Background: A metabolic condition known as diabetes mellitus (DM) is defined by an absolute or relative lack of insulin production, insulin action, or both. A risk factor for coronary heart disease (CHD) and other cardiovascular illnesses is persistent diabetes mellitus (CVD). The diagnosis and risk assessment of individuals suspected of having various CVD symptoms, such as chest discomfort and acute coronary syndrome, is done using cardiac markers.. Aim of the study: Look into myocardial affliction in kids with Type 1 diabetes. Type of study: cross-sectional comparison study. Methods: 26 controls and 50 paediatric type 1 DM patients were included in the present investigation. Complete history taking, clinical examinations, and thorough laboratory tests were performed on the patients.

Background: Since beta-thalassemia is a genetic condition with a variety of clinical manifestations, there has been an increase in interest in biomarkers that could aid in the early identification, staging, and prognosis of cardiac disease in beta-thalassemia patients. Recent advances in quantitative ultrasound technology have made speckle tracking echocardiography a viable option for precisely assessing cardiac function. Aim of the work: Determine the efficacy of Speckle Tracking Echocardiography (STE) in the early diagnosis of myocardial affection in B-Thalassemia patients utilizing various echocardiographic modalities. Methodology : 100 patients between the ages of 5 and 25 were enrolled in a cross-sectional study, 50 of them had known beta thalassemia. From March 2022 to January 2023, pediatric Cardiology and Hematology units at Banha University Hospital, Faculty of Medicine, used various echocardiographic modalities on a major patient from a hematology outpatient clinic and 50 control, seemingly healthy patients. Results: The gender split was 50% male and 50% female, with a mean age of 18.03 4.08. There was no EF affliction, all of which were within normal ranges. By using the 2D speckle tracking technique, the beta thalassemia group's global longitudinal strain (GLS) mean was -16.72 4.14 and the group of controls' GLS mean was -20.80 2.69. This difference was highly significant, with a p value of 0.001 indicating the early detection of myocardial systolic and diastolic affection despite normal EF values. Conclusion: speckle tracking echocardiography is valuable in assessment of cardiac function in patient with beta thalassemia and silent cardiac dysfunction.

Background: The autoimmune illness systemic lupus erythematosus (SLE) may damage numerous organ systems, involving the cardiovascular system. This study aimed to assess the right ventricular systolic function in SLE patients by tissue Doppler echocardiography (TDE). Methods: This prospective single-center study was done on 100 SLE individuals at Banha University Hospital. Patients had a comprehensive medical history, physical examination, laboratory tests, and tissue Doppler imaging echocardiography. Pulmonary arterial hypertension (PAH) was diagnosed regarding tricuspid regurgitation velocity gradient. Various echocardiographic parameters were measured, including right ventricular (RV) end-diastolic diameter, RV anterior wall thickness, peak systolic velocity of tricuspid annulus (S wave), TAPSE, longitudinal strain and strain rate curves and also RV fractional area change (RVFAC). Results : The study revealed a substantial difference in Pulmonary artery systolic pressure (PASP) values among three groups based on PASP (PASP ≤ 30 mmHg for group A; PASP 30-50 mmHg for group B; PASP ≥ 50 mmHg for group C). There were statistically significant differences in various echocardiography measurements, such as TAPSE, RVFAC, RV anterior wall thickness, RV end-diastolic diameter and more between the three groups. Triglyceride levels showed a substantial change among groups, being lowest in group B. Conclusion: TDE is a helpful technique for examining RV systolic function in SLE patients.

Background: Pentraxin 3 (PTX3) is an inflammatory marker, a member of long pentraxin superfamily that has an important role in inflammation and immunity. Objectives: The aim of this study was to measure serum pentraxin 3 levels in RA patients and evaluate their relation with different disease parameters. Methods: We measured serum PTX3 levels in 40 women with RA and 40 female control subjects. PTX3concentrations were measures by ELISA. Results: Median serum level of pentrexin 3 was significantly higher in RA patients compared to healthy controls (36.4 ng /ml vs 12.4 ng /ml respectively) (p<0.001). There was a significant difference in serum PTX3 levels according to the disease activity grades. PTX3 concentration was also significantly correlated with ESR(r = 0.506, p=0.001), CRP(r = 0.780, p<0.001) and Larsen Score (r = 0.784, p<0.001). There was no significant difference in serum PTX3 levels and RF& anti-CCP positive and negative patients (P >0.05). No significant correlation (p>0.05) was found between serum Pentraxin-3 and age (r = 0.095), BMI(r = 0.059), disease duration (r = 0.048) and RF titer(r = 0.240). Conclusions: PTX3 can be considered as a sensitive non-invasive biomarker reflecting the disease activity and radiological damage in RA patients.

Objectives: To assess the function of the enzyme angiotensin converting in psoriasis. Background: The powerful vasoconstrictor angiotensin-converting enzyme (ACE) is essential for maintaining healthy cutaneous homeostasis. Serum ACE levels have been reported to be higher in psoriasis patients in studies, indicating that this enzyme may be involved in the aetiology of psoriasis as well as preclinical atherosclerosis and hypertension. Data Sources: by looking through and scanning medical databases (Pub Med and Medscape) for information on patients with psoriasis and their blood levels of angiotensin converting enzyme. Study Selection: For inclusion, each study underwent an independent evaluation. If they met the requirements listed below, they were considered: Serum Levels of Angiotensin Converting Enzyme in Patients with Psoriasis were explored in English-language publications in peer-reviewed journals. Data Extraction: Studies were disqualified if they didn't meet the criteria for inclusion. A study's ethical permission, eligibility requirements, proper controls, adequate information, and stated assessment methods were all considered in the evaluation of the study's quality. A data collecting form was used to independently extract data from each qualifying research in order to record details about the study results we were interested in. Conclusion: With respect to nail psoriasis, plaque psoriasis, psoriatic arthritis, and scalp psoriasis, no significant changes were found. However, there is a higher level of serum ACE among psoriasis patients, and these individuals have a much lower proportion of ACE.

Background: Hydrocephalus is a common condition in which there is an abnormal accumulation of cerebrospinal fluid in the brain. Ventriculoperitoneal (VP) shunt surgery is a standard treatment for hydrocephalus, but post-operative obstructed distal ends of VP shunt tubes can lead to complications. Laparoscopy has emerged as a potential diagnostic and therapeutic tool in managing such obstructions. This study aimed to comprehensively outline the role of laparoscopy in the diagnosis and management of obstructed distal ends of VP shunt tubes in hydrocephalic patients. Methods: The study was conducted at Benha University and Benha Children Hospitals between 2016 and 2021. A cohort of 40 hydrocephalic patients who had undergone VP shunt surgery and experienced abdominal obstruction at the distal end of their VP shunts were included. Diagnosis involved clinical abdominal examination, neurosurgical evaluation, and radiological examinations (abdominal X-ray, abdominal ultrasound, and brain CT scan). Laparoscopic procedures were performed to address the specific obstructions. Results: Among the 40 patients, laparoscopy successfully diagnosed and treated the obstructed distal ends of VP shunt tubes in 31 cases (77.5%). The laparoscopic interventions resulted in resolution of symptoms and signs of active hydrocephalus in these patients, as confirmed by radiological assessments. Out of the remaining 9 cases, 6 required additional laparoscopic revisions, while 3 patients were prepared for ventriculo-arterial shunts. No conversions to open surgery were needed, and there were no mortality cases. Mild post-operative complications were observed in 6 patients, which resolved with antibiotic and antipyretic therapy. Conclusions: Laparoscopy is a valuable diagnostic and therapeutic approach in the management of obstructed distal ends of VP shunt tubes in hydrocephalic patients.

A cohort of 40 hydrocephalic patients who had undergone VP shunt surgery and experienced abdominal obstruction at the distal end of their VP shunts were included. Diagnosis involved clinical abdominal examination, neurosurgical evaluation, and radiological examinations (abdominal X-ray, abdominal ultrasound, and brain CT scan). Laparoscopic procedures were performed to address the specific obstructions. Results: Among the 40 patients, laparoscopy successfully diagnosed and treated the obstructed distal ends of VP shunt tubes in 31 cases (77.5%). The laparoscopic interventions resulted in resolution of symptoms and signs of active hydrocephalus in these patients, as confirmed by radiological assessments. Out of the remaining 9 cases, 6 required additional laparoscopic revisions, while 3 patients were prepared for ventriculo-arterial shunts. No conversions to open surgery were needed, and there were no mortality cases. Mild post-operative complications were observed in 6 patients, which resolved with antibiotic and antipyretic therapy. Background: Gait and other daily activities depend on proper ankle and foot function. In many rheumatic and musculoskeletal conditions, the ankle is a frequently affected joint in both young and older populations, leading to pain, restricted mobility, and a lower quality of life. The difficulty in accessing the ankle and foot anatomically and the lack of adequate training in clinical assessment methods are two factors that contribute to this problem. Although traditional radiography is currently the preferred imaging method in everyday clinical practice for determining ankle involvement, it does not allow for the evaluation of soft tissues. Additional different diagnostic modalities such as magnetic resonance imaging, computed tomography and imaging diagnostic ultrasonography may offer more comprehensive data on soft tissue and bone structures. To diagnose and evaluate patients with different auto-immune rheumatic and degenerative illnesses, ultrasound is particularly well-suited for examining soft tissue components of different joints. Conclusions: Ultrasonography is considered a reliable method for diagnosing and predicting functional disability and managing ankle pathologies. Longitudinal studies are needed to confirm these findings.

Background: Intraoperative nausea and vomiting (IONV) after caesarean section under spinal anaesthesia is a prevalent problem that might compromise patient safety and comfort. IONV must be effectively managed to optimise maternal and neonatal outcomes. Objectives: This study is to examine the most recent information regarding the management of intra-operative nausea and vomiting after Cesarean section under spinal anaesthesia using a combination of propofol and dexamethasone. Conclusions: Propofol plus dexamethasone can be considered as a preferred antiemetic strategy for cesarean section under spinal anesthesia. It offers superior efficacy in reducing IONV, which contributes to improved patient satisfaction. However, individual patient factors, contraindications, and known risks associated with dexamethasone should be taken into consideration.
