The Finger Ring: Need for Regular Removal and Replacement 指环:需要定期拆卸和更换

We present two cases of finger ring impaction. Both patients tried to remove it in rural hospitals before presenting to the burns and plastic unit of the University of Calabar Teaching Hospital. The male had the ring on the left index finger while the female had it on the left ring finger. The rings were removed by the use of circumferential cutaneous tissue infiltration (ring block) with plain lignocaine, strip of esmarch bandage exanguination of the finger, lubrication with K-Y jelly and then extraction.

A Rare Case of Multiple Epidermal Cyst Scrotum and Its Excision Under Spermatic Cord Block Obviating Spinal Or General Anaesthesia Need 一例罕见的阴囊多发性表皮囊肿及其在脊髓阻断下切除,缓解脊髓或全身麻醉需求

Epidermal cysts are the most common epithelial cysts usually with non malignant potential. These occur commonly in hair- bearing areas mostly on the scalp, also on the face, neck, back, and scrotum. Etiology is usually unknown. It can be seen commonly in individuals with history of significant Acne vulgaris. Multiple sebaceous cysts over the scrotum is a rare condition. Only eight cases of multiple epidermal cysts in scrotum have been reported so far from India in the past literature. This condition is usually asymptomatic and painless. It may turn painful if gets infected and often may be associated with discharge of pus. We present a rare case of multiple epidermal cysts of scrotum in a 36 year old patient which was managed with complete excision of the cysts done under local anesthesia followed by primary closure with better outcome in cosmesis taking care of not to jeopardize the vascular supply to local flaps and testis.

Glaucoma Progressing Stage Estimated from Visual Field Test Data with Glaucoma Patient; and Zinc(Ⅱ) Induced Regenerative Ativity from Optic Nerve Damage 从视野试验数据估计青光眼的进展阶段锌(Ⅱ)诱导视神经损伤再生能力

Glaucoma progressing stages (Stages 1~5) estimated from Visual Field Index (VFI); Mean Deviation (MD); and Pattern Standard Deviation (PSD) data has been elucidated; in which the glaucomatous pathology is in the proceeding stage 2-3 with T. Ishida's glaucoma patient compared with referring with VFI; MD; and PSD values to the literatures. Zinc(Ⅱ) induced VFI improvement should be taken into account when interpreting rates of VFI change over time that zinc promotes Retinal Ganglion Cells (RGCs) survival; in which zinc intake in RGCs survival may be zinc acetate 25-50 mg/day. Zinc(Ⅱ) induced recovery activity from optic nerve damage of the eye consists of four processes as follows. (1) Intraocular inflammatory stimulation process; Zinc concentrations 123-292 μg/g inhibit intraocular inflammation with atypical growth factor oncomodulin (Ocm) binding to its cognate receptor on RGCs. (2) RGC survival process; Zn2+ chelators enhance RGC survival and promote axon regeneration through the optic nerve. (3) Neural axon regeneration process; Zn2+ chelation promotes axon regeneration. Norepinephrine Transporters (Net) inhibitor promotes RGCs survival and axonal regeneration. (4) Eye to brain pathway process; Zn2+ chelator TPEN promotes both enduring RGC survival and considerable axon regeneration. Zinc induced recovery for NO production in RGCs that the NO conveys from the eye to the brain through the axons of RGCs; in which zinc concentration 100 μM may be suited for the optic nerve recovery. Accordingly; Zinc(Ⅱ) could enhance optic nerve damage recovery that Zn2+ may be bound with optic nerve damage proteins; in which Zn2+ ions may bind with intraocular protein; RGC survival protein; axonal protein; and optic nerve disorder proteins during recovery process by Zn2+ ions-centered tetrahedrally binding proteins molecular coordination pattern.

Epiphora; Successful Response to Systemic Steroids vs Topical Steroids 泪;全身类固醇与局部类固醇的成功反应

Purpose: Topical steroids are considered to be the first line of treatment for some forms of idiopathic punctal stenosis causing epiphora. We present a case of epiphora, which was unresponsive to traditional management, but responded to one week of systemic steroids. Observations: A 48-year-old male with no significant medical history, was referred to us for excessive tearing. He was seen earlier by an ophthalmologist for redness in one of his eyes. He was prescribed Pataday (Olopatadine) eye drops and Zylet (Tobramycin, Lotepriol) eye drops for both eyes. The patient was seen again after a week of treatment without much improvement. His treatment was changed to Alrex (Loteprednol) and Azasite (Azithromycin) eye drops. The redness of the eye disappeared in a week but patient noticed excessive tearing of his eyes. He was referred to reconstructive ophthalmologist for treatment of possible punctal stenosis. On examination the height of tear meniscus was increased and diagnostic lacrimal probing and syringing of the lacrimal drainage system demonstrated both lacrimal drainage system patent. Punctal stenosis was diagnosed. Over the next couple of weeks, patient underwent multiple syringing of the lacrimal duct system with temporary relief of symptoms. He was also prescribed Tobradex (Tobramycin+Dexamethasone) eye drops to be used for a month. Eye drops helped for the duration of treatment but symptoms returned after a couple of days of discontinuing eye drops. Patient also received ductal probing and punctal dilation in the office without much relief. Mini perforated plugs were placed in both puncta resulting in temporary relief. At the same time the intra ocular pressure started to rise and Tobradex eye drops were discontinued immediately. After almost three months of treatment and continuation of debilitating symptoms of epiphora, patient decided to consider surgical intervention and consulted an ENT surgeon for second opinion. Since he was responding to topical steroids, which was causing an increase in his intra ocular pressure, the ENT surgeon suggested Medrol Pack (Prednisolone for a week). Patient was symptom free after 4 days of treatment and by 7th day, his eyes were back to normal with complete disappearance of epiphora symptoms. Conclusions and importance: Systemic steroids can be beneficial in patients suffering from Idiopathic/Iatrogenic punctal stenosis unresponsive to topical steroids and conventional treatments.

Helicobacter Pylori Induce Gastric Upset 幽门螺杆菌引起胃部不适

Editorial: Helicobacter pylori is a micro-aerophilic, helical-form gramnegative aggressive bacteria. Accordingly, the idiom “Helico” intimates its helical appearance, “bacter” symbolizes bacteria, while “pylori” denotes stomach due to the first and common site of this bacteria living. Further, Marshall B. and Warren R. observed and described it in 1982. Then, the followed investigators studied this bacterium in detail with its consequences and complexities [1]. Gastric upset (Indigestion), dyspepsia: means impaired gastric digestion. Accordingly, the patient complains of upper abdominal pain, heartburn, belching, nausea, even feeling earlier gastric fullness than expected while eating. Furthermore, there are many causes of indigestion like gastroesophageal reflux disease, ulcer disease, gastritis, and even gastric cancer. Hence, unexplained recent onset dyspepsia in older people may need additional examinations. Moreover, one of the common causes is Helicobacter pylori infection, which needs laboratory and endoscopic examination [2]. Argument Many theories investigated the etiology and pathogenesis of Helicobacter pylori infection, concerning chronic or acute gastritis. Hence, gastric upset is the main presentation of both types of gastritis. Evidences The genotype is valuable in determining the dominant Helicobacter pylori strains as the isolates were different genetically plus heterogeneous distribution. Accordingly, the vac and cag markers operate a significant function in defining clinical consequences. These virulence agents are present in a subset of Helicobacter pylori strains isolates like cagA, iceA, vacA, and ureC. Moreover, the cagA causes cytotoxins induction by the gastric epithelial cell as Interleukin 8 [3]. The molecular intercommunication researches exhibit that the act of acarus calamus in hindering biofilm formation in Helicobacter pylori is due to the inhibitory impact of phytobio-active component, β-sitosterol, on the quorum sensing molecules-ToxB, PhnB, DnaA, plus Sip. Consequently, this opinion may suggest the molecular mechanism of Helicobacter pylori in producing the acidrelated complaints and gives a clue to a new therapy [4]. Helicobacter pylori infection causes lncRNA risk impression linked to H. pylori in gastric cancer patients and can prognosticate the prediction of these patients [5]. There was a close relationship between raised serum IgE levels in Helicobacter pylori infected patients [6]. Counterargument The laboratory investigations of Helicobacter pylori infection depend on several factors like the fluctuations of serum antibody titers in a time series, the antigene detection in stool tests, the false-positive results of lab tests, or the manner of endoscopic biopsy collection. Furthermore, other factors like the variations in Cytotoxin-Associated Gene A (CagA) in East Asian patients. Argument Many theories investigated the etiology and pathogenesis of Helicobacter pylori infection, concerning chronic or acute gastritis. Hence, gastric upset is the main presentation of both types of gastritis. Evidences The genotype is valuable in determining the dominant Helicobacter pylori strains as the isolates were different genetically plus heterogeneous distribution. Accordingly, the vac and cag markers operate a significant function in defining clinical consequences. These virulence agents are present in a subset of Helicobacter pylori strains isolates like cagA, iceA, vacA, and ureC. Moreover, the cagA causes cytotoxins induction by the gastric epithelial cell as Interleukin 8 [3]. The molecular intercommunication researches exhibit that the act of acarus calamus in hindering biofilm formation in Helicobacter pylori is due to the inhibitory impact of phytobio-active component, β-sitosterol, on the quorum sensing molecules-ToxB, PhnB, DnaA, plus Sip. Decision Making Capacity or How to Accept Patient’s Refusal for Orthodontic Treatment 如何接受患者拒绝正畸治疗的决策能力

Introduction: Communication in dentistry is bilateral process which usually is based on response (understanding) by the person. That’s why the Oral Healthcare Providers (OHP) should be convinced the consent given by the patient is valid. It means that at the beginning of the treatment the orthodontist will ask a lot of questions and have expectations to receive appropriate answers. There is a specific lack of awareness about the first orthodontic consultation at 7y of age, occurrence and prevention of most of the common tooth jaw discrepancies which affect the oral health, self-confidence and overall development of the child. A variety of socio-demographic, educational, personal and other factors mostly divided into objective and subjective factors influences the perception of facial attractiveness. The orthodontic treatment lays down on the personal desire and attitudes, depends from the motivation but is not without a risk for the patient. The aim of the current research is to present the most objective and subjective factors identifying the patient’s refusal. Material and methods: It’s a case report based on preliminary discussion and orthodontic consultation over the cephalometric analysis and cast models. Orthodontic treatment protocol was followed and given informed consent by the individual was received. Results and discussions: An electronic search was conducted using the Medline database (PubMed), Science Direct, and Scopus. In this case report were described the treatment options for Class III malocclusion with an emphasis on maxillary protraction and existing impacted canine 13. The decision making capacity was evaluated and also what are the objective and subjective factors and how to proceed with patient refusal. Conclusions: Despite the orthodontist’s efforts to improve the management of the dental practice and to attract new patients, these challenges should never been from the first importance. Contemporary dentistry requires that the patient’s right to refuse should be respected and this refusal must be accepted. Because orthodontic treatment is expensive, the process of returning money or sharing responsibility for the treatment depend on the socio-cultural characteristics of both the patient and the doctor. The whole situation requires a very delicate approach, as it affects the image of the dental community in society at whole.

Introduction: The training of dental students in communication skills has been faced with a new challenge related to the limitations of COVID 19. Obtaining informed consent and deciding on the need for treatment are part of the twoway dental-patient communication process. While training in Public Dental Health, the students learn the basic rules of patient-oriented communication, discussing models for successful conversation with the patient and the types of verbal and nonverbal communication. The main educational goal is to build dental professionals who can adequately accept the needs of patients, help to realize them and generate trust. The specific educational goal is to organize and discuss specific, evidence-based clinical skills for communicating with patients. Materials and methods: A literature search has been performed and alternatives have been discussed with colleagues. The information search has been conducted using the Medline database (PubMed), Science Direct, and Web of Science. Discussion: The development of the social element has been observed in the relationship dentist - patient. Against the background of growing commercialism in the provision of dental services, patients are feeling increasingly uncertain whether the treatment offered is the needed one. Therefore, the patient must be at the centre of modern dentistry where the primary role has the Medical University in educating ethical principles and building doctors dedicated to humanity. Conclusion: The challenges in teaching students to communicate effectively with the patient are significant, especially in a global pandemic. The roots of philanthropy and empathy are laid back in time. They were described by the first teachers, physicians and philosophers in Medicine such as Hippocrates, Socrates, Plato, Aristotle and others. It is our responsibility just to repeat them.

Mesh related visceral complications are, in general, considered to be infrequent and rarely reported in literature, but in the last decade, incidence of such complications have increased. Our study highlights the case of an elderly patient, who developed rare complication of Mesh migration after laparoscopic inguinal hernioplasty.

Consumption of Meat and the Human Health 肉类消费与人类健康

It has grown into a significant part of the country's economy as well as a primary supply of white meat and nutrition for the average family. People were interrogated by questionnaire. In this, we included all types and every age of people. Many questions were asked. Industry trends in the meat sector contribute to various meat varieties such as beef, mutton, poultry, and camel, among others. The majority of chicken production takes place on large-scale farms throughout Pakistan's provinces. Chicken is emerging meat industry but beef is still at top so far. Fishery is an important part of Pakistan's economy, as it is a significant source of revenue for coastal residents. Red meat is a nutrient-dense diet that contains significant levels of protein, vital amino acids, vitamins, and minerals, including vitamin A, iron, and zinc, all of which are commonly deficient in the world. However, several studies have connected red meat consumption to a variety of health risks, including heart disease, colon cancer, kidney illness, and digestive disorders. So it is requirement to eat only proper cooked meat to escape its harmful effects.

Complications of Abdominal Surgery, Study Carried Out in the Eastern Region of the Democratic Republic of the Cong 在刚果民主共和国东部地区进行的腹部手术并发症研究

Background: In some regions like ours, the complications from abdominal surgery are important indicators of the quality of surgical care, particularly in general surgery. The objective of our work is to study the complications of abdominal surgery. Methods: This is a retrospective and descriptive study carried out in the general surgery Department of the Skyborn Hospital Center, over a period of 5 years, period from March 2016 to April 2021, with a total of 286 patients. Results: A total of 286 patients having participated in our study, we found that the male sex occupies 186 cases, the age group of the age group between 21 and 40 years occupies the first place, the cesarean with 42% was the first the surgical history, the surgical site infection and wound infection was the first postoperative complications with 64%. 91.6% of patients left the hospital without any particularity and 75.4% of patients received surgical treatment for the management of complications. Conclusion: The delay in the management or non-prevention of complications is an important factor in the occurrence of complications from abdominal surgery. Good postoperative management of the operative site and the surgical wound can prevent and avoid complications of abdominal surgery

The objective of our work is to study the complications of abdominal surgery. Methods: This is a retrospective and descriptive study carried out in the general surgery Department of the Skyborn Hospital Center, over a period of 5 years, period from March 2016 to April 2021, with a total of 286 patients. Results: A total of 286 patients having participated in our study, we found that the male sex occupies 186 cases, the age group of the age group between 21 and 40 years occupies the first place, the cesarean with 42% was the first the surgical history, the surgical site infection and wound infection was the first postoperative complications with 64%. 91.6% of patients left the hospital without any particularity and 75.4% of patients received surgical treatment for the management of complications. Conclusion: The delay in the management or non-prevention of complications is an important factor in the occurrence of complications from abdominal surgery.
