Tibia is the most commonly fractured long bone. In adults it takes 4 to 6 months to heal depending on the severity ofinjury and method of treatment. Rate of complications are also high. Locked intramedullary nailing is the preferredtreatment for tibial shaft fractures. When locked proximally and distally, removal of one set of screws, especially intransverse and short oblique fractures are required for dynamization before weight bearing. Fail to do this timely,many of these patients present with nonunion and implant failure. The purpose of the study was to enhance healing oftibial shaft fracture by early weight bearing, at the same time minimizing the complications and early return to work.Stable open type- I, II & closed tibial shaft fractures from the isthmus to 4 cm above the distal articular surface of thetibia were fixed by locking nail without proximal screws. This prospective study was done from June 2009 to July2014 on 410 patients treated at several hospitals of Faridpur city. All the operations were done in closed methodwithout any C-Arm monitoring. Patients were allowed to move with crutch and controlled partial weight bearingwithin two to four weeks of operation. All patients were followed up for five years. In most cases the nail wereremoved in one to two years. Early weight bearing causes compression and intermittent micro movements at thefracture site. The fractures heal as early as in eight to twelve weeks and no nonunion or implant failure wereencountered. Knee pain was complained by 41 patients (10%), disappeared after removal of the nail. Superficialinfection developed in 8 cases (1.95%), controlled in 2 to 3 weeks and deep infections were encountered in 8 cases(1.95%) with delaying union but eradicated spontaneously as the fracture healed. Deep infection occurs only in openfractures. Although five years period is not enough to comment about development of secondary osteoarthritis, weencountered no such cases till now. So, closed reduction and internal fixation (CRIF) of stable open type-I, II &closed tibial shaft fractures from the isthmus to 4 cm above the distal articular surface of the tibia by interlockingnailing with distal locking screws only, allows early controlled weight bearing, thereby early fracture healing and lesschance of implant failure. Hepatic cysts are very rare in children and most of them are simple and solitary, asymptomatic, benign, and do notrequire intervention. It is usually found as a mere coincidence on abdominal imaging techniques, such asultrasonography (USG), computed tomography (CT), and magnetic resonance imaging (MRI). The incidentaldiagnosis of cystic lesions of the liver increased day by day due to increased use and sensitivity of abdominalimaging. Predominantly simple cysts are most common and diagnosed by characteristic Ultrasonogram findings.Serodiagnostic tests and contrast-enhanced ultrasound (CEUS) are useful when USG, CT, and MRI show ambiguousfindings in differentiating other cystic lesions (complicated cysts, echinococcosis, and cyst adenoma/cystadenocarcinoma) and reduce the need for invasive procedures. Although most cysts are benign, some are malignant orpremalignant, so early detection of cystic lesions is important to manage them properly. Treatment is needed insymptomatic or complex cysts or those having malignant or premalignant features. On 31 December 2019, the WHO China Country Office was informed of cases of pneumonia of unknown etiology detected in Wuhan City in Hubei Province of its territory. The Chinese authorities isolated a new type of coronavirus on 7 January 2020. The consequences of this viral infection were later termed as COVID-19 disease. Direct and indirect psychological and social effects can affect mental health now and in future. COVID-19 can infect brain and can affect brain function. During and after this pandemic, harmful behaviors like suicide and self-harm are likely to increase. SARS (2003) had 30% increase in suicide in those over 65 years. Half of the recovered patients remained anxious; 29% health care workers experienced emotional distress. When a person becomes infected with COVID-19 disease, s/he is also at higher risk for sleep problems owing to acute stress responses. Anxiety and depressive symptoms constitute common reactions for people with COVID-19 diagnosis, especially for those who may be hospitalized, due to concerns for one’s own health or the health of others, the need for physical isolation. Other concerns include the potential risk of death, concerns over the risk of infecting others, and concerns over leaving family members alone who may need care. Stress-related psychiatric conditions including mood and substance use disorders which are associated with suicidal behavior. COVID-19 survivors may also be at elevated suicide risk during and after the pandemic. The suicidal behaviors are likely to be present for a long time and peak later than the actual pandemic. Lifestyle during the pandemic is characterized by prolonged inactivity, increased eating and consequent obesity, alcohol and other substance abuse. Fractures of the distal tibia are unique in that the bone is subcutaneous with poor muscular coverage. Theconsequence of its decreased vascularity leads to complications like delayed bone union, wound complications suchas dehiscence and infection. These fractures can be managed with various techniques. Open reduction and internalfixation by plating, have been used with varying results. In current orthopedic practice, minimally invasivepercutaneous plating osteosynthesis (MIPO) and interlocking nailing are the preferred techniques for fractures of thedistal third tibia. In our study, twenty adult patients with closed fractures of distal tibia were treated with LCP usingMIPO technique & were followed for a period of one year (January 2014 to December 2014). Average age of patientsstudied was 40.5 years (18-55) with majority being high energy trauma (65% vs. 35%). Mean duration for surgeryfrom day of identification to go for surgery was 4-7 days. Results were evaluated clinically and radiologically. In 18cases (90%) fractures united & 2 cases (10%) had delayed union. All fractures were united by 20 weeks. Superficialskin infection was found in 2 cases, Valgus mal-alignment in 2 cases &1 case had ankle stiffness. Nephrotic syndrome (NS) is one of the most common childhood kidney diseases where defective immune systemincreases the chances of various infections. Forecasting the prognosis and proper planned management based on therecent data of risk factors and infection patterns can reduce the associated morbidities and mortalities. Weinvestigated the incidences of infection and associated risk factors among the hospitalized children to provide anupdated view of the current infection pattern of children with NS. This observational study was conducted at theDepartment of Pediatrics, Bangabandhu Sheikh Mujib Medical College Hospital, Bangladesh, from July 2017 to June2020. A total of 167 children aged <_12 years admitted at BSMMCH during this period with NS were included in thisstudy. The hospital course and associated investigation reports of the study participants were analyzed. The incidenceof infection among the patients with NS was 21.56%. The mean age of the participants was 5.43 2.66 years, and73.01% of them were under immunosuppressive therapy. Ascites was a significant presentation among the infectiongroup. There were no significant differences in Hb%, serum albumin, serum cholesterol and 24h urinary protein levelbetween the two groups. Klebsiella and Escherichia coli were predominant isolates in the blood and urine culture,respectively. Serum albumin level <1.5 g/dL was associated with infection in risk analysis. Peritonitis, UTI andpneumonia were common infections and patients with associated infections had a significantly longer hospital stay (p= <0.001). Identifying the risk groups with ascites and low serum albumin levels and hospital-based predominantinfectious organism patterns can help develop planned management of patients with NS associated various infections. Varicella zoster virus (VZV) infection primarily causes varicella or chickenpox. Herpes zoster (HZ) or shingles is themanifestation due to reactivation of same virus. Simultaneous occurrence of HZ and Chickenpox in a patient is anuncommon event. A 14 years old apparently healthy girl came to the medicine outpatient department of PirojpurDistrict Hospital with the complaints of burning pain in the right side of forehead followed by maculopapular rashand blister lesion with painful right red eye. The lesions were anatomically distributed along the dermatome of rightsided ophthalmic division of trigeminal nerve. Simultaneously she developed some small vesicular lesion in hertrunk. At that time she had fever, bodyache and malaise. The entire lesion was healed with crusting within seven toten days. She had no significant past history. Patients on maintenance hemodialysis are vulnerable to multitude of health-related issues, malnutrition being one ofthem. It is a key predictive factor for morbidity and mortality among patients on maintenance hemodialysis. Theprimary objective of the study was to assess the severity of malnutrition in patients undergoing maintenancehemodialysis, using modified subjective global assessment score and correlate it with biochemical parameters likeblood hemoglobin, serum creatinine, serum albumin, serum pre-albumin, serum triglyceride and serum totalcholesterol. Cross-sectional study from July 2016 to June 2017 was conducted at hemodialysis unit of SSMC & MH,BSMMU, BIRDEM hospital and NIKDU, Dhaka, among 80 maintenance hemodialysis patients. Adult patients overthe age of 18 years and on regular (>_2 sessions per week) hemodialysis for more than 3 months without any acuteinfection were enrolled. Nutritional status of the patients was evaluated using modified subjective global assessmentscore and compared with biochemical parameters for any correlation between them. Based on the modified SGAscores, of the 80 patients, 10% were well nourished and the rest were mildly malnourished, showing a high degree ofmalnutrition among respondents. There was strong positive correlation between modified subjective globalassessment score and age. Modified SGA score also showed statistically significant negative correlation with serumcreatinine, serum albumin and serum pre-albumin. Data obtained from this study confirm that modified subjectiveglobal assessment score can be used as a reliable index for identifying patients at risk for malnutrition and itcorrelates well with some of the biochemical profile. We report a case of 14 years old girl who presented with intractable diarrhoea, low-grade irregular fever, multiplepainful oral ulcers, recurrent boils over different parts of the body for 1 year, and weight loss for 6 months. Previouslyshe was erroneously diagnosed as intestinal tuberculosis. The patient did not respond to antitubercular drugs. She alsogot prednisolone and mesalamine as the treatment of inflammatory bowel disease. Now she is diagnosed as a case ofAIDS-stage 3. Left Main (LM) coronary artery disease and left ventricular dysfunction both are important predictors of in-hospital mortality. To avoid the harmful effects of cardio pulmonary bypass (CPB), many cardiac surgeons are using Offpump Coronary Artery Bypass (OPCAB) as an effective alternative to conventional CABG (CCAB) even in these sub-groups of patients. This study performed in the National Institute of Cardiovascular Diseases (NICVD) evaluated the early outcomes of OPCAB in terms of mortality and major post-operative morbidities and compared them with that of CCAB in patients with left main coronary artery diseases with left ventricular (LV) dysfunction. Total 120 patients with left main coronary artery disease with reduced left ventricular ejection fraction (<_40%) were allocated into two groups: a) 60 patients who underwent OPCAB and b) another 60 patients who underwent conventional CABG between January 2012 and December 2017. All risk factors and co-morbidities were homogenously distributed between the two groups. Majority of the patients had triple vessel disease. Nearly three-quarters (73.3%) of patients in OPCAB group and 80% in CCAB group received 3 grafts (p=0.470). The mean total operative time (p< 0.001), intubation times (p<0.001), blood losses (p < 0.001); requirements for blood and blood products (p < 0.0010), intensive care unit stays (p<0.001) and hospital stays (p< 0.001) were all significantly lower in the OPCAB group. OPCAB is a safe and effective operative revascularization procedure for patients with left main coronary artery disease with left ventricular dysfunction and is associated with reduced morbidity. Chronic idiopathic urticaria (CIU) is a recalcitrant skin disease characterized by pruritic wheals lasting more than six weeks in the absence of a physical cause. A case-control type of analytical study was conducted from January 2012 to August 2012 on patients of chronic idiopathic urticaria attending the outpatient department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University, Dhaka. The objective of the study was to see the association between chronic idiopathic urticaria and Helicobacter pylori infection. In serum, IgG positive individuals, female covers about two-thirds (14 respondents with 73.3%) than male counterparts (5 respondents with 26.3%). Among the IgG negative individuals, females were again more in percentage (63.6%) and males were 36.4%. Serum IgG for H. Pylori positive respondents were highest among the students (42.1%) followed by housewives (31.6%) and in IgG negative individuals, the same trend exists. When we considered education level for IgG positivity, it was found that graduate and above educated respondents showed higher results (42.1%) and SSC level showed next incidences (26.3%). In IgG negative group, the same trend exists. We found the same positivity for IgG of H. pylori among the different food intake, heat, and sunlight exposed history respectively whereas food intake history covers more in IgG negative individuals. Most of the IgG positive respondents were ill-looking (94.7%), though IgG negative individuals were also ill-looking. Chronic idiopathic urticaria is a disorder that often baffles dermatologists and allergists alike. Without a known cause, it is nearly impossible to prescribe an effective treatment.
