In-depth interviews were conducted with 83 residents of the dental profile of graduates of medical universities in Moscow, Krasnoyarsk and Ufa. As a result of studying the opinions of graduates of medical universities, certain difficulties were identified in the prospective development of professional growth and the passing the accreditation procedure. The dissatisfaction of the respondents was associated with the imperfection of the system of training and motivation of graduates to improve their professional skills and competencies. In this regard, measures are needed to improve the quality of training of medical personnel, as well as ways to improve and increase the availability of quality medical education.

Aim: to assess the dynamics of histolological changes in the lung under acute baclofen poisoning. Materials and methods: The study was performed on 20 male Wistar rats weighing 200-215 g and aged 20 weeks. The animals were divided into four groups (5 animals in each) – controls, group 1 (baclofen, 3h.), group 2 (baclofen, 4,5 h.) and group 3 (baclofen, 24 h.). Results: The thickness of intraalveolar septi in group 1 (baclofen, 3 h.) was considerably lower than in the group of controls. 4,5 hours after baclofen administration the thickness of intraalveolar septa was significantly higher than in group 1 (baclofen, 3 h.) and in the group of controls. 24 hours after baclofen administration the thickness of intraalveolar septa was significantly higher than in group 1 (baclofen, 3 h.) and in the group of controls. The diameter of the alveoli was considerably higher in all study groups than in the group of controls. The diameter of the alveoli 3 hours after baclofen administration was higher than 4,5 and 24 hours after baclofen administration, but the difference was not significant. Conclusion: In baclofen poisoning, a certain complex of general pathological processes in lungs develops. It includes circulatory disorders, increased vascular permeability, infiltration of intraaveolar septa. The quantitive assessment of the changes can be used to assess the severity of general pathological process in acute baclofen poisonings and to determine the exact time of the poisoning.

Background: Antibiotic resistance is an issue of concern to physicians, scientists and patients. It has an impact on the increase of morbidity, length of hospital stay and mortality of patients. There is also a correlation between the resistance of bacteria to antibiotics and their geographical location. Objective: Analysis of the prevalence of nosocomial skin and soft tissue infection pathogens and the rate of development of antimicrobial resistance in the Siberian and Far Eastern regions of Russia. Methods: Data on microbial antimicrobial susceptibility and prevalence of major genetic determinants of antimicrobial resistance were analyzed. The work was carried out on the basis of data from the AMRmap online platform. Comparison of indicators was based on samples obtained from adult surgical patients with infected skin and soft tissues throughout 2016-2020 in the Siberian and Far Eastern regions of Russia. Results: 5 microorganisms were identified in adult surgical patients with infected skin and soft tissues on the territory of the Siberian region and the Far Eastern region (Russia) in 2016-2020. They were Klebsiella pneumoniae, Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli, Acinetobacter baumannii. The authors used the AMRmap online platform to study the sensitivity and resistance to various antibacterial drugs applied to the three most important pathogens in the regions. Conclusions: The results indicate a rapid spread of resistance genes in the studied strains of Klebsiella pneumoniae, P. aeruginosa, S. aureus, E. coli, Acinetobacter baumannii. Systems for monitoring resistance to antibacterial drugs by laboratory methods need to be improved. It is also necessary to create new databases and improve the existing ones. These will help to select the optimal drug treatment strategy for patients according to their resistance profile.

An important task of gerontology and lymphology is to improve the function of the lymph nodes and reduce the risk of polymorbidity. In our experiment, we have shown the possibility of phytonutrient control over the lymph node work by changing of structure and trace-element profile. The purpose of the work is to assess the effect of a phytocomposition on the rate of aging of somatic lymph node by characterizing the structure and trace elements. Materials and Methods: The experiment was performed on Wistar rats with conditional division into old and young animals. Half of old animals received bioactive phytocomposition. The method of X-ray fluorescence analysis using synchrotron radiation (RFA SR) was used to determine trace elements (Mn, Fe, Cu, Zn, Se) in the inguinal lymph node. Results: Aging leads to immune failure accompanied by destabilization of compartments and trace element deficiency. Phytonutrient delivery format (flavonoids and trace elements) in the form of a plant food supplement helps to delay the onset of lymph node aging and improving trace element status. We have indicated the significant contribution of trace elements in restoring the work of the lymph node based on the total intensity coefficient of age changes. Conclusion: Phytonutrients improve functioning of the lymph node in aging. Thus, they can be employed as prospective anti-aging compounds in medical and wellness programs.

Despite scientific and technological progress and the emergence of modern diagnostic methods, the number of neglected cases of oncopathology has been increasing annually, and its mortality rate has been also increasing. Thus, over the past five years in Russia it increased by 12.4%. Moreover, the one-year mortality of patients with oral mucosa oncological pathology (OMC) is over 50 %, the 5-year survival rate is 24 % for men and 37 % for women. One of the demanded areas of effective medicine is the use of minimally invasive organ-preserving diagnostic tests, which unfortunately have not been widely used in practical dentistry. The purpose of the study is to improve early diagnostic efficiency for detecting pathological processes in the oral mucosa through the combined use of optical and cytological methods for its study. We examined 350 patients of both sexes aged from 20 to 67 years within consultation appointments with the aim of specifying the diagnosis and determining the algorithm of treatment. Methods: clinical, analytical, physical (autofluorescence dentistry), laboratory (liquid cytology), statistical. Conclusion: combined use of methods of autofluorescence stomatoscopy and liquid cytology increases efficiency of diagnostic stage, reduces risk of neglected stages of pathological processes, improves quality of patient's life.

Chemical peels are recently used in gynecology for the treatment of pigmentation in the anogenital area. Trichloroacetic acid is a peel used most commonly in cosmetic dermatology. It has a great potential to lighten and rejuvenate the skin in the anogenital area. We present a case of pigmentation of the vulva due to photoaging and pregnancy and its successful and efficient treatment with trichloroacetic acid. We describe the precise technique of trichloroacetic acid peel approach in the anogenital area.

The article presents results of a program of psychological support provided to medical residents. The program was implemented at Astrakhan Medical University (Astrakhan, Russia) in February 2023. This program was aimed at psychological correction of communicative skills and empathic abilities, as professionally important qualities of a medical professional. 38 residents of the specialty Infectious diseases were involved in the program. We developed and implemented the program of psychological support, which consists of four stages: 1) psychological diagnosis; 2) theoretical block; 3) psychological correction in form of individual and group trainings on empathy level and communication skills (conversation, cognitive-behavioral psychotherapy, transactional analysis, Ericksonian hypnosis techniques); 4) re-diagnosis in order to assess psychological indicators. Mastering the techniques of managing the emotional state positively influences the psychological well-being of medical residents. Our program made it possible to improve the soft competencies required for medical professionals.

The article investigates special features of health care policies under martial law conditions. It determines that the national model of the health care system management has been in an active phase of the medical reform since 2015. The reform covers about 20,000 health care facilities. Under COVID19 pandemic conditions, the national model of health care management operates on the principles of synchronous action of all entities at all levels of management. In the conditions of the full-scale war, since February 24, 2022, the national model of the health care system has undergone structural and functional changes and its facilities operate as critical infrastructure institutions in an asynchronous mode. The health care institutions management in an asynchronous mode is mainly entrusted to local self-government bodies, and ensuring the sustainability of medical institutions depends on the operational situation in the area where medical institutions are located. At the central level of branch administration, a number of measures of legal, economic and organizational support for the implementation of Ukraine’ state policy aimed at the availability and quality of medical care under martial law conditions have been introduced.

It determines that the national model of the health care system management has been in an active phase of the medical reform since 2015. The reform covers about 20,000 health care facilities. Under COVID19 pandemic conditions, the national model of health care management operates on the principles of synchronous action of all entities at all levels of management. In the conditions of the full-scale war, since February 24, 2022, the national model of the health care system has undergone structural and functional changes and its facilities operate as critical infrastructure institutions in an asynchronous mode. The health care institutions management in an asynchronous mode is mainly entrusted to local self-government bodies, and ensuring the sustainability of medical institutions depends on the operational situation in the area where medical institutions are located. The article focuses on the need for the integration of civil and military medical institutions into the unified medical space of Ukraine. It substantiates the development of a holistic system of medical and evacuation support for the troops in combination with the resource capabilities of the civil health care system. It also defines the necessary prerequisites for the development of such a system, namely unity of views on the organization, tactics and procedure for the use of forces and means of medical services, and the capacity of civilian health care facilities for medical and evacuation support of military personnel. The Article describes specific management approaches regarding the medical and evacuation support capabilities of the Armed Forces of Ukraine (AFU) that will contribute to the implementation of efficient medical and evacuation measures and adequate support as well as measures for the medical protection of military personnel, etc. It also proves the importance of defining and clearly demarcating levels of medical care and required resources and considers the possibility of working out interdepartmental civil and military cooperation as well as refining the system of medical and evacuation support for servicemen of the Armed Forces of Ukraine in the conditions of the unified medical space of Ukraine.
