Background: Dengue-COVID-19 overlap is mixture of both diseases sharing few similarities in pulmonary and extrapulmonary involvement. Due to high prevalence of both diseases later being pandemic disease, and overlapping laboratory and clinical parameters we have conducted a study to observe dengue-COVID-19 overlap in Indian settings in tertiary care hospitals. Methods: Prospective, observational study, included 600 COVID-19 cases with dengue NS1 or Dengue IgM positive, with lung involvement documented and categorized on HRCT thorax at entry point. All cases were subjected to dengue IgG antibody titers and dengue IgM/IgG antibody titer analysis after 12 weeks of discharge form hospital. Results: Dengue- COVID-19 overlap was documented in 16.33% (98/600) cases. CT severity has documented significant correlation with Dengue-COVID-19 overlap cases. [p<0.00001] Hematological evaluation, white blood cell count & platelet count were having significant association with Dengue-COVID-19 overlap [p<0.0076] & [p<0.00001] respectively. Clinical parameters as hypoxia have significant association with dengue- COVID-19 overlap. [p<0.00001] Inflammatory markers as IL-6, CRP and LDH has significant association in dengue-COVID-19overlap [p<0.00001] respectively. In study of 98 cases of ‘Dengue-COVID-19’, post covid lung fibrosis [p<0.004] and serological assessment in dengue IgM/IgG and covid antibody titers has significant association [p<0.00001] Conclusion: Dengue-COVID-19 overlap is clinical syndrome with overlapping clinical and laboratory workup of both the illnesses. High index of suspicion is must in all covid cases in tropical settings where dengue is endemic; and all cases with leucopenia and thrombocytopenia with fever should be screened for dengue serology. False positive dengue serology or dengue antigen crossreactivity is known to occur in underlying COVID-19 illness, and have impact on clinical outcome as it will result in delay in covid appropriate treatment initiation and many cases require intensive care unit treatment due to progressed covid pneumonia. Objective: Males and females have been reported to show significant variation with respect to clinical profile of sickle cell disease (SCD) (e.g. cardiovascular, musculoskeletal and chronic end-organ complications). Clinical features of SCD are also influenced by lifestyle and dietary habits. Thus, the study was conducted to examine whether the distributions of other manifestations (e.g. priapism, pain frequency, leg ulcers rates) and habits (lifestyle, dietary) of SCD patients in Osun state, Nigeria are influenced by gender or genotype. Material and methods: Questionnaires were administered to obtain information on age, gender, concoction/herbal preparations, and fruit/vegetable consumption, as well as zinc contents of 24-hour meal recall, physical exercise, lifestyle parameters and clinical profile of SCD. Descriptive (relative frequency) and inferential (Chi-square (χ2) test of independence) statistics were employed. Statistical significant was set at p<0.05. Results and discussion: HbSS rather than HbSC showed higher distributions of pain frequency and more frequent hospitalization rates at p= .001 and p=.001 respectively. There was no significant relationship between hemoglobin genotype and the following variables: leg ulcers, priapism, fruit intake, herb/concoction consumption, 24-hour dietary zinc content. Moreover, all the variables were not influenced by gender not only among HbSS but also HbSC patients. Conclusion: There are indications from data obtained that clinical profile of SCD such as priapism, leg ulcers, high hospitalization rates and pain episodes are present among HbSS and HbSC SCD patients in Osun state Nigeria. Although only pain episodes and hospitalization rates were significantly higher among HbSS than HbSC, for either variable though there was no gender bias in its distribution. Similarly the distributions of other variables (such as exercise, dietary, etc) for each of SCD (HbSS, HbSC) categories were not influenced by gender. Objective:Application of Laryngeal Mask Airway (LMA) as a ventilation method has been rarely explored in short and medium period orthopedic operations. In this study, we evaluated ventilatory efficacy of LMA in patients admitted to Ho Chi Minh City Orthopedic and Rehabilitation Hospital, Vietnam via a number of patient indicators including changes of pulse, blood pressure, airway pressure, SpO2 and pressure of CO2 at the end of an exhaled breath (EtCO2). Method: A total of 154 emergency and elective surgery cases with anesthesia were selected for the study. Result: One minute after LMA insertion to discontinuation of anesthetic, we recorded slower pulse of patients but only twenty-six cases (16.9%) required intravenous administration of atropine. Blood pressure of patients dropped after 1 minute following LMA insertion but then recovered afterward and only nine cases (5.8%) needed administration of ephedrine. Conclusion: After surgery, only eleven (7.15%) patients experienced oral mucosal bleeding but no further special treatment was used. Background:The effect of vitamin D in regulating lipid profiles is one of the proposed mechanisms for the association between vitamin D deficiency and cardiovascular heart disease (CHD). However, the relationship between vitamin D status and lipid profiles remained unclear. Objective: The objective of this study is to evaluate the association of vitamin D status on serum lipids among healthy adults. Material and Methods: This is cross-sectional study, involving 120 healthy adults with age of 18-50 years from the Malay adult in Kota Bharu, Malaysia which were selected via random sampling. To decrease the seasonal variability, the study was conducted within 2 months, between July and August 2015. Serum 25(OH)D, serum parathyroid hormone (PTH), serum triglycerides (TG), serum total cholesterol (TC), serum low density lipoprotein cholesterol (LDL-C) and serum high density lipoprotein cholesterol (HDL-C), fasting blood sugar (FBS) and serum insulin were measured. Results: The mean serum 25(OH) D was 23.50 ± 8.74 nmol/L. Based on the definition of vitamin D deficiency as serum 25(OH)D less than 30mmol/L, the proportion of vitamin D deficiency among our study subjects was 76.7%. Our data revealed that serum 25(OH) D had significant inverse association with HDL-C and significant positive association with TC, TG and LDL-C. Conclusions: Our study shows highly prevalence of vitamin D deficient among healthy Malay adult population in Kota Bharu, Malaysia. Serum 25(OH) D was positively associated with TC, TG and LDL-C and negatively associated with HDL-C. Perhaps optimizing the level of vitamin D might improve the lipid profiles among healthy Malay adults. Bangladesh Journal of Medical Science Vol. 23 No. 01 January’24 Page : 214-220
January'24 Page : 214-220

Objective: A health problem that is a priority for medical care and treatment for the elderly is pain in the musculoskeletal system. Non-pharmacological management in the form of treating musculoskelatal pain in the elderly, one of which is infrared therapy. Infrared therapy is able to reduce pain in the elderly and requires close supervision because the elderly experience a decrease in skin sensitivity to heat. The effectiveness of reducing pain varies in the elderly. One of the influencing factors is age. This study’s objective is to determine differences in the effectiveness of infrared therapy to reduce musculoskeletal pain in the elderly group. Materials and methods: This study used an experimental research method with a nonrandomized pretestposttest control group design. The research location was conducted at the Posbindu Institut Teknologi Sains dan kesehatan PKU Muhammadiyah Surakarta, in September - October 2022. A sample of 50 elderly people was divided into elderly (> 60 years: 30) and pre-elderly (45-60 years: 20) selected using a purposive technique sampling. The dependent variable is musculoskeletal pain. The independent variable is infrared therapy. Data were analyzed by Wilcoxon and Mann-Whitney test. Results and Discussion: The Wilcoxon test showed that there was an effect of infrared therapy in reducing musculoskeletal pain in the elderly and preelderly groups (p value <0.001 and <0.001). The average reduction in pain in the elderly group = 3.1; SD = 1.3 and pre-elderly = 2.95; SD = 1.2. The Mann-Whitney test showed no difference in reducing musculoskeletal pain using infrared therapy in the elderly and pre-elderly (p = 0.712). Conclusion: Infrared therapy is effective in reducing musculoskeletal pain and reducing musculoskeletal pain for the two elderly groups are similar Bangladesh Journal of Medical Science Vol. 23 No. 01 January’24 Page : 108-114

Background: Biological and technical hardware complications in implant-retained prostheses are well documented in the literature. The most frequently reported failures are on the immediate or delayed factors after implant placement. Even when the implant has successfully osseointegrated, there is a chance that the prosthesis can fail if the treatment planning and occlusal aspects are not well studied. Management of failed cases is rarely reported. In this case report, the authors present a failed implant prosthesis and how it was managed well with a more permanent solution. Case presentation: A 55-year-old, male retired bank manager, reported with the chief complaint of frequent fracture of the maxillary acrylic hybrid implant prosthesis that was delivered 6 months ago. He wanted a replacement for the fractured upper maxillary acrylic hybrid prosthesis. On clinical evaluation, the upper maxillary implant prosthesis was opposing the natural tooth, an anterior bridge, and two posterior implants in the lower right and left of the first and second molar region. The patient was happy with the lower restorations. Conclusion: The design of the final restoration based on biological, mechanical, technological, and patient factors should be ideally planned during the initial treatment planning stage itself. In case a prosthesis fails the underlying cause should be analyzed and retreatment planning is to be done. Bangladesh Journal of Medical Science Vol. 23 No. 01 January’24 Page : 266-271

The current study is aimed at evaluation of the impact of different surgical approaches while correcting congenital heart diseases (CHD). Clinical outcomes like; operative time, complications, hospital stay were evaluated in children undergoing surgical repair of atrial septal defect (ASD) and ventricular septal defect (VSD). Patients and methods: Between 2015 and 2019, a total of 111 patients surgically managed for CHD with male: female ratio of…. and mean age 10.42±3.24 years. Two surgical approaches: open either right anterolateral thoracotomy or sternotomy and thoracoscopic were used. The study conducted to determine the operative time, ICU stay, complications, recovery period and long term follow up of the patients in both the groups. Results: 67 patients with ASD and 44 patients with VSD underwent cardiac surgery. Mean operation time was significantly longer in TS patients in comparison with TT and ST groups. However, mean intensive care unit (ICU) stay and hospital stay were significantly shorter in TS group in comparison with TT and ST patients. Conclusion: TS approach in repair of congenital heart defects not only resulted of good cosmetic effect, but also reduces time in ICU and hospital stay in comparison with “classical” operational techniques. In this article we tried to show the whole results of research. Bangladesh Journal of Medical Science Vol. 23 No. 01 January’24 Page : 115-120

Background: Sudden stroke had caused the caregivers minimal time to prepare for their role as carers. Some stroke caregivers were overwhelmed to adjust and accept new responsibilities. They need to reorganise their functions and establish new daily life routines in response to the chronic illness that happens with stroke survivors. Objective: This study aimed to determine and explore coping strategies employed by stroke caregivers. Methods: This mixed-method study involved 32 stroke caregivers from a community-based stroke rehabilitation centre in Kuching, Sarawak. Results: From quantitative analysis, most stroke caregivers employed problem-focused coping strategies (mean=56.66, SD=10.79), followed by emotion-focused coping strategies (mean=55.63, SD=10.29) in confronting the challenges of caring for stroke survivors. Data from qualitative findings revealed eight themes of coping strategies: adjustment to daily life, selfmotivation, sharing with other people, acceptance, avoiding thinking about the problem, hoping stroke survivors will get better, emotion suppression and self-blame. Conclusion: This study’s outcomes provide crucial information about the burdens held by stroke caregivers when taking care of stroke survivors. Stroke caregivers need to adapt better and suitable coping strategies. This will prevent them from becoming overwhelmed by the accumulated stress from caring burdens. The long-term effect of coping strategies should positively assist stroke caregivers in managing the burdens effectively and fostering satisfaction in their life.
