Virtual Screening and Inhibition of Middle East Respiratory Syndrome Coronavirus Replication by Targeting Papain-like Protease 以木瓜样蛋白酶为靶点虚拟筛选及抑制中东呼吸综合征冠状病毒复制

Infection by the emerging, potentially zoonotic Middle East Respiratory Syndrome Coronavirus (MERS-CoV) presents a severe health hazard to humans and is often fatal. Given the lack of particular medicines against MERS-CoV, drug discovery studies are needed to bridge this knowledge gap. In this study, we introduce virtual screening-guided identification of MERS-CoV Papain-like Protease (PL pro )-binding drugs. After a two-step virtual screening method, enzyme assays and antiviral testing with a MERS-CoV plaque reduction assay were used to further investigate the five compounds with the highest computational score. The top five screened compounds showed a 10.2–40% decrease in MERS-CoV PL pro activity. The top two compounds showed promising inhibition of MERS-CoV replication, reducing virus plaque formation by 30.6% and 24%. Compounds 1 and 4 in this study can be further modified to target binding with MERS-CoV PL pro active triad residues. Furthermore, the compounds produced stable interaction with the protein and protein conformation. With their reported inhibition of MERS-CoV enzyme and virus replication, supported by favorable absorption, distribution, metabolism, and excretion and toxicity profiles, the two reported benzimidazole and piperazine derivatives could be considered lead compounds against MERS-CoV.

Filgrastim is a recombinant human Granulocyte Colony-stimulating Factor (G-CSF) approved by the US Food and Drug Administration [1]. It is used to treat patients with neutropenia caused by chemotherapy [2–5], human immunodeficiency virus infection [6], bone marrow transplant [7–9], or in cases of congenital [2,7], cyclic [2,10], or idiopathic neutropenias [2,11]. It is a hematopoietic growth factor that influences leukopoiesis [12–14], and affects the proliferation and differentiation of neutrophils within the bone marrow [15,16] and possibly other sites (e.g., spleen) [17,18]. It binds directly to G-CSF receptors on neutrophil progenitor target cell surfaces [19,20] and stimulates neutrophil proliferation, thereby increasing neutrophil counts and activity [21,22].

Pharmacists serve as an essential part of our healthcare system. The role of community pharmacists has recently evolved, beyond dispensing medication, toward the enhancement of patient care and pharmacist–patient interaction. Their knowledge of patients’ medical history and thus, identifying drug interactions and dosing schedules, plays an important role in limiting dangerous and fatal drug interactions. Pharmacists taking this path promise to consider the interest toward their patients and community, by applying knowledge, experience, and skills to provide the best outcomes for patients [1].

Strengthening Mortality Statistics for Health Programs in Malaysia: Lessons from the Field 加强马来西亚卫生方案死亡率统计:来自实地的经验教训

Malaysia is one of a few Asian countries with a longstanding history of vital registration systems. Yet, issues with data quality have limited the utility of available vital statistics for public health policy, program evaluation, and research [1,2]. Although death registration is complete for states located on Peninsular Malaysia, the problem of marginal underreporting of deaths still persists in the eastern states of Sabah and Sarawak [3]. However, initiatives have been recently launched by the Malaysian National Registration Department [Jabatan Pendaftaran Negara (JPN)] to resolve issues with registration rules and infrastructure in these states. The more significant concern lies with the poor quality of registered causes of death. About 50% of all deaths in Malaysia occur in hospitals and are registered with medically certified causes, which are of good quality. However, the remaining deaths that occur at home are registered with causes reported by family members. For statistical purposes, these are termed as “nonmedically certified deaths.” Each year, only about a third of these deaths are reported to be from specific causes, and the remaining 65–70% of these deaths are registered annually with vague and ill-defined causes including “old age,” “heart failure,” and “sudden death” [3]. These limitations in vital registration data necessitated the use of generic model-based adjustments to fill these data gaps, while estimating national mortality and burden of disease estimation for the years 2000 and 2008 [4,5]. These persistently high proportions of illdefined causes can still be observed even for the most recent period from 2009 to 2017 (Figure 1).

Recurrent Urinary Tract Infections: A Red Herring for Primary Appendiceal Carcinoma 复发性尿路感染:原发性阑尾癌的转移焦点

Recurrent Urinary Tract Infections (UTIs) are a common issue in females owing mostly to the anatomy of their urinary tract. It can also hint at a pathology pertaining to the pelvic organs such as tumors or infections that infiltrate the urinary bladder. Sometimes, albeit rarely, it can be a red herring, distracting the clinician’s attention from a more sinister cause, such as an appendiceal carcinoma. We discuss such a case where a primary mucinous appendiceal carcinoma was invading the bladder causing recurrent UTIs.

Disorders of Sex Development (DSD) is an umbrella term for a large number of rare conditions on atypical sex development, leading to sexual ambiguity and infertility. DSD is defined by congenital conditions in which development of chromosomal, gonadal, or anatomic sex is atypical [1]. In this condition, the neonate external genitalia do not emerge to be illustrative (whether male or female) [2].

Uncommon Cause of Endotracheal Tube Obstruction during Anesthesia Recovery 麻醉恢复过程中气管插管梗阻的不常见原因

Intraoperative ventilatory failure is a critical event that has been associated with various causes. However, Endotracheal Tube (ETT) obstruction remains a difficult one to recognize. It can result from blood impaction, mucus plug, manufacturing defects, and tube malfunction [1]. Once the diagnosis of ETT obstruction has been reached, a prompt response is required to restore ventilation. Replacement of the ETT is a cornerstone in management. This case report describes a sudden ventilatory failure at the end of a surgical procedure in a child under general anesthesia. A blood clot obstructing the ETT was detected and successfully managed.
术中呼吸衰竭是一种与多种原因相关的关键事件。然而,气管插管阻塞仍然是一个难以识别的问题。它可以由血液嵌塞、粘液堵塞、制造缺陷和管道故障引起[1]。一旦诊断为ETT梗阻,需要及时采取措施恢复通气。更换ETT是管理的基石。本病例报告描述了一个在全身麻醉下的儿童手术结束时突然呼吸衰竭的病例。发现血栓阻塞ETT并成功处理。

Perforated Bowel after Obstruction in a Neonate with Milk Curd Syndrome following Ready to Feed Infant Formula 在准备好喂养婴儿配方奶粉后,患有乳凝综合征的新生儿梗阻后肠穿孔

Constipation is not an uncommon disorder in infants. In neonates, “constipation” may be caused by intestinal obstruction, which could be congenital or iatrogenic. Milk Curd Syndrome (MCS) is a rare iatrogenic form of infantile intestinal obstruction because of inspissated milk. The condition is potentially preventable by using better feeding formulas and feeding techniques. MCS must always be suspected in formula-fed babies who have prolonged constipation. The condition is curable with morbidity-free survival after surgical intervention. In this report, we present a case of transverse colon perforation caused by MCS in a premature baby with morbidity-free survival after surgical intervention.
