Pay attention to eye health for children during the period of COVID-19 新冠肺炎疫情期间注意儿童眼部卫生

At present, COVID-19 continue to spread in China and are highly contagious. Ocular symptoms are represented by conjunctivitis. When children in the incubation period come to the clinic, it is very easy to cause infection by medical staff and leading to the spread of epidemic. During the epidemic, the principle of non-severe, treatment should be elective. However, when children need to go to the hospital for medical treatment due to the illness, protective measures shall be taken. Coincides with the peak children’s visits during the holidays, in order to do a good job of treatment management and medical staff protection, to prevent the spread of the epidemic and protect the health of medical staff. This paper reviews the main features of COVID-19, how to protect pediatric ophthalmologists and their clinical work of COVID-19 for clinicians’ reference. 当前,新冠肺炎疫情在中国持续蔓延,具有高度传染性。眼部症状以结膜炎为代表。当处于潜伏期的儿童来到诊所时,很容易引起医护人员的感染,导致疫情的传播。在疫情期间,以非重症为原则,治疗应选择性。但是,当儿童因患病需要去医院就医时,应当采取防护措施。恰逢假期儿童就诊高峰,为做好治疗管理和医护人员保护,防止疫情蔓延,保障医护人员健康。本文就新型冠状病毒肺炎的主要特点、如何保护儿童眼科医生及临床工作进行综述,供临床医生参考。关键词:COVID-19;眼科、儿科;传染性的;保护

Clinical analysis of pediatric ocular trauma 儿童眼外伤的临床分析

Objective To analyze the clinical characteristics of pediatric ocular trauma. Methods A retrospective study. Clinical data of 165 eyes of 164 cases with ocular trauma aged 0-14 years in the Second Affiliated Hospital of Chongqing Medical University from Apr. 2009 to Apr. 2019 were analyzed. All patients were compared according to age, sex, time of injury, population distribution, causes, types of trauma, complications and efficacy of treatment. Results The incidence of pediatric ocular trauma in boys was higher than that in girls and the incidence was higher in rural areas than that in cities. The ocular trauma was more common in children aged 3-8 years. Penetrating injury was the most common type, and traumatic cataract often occurred in ocular trauma. The following types were ocular contusion and ocular adnexal trauma. There were varieties of vulnerant, and the most injury were caused by implements, including knives, sticks, broken bowl, glass, pebble and so on. The visual acuity were improved to different extent after timely treatment. Conclusion The focus of prevention and treatment of pediatric ocular trauma in different age groups are different. 目的分析小儿眼外伤的临床特点。方法回顾性研究。分析重庆医科大学附属第二医院2009年4月至2019年4月收治的0 ~ 14岁眼外伤患者164例165只眼的临床资料。比较所有患者的年龄、性别、受伤时间、人群分布、原因、创伤类型、并发症及治疗效果。结果儿童眼外伤男童发生率高于女童,农村发生率高于城市。眼外伤多见于3 ~ 8岁儿童。穿透性白内障是最常见的类型,外伤性白内障多见于眼外伤。以下类型为眼挫伤和眼附件外伤。凶器种类繁多,最常见的是刀具、棍棒、破碗、玻璃、卵石等器具。经及时治疗,患者的视力均有不同程度的改善。结论不同年龄组儿童眼外伤的防治重点不同。早期治疗可改善视力预后。关键词:外伤,眼,儿科;并发症;预防;治疗

The small incision manual cataract surgery for hard nucleus cataract——the superiority on improvement of quality of visual acuity and life 硬核白内障小切口人工白内障手术——提高视力质量和生活质量的优势

Objective To observe the visual acuity, corneal astigmatism and quality of life of patients with hard nucleus cataract after small incision manual nuclear extraction (non-phacoemulsification). Methods Prospective cohort study.Total of 440 eyes of 440 cases with hard nucleus cataract from Jan.2014 to Jan.2019 in Yuyao Maternal and Child Health Hospital were randomly divided into two groups.There were 198 cases in small incision manual nuclear extraction group (manual group) and 242 cases in phacoemulsification group (phaco group). Follow-up time was 3 months. Results At 1 month after operation, the complication rate in the manual group was lower than that in the phaco group (P=0.028). At 3 months after operation, there was no significant difference in distribution of uncorrected visual acuity between the two groups (P=0.776, 0.712, 0.810). The corneal astigmatism of the manual group was lower than that of the phaco group (P=0.000). There was no significant difference in general health status, far work, ophthalmodynia, social function, social role limitation, independence, or mental health questionnaire score between the two groups (P=0.230, 0.601, 0.259, 0.296, 0.544, 0.492, 0.547). The score of overall vision, near work, color vision and peripheral vision in the manual group were higher than those of the phaco group (P=0.000). Conclusion Small incision manual cataract surgery for the treatment of hard nucleus cataract can not only improve the visual acuity, but also reduce corneal astigmatism and the incidence of complications, and improve the visual related quality of life of patients. 目的观察硬核白内障患者小切口人工摘除(非超声乳化)后的视力、角膜散光及生活质量。方法前瞻性队列研究。选取余姚市妇幼保健院2014年1月~ 2019年1月440例硬核性白内障患者440只眼,随机分为两组。小切口人工核摘除组198例,超声乳化术组242例。随访时间3个月。结果术后1个月,手工组并发症发生率低于phaco组(P=0.028)。术后3个月,两组未矫正视力分布差异无统计学意义(P=0.776, 0.712, 0.810)。手工组角膜散光明显低于超眼组(P=0.000)。两组在一般健康状况、远工、眼痛、社会功能、社会角色限制、独立性、心理健康问卷得分方面差异均无统计学意义(P=0.230、0.601、0.259、0.296、0.544、0.492、0.547)。手工组的整体视觉、近景视觉、色觉和周边视觉评分均高于phaco组(P=0.000)。结论小切口手工白内障手术治疗硬核性白内障不仅能提高患者的视力,还能减少角膜散光和并发症的发生,提高患者视力相关的生活质量。关键词:白内障;硬核;分娩,核,手动,非超声乳化,小切口;超声乳化;生活质量

Analysis of traumatic glaucoma in children 儿童外伤性青光眼的分析

Objective To analyze the clinical characteristics of traumatic glaucoma in children. Methods This is a retrospective study.The data of 67 eyes of 67 cases with secondary glaucoma in 425 children with ocular trauma, aged 1 to 14 years, from Jan. 2014 to Dec. 2018 in First Affiliated Hospital of Zhengzhou University were analysed retrospectively. There were 52 eyes of 52 cases with secondary glaucoma caused by closed ocular trauma in closed ocular trauma group, and 15 eyes of 15 cases with secondary glaucoma caused by open ocular trauma in open ocular trauma group. The clinical characteristics, treatment methods and efficacy of treatment of the two groups were analyzed. The follow-up time was 1 month- 2 years. Results There were 251 cases of closed ocular trauma and 174 cases of open ocular trauma in children. The incidence of secondary glaucoma caused by closed ocular trauma (20.72%, 52/251) was higher than that of open ocular trauma (8.62%, 15/174), and the difference was statistically significant (χ2=11.323, P=0.001). The onset time of secondary glaucoma caused by closed ocular trauma was earlier than that of open ocular trauma. Hyphema (40.38%, 21/52) was the main cause of secondary glaucoma due to closed ocular trauma, and the mixed mechanism was the main cause of secondary glaucoma due to open ocular trauma (73.33%, 11/15). The two groups received conventional medicine first, and 22 cases (42.31%, 22/52) of closed ocular trauma group and 3 cases (20.00%, 3/15) in open ocular trauma group were converted to surgery because of lose control of IOP. The difference in surgery rate was not statistically significant between the two groups(χ2=2.477, P=0.116). At the last follow-up, the mean IOP of closed ocular trauma group was higher than that of open ocular trauma group (t=2.828, P=0.006). There was no statistical significance in BCVA between the two groups at the last follow-up (t=-1.244, P=0.218). Conclusion Secondary glaucoma caused by closed ocular trauma is more common than open ocular trauma. Treatment with medicine is still the first choice, but filtering surgery is an effective way to control secondary glaucoma with closed ocular trauma. Efficacy of macular massage combined with negative pressure suction for giant idiopathic macular hole 黄斑按摩联合负压吸引治疗巨大特发性黄斑孔的疗效观察

Objective To evaluate the efficacy of 25 G vitrectomy combined with macular massage and negative pressure suction for the treatment of giant idiopathic macular hole. Methods Clinical data of 27 eyes of 27 patients with giant idiopathic macular hole, of which the diameter was 500-800 μm, from Apr.2016 to Dec.2018 in Hanyang Eyegood Ophthalmic Hospital was analyzed retrospectively. Macular massage and negative pressure suction with flute needle were performed after 25 G vitrectomy and peeling of the inner limiting membrane. Results Macular hole closure at 3-5 days postoperatively occurred in 25 eyes(92.6%). Best corrected visual acuity (BCVA, logMAR) at 3-5 days postoperatively were ameliorated from 1.28±0.30 to 1.04±0.37(t=3.860, P=0.032). The postoperative intraocular pressure (IOP) was similar to the preoperative IOP within normal limits(t=2.147, P=0.226). No complication such as iatrogenic injury of macular, bleeding, retinal detachment or secondary glaucoma was recorded in the intra-operative and the follow-up period. Conclusion 25 G vitrectomy combined with macular massage and negative pressure suction contributes to the anatomical repair and visual function recovery of patients with giant idiopathic macular hole. 目的评价25g玻璃体切除术联合黄斑按摩负压吸引治疗巨大特发性黄斑裂孔的疗效。方法回顾性分析2016年4月至2018年12月汉阳Eyegood眼科医院27例直径500 ~ 800 μm的巨大特发性黄斑裂孔患者27只眼的临床资料。玻璃体切除25 G,剥去内限定膜后,行黄斑按摩及笛子针负压吸。结果术后3 ~ 5 d黄斑孔闭合25眼(92.6%)。术后3 ~ 5 d最佳矫正视力(BCVA, logMAR)由1.28±0.30改善至1.04±0.37(t=3.860, P=0.032)。术后眼压(IOP)与术前相近,在正常范围内(t=2.147, P=0.226)。术中及随访期间无黄斑医源性损伤、出血、视网膜脱离、继发性青光眼等并发症发生。结论25g玻璃体切除术联合黄斑按摩和负压吸引有助于巨大特发性黄斑裂孔患者的解剖修复和视觉功能恢复。关键词:玻璃体切除术;按摩,黄斑;吸力,负压;空洞,黄斑,特发性,巨大

Safety and protective measures on performing ophthalmic surgeries during the epidemic of COVID-19 新冠肺炎疫情期间眼科手术的安全防护措施

Objective To investigate the safety of ophthalmic surgery during the epidemic of COVID-19 and to make related protective measures. Methods The clinical data of 12 eyes of 11 patients who underwent ophthalmic surgeries from Jan. 25th to Feb. 28th in Tianjin Medical University General Hospital were collected during the outbreak of COVID-19. There were 1 eye of 1 case with rupture of eyeball, 1 eye of 1 case with corneal perforating injury, 1 eye of 1 case of corneal perforating injury with traumatic cataract, 1 eye of 1 case with intraocular foreign body, 2 eyes of 2 cases of canalicular laceration, 4 eyes of 4 cases of rhegmatogenous retinal detachment, and 2 eyes of 1 case of glaucoma. All patients were monitored temperature and made epidemiological investigations. According to the national epidemic prevention and control requirements and hospital’s emergency management process, an ophthalmologic diagnosis and treatment process had been formulated. All patients were strictly managed in accordance with this process. Body temperature, respiratory tract and other general conditions were monitored on the perioperative period. Results The temperature monitoring, epidemiological investigation, clinical and imaging examinations before consultation in all cases were normal, and all cases were treaded by infectious experts consultation to eliminate COVID-19. Ophthalmic surgeries were performed in 12 eyes of 11 patients with ocular trauma and acute eye diseases, included 6 eyes of emergency surgery, 6 eyes of limited-term surgery, and strictly followed the ophthalmologic diagnosis and treatment process for surgical treatment. All patients recovered well postoperatively, and no serious complication occurred. No COVID-19 occurred among patients, family members and medical staff during the hospitalization and follow-up period. Conclusion The ophthalmic surgeries can be safely performed after strict screening and in accordance with the requirements of prevention and control workflow during the epidemic of the novel coronavirus pneumonia. 目的探讨新型冠状病毒病疫情期间眼科手术的安全性,并制定相关防护措施。方法收集1月25日至2月28日在天津医科大学总医院接受眼科手术的11例患者12只眼的临床资料。其中眼球破裂1眼,角膜穿孔损伤1眼,角膜穿孔损伤合并外伤性白内障1眼,眼内异物1眼,小管撕裂2眼,孔源性视网膜脱离4眼,青光眼2眼1例。对所有患者进行体温监测和流行病学调查。根据国家疫情防控要求和医院应急管理流程,制定了眼科诊疗流程。所有患者均严格按照此流程进行管理。围手术期监测体温、呼吸道等一般情况。结果所有病例会诊前体温监测、流行病学调查、临床及影像学检查均正常,所有病例均经传染病专家会诊处理,消除COVID-19。对11例眼外伤及急性眼病患者进行了12眼眼科手术,其中急诊手术6眼,限期手术6眼,严格按照眼科诊疗流程进行手术治疗。所有患者术后恢复良好,无严重并发症发生。住院及随访期间,患者、家属及医护人员均无新冠肺炎病例发生。结论新型冠状病毒肺炎疫情期间,严格筛查并按照防控工作流程要求,可安全开展眼科手术。关键词:COVID-19;眼科手术;保护;安全

Observation on the efficacy of application of 3D printing technique in repair surgery for the orbital blowout fracture 3D打印技术在眼眶爆裂骨折修复手术中的应用效果观察

Objective To observe the efficacy of application of 3D printing technique in repair surgery for orbital blowout fracture. Methods A retrospective study. The clinical data of 13 patients with orbital blowout fracture from Jul.2016 to Dec.2017 in Peking University International Hospital were analyzed. All patients received thin-slice spiral CT scanning. The data were used to three-dimensional reconstruction and obtained the model of orbital wall fracture. With mirror image repair technique, the fracture area of the affected side was reconstructed with the normal orbital wall shape of the opposite side. The model of fractured orbit and the patch of repair model with individualized shape were printed with 3D printer which was used as template to guide bioresorbable plate shaping during the surgery. The orbital fracture was repaired with the after-shaping bioresorbable plate which accorded with the anticipated anatomical shape. The efficacy of repair was observed after the surgery. Results The postoperative CT showed that the efficacy of fracture repair was satisfactory and the repair matereial fitted well with fracture area. All patients had no visual impairment after surgery. The average measurement of enophthalmometric was (0.54±0.78) mm. Among 5 cases with diplopia before surgery, 4 cases recovered completely and only 1 case had residual slight peripheral diplopia. Conclusion Application of 3D printing technique to guide and repair orbital fracture can achieve satisfactory repair effect. 目的观察3D打印技术在眼眶爆裂性骨折修复手术中的应用效果。方法回顾性研究。分析2016年7月至2017年12月北京大学国际医院收治的13例眼眶爆裂性骨折患者的临床资料。所有患者均行薄层螺旋CT扫描。将所得数据进行三维重建,得到眶壁骨折模型。采用镜像修复技术,将患侧骨折区重建为对侧正常眶壁形状。利用3D打印机打印出眼眶骨折模型和个性化形状的修复模型贴片,作为指导手术过程中生物可吸收板成型的模板。采用符合预期解剖形态的后整形生物可吸收钢板修复眼眶骨折。术后观察修复效果。结果术后CT检查显示骨折修复效果满意,修复材料与骨折部位吻合良好。所有患者术后均无视力损害。眼内测量平均值为(0.54±0.78)mm。术前复视5例,4例完全恢复,仅1例残余轻度外周性复视。结论应用3D打印技术引导修复眶内骨折可取得满意的修复效果。关键词:骨折,眼眶,爆裂;平板,可吸收,生物;印刷、3 d

Observation on efficacy of pranoprofen eye drops in the treatment of Meibomian gland dysfunction 普萘洛芬滴眼液治疗睑板腺功能障碍的疗效观察

Objective To observe the efficacy of pranoprofen eye drops in the treatment of Meibomian gland dysfunction. Methods Clinical case-control study.Total of 62 eyes of 42 cases with Meibomian gland dysfunction in Huaxian People’s Hospital from May 2017 to Jun.2017 were randomly divided into two groups.There were 31 eyes of 21 cases in artificial tears group and 31 eyes of 21 cases in the pranoprofen eye drops group.The two groups were treated with artificial tears(4 times a day) or pranoprofen eye drops (3 times a day) for 1 month, respectively.The efficacy was observed at 1 month after treatment. Results The tear break-up time in the artificial tear group was ameliorated compared with that before treatment(t=2.211, P=0.033). The ocular symptoms, break-up time and corneal fluorescein staining in the pranoprofen group were significantly ameliorated compared with those before treatment(t=2.102, 2.401, 2.385, P=0.046, 0.018, 0.022). The difference of corneal fluorescein staining was not statistically significant between the two groups. Conclusion Non-steroidal anti-inflammatory eye drops therapy is effective for patients with Meibomian gland dysfunction. Key words: Dysfunction, Meibomian gland; Eye drops, pranoprofen; Treatment
目的观察普萘洛芬滴眼液治疗睑板腺功能障碍的疗效。方法临床病例-对照研究。选取2017年5月至2017年6月在华县人民医院就诊的42例睑板腺功能障碍患者62只眼,随机分为两组。人工泪液组21例31眼,丙洛芬滴眼液组21例31眼。两组患者分别应用人工泪液(4次/ d)或普萘洛芬滴眼液(3次/ d)治疗,疗程1个月。治疗1个月后观察疗效。结果人工撕裂组泪液破裂时间较治疗前明显缩短(t=2.211, P=0.033)。与治疗前相比,普萘洛芬组患者眼部症状、破裂时间、角膜荧光素染色均有显著改善(t=2.102、2.401、2.385,P=0.046、0.018、0.022)。两组间角膜荧光素染色差异无统计学意义。结论非甾体类抗炎滴眼液治疗睑板腺功能障碍有较好的疗效。关键词:功能障碍;睑板腺;滴眼液、普萘洛芬;治疗
