Nursing roles in disaster zones: Experiences and lessons from Turkey's 2023 earthquakes 护理在灾区的作用:土耳其 2023 年地震的经验和教训

BackgroundDisasters affect human health and well‐being globally. Nursing plays a vital role in disaster preparedness and response, ensuring efficient early care coordination and delivering effective field treatment.AimThis study investigates the challenges an Israeli humanitarian delegation encountered during their response to major earthquakes in Turkey in 2023. It explicitly focuses on difficulties in preparation, operations, and collaboration with local teams. The study further analyzes the findings and extracts valuable lessons from the mission.MethodsUsing a qualitative descriptive design, 22 out of 32 nurses involved in delegation participated in three focus group discussions within two months of returning to Israel. The discussions were recorded, transcribed verbatim, and analyzed thematically. The study followed the COREQ guidelines, ensuring comprehensive reporting and methodological rigor in qualitative research.FindingsThe study's main findings spanned predeparture preparation, mission challenges in the disaster zone, and postmission lessons, each highlighted by subthemes and participant quotations. A strong sense of mission was evident among the participants, along with frustration at inefficient time management prior to deployment. Many participants noted additional challenges, related to the difficulty of working in multiple languages and across cultures, and the opportunities for resolution. Finally, participants called for better psychological support following the mission.ConclusionNurses in disaster zones offer valuable insights to enhance preparation, cross‐cultural communication, and postmission implementation.Nursing and health policy implicationsNurse managers and healthcare policymakers can utilize this study's findings to develop future nursing training programs in disaster‐related skills. Additionally, it can help foster collaboration among international healthcare teams.

AimThis study investigated the levels of depression and anxiety in nurses and nursing assistants working in long‐term care facilities during the COVID‐19 pandemic. We also explored the potential causes of depression and anxiety in nurses and nursing assistants working in long‐term care facilities during the pandemic.BackgroundThe COVID‐19 pandemic has had a considerable impact on long‐term care facilities. The high infection and mortality rates for COVID‐19 have resulted in an increased workload for caregivers.IntroductionThe COVID‐19 pandemic exposed caregivers working in long‐term care facilities to higher risks of anxiety and depression. Additionally, the high risk of infection in the work environment and concerns about spreading COVID‐19 to family members and long‐term care facility residents led to various forms of stress among caregivers.MethodsThe present study was a cross‐sectional study. Questionnaires were used to investigate depression and anxiety among regarding nurses and nursing assistants working in long‐term care facilities during the pandemic.ResultsThe depression and anxiety levels of the nurses were higher than nursing assistants, but had no statistically significant difference (p = 0.551). The factors influencing levels of depression and anxiety in nurses contained facility affiliation and experience working. In terms of nursing assistants, age, marital status, and facility affiliation were correlated with the levels of depression and anxiety.DiscussionThe pandemic has severely impacted caregivers. In the process of implementing pandemic prevention measures and providing care for COVID‐19 patients, safeguarding the psychological health of caregivers is also essential.ConclusionThe levels of depression and anxiety in nurses were higher than in nursing assistants working in long‐term care facilities during the pandemic.Implication for nursing and health policyLong‐term care facilities managers are recommended to enhance the education and training process for caregivers. Managers are also recommended to ensure provision of sufficient amounts of pandemic prevention equipment and resources.

Aim: This study compared nurses' self-competency assessments and their managers' nurse competence assessments.

Background: Nurse competence is important for nursing care quality and patient safety. Executive (charge) nurses play a main role in the nurse competency assessment.

Methodology: In this study conducted at a university hospital between January and December 2020, the self-efficacy evaluations of nurses (n = 162) working in services other than the emergency department, operating room, and outpatient clinics of the university hospital were compared with the evaluations of nurse executives (charges) on these nurses (n = 21). Data were collected using the personal information form and the Nurse Competence Scale. A cross-sectional study design was used. The Strengthening the Reporting of Observational Studies in Epidemiology guideline was used for reporting.

Results: Service nurses' self-assessments and nurses treating charge's assessment of clinical nurses evaluated the competence levels as excellent in both groups. The self-competency assessments of the nurses were higher than the charge nurses' competency assessments.

Discussion: In this study, the lower competence assessment of nurses by the charge nurses was may be due to their ability to observe nurses in their teams in general and that they have more experience in evaluation.

Conclusion: It is suggested that professional development and competence programs be created to further develop the competencies of nurses.

Implications for nursing and health policy: Nurse competency assessments support planning to ensure patient safety. Patient care burden and planning of risky patients according to the nurse competency level will provide safe patient care.

Relationship between nurses' resilience and quality of professional life. 护士的复原力与职业生活质量之间的关系。

Aim: This study aimed to investigate the correlation between the resilience levels of nurses and qualit of their professional life.

Background: Nurses' resilience level can affect their professional qualityof life. A higher resilience level enables them to manage stress and overcome challanges more effectively leading to enhanced patient care and satisfaction. Coversly, lower resiliance can lead to burnout, negatievly impacting both the nurses' wellbeing and the quality of service delivered to patients.

Introduction: It is crucial to understand the correlation between the professional life quality and resilience of nurses working in difficult conditions.

Methods: This correlational study was conducted between September 2021 and February 2022. A total of 276 nurses were included in the study. Data were collected using the Demographic Data Form consisting of sociodemographic characteristics, the Professional Quality-of-Life Scale, and the Resilience Scale for Adults.

Results: The resilience score was above average, and the professional quality of life was high. Compassion satisfaction and compassion fatigue, which are components of professional quality of life, were moderate, and the burnout level was low. A robust positive correlation was observed between resilience and the professional quality of life. The linear regression model showed that choosing the profession unwillingly, dissatisfaction with the unit in which one works, and the level of psychological resilience are independent factors of professional quality of life.

Conclusion: According to the findings, high psychological resilience increases the quality of professional life. Resilience, choosing nursing willingly, and liking the unit in which one works are determinants of professional quality of life.

Implications for nursing policy: Enhancing the professional quality of life of nurses has important individual, institutional, and social consequences. Enhancing resilience has the potential to elevate the quality of professional life. Hence, it is important to develop national and international policies and models. Additionally, managers bear significant responsibilities in ensuring a positive work environment. Undoubtedly, having nurses work in units they like, feel happy in, and prefer will increase their professional quality of life and make positive contributions to patient care, the institution, and society.

Aim: This study aimed to understand Indonesian nurses' familiarity with the concept of reasonable adjustments, and the frequency of its application within their practice.

Background: People with intellectual disability and/or autism are exposed to significant barriers when accessing healthcare and have poorer health outcomes than those without developmental disabilities. Reasonable adjustments can improve accessibility and quality of healthcare for people with disabilities and involves adapting practices and environments to promote equitable healthcare.

Introduction: There is a scarcity of literature focused on the application of reasonable adjustments in the Indonesian nursing context. A greater understanding of the application of reasonable adjustments in this context can help inform nursing curricula and policy.

Methods: A cross-sectional, descriptive survey -was undertaken and is reported in accordance with the Strengthening the Reporting of Evaluations and Non-randomised Designs. Following descriptive analysis, bivariate analyses were undertaken between key demographic, workplace, and self-reported capability variables, and familiarity and implementation of reasonable adjustments.

Results: The majority of respondents were not familiar with the concept of reasonable adjustments and self-reported sometimes applying it within their practice. Higher levels of educational and clinical exposure to intellectual disability and/or autism, and self-capability variables, were significantly associated with familiarity with reasonable adjustments and their application.

Conclusions: An increase in nursing curricula focused on caring for people with intellectual disability and/or autism, including content focused on applying reasonable adjustments, is indicated.

Implications for nursing policy: Given that, internationally, people with intellectual disability and/or autism have disproportionately negative health outcomes and experiences, findings highlighting gaps in understanding and application of reasonable adjustments of Indonesian nurses have substantial implications for nursing policy and curriculum.

Aim: This study aimed to explore anesthesia nurses' (ANs) perceptions of roles, competencies, and attitudes towards patient safety culture, along with predictive factors for patient safety culture.

Introduction: The 2021 guidelines from the International Council of Nurses aim to ensure global access to safe surgical and anesthesia care by 2030. However, in Taiwan, the roles and competencies of ANs are still evolving. Many have a limited understanding, posing potential risks to patient safety.

Methods: A cross-sectional study was conducted, involving ANs from five hospitals within a healthcare foundation. ANs' self-perceived roles and competencies were assessed using a structured questionnaire based on the CanMEDS framework from the International Federation of Nurse Anesthetists. Additionally, data were collected using the Chinese version of the Safety Attitudes Questionnaire. Reporting followed the STROBE guideline.

Results: Among 200 ANs, a consensus emerged favored all roles and competencies, with positive attitudes toward patient safety culture. Remarkably, self-perceived competencies, working in regional hospitals, and working in medical centers significantly predicted ANs' attitude toward patient safety culture.

Discussion: The study results can assist ANs in gaining a deeper understanding of their roles and competencies. Considering predictive factors, strengthening ANs' competencies may contribute to enhancing patient safety culture.

Conclusion and implication for nursing and health policy: Our results informed nursing leaders and policymakers in Taiwan and other countries regarding ANs' perceptions of roles and competencies. Nurse managers could consider the specific factors influencing ANs' attitudes toward patient safety culture and make great efforts to develop strategies aimed at enhancing their competencies.

Aims: This study aimed to test whether stress could mediate the association between abusive supervision and nurses' work engagement, absenteeism, and turnover intention.

Background: Abusive supervision has been attributed to suboptimal work performance and reduced productivity among employees in different sectors. While existing nursing literature links abusive supervision to a wide range of work-related outcomes in the nursing workforce, little is known regarding the mechanism underlying this relationship.

Design: Data for this descriptive study were collected from 770 direct-care nurses from seven acute care hospitals in the Philippines, utilizing five standardized scales.

Results: Abusive supervision had direct positive effects on absenteeism (β = .189, p < .001) and intent to leave (β = .138, p < .001) and a direct negative effect on job engagement (β = -.131, p < .001). The relationships between abusive supervision and absenteeism (β = .175, p < .001), intent to leave (β = .131, p < .001), and work engagement (β = -.122, p < .001) were partially mediated by stress.

Conclusions: Stress mediated the relationship between abusive supervision and nurses' work outcomes, including turnover intention, absenteeism, and work disengagement.

Implications for nursing and health policy: The evident connection between abusive supervision, stress, and work-related outcomes underscores the importance of focusing on enhancing managerial supervisory styles as a potential organizational strategy to enhance staff retention and well-being.

Background

There is increasing global evidence on the impact of advanced nursing and midwifery practitioners, and Kenya's healthcare system has an excellent opportunity to develop scopes of practice and other regulatory frameworks for the integration of these roles.

Objective

The primary purpose of this gap analysis was to explore the existing evidence on opportunities and threats toward the integration of the advanced practice nursing (APN) and advanced practice midwifery (APM) roles in Kenya's healthcare system.

Methods

The study team conducted a structured electronic database search of PubMed, CINAHL, Scopus, EBSCOhost Academic Search Complete, and PsycINFO to retrieve articles and credible websites for reports highlighting the opportunities and threats toward the integration of the APN and APM roles in Kenya's healthcare systems. The retrieved articles were screened for relevance and synthesized for reporting using the traditional literature review approach.

Results

The Kenya Health Policy Framework 2014–2030, growing population needs, and implementation of universal health coverage provide an opportunity to harness and leverage advanced practice roles in nursing and midwifery. There is also momentum to develop advanced practice because of strategic alliances and global evidence showing the contributions and quality of services offered by advanced practice nurses and advanced practice midwives. However, lack of financial support, structural challenges, and lack of national policies, regulations, and legislation continue to obstruct progress.

Conclusion and implications for nursing policy

Developing scopes of practice for APN and APM in Kenya will benefit the professions, the country's healthcare delivery system, and the population. Achieving universal health coverage depends on a health workforce trained and practicing at optimal levels in tandem with education and training to deliver quality care.
