Dr. Douglas B Clement, Richmond, British Columbia, is President of the Canadian Academy of Sports Medicine and a member of the Canadian Association of Sports Sciences and American College of Sports Medicine Current training methods in sport expose the athlete to higher and higher levels of stress and subsequent injury is all too common The athlete who presents with gradually increasing pain in the lower leg, followmg repetitive running or jumping offers a challenge in proper diagnosis and adequate treatment. Slocum’ has probed this challenging differential diagnosis and Devas2 has commented that one of the most disabling conditions presenting in this manner is a stress fracture of the tibia Traditionally this diagnosis is dependent on the radiological findings that Wilson and Katz3 describe as a combination of a fracture line, focal sclerosis of bone with endosteal callus or periosteal reaction with external callus. Radiographically the incidence is slightly lower, and collected series of pre-employment films have shown the incidence to range from 2.08% to 4.4%.1, 3, 18 This lower incidence reflects x-ray technique, the ability of the radiologist, and the fact that the films are taken with the patient supine. Wiltse24 states that many patients with known bilateral spondylolysis can be converted to spondylolisthesis by applying a load to the lumbosacral joint at the time of x-ray. He therefore recommended that patients with spondylolysis have weight-bearing films. Although the etiology of spondylolisthesis remains in question most evidence supports

Rugby football and American football are frequently compared and contrasted by their respective proponents as to playing skills required, and the relative dangers involved in playing each sport. It is the purpose of the present study to report the incidence, nature, and severity of injuries sustained in rugby football by players of seven Boston area rugby clubs in the Spring 1971 season. Historically, contemporary studies of athletic injury protection have done little more than provide some descriptions of existing injury patterns. Unfortunately, these descriptions fall short of pointing out the most efficient ways of preventing athletic injuries. An early investigation by Lloyd, Deaver and Eastwood3 suggests that it is not enough to know what the hazards are in athletics; it is also important to know how to eliminate these hazards. Subsequent studies tell what parts of the body are injured, how these injuries occur, the type of injury and the activity engaged in; but most do not mention the protective equipment worn by the player. Athletic program administrators believe that the protective equipment being worn today is protecting their student-athletes from injury but they have no substantial

Cervical injuries can be prevented by per~~crvic formance of neck exercises, including isometric maneuvers with fixed restriction of motion. Weighted resistance is accomplished by bridging the cervical spine in all directions. A proper fitting helmet dissapates and/or absorbs impacts to the head. Styrofoam in a helmet will not restore its contour when the helmet is cracked. The trend today is toward a crown cord suspension helmet. There is very little loss of visual field with an increase in the number of bars of the protective face mask. In early-season injuries, where there is loss of conciousness, the possibility of heatstroke should be kept in mind as well as cerebral trauma. An acute, severe injury may require the immediate establishment of
