Limitations of some epidemiological studies on low-dose lowrate exposures to ionizing radiation include dose comparisons disregarding natural radiation background, unfounded classification of sporadic diseases as radiogenic and conclusions about causality of dose-effect relationships. Other bias, confounders and inter-study heterogeneity have been pointed out. Some dose-effect correlations can be explained by a dose-dependent selection, self-selection and recall bias. It can be reasonably assumed that individuals knowing their higher doses would be more motivated to undergo medical examinations being at the same time given more attention. Reported dose-effect relationships between low-dose low-rate exposures and non-neoplastic diseases call in question the causality of such relationships for cancer detected by the same researchers. Reliable evidence in regard to biological effects of low radiation doses can be obtained in large-scale animal experiments with registration of life duration. The monitoring of human populations exposed to low-dose radiation is important but conclusions should be made with caution considering potential bias and economical motives to strangulate nuclear energy production in accordance with the interests of fossil fuel producers. Of note, health burdens are the greatest for power stations based on coal and oil; the burdens are smaller for natural gas and still lower for the nuclear power. This case report is about spontaneous foreskin retraction but it is relevant also to the status after circumcision. Non-therapeutic male circumcision remains controversial; there is evidence in support of its benefits; however, good hygiene may achieve some of the aims. Late consequences are understudied, in particular, those related to atrophy of the ageing genital skin and mucous membranes. A superficial scald burn of the genital area occurred to a 3-years-old boy in conditions of child neglect. Immediate medical help was not sought. In the following years, symptoms of balanoposthitis were repeatedly noticed. Along with the pubertal development, the foreskin became completely and permanently retracted. There has never been any strangulation or compression in terms of paraphimosis. The symptoms started appearing at the age of about 58 years: recurring superficial erosions in the frenular area, preputial rim and adjacent surfaces. The penis binding with a soft cloth and occasional topical treatment is necessary as atrophic skin is sensitive and vulnerable. Derangements of blood supply may contribute to atrophic changes; therefore, vascular diseases and diabetes mellitus must be included in the differential diagnosis. Potential adverse effects discussed in this report emphasize the fact that non-therapeutic neonatal circumcision is an invasive procedure performed without patients’ consent.

Background: The investigation of potential adverse health effects of occupational exposures to ionizing radiation, on nuclear plant workers, is an important area of research. In this study, we aimed to calculate the incidence and risk of cancer development and mortality during last five years (2015-2019). Material and Methods: 660 nuclear industry workers were included into this study. For this cohort, the cancer mortality has been assessed by data obtained from national health registry excluded for the probability of known causes of death. The associations between cumulative occupational radiation exposures (radon, gamma radiation and long-lived radionuclides) and cancer mortality were calculated. Results: Radon and Gamma exposure, in mSV, was significantly higher among workers who developed cancer [8.9 (0; 3,224.5) vs 19.9 (0; 128.4), p=0.03] and [15.0 (2.1; 110.0) vs 24.5 (0; 470.1), p=0.02]. However, no significant association was found between long-lived radionuclides and risk of cancer (p=0.07). Considering economic sanction as a dependent variable, we found no significant effect on mortality rate among workers. Conclusion: In conclusion, significant association has been observed between the risk of cancer development and radon and gamma exposure among nuclear industry workers, but no association was found between cancer and long-live nucleoids exposure.

A 23 year old female was admitted to Internal Medicine department due to bilateral face swelling of both preauricular area and multiple tender small lesions in the anterior aspect of both calves. A few similar lesions were found on the posterior aspect of her arms. She had a prodrome of two days fever before and upper dysphagia and odynophagia a day before her admission. She reported of mild alcohol consumption and childhood asthma. She did not take any medication prior to her admission except oral contraceptives, which were prescribed for a few weeks, until 4 months ago. On physical examination, the patient seemed to have bilateral tender swellings of both preauricularareas ,that were painful while opening her mouth (Figure 1). She also had bilateral tender erythematous lesions on both calves and a few similar lesions involving the extensor portion of her arms (Figure 2). The lesion borders were poorly defined, and lesions vary from 2-5 cm. The rest of physical examination was within the normal range. Laboratory examination included elevated CRP (133 mg/dL, norm<0.5 mg/dL). CBC, liver functions and renal functions were all within the normal range. Blood cultures, serology for HAV, HBV, HCV, EBV, CMV and HIV and Mantoux test were negative. Antinuclear antibody was weak positive (+1), complement levels (c3 & c4) were in normal range and Rheumatoid Factor was negative. Beta HCG was negative (<0.6). Chest X-ray was without pathologic findings.

Lymphangiomas represent benign embryonic vascular malformations of the lymphatic architecture. A symptomatic small bowel lymphangioma in an adult is a rare finding. Here we present a case of a 93 year-old male who had a 3-month history of intermittent melena. After an unrevealing upper endoscopy, colonoscopy was significant for blood emanating from the ileo-cecal valve. Video capsule endoscopy was significant for bright blood and clots in jejunum. Small bowel enteroscopy demonstrated multiple jejunal sub-mucosal masses. The bleeding lesion was biopsied and then successfully treated with Argon Plasma Coagulation (APC). Histopathology was consistent with a lymphangioma.

This article presents a continuation of report series on alcohol abuse, reproductive coercion and intimate partner violence. Abortion rate in the former Soviet Union has been the highest in the world, caused not only by the insufficient availability of modern contraception but also by irresponsible behavior. The overconsumption of alcohol is generally perceived as a contributing factor. Currently there is an improvement tendency. The social background of certain behavioral stereotypes such as reproductive coercion and binge drinking are discussed. The conclusion is that use of condoms is dependent on personal traits and the level of social development; however, alcohol consumption at sexual encounters and heavy binge drinking are risk factors for the non-use of condoms and other irresponsible behaviors. In some less developed societies with a widespread rape myth acceptance, sexual violence was a way of acquiring wives [15,16]. The fact that some victims married rapists was seen as indication that women enjoy it; in fact, however, existing accounts demonstrate various degrees of trauma [16]. High social positions held by sexual offenders or their relatives sometimes impede reporting [15]. Later on, the stepfather physically abused his stepson, while the mother sometimes participated in battering.

Dentures are prosthetic devices constructed to aid as a replacement for the missing teeth. However, the problems come when there is no right fitting as it can turn into a potpourri of problems for the patient. Discomfort in mastication, speech, pain and inflammation are just some of the ill-effects of an ill-fitting denture. Epulis fissuratum refers to reactive tissue response to excessive mechanical pressure imparted by the poor fit of prosthesis. In this article, we discuss one such case of a 63-year-old female. Vestibular paroxysmia is the name given to vascular compression of the vestibulocochlear nerve. Substantial evidence has been discovered in support of vascular compression of the trigeminal nerve as the etiology for trigeminal neuralgia, and effective therapies have been targeted to address this pathophysiology. Perhaps due to the common and often vaguely-described symptoms of dizziness and tinnitus, vascular compression of the vestibulocochlear nerve as a cause of symptoms has remained controversial. Recent clinical studies, however, have better defined diagnostic criteria for vestibular paroxysmia. In this report we discuss a case of vestibular paroxysmia, highlighting some findings of the condition that also uniquely separate it from other more common vestibular disorders. Finally, we discuss current clinical management of vestibular paroxysmia.

Attention Deficit-Hyperactivity Disorder (ADHD) often continues into adulthood. Recent neuroimaging studies found lowered baseline dopamine tone in the brains of affected individuals that may place them at risk for Substance Use Disorder (SUD). This is an observational case study of the potential for novel management of Adult ADHD with a non-addictive glutaminergic-dopaminergic optimization complex KB200z. Low-resolution electromagnetic tomography (LORETA) was used to evaluate the effects of KB220z on a 72-year-old male with ADHD, at baseline and one hour following administration. The resultant z-scores, averaged across Eyes Closed, Eyes Open and Working Memory conditions, increased for each frequency band, in the anterior, dorsal and posterior cingulate regions, as well as the right dorsolateral prefrontal cortex during Working Memory, with KB220z. These scores are consistent with other human and animal neuroimaging studies that demonstrated increased connectivity volumes in reward circuitry and may offer a new approach to ADHD treatment. However, larger randomized trials to confirm these results are required.

The typical HIV-infected patient presenting with acute coronary syndrome (ACS) is a man in his mid to late 40s. The most common presentation is an acute myocardial infarction (MI), most often with ST segment elevation. Coronary anatomy seems to be variable, with some studies showing a higher prevalence of single-vessel disease and others showing a higher prevalence of 2- and 3-vessel disease than in controls not infected with HIV.
