Developing an Evaluation Framework Based on Entrustable Professional Activities for Advanced Practice Nurses in China 基于可信赖专业活动的中国高级执业护士评估框架的构建。

<div> <section> <h3> Aim</h3> <p>To address conceptual ambiguity, unclear scope, and regulatory gaps hindering advanced practice nurse role implementation in China, this study developed an evaluation framework based on entrustable professional activities to facilitate the introduction, implementation, and development of the advanced practice nurses while guiding policy formulation, educational design, and credentialing.</p> </section> <section> <h3> Background</h3> <p>Globally, advanced practice nurses are recognized for delivering high-quality, safe, and cost-effective care. Many countries have formalized this role, often marked by prescriptive authority. However, China lacks a clearly defined advanced practice nurse role and national evaluation standards, creating barriers to effective role integration.</p> </section> <section> <h3> Methods</h3> <p>A scoping review identified international evaluation frameworks and their core components. A two-round Delphi study with 27 experts established relevant entrustable professional activities, and structured consensus meetings refined the framework.</p> </section> <section> <h3> Results</h3> <p>Delphi consensus was achieved on an entrustable professional activity framework consisting of 20 indicators across five domains: assisting in clinical assessment, diagnosis, and treatment; engaging in advanced nursing practice; communicating, consulting, coordinating, and collaborating; managing teaching and promoting research innovation; and preventing and managing chronic diseases. Background

Traditional nursing delegation frameworks treat care tasks as discrete, isolated units, failing to reflect the complex realities of contemporary nursing practice.

Introduction

The Nursing Care Entanglement Theory (NCET), complemented by complexity and human factors perspectives, posits that nursing activities are deeply interwoven, contextually shaped, and relational, challenging conventional task-based delegation models.

Methods

This viewpoint article examines current delegation frameworks through the lens of NCET and related theoretical perspectives to uncover conceptual and policy-level limitations.

Findings

Existing delegation models oversimplify nursing practice, leading to fragmented care processes, missed clinical cues, and increased risks to patient safety across diverse healthcare settings.

Implications for Nursing and or Health Policy

Policy makers, healthcare organizations, and regulatory bodies must redesign delegation frameworks to acknowledge care entanglement, ensuring comprehensive assessment of delegatee capabilities and preserving nursing care integrity.

<div> <section> <h3> Aim</h3> <p>To examine how nurses’ acceptance of artificial intelligence influences job crafting over time and to explore the longitudinal mediating role of decent work perception in this relationship.</p> </section> <section> <h3> Background</h3> <p>The integration of artificial intelligence into clinical settings is accelerating, yet its impact on nurses’ proactive work behaviors remains underexplored. Understanding the mechanisms through which artificial intelligence acceptance affects job crafting is critical for optimizing workforce adaptation.</p> </section> <section> <h3> Methods</h3> <p>This longitudinal study employed a three-wave cross-lagged panel design. A total of 598 nurses from six tertiary hospitals in China completed surveys on artificial intelligence acceptance, decent work perception, and job crafting over a six-month period. Structural equation modeling and bootstrap analysis were used to test longitudinal mediation.</p> </section> <section> <h3> Results</h3> <p>Artificial intelligence acceptance showed declining stability over time, while decent work perception increased, and job crafting stabilized. Cross-lagged analyses revealed that artificial intelligence acceptance positively predicted job crafting both directly and indirectly via enhanced decent work perception. The mediating effect accounted for over one-quarter of the total effect.</p> </section> <section> <h3> Discussion</h3> <p>These findings challenge static views of technology adoption, suggesting that artificial intelligence implementation triggers dynamic psychological and behavioral responses among nurses.</p> </section> <section> <h3> Conclusion</h3> <p>Early acceptance of artificial intelligence technology significantly predicts subsequent job crafting, both directly and through the mediating role of decent work perception.</p> </section> <section> <h3> Implications for nursing</h3> <p>Enhancing nurses’ early acceptance of artificial intelligence and strengthening their perception of decent work may foster more proactive job crafting behaviors.</p> </section> <section> <h3> Implications for Nursing Policy</h3> <p>Nurse managers and policymakers should incorporate psychological support and dignity-enhancing strategies into artificial intelligence integration plans. Aim

To investigate the mediating effects of basic psychological needs and the moderated mediating effect of perceived organizational support on the relationship between career growth and job embeddedness among nurses.

Background

Job embeddedness is an important predictor of nurse retention. However, little is known about the underlying mechanism of when and why career growth affects nurses’ job embeddedness.

Methods

This cross-sectional online survey enrolled 858 nurses who worked at hospitals from 12 hospitals across 6 cities in China. Participants completed an online questionnaire that included the Nurse Career Growth Scale, Basic Needs Satisfaction in General Scale, Chinese version of the Survey of Perceived Organizational Support, and Job Embeddedness Scale. Data were analyzed using Hayes's PROCESS macro. The Reporting of Observational Studies in Epidemiology (STROBE) guidelines were used.

Results

Career growth was positively associated with job embeddedness, and this positive relationship was mediated by basic psychological needs. The indirect effect of career growth on job embeddedness via basic psychological needs was stronger among nurses with higher perceived organizational support than among nurses with lower perceived organizational support.

Discussion

Career growth is a valuable factor for predicting nurses’ job embeddedness. Basic psychological needs and perceived organizational support further explain how career growth facilitates job embeddedness.

Conclusion

The indirect effect of career growth on job embeddedness via basic psychological needs was strengthened as the level of perceived organizational support increased.

Implications for Nursing and Health Policy

Policymakers and nurse managers should put effort into promoting nurses’ career growth, satisfying their basic psychological needs, and establishing supportive work conditions to embed nurses in their present jobs.

Aim

This study investigated the role of perceived stigma in predicting organizational commitment among nurses, examining job satisfaction as a potential mediating variable.

Background

Perceived stigma in the healthcare environment is increasingly recognized. However, the mechanisms linking stigma to organizational commitment, such as job satisfaction, remain underexplored.

Methods

A cross-sectional study was conducted from March to June 2025 among 313 nurses from various healthcare settings in Egypt. Data were collected using validated instruments measuring perceived stigma, job satisfaction, and organizational commitment. Pearson correlation analyses were used, and a mediation analysis was performed using the PROCESS macro to determine the significance of indirect effects.

Results

Perceived stigma was negatively correlated with job satisfaction (r = –0.140, p < 0.05) and its subscales and showed a weak correlation with the organizational commitment subdomains. Mediation analysis indicated that stigma significantly predicted lower job satisfaction (β = –0.312, p = .013), which in turn was positively associated with organizational commitment (β = 0.227, p < .001). The direct effect of stigma on commitment was positive and significant (β = 0.213, p = .028). In contrast, the indirect effect through job satisfaction was negative and significant (β = –0.071, 95% CI [–0.141, –0.015]), supporting a partial mediation model.

Discussion

These findings underscore that while stigma may have a complex direct influence, its negative effect on commitment operates significantly through the reduction of job satisfaction.

Conclusion

Job satisfaction partially mediates the relationship between perceived stigma and organizational commitment.

Implications for nursing and health policy

Healthcare policies and managerial interventions designed to reduce workplace stigma and proactively enhance job satisfaction are essential strategies for strengthening organizational commitment and improving nurse retention.

<div> <section> <h3> Aim</h3> <p>This study aimed to examine the moderator role of cultural intelligence in the relationship between the caring culture and caring behaviours of nurses.</p> </section> <section> <h3> Background</h3> <p>Nurses working in internal medicine units usually care for patients with chronic diseases. The caring culture of nurses and the development of cultural intelligence are extremely important because of the difficulties in the chronic disease process.</p> </section> <section> <h3> Design</h3> <p>This was a descriptive, cross-sectional, correlational study.</p> </section> <section> <h3> Methods</h3> <p>The study was conducted with 306 nurses between July 10, 2024, and March 10, 2025. Data were collected using a personal information form, the caring behaviours inventory-24, the caring culture scale, and the cultural intelligence scale. The data were analysed with IBM SPSS vn. 23 and PROCESS vn4.2 by Andrew F. Hayes, model 1 software. STROBE was used in the reporting of results.</p> </section> <section> <h3> Results</h3> <p>Caring behaviour (115.07 ± 24.2), caring culture (77.11 ± 14.18), and cultural intelligence (101.17 ± 22.09) points of nurses were at a moderate level. A statistically significant positive correlation was found between caring culture and caring behaviour (<i>r</i> = 0.594, <i>p</i> < 0.001), between cultural intelligence and caring behaviour (<i>r</i> = 0.351, <i>p</i> < 0.001), and between caring culture and cultural intelligence (<i>r</i> = 0.460, <i>p</i> < 0.001). Cultural intelligence was not found to have a moderator role in the relationship between caring culture and caring behaviour (β1 = 0.08, <i>p</i> > 0.05).</p> </section> <section> <h3> Conclusion</h3> <p>A high level of cultural intelligence may contribute to increasing the perceptions of caring culture and caring behaviours of nurses.</p> </section> <section> <h3> Implications for Nursing Policy, Health Policy, and/or Social Policy</h3> <p>To further improve nurses’ perception of the culture of care, in-house cultural care training should be organized.
