training clinic.

Results

Independent samples t-tests and chi-squared tests for independence indicated that clients who dropped out of psychotherapy were more likely to be non-students, live farther away from the treatment clinic and no-show at least once in the first four sessions of therapy.

Discussion

To address dropout risk, it is important that clinicians be attuned to possible indicators of structural and motivational barriers to treatment engagement.

Objective

This study aimed to examine the impact of a brief body scan session on individuals' anxiety and pain levels considering individuals' levels of symptom severity (low vs. high).

Method

The sample was composed of 355 undergraduate or graduate students. Participants completed a series of questionnaires, performed a 14-min body scan exercise and completed some of the questionnaires a second time. Two questionnaires were aimed at identifying individuals reporting high and low levels of symptomatology (i.e. anxiety and somatic symptoms) in their everyday life, while other questionnaires were aimed at assessing the effectiveness of the body scan session in reducing current symptoms.

Results

As hypothesised, body scanning was more effective in decreasing anxiety and pain for students with high-symptom severity than for those with low-symptom severity.

Discussion

We demonstrated that individuals with high levels of symptoms, and especially anxiety symptoms, can benefit from a brief form of body scan.

Aim

Black queer men in the UK are increasingly at risk of mental health problems related to anxiety and depression, often triggered by traumatic experiences. This study explored the efficacy of integrative practice with Black queer men who experienced trauma from a decolonised, intersectional and queer-affirming perspective.

Method

One-to-one semi-structured interviews with nine integrative practitioners were completed. Data collected from these interviews were analysed using Braun and Clarke's six stages of reflexive thematic analysis (RTA).

Findings

RTA identified four subordinate themes: (1) understanding the experience of Black queer men, (2) developing the working alliance and use of self in practice, (3) effective integrative practice and (4) intersectional differences and considerations. Findings discussed the varying forms of trauma Black queer male clients experienced that impacted their mental health and how developing a strong working alliance with Black queer men was crucial when supporting them to recover from trauma. Additionally, the participants described a breadth of trauma-informed integrative counselling and coaching practice. A decolonised approach utilising the intersection of race, gender and sexuality of Black queer men was successfully integrated into the approach used by practitioners in this study.

Conclusion

Overall, this study evidenced that a decolonised perspective to integrative practice effectively supports Black queer men to recover from trauma. Future research should focus on an integrated approach with Black queer men and other gender and sexual ethnic minority clients who experienced trauma.
