The purpose of this prospective single-center study was to determine the feasibility of minilaparotomy for aortoiliac segment and its effect on intraoperative and postoperative outcomes. Materials and Methods: From August 2020 to July 2022, the authors used minilaparotomy in 24 patients with aortoiliac atherosclerotic occlusive disease. The comparison group included 27 patients undergoing laparotomy. The age of the patients ranged from 58 to 83 years, among them 37 men and 14 women. In the group with traditional access, 9 aorto-bifurcational and 18 linear operations were performed; in the minimal incision group, there were 7 bifurcational and 17 linear interventions. Results and Discussion: The duration of surgery was 196.5±26.7 minutes for operations with traditional access, and 146.3±16.7 minutes in the group with minimal incision. On average, minimal incision surgeries were shorter by 52.3±14.4 minutes (p≤0.05). For patients from the group with minimal incision, the length of stay in intensive care was 1.27±0.4 days, and with traditional access 2.3±0.3 (p≤0.05). Postoperative bed stay for the group with mini-access was 6.4±0.5 days, for patients with classic access 9.2±1.2 days (p≤0.05). 4 patients in the comparison group had postoperative paresis, and in one of them it was complicated by eventration. In the long-term postoperative period, two patients in the comparison group required surgery for an incisional hernia. Conclusions: Minimal incision access to the aorta can reduce surgical trauma and intervention time, shorten the patient’s stay in the intensive care unit and postoperative bed, and minimize perioperative and postoperative complications. During repeated reconstructive interventions on the aorta, a “high” mini-access to the aorta allows to quickly reach the optimal area of the aorta for anastomosis and minimize organ trauma, as well as prevent adhesions in the abdominal cavity.

Ocular manifestations in autoimmune diseases are mainly related to the formation of a chronic inflammatory process in the body. This is caused by an error in the body's histocompatibility system, which results in the lymphocyte attacking its own cells, which the lymphocyte considers foreign. Various autoimmune diseases with ocular symptoms may have different inflammation-related pathophysiology, ranging from increased vascular permeability in diabetic retinopathy to lymphocytic pressure in the exocrine glands in Sjögren's syndrome. The inflammatory process in the eye tissues leads to symptoms such as pain, redness, photosensitivity, tearing and reduced visual acuity. One of the most common autoimmune diseases associated with ocular symptoms is systemic lupus erythematosus, in which 50% of patients experience ocular symptoms. Ocular manifestations may worsen or improve depending on disease activity. Ophthalmological examination is extremely important for patients with autoimmune diseases because it allows for the early detection and treatment of eye-related diseases, monitoring the side effects of medications taken, and identifying other related health problems.

Cancers during pregnancy are a relatively rare phenomenon, but the risk of developing them increases with a woman's age. As more women choose to become pregnant at an older age, the incidence of cases may rise in the future. This article discusses the diagnosis and treatment of the most common cancers in pregnant women: breast cancer, cervical cancer, ovarian cancer, thyroid cancer, colorectal cancer, lung cancer, lymphomas, and melanoma. The aim of this work is to present optimal methods for the treatment and diagnosis of cancers during pregnancy, as the approach for pregnant patients is limited in options. Methods: We analyzed scientific papers on the safety of diagnostics and treatment of breast cancer, cervical cancer, ovarian cancer, thyroid cancer, colorectal cancer, lung cancer, lymphomas, and melanoma. We used PubMed, Google Scholar, and specialized literature to search for information. Conclusions: Due to the limited options for treatment and diagnosis in pregnant women, each case of illness should be considered individually, with an assessment of potential risks and benefits for both the mother and the child. Increased oncological vigilance should be maintained in pregnant women since some pregnancy symptoms often overlap with the initial symptoms of cancer.

Pediatric and adolescent scoliosis is one of the most common and prevailing orthopedic pathologies leading to spinal deformation. Cases of scoliosis where curving of the spine is <45% treated mostly surgically. However, when the spine is straightened with an implant, there are risks and complications. Hence, interest to conservative treatment methods has increased significantly. This article discusses advantages and disadvantages of non-invasive methods for treating scoliosis. The disadvantage is the duration of treatment: treating scoliosis using conservative methods such as physical therapy and wearing bondages can take a long time to achieve results. Also, there is a need for regular control: conservative treatment of scoliosis requires regular control by doctors and specialists to assess progress and change treatment plan if necessary. The most promising methods of conservative treatment, including correction of the muscular system, have been identified – adaptation physical exercises and gymnastics due to physical impact on kyphotic and scoliotic deformity. This includes breathing and physical exercises, therapeutic horse riding and wearing rigid torsion brace.

Purpose of the Study: To analyze the effectiveness of using sodium deoxyribonucleate (SDN) for pelvic adhesions in rats in experiment. Materials and Methods: The experiment was carried out on 90 laboratory rats. Three groups of 30 animals each were formed: group 1 – control; group 2 – rats with a simulated adhesive process in the pelvis without subsequent treatment; group 3 – animals with a model of adhesive process in the pelvis receiving intramuscular SDN for 10 days. The adhesive process in the pelvis was modeled in rats by ligating the uterine horns on both sides. The expression of CD4, CD8, CD20, CD138 was studied using DP-SOFT software program and subsequent statistical processing. Results: In animals of the control group, a small number of cells with expression of CD4+, CD8+ and CD138 markers were found in the parietal peritoneum (PP). In perimetrium (PM), no positively stained cells of the studied immunophenotype were found. In group 2, animals showed an increase in the number of cells in the PP positively expressing CD4+ and CD138+, and the appearance of CD56+ cells. An increase in the number of CD138+ and CD4+ cells was found in the rats’ PM. In group 3, compared to the control, the count of CD4+ cells, CD56+ cells and CD138+ cells significantly increased. In the PM of the operated animals, we found an increase in the number of cells positively expressing CD4+ and CD138+, and the appearance of CD56+ cells. In the PM of rats, an increase in the number of CD138+ and CD4+ cells was noted, which indicates activation of the humoral immune system. In group 3, the count of CD4+, CD56+ and CD138+ – cells in the PP significantly increased compared to the control. PM was characterized by a similar distribution of cells. Conclusion: After surgery, the adhesive process causes a change in the ratio of leukocyte subpopulations in the affected area towards an increase in their number in the parietal peritoneum rather than in the perimetrium. The use of SDN has an immunomodulatory effect and stimulates immune responses by increasing the number of immunocompetent cells in the surgical site.

The authors presented a clinical case of left subclavian steal syndrome in a 62-year-old man. The main complaints were dizziness, tinnitus, memory loss and unsteady gait. The patient had severe concomitant pathology: coronary heart disease, hypertension, heart failure. X-ray endovascular technologies (balloon dilatation and stenting) made it possible to completely restore blood flow in the affected segment. After treatment, the above complaints disappeared. Minimally invasive X-ray endovascular techniques have particular benefits in patients with severe comorbid pathology.

Lymph nodes play an important role in the life of humans and animals. In the modern concept, the lymph nodes are the main homeostatic organs of the internal environment of the body, the lymph nodes are part of the lymphatic bed, on the other hand, they are the organs of the lymphoid (immune) system. Purpose: The work presents data on the morphofunctional state and microstructure of regional lymph nodes in experimental hypothyroidism and under correction. Materials and Methods: Experimental hypothyroidism in animals was induced by per os administration of mercazolil at a dose of 20 mg per 100 g of body weight with drinking water for 21 days. To correct thyroid disorders, starting from the 22nd day of the experiment, the animals received for 30 days an iodine-containing dietary supplement "Renaissance Plus" balm in powder at the rate of 2 μg/100 g of body weight and a decoction of the roots of the white cinquefoil (Potentilla alba L.) 50 ml per 100 g per day with drinking water. Results: The results of the study showed that in conditions of hypothyroidism, the lymph node becomes compacted with a decrease in the main structural and functional zones against the background of reduced lymphopoietic function. In the lymphoid nodule, there is a decrease of 1.43 times in the number of lymphoblasts, 1.56 times in the number of medium lymphocytes, 1.50 times in the number of small lymphocytes due to an increase of 1.29 times in the number of macrophages per unit area of the lymphoid nodule in the lymph node. There was a static decrease in the number of small lymphocytes in all structural and functional areas; the number of blasts decreases in the lymphoid nodule and increases in the paracortex and Billroth's strand; the number of macrophages increases in the lymphoid nodule, cerebral sinus, the number of mature plasma cells decreases in the paracortex and Billroth's strand, and there is also inhibition of cellular reactions in the lymph node, which reflects a decrease in the lymphopoietic function of immune activity and as a result of the immunosuppressive action of mercazolil. Conclusions: After an administration of corrigents, the area of the medulla and paracortex was restored to the control level and the ratio of cortical and medulla in the overall structure of the lymph node was leveled.

Purpose: The work presents data on the morphofunctional state and microstructure of regional lymph nodes in experimental hypothyroidism and under correction. Materials and Methods: Experimental hypothyroidism in animals was induced by per os administration of mercazolil at a dose of 20 mg per 100 g of body weight with drinking water for 21 days. To correct thyroid disorders, starting from the 22nd day of the experiment, the animals received for 30 days an iodine-containing dietary supplement \"Renaissance Plus\" balm in powder at the rate of 2 μg/100 g of body weight and a decoction of the roots of the white cinquefoil (Potentilla alba L.) 50 ml per 100 g per day with drinking water. Results: The results of the study showed that in conditions of hypothyroidism, the lymph node becomes compacted with a decrease in the main structural and functional zones against the background of reduced lymphopoietic function. In the lymphoid nodule, there is a decrease of 1.43 times in the number of lymphoblasts, 1.56 times in the number of medium lymphocytes, 1.50 times in the number of small lymphocytes due to an increase of 1.29 times in the number of macrophages per unit area of the lymphoid nodule in the lymph node. There was a static decrease in the number of small lymphocytes in all structural and functional areas; the number of blasts decreases in the lymphoid nodule and increases in the paracortex and Billroth's strand; the number of macrophages increases in the lymphoid nodule, cerebral sinus, the number of mature plasma cells decreases in the paracortex and Billroth's strand, and there is also inhibition of cellular reactions in the lymph node, which reflects a decrease in the lymphopoietic function of immune activity and as a result of the immunosuppressive action of mercazolil. Purpose of the Study: In this article, the team of authors shares their own experience of performing the procedure of therapeutic angiogenesis with plasmid DNA in the department of surgery. Materials and Methods: From November 2020 to December 2021, the department performed 64 procedures of therapeutic angiogenesis using plasmid DNA in 32 patients with distal steno-occlusive lesions of lower limb arteries, with CAI II-A - CAI III according to Pokrovsky-Fontaine. All patients included in the statistics of this work were questionnaired (SF-36) and thoroughly interviewed during the writing of this article. Results: Of the 32 patients, only two (6.25%) noted a deterioration in their condition, of which one patient with CAI III and diabetes underwent hip amputation after 1.5 months. In the second patient, three months after the repeated procedure of therapeutic angiogenesis, the distance of pain-free walking decreased from 350 to 250 meters, which can be associated with severe nicotine addiction and inability to quit smoking. Four patients did not note any significant changes in their condition in the period from 2.5 to 6 months after repeated administration of the drug. 12 patients (37.5%) noted moderate positive changes in the form of an increase in pain-free walking distance, a decrease in pain, and an improvement in quality of life. 14 patients (43.75%) claimed a significant increase in pain-free walking distance and assessed the results of the therapy as good. The greatest positive dynamics were observed in the groups CAI II-A and CAI II-B. The peak of positive dynamics of pain-free walking distance was noted 3-6 months after the second administration of the drug. Conclusion: As a result of treatment with plasmid DNA, we noted a significant positive effect from the procedure, especially in patients with intermittent claudication. In a group of patients with critical ischemia, the procedure of therapeutic angiogenesis became the only means to preserve the limb.

The article presents almost forty years of experience in the treatment and follow-up of congenital hip dislocation in 839 children at Tver Children's Regional Clinical Hospital (Russia). An analysis of conservative and surgical treatment methods was carried out taking into account concomitant pathology. The children were aged from 3 months to 13 years, girls predominated (78%). Bilateral damage to the hip joints was observed in 278 children (33%), unilateral - in 561 children (67%). Surgical treatment was performed in 312 patients (37.2%).The majority of patients admitted with a diagnosis of congenital hip dislocation were treated conservatively (527 - 62.8%). Long-term results of treatment (12-27 years from the start of treatment) were monitored in 174 patients. 123 of them (70.7%) obtained good anatomical and functional treatment results (no complaints or functional limitations, anatomically correct formation of the hip joints according to radiation diagnostics), 45 (25.8%) - the treatment results were assessed by us as satisfactory (patients considered themselves healthy, but X-rays revealed subluxation of the femoral head without degenerative-dystrophic changes). In 6 surgically treated patients (3.4%), deforming coxarthrosis occurred, which required endoprosthetics at the age of 27-33 years. Dysplastic changes in the musculoskeletal system require an integrated approach involving specialists of various profiles at all stages of the child’s treatment.

Combustion of fossil fuels, industrial emissions, biomass and waste combustion results in an increase in the amount of harmful substances such as sulfur dioxides, nitrogen oxides, carbon monoxide, ozone, particulate matter or aromatic hydrocarbons in the environment. The air full of pollutants that we all breathe affects our respiratory and cardiovascular systems and is also a factor leading to the development of cancer. The polluted air we breathe in every day drives the influx macrophages that release inflammatory mediators such as interleukin-1β, accelerating the expansion of cells with EGFR mutations. Overexpression mutated EGFR gene is characteristic of non-small cell lung cancer. Adenocarcinoma is one of the most common lung cancers. It accounts for 40% of all types of lung cancer. In the European Union, Bulgaria and Poland are among the most polluted countries, while Sofia, Nicosia and Warsaw are the three most polluted capitals. Solid particles suspended in the air have been recognized by the International Agency for Research on Cancer (IARC) as a factor group I carcinogen. The concentration of PM10 measured at regional background stations, i.e. outside the direct impact of traffic pollution and outside city centers, in Poland is almost the highest in Europe and amounts to over 60 µg/m3 with the standard up to 20 µg/m3. An adult person inhales 10-12 m3 of air per day, and with it inhales the pollutants contained in it into the body. The aim of our work was to examine the correlation between air pollution and lung cancer and to conclude that anyone may be at risk.
