The association of bacterial biofilm and middle ear mucosa in patients with mucosal chronic suppurative otitis media 粘膜慢性化脓性中耳炎患者的细菌生物膜与中耳粘膜的关系

Objectives

To evaluate the bacterial biofilm’s role in mucosal chronic suppurative otitis media (CSOM) utilizing scanning electron microscopy (SEM).

Methods

This study involved 123 participating patients with active and inactive mucosal CSOM who underwent tympanomastoid surgery. SEM was used to examine middle ear mucosa biopsies for the development of biofilms. Middle ear discharge or mucosal swabs from patients were cultured to detect any bacterial growth. The biofilm formation was correlated to the culture results.

Results

The biofilm was present in 69.9 % of patients (59% of them were with active mucosal CSOM) and absent in 30.1% of the patients (70% of them were with inactive mucosal CSOM), being more statistically significant in active mucosal CSOM (p-value = 0.003). A correlation that was statistically significant was found between active mucosal CSOM and higher grades (3 and 4) of biofilms (p-value <0.05). The mucosal CSOM type and the results of the culture had a relationship that was statistically significant (p-value <0.001). 60% of patients had positive culture (70% of them were with active mucosal CSOM). There was a statistically significant relation between Pseudomonas aeruginosa bacterial growth and active mucosal CSOM (p-value = 0.004) as well as higher grades of biofilms in mucosal CSOM.

Conclusion

Mucosal CSOM, especially the active type, is a biofilm-related disease. There is a significant relation between the state of mucosal CSOM (active or inactive) and culture results with predominance of Pseudomonas aeruginosa bacterial growth in active mucosal CSOM and in higher grades of biofilms in mucosal CSOM.

Comparison of the MIPS technique versus linear incision with tissue preservation in osseointegrated implants in the Ponto model® Mips 技术与线性切口技术在骨结合植入 ponto ® 模型中的组织保留方面的比较。

Background and objectives

The surgery of osseointegrated implants has undergone different modifications over the years with the aim of achieving better results and facilitating the surgical technique. Today the most commonly used technique is the linear incision with tissue preservation and placement of the abutment and implant. The long-term success of this technique has served as the basis for the development of the so-called minimally invasive surgical approach (MIPS). This study compares the short-, medium- and long-term results between the classic linear incision technique and the MIPS technique.

Material and methods

A prospective study was conducted on patients who had an osseointegrated implant placed between February 2016 and February 2020. A total of 59 surgeries were performed, 32 surgeries according to the linear incision technique with tissue preservation and 27 with MIPS technique. Outcomes were evaluated at one week, one month and one year.

Results

Statistically significant differences were achieved between the 2 groups at one week after surgery. Eighty per cent of the MIPS patients had Holgers grades 0–1 compared to 35% of the linear technique patients (p = 0.001). No statistically significant differences were observed at one month (p = 0.457) and one year (p = 0.228). One case with grade 4 was recorded which resulted in implant extrusion one month after surgery with the MIPS technique. A new osseointegrated implant was placed 2 months after the fall using the same MIPS technique with good results. We were also able to verify that the duration of surgery was much shorter with the MIPS technique and better tolerated in terms of postoperative discomfort by the patient.

Conclusions

In our experience, the MIPS technique is the technique of choice for surgery of osseointegrated Ponto model implants as it is simpler, faster and presents fewer problems in the immediate postoperative period, with similar long-term postoperative results.

Normal hearing and verbal discrimination in real sounds environments. 在真实声音环境中的正常听力和语言辨别能力。

Introduction: Human beings are constantly exposed to complex acoustic environments every day, which even pose challenges for individuals with normal hearing. Speech perception relies not only on fixed elements within the acoustic wave but is also influenced by various factors. These factors include speech intensity, environmental noise, the presence of other speakers, individual specific characteristics, spatial separatios of sound sources, ambient reverberation, and audiovisual cues. The objective of this study is twofold: to determine the auditory capacity of normal hearing individuals to discriminate spoken words in real-life acoustic conditions and perform a phonetic analysis of misunderstood spoken words.

Materials and methods: This is a descriptive observational cross-sectional study involving 20 normal hearing individuals. Verbal audiometry was conducted in an open-field environment, with sounds masked by simulated real-word acoustic environment at various sound intensity levels. To enhance sound emission, 2D visual images related to the sounds were displayed on a television. We analyzed the percentage of correct answers and performed a phonetic analysis of misunderstood Spanish bisyllabic words in each environment.

Results: 14 women (70%) and 6 men (30%), with an average age of 26 ± 5,4 years and a mean airway hearing threshold in the right ear of 10,56 ± 3,52 dB SPL and in the left ear of 10,12 ± 2,49 dB SPL. The percentage of verbal discrimination in the "Ocean" sound environment was 97,2 ± 5,04%, "Restaurant" was 94 ± 4,58%, and "Traffic" was 86,2 ± 9,94% (p = 0,000). Regarding the phonetic analysis, the allophones that exhibited statistically significant differences were as follows: [o] (p = 0,002) within the group of vocalic phonemes, [n] (p = 0,000) of voiced nasal consonants, [r] (p = 0,0016) of voiced fricatives, [b] (p = 0,000) and [g] (p = 0,045) of voiced stops.

Conclusion: The dynamic properties of the acoustic environment can impact the ability of a normal hearing individual to extract information from a voice signal. Our study demonstrates that this ability decreases when the voice signal is masked by one or more simultaneous interfering voices, as observed in a "Restaurant" environment, and when it is masked by a continuous and intense noise environment such as "Traffic". Regarding the phonetic analysis, when the sound environment was composed of continuous-low frequency noise, we found that nasal consonants were particularly challenging to identify. Furthermore in situations with distracting verbal signals, vowels and vibrating consonants exhibited the worst intelligibility.

Diagnostic value of vestibular evoked myogenic potentials in benign paroxysmal positional vertigo 良性阵发性位置性眩晕的前庭诱发电位诊断价值。

Objectives

Vestibular evoked myogenic potentials (VEMPs) are useful for studying the disturbances along nerve pathways implicated in the transmission of neurological information from otolithic organs related to vestibular function. This study aims to determine the differences in VEMPs in patients affected with benign paroxysmal positional vertigo (BPPV).

Methods

We recruited 36 patients, 9 diagnosed with recurrent BPPV (rBPPV), 9 with only one episode of vertigo (iBPPV), and 18 as a control group. We performed cervical and ocular VEMPs (cVEMPs and oVEMPs).

Results

We observed differences in asymmetry ratio, which was 41.82% in cVEMPs in iBPPV and 68.27% in oVEMPs in rBPPV, while no asymmetry was found in control cases. Also, there was a lack of both VEMP responses in 22.2% of cases and an absence of cVEMP in 11.1% in iBPPV; in rBPPV, 11.1 % presented no responses in cVEMPs or oVEMPs, 22.2% showed no oVEMP, and 11.1% showed no cVEMP. These values were normal in the control group.

Conclusion

The value of VEMPs in BPPV demonstrates the implication of vestibular damage, mainly utricle damage. For better sensitivity in detecting otolith abnormalities, we should perform oVEMPs and cVEMPs in recurrent BPPV and early stages of BPPV.

This study aims to determine the differences in VEMPs </span>in patients<span> affected with benign paroxysmal positional vertigo (BPPV).</span></span></p></div><div><h3>Methods</h3><p>We recruited 36 patients, 9 diagnosed with recurrent BPPV (rBPPV), 9 with only one episode of vertigo (iBPPV), and 18 as a control group. We performed cervical and ocular VEMPs (cVEMPs and oVEMPs).</p></div><div><h3>Results</h3><p>We observed differences in asymmetry ratio, which was 41.82% in cVEMPs in iBPPV and 68.27% in oVEMPs in rBPPV, while no asymmetry was found in control cases. Also, there was a lack of both VEMP responses in 22.2% of cases and an absence of cVEMP in 11.1% in iBPPV; in rBPPV, 11.1 % presented no responses in cVEMPs or oVEMPs, 22.2% showed no oVEMP, and 11.1% showed no cVEMP. These values were normal in the control group.</p></div><div><h3>Conclusion</h3><p><span>The value of VEMPs in BPPV demonstrates the implication of vestibular damage, mainly utricle damage. Effect of antihypertensive treatment on the quality of life of patients with chronic rhinosinusitis with nasal polyps 降压治疗对伴有鼻息肉的慢性鼻炎患者生活质量的影响。

Aim

Nasal polyposis (CRSwNP) shares type 2 inflammation biomarkers with asthma, allergy or arterial hypertension (AH), including periostin, a predictive marker of severity and post-surgical recurrence of polyposis. Antihypertensives have been shown to decrease periostin expression. We set out to evaluate the effect of antihypertensives on the quality of life of patients with CRSwNP.

Materials and methods

Retrospective study of 43 patients with CRSwNP and ah with at least 1 year of follow-up and antihypertensive treatment prescribed after the diagnosis of CRSwNP. Phenotypes were analyzed (F1: isolated CRSwNP; F2: CRSwNP with asthma and/or NERD) and aspects related to quality of life (SNOT-22), clinical severity (VAS), polypoid size (NPS), exacerbations and surgical needs after the initiation of antihypertensive treatment.

Results

The predominant phenotype was F1 (62.8%). The number of exacerbations was 19.2% for F1, compared to 31.3% for F2. 34.8% underwent surgery after the start of antihypertensive treatment (F1 = 27.9% and F2 = 6.97%). A significant reduction in polypoid size, SNOT22 (16.4 ± 19.6 points), and VAS scales (p < .05) was obtained.

Conclusions

polypoid size, and reduce the risk of postoperative recurrence.

Objective

to assess clinical safety and postoperative audiological outcomes in postlingual deafness Spanish speaking patients, who underwent surgery with Nurotron™ cochlear implant.

Material and methods

Retrospective descriptive case series study. We performed follow-up of complications and audiological measurements before and after cochlear implantation. Patients with bilateral severe to profound sensorineural hearing loss or patients with unilateral deafness with/without tinnitus were included. Repeated-measures within-subjects for assess pure tone thresholds and speech performance (bilingual test) with a detailed monitoring to establish security or adverse effects were performed. Analysis of variance tests, repetitive measures, were used for statistical analysis.

Results

31 patients were included, 17 (54.8%) men and 14 (45.2%) women. Mean age at the time of surgery was 49.82 ± 18.8 years. The mean follow-up of the group was 31.56 ± 9.57 months (minimum = 19.6 months and maximum = 52.50 months). As major complication one patient (3.23%) had a hard failure that required removal and re-implantation. 25.8% of the patients presented minor complications, the most frequent being vertigo/unsteadiness in 22.6%.

The mean of language discrimination (free field at 65 dB SPL) was 62.19% ± 16.66; being 69.82% ± 7.35 in the group of severe to profound bilateral sensorineural hearing loss. A statistically significant reduction was observed in patients with tinnitus, assessed using the visual analogue scale, preoperative = 7.2 ± 1,6 vs postoperative (18months postoperative) = 1.7 ± 1.3 (p < .001).

Conclusions

The Nurotron™ cochlear implant shows satisfactory audiological results, in accordance with what has been reported in the literature. Minor complications were similar to previous studies, but the percentage of hard failure should continue to be observed, which was higher than other reports with comparable follow-up.

Analysis of selective screening for congenital cytomegalovirus in a secondary hospital: Problems and solutions 一家二级医院先天性巨细胞病毒选择性筛查分析:问题与解决方案。

Universal hearing screening offers unique possibilities for detection of congenital deafness as a consequence of congenital cytomegalovirus (CMVc) infection, so its selective study in the case of a failed test could be a non-negligible screening opportunity while other guidelines covering the possibility of universal screening are adopted. The aim of this study is to analyse the possibility of selective screening for CMVc after an altered hearing test in a regional hospital. During the period studied, the results obtained were unsatisfactory, especially in children born outside the hospital of residence, showing an excessive delay in hearing screening in many cases and in the few cases where CMVc screening could be performed, only 30% had the test ordered in a timely manner. The reasons for this are varied and the solution is to include selective screening for CMVc in the hearing screening programme. This implies shortening the timing of the hearing screening protocol to allow CMVc testing in saliva or urine (preferably) before 21 days of age and providing screening programmes with the necessary staff and time to perform it properly.
